• ĐTĐ là nguyên nhân tử vong đứng hàng thứ 4-5
ở các nước phát triển.
• ĐTĐ týp 1 và týp 2 là YTNC quan trọng của
bệnh tim mạch
• Xơ vữa là nguyên nhân của 80% tử vong sớm
và 75% lý do nhập viện.
• ¾ của các tử vong này là do BĐMV và ¼ do
bệnh ĐM não và ĐM ngoại biên.
Các YTNC chính của bệnh tim mạch
– Hút thuốc lá
– THA
– ĐTĐ
– RL lipid máu
– Béo phì
• Hoa kỳ: Tần suất các YTNC này giảm trong vài thập niên qua, ngoại trừ ĐTĐ và béo phì

BỆNH TIM MẠCH VÀ ĐTĐ
• Bệnh ĐMV:
– 30% trường hợp HC vành cấp nhập viện có ĐTĐ
– BN ĐTĐ bị bệnh ĐMV ở tuổi trẻ hơn BN không ĐTĐ
• NMCT ở bn ĐTĐ:
– Có thể im lặng (suy tim, loạn nhịp, tăng đường máu cấp, toan cetone..)
– Tỉ lệ tử vong cao hơn trong g/đ cấp và sau nhồi máu
– Thường có biến chứng: loạn nhịp, suy tim, nhồi máu tái phát.
– Tổn thương nhiều mạch máu, nhiều vị trí.
• Thiếu máu cơ tim im lặng: 32-42% bn NMCT/ĐTĐ có triệu chứng không điển hình (khó thở, mệt lả, lú lẫn, buồn nôn và nôn), so với 6-15% ở bn không ĐTĐ.

Hyperglycemia and Acute Coronary Syndrome Scientific Statement From the American Heart Association Diabetes Committee of the Council
on Nutrition, Physical Activity, and Metabolism
1. Hyperglycemia is common and associated with a markedly increased mortality rate and heart failure in patients with and without diabetes who are hospitalized with acute coronary syndromes (ACS). There is an incremental risk when the glucose rises after admission. Blood sugars of >110 mg/dl are associated with a greater than threefold increase in mortality. 2. There are critical gaps in our understanding of the relationship between hyperglycemia and poor outcomes. These gaps include the need to specifically define hyperglycemia, develop optimal ways of measuring and tracking glucose values during ACS hospitalization, and better understand the physiological mechanisms responsible for poor outcomes associated with hyperglycemia. The most important issue, however, is whether elevated glucose is a direct mediator of adverse outcomes in ACS patients or just a marker of greater disease severity. 3. The potential mechanisms for an increasing glucose to increase morbidity and mortality include negative effects on contractility, ischemic preconditioning, endothelial function, inflammatory cytokines, thrombosis, and the QT interval. 4. In the inpatient setting, where the duration of care is relatively brief, there is no single laboratory test (such as glycosylated hemoglobin) that can accurately assess the degree of glucose control. Instead, multiple glucose results should be analyzed; these results may be obtained either from plasma samples or from capillary blood (finger sticks), and represent a variety of fasting and nutritional conditions. 5. There have been several trials of various insulin regimens in diabetics and patients with hyperglycemia, none of which is conclusive. Hypoglycemia is also a major risk factor for a poor outcome. Whether strict glucose control in hyperglycemic patients with ACS will result in similar reductions in mortality and in-hospital complications remains to be established, and needs to be investigated in well-designed randomized clinical trials.

