SECTION II - INTRODUCTION AND SUMMARY OF WORK

01 INTRODUCTION

The Metropolitan Washington Airports Authority (Airports Authority) is responsible for the operation, maintenance and repair of the vertical and horizontal conveyance systems located at Ronald Reagan Washington National Airport and Washington Dulles International Airport. Collectively these jobs sites will jointly be referred to as the “Airports” and herein separately referred to as “DCA” and “IAD” respectively (Appendix A). The units covered under this contract are located throughout the Airports and are owned and operated by the Airports Authority and by private tenants (Airlines and Concessionaires). They are used by the traveling public and private business enterprises (for passenger and freight transport). The units range in age from newly installed to nearly twenty (40) years old.

Ronald Reagan Washington National Airport Ronald Reagan Washington National Airport served 2.4 million passengers in 2013. Units at DCA passed ASME five year inspections in March of 2011, Periodic inspections are next due in November 2014.

Washington Dulles International Airport Washington Dulles International Airport served 17,235,163 passengers in 2002. Units at IAD passed ASME five year inspections in March of 2011, Periodic inspections are next due in June 2014.

The Conveyance units at IAD vary in manufacturer, model, age and size and are subject to intense use and passenger load that most conveyance units do not experience. Consequently, these conveyance units require a significant higher degree of quality in their preventive maintenance, service, repair, and inspection and testing than typical commercial installations.

The term of this Contract is intended to consist of a two (2) year base period with three (3) one year options.

02 SUMMARY OF WORK

The Contract that will be awarded by this solicitation shall provide routine and “as needed” ASME safety code inspections and tests, technical consulting, preventive maintenance evaluation and equipment data trend analysis for the Vertical and Horizontal Pedestrian Conveyance Systems (elevators, escalators, moving walkways and handicap accessibility/material lifts) at both DCA and IAD for the term of this Contract. The Contractor shall furnish all labor, materials, tools, supplies, equipment, transportation and supervision necessary to provide recurring ASME safety code inspections and technical oversight in the maintenance, care, operation and enhancement of the entire Vertical and Horizontal Pedestrian Conveyance Systems at the Airports (Appendix B). The Contractor shall not only have the responsibility and obligation to ensure all elevators, escalators, moving walkways, and handicap accessibility/material lifts covered in this Specification meet the latest requirements of the American National Standards Institute/American Society of Mechanical Engineers Safety Codes (ANSI/ASME) A17.1, A17.2.1, A17.2.2, A17.2.3, and A17.3 (referred to herein as "ASME A17"), and perform all of the specified consulting, and technical evaluations to assure that all vertical and horizontal pedestrian conveyance systems at the Airports are operating efficiently according to manufacturer’s recommendations.

Any item(s) of work, inspections, parts and service listed in this Specification shall not be construed as limiting the Contractor to those items but rather as minimum requirements.

The conveyance units currently covered by this contract for Dulles consist of 250 units; 110 hydraulic elevators, 6 traction elevators, 17 rope hydraulic elevators, 16 moving walks, and 101 escalators.

SECTION III - DEFINITIONS

ACCEPTANCE INSPECTION AND TEST - The act of performing all tasks required by ASME A17 when accepting the installation of new or renovated equipment.

AIRPORT – Shall refer to either Ronald Reagan Washington National Airport, aka, “DCA” or “Reagan”, and Washington Dulles International Airport, aka, “IAD” or “Dulles” as described in the Specification.

AIRPORTS – Shall mean the DCA and IAD together.

AOA - Aircraft Operation Area - The portion of the Airport used or intended to be used for landing, takeoff or surface maneuvering of aircraft. This is a security area requiring security badging. Workers in this area are required to obtain and display an AOA photo ID credential. Drivers in this area are required to obtain an Aerodrome Vehicle Operator's Permit. (See Appendix D)

ASME A17- Shall refer to and include latest requirements of American National Standards Institute/American Society of Mechanical Engineers Safety Codes (ANSI/ASME) A17.1, A17.2.1, A17.2.2, A17.2.3, and A17.3

AIRPORTS AUTHORITY - The Metropolitan Washington Airports Authority

AIRPORTS AUTHORITY BUSINESS OFFICE HOURS -

DCA 6:00 A.M. through 4:30 P.M. EST/DST, Monday through Friday, excluding weekends and holidays.

IAD 7:30 A.M. through 4:00 P.M. EST/DST, Monday through Friday, excluding weekends and holidays.

CO – Contracting Officer

COTR – Contracting Officers Technical Representative. DCA and IAD have separate and distinct COTR’s and each are responsible for their respective job sites only.

CORRECTION - The elimination of a deficiency.

DCA - Ronald Reagan Washington National Airport

FIVE YEAR INSPECTION AND TEST - Inspections and tests performed that include all items required as listed in ASME A17. To be performed at intervals not longer than five (5) years.

