With your permission, your child will accompany the class. The students will have a variety of experiences in the participation of the Lakbay Aral and exhibitions at the Manila and nearby provinces Industrial and Power plants, agencies as well as related engineering services, including: Batangas City: SIEMENS POWER PLANT Taguig City: METALS INDUSTRY RESEARCH & DEVELOPMENT CENTER Laguna: MAK-BAN GEOTHERMAL POWER PLANT Quezon City: COLUMBIA WIRES & CABLES CORP. Laguna: CONCEPCION – CARRIER AIRCONDITIONING SYSTEM, INC. Pampanga: YOKOHAMA TIRES PHILS. Bulacan: MOLDEX PRODUCTS INC. Bulacan: PHILIPPINE SPRING WATER RESOURCES, INC. Bulacan: ENERTECH BOILER INDUSTRY, INC. The expenses for this activity including Transportation (from dorm to the company and vice versa), and Board & Lodging) amounting to Php 6694.166 per student (excluding boat fare and allowances). We will stay at MAGARA DORM located at A. Bonifacio St. Brgy. Bangkal, Makati City (Mobile number 0915-3719180). The Fifth and Fourth year Mechanical Engineering students of the University will be having their field trip as part of their curriculum ME 5118 – Industrial Processes, Inspection trips and seminars. This activity will be scheduled on February 14-18, 2011. The venues are in Batangas, Laguna, Bulacan and Pampanga where most companies are located. This is in line with an objective of giving them insights on the operation and the actual appearance of the equipment and devices use in the company.  Likewise, this will give them an idea where to look a job after graduation in their chosen field of specialization.

In view of this, may I request your good office to allow the students conduct the field trip at their own expenses and the undersigned on Official Business to accompany the students.

We will stay at MAGARA DORM located at A. Bonifacio St. Brgy. Bangkal, Makati City. Rest assured that necessary papers for the benefits of our students will be prepared and secured.

We are looking forward for a positive response regarding this request.

Therefore, any loss or injury suffered by me or to my property because of my participation in the activity or use of equipment or facilities during this field trip is my sole responsibility.  The Romblon State University, its Board of Regents, the field trip leader (s), and the field trip coordinator (s) will not be held liable for any such loss or damage to my person or property.