6. Recommendations based on present expert opinion with various degrees of evidence include the following: a. Glucose level should be a part of the initial laboratory evaluation in all patients with suspected or confirmed ACS. b. In patients admitted to an intensive care unit (ICU) with ACS, glucose levels should be monitored. It is reasonable to consider intensive glucose control in patients with plasma glucose >180 mg/dl, regardless of prior diabetes history. Efforts to optimize glucose control may also be considered in patients with milder degrees of hyperglycemia. Until further data are available, approximation of normoglycemia appears to be a reasonable goal (suggested range for plasma glucose 90-140 mg/dl), as long as hypoglycemia is avoided. c. Insulin, administered as an intravenous infusion, is currently the most effective method of controlling glucose among patients hospitalized in the ICU. Care should be taken to avoid hypoglycemia, which has been shown to have an adverse prognostic impact. Treatment should be instituted as soon as feasible, without compromising the administration of life-saving and evidence-based treatments. d. In patients hospitalized in the non-ICU setting, efforts should be directed at maintaining plasma glucose levels <180 mg/dl with subcutaneous insulin regimens. e. ACS patients with hyperglycemia, but without prior history of diabetes should have further evaluation (preferably before hospital discharge) to determine the severity of their metabolic derangements. This evaluation may include fasting glucose and glycosylated hemoglobin assessment and, in some cases, a post-discharge oral glucose tolerance test. f. Before discharge, plans for optimal outpatient glucose control should be determined in those patients with established diabetes, newly diagnosed diabetes, or evidence of insulin resistance.

UNFRACTIONATED HEPARINE (UFH), LMWH,
DIRECT THROMBINE INHIBITOR
• LMWH là sự lựa chọn tốt hơn, không những trong HCVC không có ST chênh lên, mà còn trong NMCT có điều trị tiêu sợi huyết:
- Tác dụng ổn định
- Biến chứng huyết khối do heparine (Heparine – Induced
Thrombosis) thấp hơn nhiều
- Dễ sử dụng
Chưa có nghiên cứu so sánh riêng cho bn ĐTĐ
• DIRECT THROMBINE INHIBITOR: tác dụng giảm tỉ lệ NMCT và tử vong trong HCVC nhiều hơn UFH và LMWH nhưng biến chứng chảy máu nhiều hơn.

The SANDS trial (Stop Atherosclerosis in Native Diabetics Study) was conducted
by Barbara Howard, PhD, from the MedStar Research Institute (Hyattsville, MD)
and colleagues and reported in the April 9, 2008, issue of the Journal of the
American Medical Association.
The 3-year trial randomized 499 American Indian men and women over age 40 with
type 2 diabetes to either aggressive treatment (n=252) of LDL cholesterol and
systolic blood pressure or to standard treatment (n=247).
• The aggressive treatment was intended to bring blood levels of LDL cholesterol
and systolic blood pressure down to 70 mg/dL and 115 mm Hg, respectively.
• Standard recommendations set by the National Cholesterol Education Program
(NCEP) and the National Heart, Lung, and Blood Institute (NHLBI) call for
achieving an LDL of 100 mg/dL and a systolic blood pressure of 130 mm Hg.

TIÊU HUYẾT KHỐI - CAN THIỆP QUA DA
• Tiêu huyết khối được khuyến cáo sử dụng nếu thời gian vận chuyển bn đến Cath lab > 2 giờ
• Bn ĐTĐ ít có cơ hội được tái tưới máu kịp thời vì: TMCB im lặng, biểu hiện lâm sàng trễ hoặc không điển hình, nhiều bệnh lý phối hợp…
• Can thiệp qua da ít được chỉ định hơn do tổn thương lan tỏa nhiều mạch máu vành
NONG ĐMV QUA DA
(KHÔNG ĐẶT STENT)
• National Heart, Lung and Blood Institute - NHLBI(1985-1986): không khác nhau về tỉ lệ thành công thủ thuật và biến chứng giữa bn ĐTĐ và không ĐTĐ
Bn ĐTĐ có tỉ lệ tử vong tại viện và NMCT cao hơn (10% so với 4,8%, p=0,02)
• Emory University (1980-1989): không khác biệt về nguy cơ tử vong, NMCT hoặc PT cầu nối
NONG ĐMV QUA DA
(KHÔNG ĐẶT STENT)
• Emory University: Tỉ lệ sống còn sau 5 năm thấp hơn trong nhóm bn ĐTĐ (88% so với 93%). Tỉ lệ NMCT, cần PT cầu nối về sau cao hơn.
• NHLBI: theo dõi sau 9 năm:
- Tỉ lệ tử vong trong nhóm ĐTĐ cao gấp đôi bn không ĐTĐ (36% so với 18%).
- Tỉ lệ tái hẹp cũng cao hơn: 47% so với 32% (p<0,01): khẩu kính mạch nhỏ hơn, tăng sản nội mạc và tái cấu trúc mạnh hơn trong ĐTĐ
CAN THIỆP QUA DA (CÓ ĐẶT STENT)
• Giảm biến chứng cấp và tái hẹp muộn,
ngay cả trong nhóm nguy cơ cao (ĐTĐ): giãn lòng mạch, chống co hồi mạch máu, giảm tái cấu
trúc.
• Tỉ lệ tái hẹp vẫn còn cao ở bn ĐTĐ (25-
50%), do tăng sản tân sinh thành mạch.
• ĐTĐ (không lệ thuộc insulin) là yếu tố dự
báo độc lập của tái hẹp
CAN THIỆP QUA DA (CÓ ĐẶT STENT)
• EPISTENT (Evaluation of Platelete IIb/IIIa
Inhibitor for Stenting): tỉ lệ cần tái thông lại
mạch máu đích (TVR) sau đặt stent:
- Stent + Placebo : 16,6%
- Nong + Abciximab : 18,4%