HCS - Hazardous Communication Standard also known as “HAZCON”.

IAD – Washington Dulles International Airport.

INSPECTION AND TEST CHECKLIST FORM - ASME checklist forms, which shall be used by the Contractor to document the inspection and re-inspection of elevators, escalator moving walks and handicap accessibility and material lifts. (A050C7, A134C1 or A141C1)

JOB SITE - The area within the Airports Authority’s property lines for DCA and IAD or portions of such area, which are defined within the Specification.

METROPOLITAN WASHINGTON AIRPORTS AUTHORITY (Airports Authority) - The public body responsible for the operation and management of both Ronald Reagan Washington National Airport (DCA) and Washington Dulles International Airport (IAD).

OSHA - U. S. Occupational Safety and Health Administration. The Federal Government agency responsible for providing the rules and regulations on safety and health requirements in the work place.

PERIODIC INSPECTION AND TEST - Inspections and tests performed that include all items required for a routine inspection and test plus additional items as listed in ASME A17. To be performed at intervals not longer than one (1) year.

PRIMARY TERMINAL OPERATING HOURS – Both DCA and IAD terminals are operational to the public 24 hours a day.

DCA: The hours of 6:00 AM through 11:00 PM EST/DST, daily 365 days a year (includes weekend and holidays).

IAD: 24-hours a day, 7 days a week, 365 days a year (includes weekend and holidays).

QUALITY CONTROL PROGRAM - A method used by the Contractor to assure that quality services are provided to satisfy the Contract requirements.

QUALITY ASSURANCE - A means by which the Airports Authority is able to confirm that the quantity and quality of services received conformed to Contract requirements. These methods/procedures are not intended to aid the Contractor in the performance of the Contract requirements and shall not be a substitute for Contract quality control.

ROUTINE INSPECTION AND TEST - Inspections and tests performed that include all items required as listed in ASME A17. To be performed at intervals not longer than six (6) months.

SERVICES - Includes services performed, workmanship, and material furnished or utilized in the performance of services.

SIDA – Security Identification Display Area

STATE - The Commonwealth of Virginia

SUPERVISOR - Supervises individuals and/or groups/teams of employees/subcontractors.

TYPE I INSPECTION - Inspection services requested by the Airports Authority, the Contractor shall respond on site within 4 hours, perform the services requested and provide the COTR with all checklists and reports immediately after the conclusion of the inspection and prior to the Contractor’s departure from the site.

TYPE II INSPECTION - Inspection services requested by the Airports Authority, the Contractor shall respond on site within 24 hours, perform the services requested and provide the COTR with all checklists and reports within 2 business days.

VANDALISM - Willful or malicious abuse and/or destruction of property.

WORK CONTROL DESK – Unit where Contractor shall check-in and checkout when arriving or departing the job site.

SECTION IV - BASE SERVICES

01 DESCRIPTION OF SERVICES

The Airports Authority’s goal for the Vertical and Horizontal Pedestrian Conveyance Systems is to provide safe, high quality, reliable and uninterrupted services to all Airport users and to achieve excellent customer satisfaction. The Airports Authority’s objectives to accomplish this goal are as follows:

A. To accomplish ASME A17 safety code inspections, post incident inspections, equipment condition surveys and equipment maintenance evaluations through a single Contractor.

B. Excluding scheduled Preventive Maintenance, Safety Code Inspections and Vandalism/Abuse Outages, the Airports Authority expects to achieve system availability of 99 percent during primary terminal operating hours.

C. Provide response to Inspection Service Calls and perform the necessary inspections and/or tests effectively and efficiently.

D. To establish and implement a quality assurance program that will ensure complete and thorough inspections, tests and equipment maintenance evaluations that will result in continuous improvements in the Vertical and Horizontal Pedestrian Conveyance Systems performance.

E. To incorporate a third party preventative maintenance and equipment condition evaluation during the safety code inspection process.

F. Complete all safety code inspections and verify/document compliance within 30 days of the scheduled safety code inspections start date.

G. To know the operational status and functional performance of the Vertical and Horizontal Pedestrian Conveyance Systems at all times.

H. To provide accurate equipment inspection and test status and activity documentation.

02 INSPECTION PROGRAM

A. The Contractor shall furnish all labor, materials and equipment necessary to implement, schedule, execute, track and document the ASME A17 safety code inspection and certification program for all Airports Authority owned and operated elevators, escalators, moving walkways, handicap accessibility lifts and material lifts.

B. The inspection program shall cover the latest required inspections and tests (routine, periodic and 5 year) as listed and described in ASME A17 on all existing vertical/horizontal conveyance systems (Appendix B) located at the Airports.

C. The Contractor shall be capable of fulfilling all requirements of the Specification simultaneously at both DCA and IAD.

D. All inspections and tests shall be performed in accordance with and meet the latest requirements of the American Society of Mechanical Engineers (ASME A17) Safety Code for Elevators and Escalators.

E. Inspectors shall perform the inspections following the guidelines of the latest American National Standards Institute/American Society of Mechanical Engineers Inspectors Manuals (ASME A17)

F. Unless otherwise arranged with the COTR, all fire detection devices shall be inspected and tested during the periodic safety code inspection and meet the latest requirements of the American Society of Mechanical Engineers (ASME A17) Safety Code for Elevators and Escalators.

G. Prior to performing an inspection the inspector shall review the Airports Authority’s equipment history log and compare against current equipment conditions.

H. The Contractor shall document all inspections using the latest version inspection and re-inspection ASME checklist forms.

I. During the inspections the Contractor shall also identify and document any maintenance related deficiencies to include but not limited to; parts, assemblies, items or adjustments which are approaching the end of their useful service life, evidence of improper or lack of maintenance, abnormal wear due to maladjustments, and general condition of the equipment.

J. The COTR shall be notified in writing immediately of any code violation or discrepancy, requiring a unit to be placed out of service or which may cause damage or personal injury.

03 INSPECTION SCHEDULE

DCA Specific:Periodic (Annual) Code Inspections – April Routine (Semi-Annual) Code Inspections – October 5 Year - Code Inspections - 2016

IAD Specific: Periodic (Annual) Code Inspections – June Routine (Semi-Annual) Code Inspections – December 5 Year Code Inspections – June 2016 (Traction Elevators)

June 2017 (Rope Hydraulic Elevators)

Note:� Routine and Periodic Code Inspections are defined as:

Routine Code Inspection – ASME inspection and test to be performed at intervals not longer than six (6) months.

� Periodic Code Inspection – ASME inspection and test to be performed at intervals not longer than one (1) year (includes routine inspection and test as well as additional items required for periodic inspection and test).

� At Dulles Periodic and Routine Inspections shall be executed on the conveyance units in three (3) consecutive phases;

1. Elevators; 2. Moving walkways; 3. Escalators

� Each phase at Dulles must be completed before proceeding to the next. Fire alarm testing shall follow separately to the elevators and/or escalators after completion of their periodic inspections and before proceeding to the next group of conveyance units. Fire alarm verification shall

include witness of the smoke and ionizing detector actuation and operation of the firemen key switch phase 1 and 2.

� The inspections at Dulles are all subject to restrictions of coordination, accessibility, stoppage of work and/or rescheduling by the COTR. Twenty (20) vital conveyance units must be tested off hours between 10 PM to 5A, consisting of 6 hydraulic elevators and 3 traction elevators, and 11 escalators.

04 PRE-INSPECTION MEETING

Prior to the start of the scheduled inspections the Contractor shall attend and participate in an on-site meeting at the Airport with the respective COTR and the assigned support personnel. The purpose of this meeting is to coordinate the upcoming inspection process with the Airports Authority’s Vertical and Horizontal Pedestrian Conveyance Systems maintenance Contractor and Airports Authority maintenance sections.

05 INSPECTION PROCEDURES (Airports Authority Maintained Units)

A. The Contractor shall, when authorized by the COTR, coordinate and schedule the inspections with the Airports Authority’s vertical and horizontal pedestrian conveyance system on-site maintenance Contractor.

B. The Contractor shall follow all check-in/check-out procedures as described in Section VII of this Specification, there shall be exceptions to this requirement.

C. The Contractor’s inspector shall meet with the COTR, at the end of each day while on site performing inspections. The estimated duration of these meetings is fifteen (15) minutes. The purpose of these meetings it to provide the COTR with the inspection check lists, progress of the inspections, discuss any issues concerning the inspection program, staff, equipment or other relevant issues.

06 INSPECTION PROCEDURES (Tennant Maintained Units at DCA)

A. The Contractor shall, when instructed by the COTR, coordinate and schedule inspections directly with the (Owner) of the equipment and notify the Airports Authority at least ten (10) working days prior to each scheduled inspection. When scheduling periodic or five-year inspections the Contractor shall, when instructed by the COTR coordinate directly with the (Owner) organization to ensure that the (Owner’s) maintenance Contractor provides mechanic/technician support. Five (5) year inspections shall be scheduled in writing at least sixty (60) days in advance.

B. Contractor’s inspector shall in addition to completing the appropriate ASME inspection checklist (A050C7, A134C1 or A141C1) prepare and submit within two (2) days of the inspection and test an exception report simultaneously with the checklist, detailing code violations or deficiencies noted during the inspection plus a notation on which violations will require the unit to be placed out of service or re-inspection by the Inspector.

C. The COTR shall provide the Contractor with information on corrections of discrepancies as received from the “owners" and/or service Contractor, at the Airports Authority's request, the Contractor shall re-inspect certain units to verify the correction of identified deficiencies, provide inspection reports and repeat code violation notification letters following procedures described above.