• Giảm tỉ lệ tái hẹp, các biến cố tim mạch,

• TAXUS IV: tỉ lệ tái hẹp sau 9 tháng:
DES/không ĐTĐ: 8,5%
BMS/không ĐTĐ: 24,4% (p<0,001)
DES/ĐTĐ-thuốc uống: 5,8%
BMS/ĐTĐ-thuốc uống: 29,7% (p=0,003)

PHẪU THUẬT CẦU NỐI
• Bn ĐTĐ có nguy cơ biến chứng cao hơn và tiên lượng dài hạn xấu hơn sau PT cầu nối ĐMV
• Ngắn hạn sau PT: nguy cơ tử vong, đột quỵ, nhiễm trùng vết mổ, suy thận, và PT cũng cao hơn
• Duke University: tỉ lệ sống còn dài hạn sau PT cầu nối : 74% so với 86% ở bn không ĐTĐ (p=0,02)
• Đức: khi tái hẹp sau PT cầu nối: tỉ lệ tử vong: – Nong lại : 7,2%
– PT cầu nối lại : 37,4% (p<0,006)
KIỂM SOÁT ĐƯỜNG HUYẾT
TRONG PT CẦU NỐI
• Kiểm soát đường huyết hậu phẫu tốt hơn với
Glucose-Insulin-Potassium so với Insulin TDD
(138±4 so với 260±6, p<0,0001)
Giảm tỉ lệ rung nhĩ chu phẩu (16,6% so với 42%, p=0,0017)
Thời gian nằm viện sau mổ ngắn hơn (6,5±0,1 so với 9,2±0,3,
p=0,003)
Giảm các đợt TMCB tái phát (5% so với 19%, p=0,01)
Ít nhiễm trùng chân và xương ức hơn (1% so với 10%, p=0,03)
STENT – PT CẦU NỐI
ARTS (Arterial Revascularization Therapy
Study): tỉ lệ tử vong sau 1 năm ở bn ĐTĐ:
Stenting : 6,3%
(p=0,013)
PT cầu nối : 1,6%
KẾT LUẬN Điều trị bệnh nhân ĐTĐ / HCCH:
• Kiểm soát đường huyết (HbA1C < 7%). Càng sớm khi HCVC/NMCT hoặc hậu phẩu (Insulin TM)
• Dự phòng xơ vữa ĐM và các biến chứng mạch máu bằng Statin (hoặc Fibrate), ngay cả khi chưa có biểu hiện BMV
• Dự phòng huyết khối sớm (Kháng TC).
• Sử dụng ƯCMC, chẹn bêta giao cảm trong và sau NMCT.
• Phẩu thuật cầu nối >>>> Can thiệp qua da (stent phủ thuốc +++)
• Loại bỏ các YTNC tim mạch (THA, béo phì, thuốc lá...)