D. The Contractor’s inspector shall meet with the COTR, at the end of each day while on site performing inspections. The estimated duration of these meetings is fifteen (15) minutes. The purpose of these

meetings it to provide the COTR with the inspection check lists, progress of the inspections, discuss any issues concerning the inspection program, staff, equipment or other relevant issues.

SECTION V - SUPPLEMENTAL SERVICES

01 DESCRIPTION OF SERVICES

The Airports Authority may, during the term of this Contract, request that the Contractor perform supplemental services which are outside the requirements of the Base Contract, i.e., provide post-accident/injury inspections and tests (Type I), technical on-site consulting/inspections to the Airports Authority on projects (Type II) such as additions to equipment, rehabilitation/refinishing or repair work required to bring equipment up to standards with the provisions of current applicable laws (Federal, state local, NFPA, ASME/ANSI, BOCA etc. including Americans with Disabilities Act (ADA), OSHA, and ASME A17 Safety Code) and provide data analysis and trending presentations.

02 CONTRACT SERVICES CALL ORDER

A. All supplemental Services work will be requested and approved in advance with a signed Services Call Order by the COTR & CO (if over $10,000.00) using the “Contract Services Call Order” form shown in Appendix I. The Services Call Order will contain a detailed description of the services/materials that are required from the Contractor. The Contractor shall be required to provide the COTR/CO a detailed cost proposal and/or estimate including an itemized breakdown for all labor, parts and materials, as well as a schedule with critical milestones for completing the Services Call Order.

B. Fixed price unit rates included in the contract Price Schedule under Supplemental Services will be used in preparing these estimates. The Contractor shall provide the Airports Authority an accurate written estimate of the cost and time for the Contractor to complete each of the tasks requested by the Airports Authority. Both the cost breakdown and schedule shall be made part of the Services Call Order. The Contractor shall not proceed with any work described in such Services Call Orders until authorized in advance and in writing by the COTR and CO (if over $10,000.00).

C. The contractor shall be reimbursed for the labor and materials to complete such tasks in accordance with the provision of Section XIII of this SOW entitled "Method of Payment". The Authority shall incur no obligation for work outside of Base Service that is not authorized in advance, in writing.

D. The Airports Authority shall incur no obligation for out of scope work that is not authorized in advance, in writing with the exception of Type I inspections on post-accident / injury inspections that may be directed verbally (followed up in writing) at Dulles to initiate the emergency service by Airports Authority staff. Call orders for Type I inspections will be issued the following business day.

03 INSPECTIONS AND TESTS

A. Type I Inspection and Test - The Airports Authority may require that the Contractor respond/perform and deliver services on specific units for various reasons including but not limited to, maintenance concerns, post-accident/ injury inspections or equipment failure evaluations and analyses. The Contractor shall respond on site within 4 hours, perform the services and provide the COTR with all inspection checklists and/or reports immediately after the conclusion of the inspection and prior to the Contractor’s departure from the site.

B. At Dulles, the Contractor shall respond on-site within two (2) hours from a post-accident / injury inspection call seven (7) days a week, 365 days a year, including holidays, to deliver services for an escalator injury.

C. Type II Inspection and Test - The Airports Authority may require that the Contractor respond/perform and deliver inspections and/or tests on specific units for various reasons including but not limited to;

maintenance concerns, equipment improvements, unique equipment operational problems, upgrades or modifications and equipment failure analyses. The Contractor shall respond on site within 24 hours, perform the services and provide the COTR with all inspection checklists, recommendations and/or reports within 2 days from date of inspection.

D. Acceptance Inspection and Test

The Airports Authority may require that the Contractor monitor and/or perform acceptance inspection of new, repaired and/or rehabilitated equipment. Perform on-site inspection to ensure compliance with construction documents, manufacturer specifications, industry standards, and/or warranty requirements.

04 TECHNICAL CONSULTING SERVICES

The Contractor shall when requested through a Services Call Order provide the following services:

A. Analyze data contained in the Airports Authority’s Equipment History, Bar Code and CMMS Databases and provide written reports detailing equipment data and trending evaluation.

B. Develop and perform equipment data analysis, comparison and trending evaluation presentations to upper level Airports Authority management. The Contractor shall prepare and provide all visual aids necessary to support the presentations. These visual aids shall include at a minimum handouts, overhead transparencies, Power Point presentations, etc. The Contractor shall at a minimum identify, make comparisons and discuss; outage trends, safety reset trends, equipment down/up time, response time and provide recommendations to improve the performance of the Airports Authority’s equipment and/or maintenance program.

C. Attend and participate in pre-construction planning, work initiation, progress, inspection, etc. meetings associated with the design and/or construction of new equipment, as well as maintenance, repair and/or upgrade of existing equipment.

D. Review and analyze equipment related maintenance, repair and construction documents and/or drawings to determine if specified design and/or work provides the Airports Authority with the most reliable, high quality, safe and cost effective equipment in compliance with all applicable safety code and regulatory requirements. Contractor shall provide the COTR with a written report that will include its assessment, description of deficiencies and recommended action.

E. Develop plans, specifications and cost estimate for new equipment and/or equipment to be repaired or rehabilitated.

F. Provide the services of two (2) Fire Technicians as a team to test smoke and heat detectors in the shafts of elevators, and obtain acknowledgement of the event at the appropriate Fire Control Panel. The Fire Technicians shall be certified by Siemens, Industries as experienced trained technicians or equivalent approved by the COTR and MWAA Staff. Validation of their certification and experience shall be submitted by the Contractor to the COTR prior to any work being performed.

SECTION VI - CONTRACT START UP

01 KEY CONTRACTOR PERSONNEL

A. The Contractor shall identify and provide the Contracting Officer and COTR with a list of names and telephone numbers of its key personnel who shall be responsible for fulfilling all the requirements of this Specification including responding to emergency callbacks. Failure to be able to contact any of the persons on the list or their refusal to respond in an emergency situation shall constitute a failure to perform and shall be subject to deductions for damages caused and costs incurred by the Airports Authority to obtain alternative response and remedy to the emergency.

B. Resumes and certifications for the Contractor’s key personnel shall be provided to the Contracting Officer and COTR fifteen (15) days prior to the start of the Contract and shall be updated whenever approved changes are made.

02 AIRPORT SECURITY BADGES

A. The Contractor shall ensure all its key personnel while on the job site visibly display at all times a valid Airports Authority AOA badge. The Contractor shall provide the COTR with a list of employees who will be applying for security badges fifteen ten (15) days prior to the start of the Contract. All Contractors employees working at the jobsite shall obtain a security badge no later than thirty (30) days from the award of the Contract.

B. Airport security badges not used for a period longer than 45 days will be deactivated. To reactivate, the Contractor shall be required to request reactivation through the Pass and ID office. The Contractor’s personnel shall have its airport security badge(s) reactivated, if necessary, prior to its scheduled performance of service.

DCA Specific: N/A

IAD Specific: In addition, Contractor shall obtain the necessary custom seal required to access the immigration and custom areas of the airport.

SECTION VII - GENERAL REQUIREMENTS

01 CONTRACT MANAGER

A. The Contractor shall have a minimum of three (3) years’ experience in successfully managing Contracts of the size and scope described in this Specification

B. The Contractor shall provide a Contract Manager. The Contract Manager shall have full authority to act for the Contractor and serve at all times to carry out all the provisions of the Contract.

C. The names of the Contract Manager and an alternate who shall assume the Contract Managers duties when the primary Contract Manager is absent shall be designated in writing to the COTR fifteen (15) business days prior to the Contract start date.

D. The Contract Manager shall be available for calls 24 hours a day, seven (7) days a week. The Contract Manager shall meet as promptly as possible with the COTR at the COTR’s request to discuss the performance of the work or other provisions of the Contract.

E. The Contract Manager shall possess sufficient computer software skills to perform trending, queries and analysis of the Airports Authority’s Equipment History, Bar Code and CMMS databases for the purpose of creating reports and developing presentations using the resulting data.

F. The Contract Manager shall possess the skills and expertise required to develop and perform equipment data analysis, comparison and trending presentations to upper level Airports Authority management.

02 INSPECTORS QUALIFICATIONS

A. All Inspections, tests and services covered by this Contract shall be performed by Certified Elevator Inspectors (CEI) certified by the National Association of Elevator Safety Authorities (NAESA). Inspectors performing work on this Contract shall also be certified by the Commonwealth of Virginia as an Elevator Inspector General through Virginia Department of Housing and Community Development. Inspectors shall have a minimum of 3 years of experience as a certified elevator inspector. A copy of the inspectors' resume and certifications shall be provided to the Airports Authority within fifteen (15) days prior to the Contract start date.

B. In the event the Contractor hires new employees to perform inspections at the Airports, a copy of the inspectors' resume and certifications shall be provided to the Airports Authority within five days prior to the employee’s intended start date.

03 CONTRACTOR PERSONNEL CONDUCT

A. The Contractor’s employees shall at all times while on the job site, whether on or off duty, shall conduct themselves in a professional, orderly and safe manner. Rudeness, fighting, being under the influence of alcohol and/or drugs or bringing and/or consuming alcohol and/or drugs, gambling, soliciting, stealing, taking pictures or bringing cameras or other photographic devices anywhere on Airports Authority property (unless fulfilling the requirements of this Contract), and any immoral or otherwise undesirable conduct shall not be permitted on the job site and shall result in immediate and permanent removal from the job site of any employee engaging in such conduct from work.

B. The Contractor agrees to transfer promptly from the Airports any employee or employees that the Airports Authority advises are not satisfactory, and to replace such personnel with employees

satisfactory to the Airports Authority; but in no event shall the Airports Authority be responsible for monitoring or assessing the suitability of any employee or agent of the Contractor.

04 QUALITY CONTROL PROGRAM

A. The Contractor shall implement an effective quality control program. This program shall insure the Contractor fulfills all the requirements of this Specification. This program shall include but not be limited to including all elements of the quality control program described in the Technical Proposal that the Contractor submitted in response to the Airports Authority's solicitation for this Contract. The Contractor shall, within fifteen (15) days prior to the Contract start date, submit its quality control plan to the COTR for review and approval.

B. This program shall also include but not be limited to the following:

1. A proactive management system based on using quality control inspections as a means of monitoring work performance to ensure services and documentation are being provided in accordance with the Contract documents.

2. Description of how inspections, scheduled and unscheduled, are to be conducted.

3. Documentation, such as inspection forms and corrective action forms that record inspections and recommended corrective action.

C. At all times during the Contract term the Contractor shall provide no less than the level of quality described in this Specification.

05 AIRPORTS AUTHORITY’S QUALITY ASSURANCE PROGRAM

A. The Contractor is responsible for the day-to-day inspection and monitoring of all Contractor work performed to ensure compliance with Contract requirements.

B. Each phase of the services rendered under this Contract is subject to Airports Authority inspections, both during and after completions of work. The Airports Authority’s quality assurance (inspection/evaluations etc.) is NOT a substitute for adequate and consistent quality control (manpower supervision/control/resource management etc.) by the Contractor.

C. The Airports Authority has the right, at all times, to inspect services performed by the Contractor’s workmanship and materials furnished/utilized in the performance of such services to the extent practicable. The Airports Authority shall perform inspections, as it deems necessary throughout the term of the Contract. However, inspections and/or walk-through shall be conducted in a manner that will not unduly interrupt/delay the Contractor’s work.

D. If any of the services do not conform to Contract requirements, the Airports Authority may require the Contractor to perform the services again in conformity with Contract requirements, at no increase in Contract amount. When defects in service cannot be corrected by performing the service again, the Airports Authority may:

1. Require the Contractor to take the necessary action to ensure that future performance conforms to Contract.

2. Reduce payment to reflect the reduced value of the services performed. The Contracting Officer shall make a determination as to an appropriate sum of money that will approximately equate to the reduced service.

E. If, after having been directed by the Airports Authority to correct a Contract deficiency, the Contractor fails to promptly perform the services again or fails to take the necessary action to ensure future performance is in conformity with Contract requirements, the Airports Authority may:

1. Perform the services (by Contract or otherwise) and charge the Contractor any cost incurred by the Airports Authority directly related to the performance of such service.

2. Terminate the Contract for default.

06 AIRPORTS AUTHORITY’S SURVEILLANCE PLAN

A. The Airports Authority may monitor the Contractor’s performance by performing, at a minimum, the checks and inspections listed below:

B. Review Airports Authority’s Contractor Call in Log for Contractor check in/out requirement compliance and response time.

C. Special Activities/Events/Projects – The COTR and/or COTR backup may witnesses and document after-hours events/activities/projects (Tower Elevator Inspections) performed by the Contractor. Monitor at a minimum: arrival/departure time, number and job title of Contractor’s employees, activity performed, and summation of Contractor’s preparedness and performance.

D. Inspection shall be performed by the COTR, or Alternate COTR.

07 RESPONSE TIMES

A. The Contractor shall be available, when requested by the Airports Authority, to respond to vertical and horizontal conveyance systems emergencies as necessary at the Airports 24 hours, 365 days a year for the term of the Contract. The Contractor shall take necessary action as directed by either the COTR, or the Conveyance contractor or MWAA Staff at Dulles, after receiving verbal or written notification of any elevator, escalator, chair lift or moving walkway problems at the Airports. The Contractor shall respond to Type I requests within 4 hours at DCA and 2 hours at IAD, and for Type II requests at either Airport within 24 hours.

B. Response time is defined as from the time the Contractor receives the call from the Airports Authority to the time the Contractor arrives at the equipment involved in the request for service.

08 CONTRACTOR CHECK-IN/CHECK-OUT PROCEDURE

DCA Specific:

The Contractor shall, immediately upon arriving to the job site, check-in with the Airports Authority’s Work Control Desk (417-8572) and immediately prior to their departure from the job site shall checkout with the Airports Authority’s Work Control Desk. During callback check-in the Contractor shall inform the Work Control Desk as to the purpose of the visit. During callback checkout the Contractor shall apprise the Work Control Desk of the status of the unit(s) of which they responded. Check-in and checkout is defined as reporting in person or by utilizing an on-site Airports Authority owned non-cellular phone. There shall be no exceptions to this requirement.

IAD Specific:

The Contractor personnel shall check-in and checkout with the COTR or his designated representative immediately upon arrival at the job site and prior to the end of each site visit respectively. Furthermore, at check-in the Contractor shall inform the COTR of its work plan for each visit, and at checkout the Contractor shall inform the COTR of the actual work performed and any special activities.

09 COMMUNICATION AND COORDINATION

The Contractor shall maintain an effective Communication and Coordination Policy with the Airports Authority utilizing email, telephones, faxes, pagers, etc. to ensure the Airports Authority is kept abreast of current equipment status, planned outages, entrapments, vandalism etc. for the term of the Contract.

10 ACCIDENTS

The Contractor shall be responsible for promptly notifying the COTR and Airport Police of any accidents arising from the performance of this Contract involving bodily injury to workers, building occupants, visitors, or other persons. The COTR will provide information necessary concerning whom to contact and the specific form of the follow-up written notice.

11 SECURITY PROCEDURES

A. The Contractor shall not permit any employee to have keys for access to locked rooms until it has been determined that permitting such person(s) to have such access shall not be contrary to the Airports Authority’s interest, and that the individual(s) is authorized to be admitted in accordance with applicable security orders, rules, regulations and instructions as determined by Airports Authority.

B. The Contractor shall secure and safeguard all keys, key cards, and any other entry devices and codes provided by the Airports Authority. The Contractor shall maintain a record of the key numbers issued to its employees. These prohibitions and requirements shall also be applicable to all individuals with regard to access, removal, and/or possession of any information, confidential data, materials, supplies, or equipment. The Contractor shall not duplicate and shall not allow any such issued items to be duplicated or removed from the job site. All keys and other entry devices used by the Contractor’s employees in the performance of the work shall be returned to the Airports Authority when the Contract expires.

C. The Contractor shall safeguard all the identification cards, issued its employees and subcontractors by the Airports Authority to fulfill the requirements of this Specification. The Contractor shall ensure these cards are visibly displayed by the respective individual at all times while employed on site at the Airport to fulfill this requirement.

D. The Contractor shall immediately report to the Airports Authority all keys issued to it by the Airports Authority that are lost or stolen. The Contractor shall be responsible for all loss, damages and expenses that the Airports Authority incurs as a result of the Contractor’s loss of Airports Authority keys. These costs include the expense of changing all locks to which the lost keys provided access. The cost of changing locks or keys to the building rooms or areas accessible by the lost or stolen keys will be deducted from the Contractor’s invoice to the Airports Authority for the services performed under the Contract.

E. The Contractor shall ensure that, under no circumstances any of its employees shall enter an area not authorized for access by the COTR for the Contractor.

F. The Airports Authority will not issue keys for sensitive security areas. Instead, the Airports Authority will require the Contractor’s employees to be accompanied at all times in this area by either Authority employees or the Contractor’s own employees who have authorized access to the area.

G. The Contractor, its subcontractors, and all its employees shall be subject to, and shall at all times, conform with any and all rules, regulations, policies, and procedure pertaining to security at the airport. Any violations of the rules, regulations, policies, and procedures may be cause for immediate termination.

H. Contractor shall be responsible for, at its own expense, obtaining the proper security clearance, fingerprinting, training, badges and custom seal required to access the restricted areas of the airport including the Air Operations Area (AOA), Security Identification Display Area (SIDA), Customs and Immigration. Identification badges issued by the Airports Authority must be visibly worn at all times while in the SIDA.

I. The Contractor shall be responsible for, at its own expense, compliance with the requirements and procedures to obtain approval of any motor vehicle to operate in the Air Operations Area (AOA).

12 SAFETY

A. The Contractor and each of his employees shall comply with all applicable OSHA and Airports Authority rules and practices, including directives issued by the Airport Manager, Airport Operations, MWAA Police and Fire Departments, Federal Aviation Administration, and Airports Authority Divisions while on the job site.

B. The Contractor shall provide and ensure that all personnel at the work site wear the safety devices/apparel described below when in areas designated by the Airports Authority.

� Approved back support and protective devices � Eye protection in compliance with ANSIZ87.1. -1968. � Approved Hearing Protection � Approved hand protection � Other safety devices/apparel when conditions warrant

C. The Contractor shall furnish documentation as directed by the COTR of the completion of the approved safety training of equipment operators and other personnel. The safety training shall comply with all OSHA standards and a sample program shall be submitted to the COTR.

D. The Airports Authority reserves the right to inspect all areas for safety violations at its discretion, direct the Contractor to make immediate improvement of necessary conditions and/or procedures, and/or stop the work if other hazards are deemed to exist.

E. In the event that the Airports Authority should elect to stop work because of any type of existing safety hazards after the Contractor has been notified and provided ample time to correct, the Contractor shall bear all costs for eliminating the hazard(s) and shall not be granted compensation for the work stoppage. The Contractor shall pay all additional expenses.

F. The operation of the Contractor’s vehicles or private vehicles by the Contractor’s employees on or about the property shall conform to posted regulations and safe driving practices. Aisles, passageways, alleyways, entrances or exits to fire protection equipment must be kept unobstructed at all times.

G. The Contractor shall be responsible for initiating, maintaining and supervising all safety precautions and programs in connection with the performance of the Contract. The Contractor shall take all necessary precautions for safety of, and shall provide reasonable protection to prevent damage, injury or loss to persons, properties, equipment and vehicles.

H. Damage caused by the Contractor to any properties shall be repaired or replace to the satisfaction of the Airports Authority at the expense of the Contractor. The Airports Authority, at its sole direction, may elect to repair or replace the damaged property, and deduct such costs from monies due the Contractor.

I. The Contractor shall, within fifteen (15) days prior to the Contract start date, submit its own detailed safety and protection plan/program that shall comply with all safety, environmental protection, property protection and health provisions of the Contract.

J. Prior to use of any products or materials, the Contractor shall provide the following submittals for review and approval by the COTR. Contractor shall furnish trees copies of each submittal.

1. Manufacturer’s product data and literature 2. Manufacturer’s installation recommendations 3. Samples, if required by the COTR 4. Material Safety Data Sheets (MSDS)

13 FIRE PREVENTION AND PROTECTION

A. Fire prevention and protection at Airports Authority facilities property is essential. The Airports Authority shall provide limited fire prevention equipment within the facilities. The availability of fire protection equipment provided by the Airports Authority shall not limit the Contractor’s responsibility or liability for maintaining a reliable fire prevention and protection program for its employees and the property serviced.

B. The Contractor shall be knowledgeable of and provide adequate and appropriate training for all employees in the proper method of reporting a fire. All pertinent information regarding fire-reporting procedures may be obtained from the COTR.

14 SMOKE FREE ENVIRONMENT

The Airports Authority’s facilities are smoke free. The Contractor and its employees shall adhere to the rules and regulations in regard to this facilities maintenance of a smoke free environment.

15 LOST AND FOUND PROPERTY

The Contractor shall turn in to the Airports Authority Police Department all property found on the property of Ronald Reagan Washington National Airport and Washington Dulles International Airport immediately, any violations or disregard of the rules, regulations and/or policies may be cause for immediate termination.

SECTION VIII - DOCUMENTATION AND REPORTING

01 INSPECTION CHECK LIST FORM

A. Inspectors shall complete the appropriate ASME inspection checklist form (latest version) for each unit, combining units on a single inspection checklist form will not be accepted. Inspectors shall provide in the comments section, a detailed description of any code violations or deficiencies noted during the inspection plus a notation on which violations will require the unit to be placed out of service or require re-inspection.

B. Units requiring an annual inspection certificate shall have the words “Issue Certificate” boldly printed or stamped in the comment section on the inspection checklist after the inspection or re-inspection has been completed and the unit is found in compliance with all safety code requirements.

C. The Contractor shall document on the inspection checklist form before and after values/settings of any equipment that requires adjustment to be in compliance with ASME A17 Safety Code for Elevators, Escalators, and Moving Walks.

D. The Contractor shall provide DCA with three copies of the completed inspection checklist by close of business on the day the inspection or re-inspection is completed; on all services at Dulles, the contractor shall provide electronic copies of the report emailed to the COTR, and both a hard copy and email for the conveyance service contractor. The COTR shall be notified in writing immediately of any code violation or discrepancy, requiring a unit to be placed out of service or which may cause damage or personal injury.

02 REINSPECTION CHECK LIST FORM

Inspectors shall complete the appropriate ASME inspection checklist form (latest version) for each unit re-inspection, combining units on a single re-inspection checklist form will not be accepted. The Contractor shall attach a copy of the original inspection check list form to the re-inspection check list form and submit to the COTR by close of business on the day the re-inspection is completed.

03 SAFETY CODE COMPLIANCE LETTER

The Contractor shall provide the COTR with a Safety Code Compliance Letter no later than 5 business days following the completion of an inspection cycle (Routine, Periodic and 5 year) certifying to the Airports Authority that all units cover under this Contract were inspected and are in compliance with ASME A17 safety code requirements. The letter shall be on Contractor’s company letterhead and shall identify, at a minimum; type of inspection, the code that the units were inspected by, date inspection performed, unit number, type of unit and unit location.

04 EQUIPMENT INSPECTION HISTORY DATABASE

A. Contractor shall maintain and update, for the term of the Contract, an automated informational database, compatible to the Airports Authority’s Computer System of each Airport, of all units on the Airports requiring inspections covered under the ASME A17 code. The Computer Software that shall be used to maintain this data shall be of Microsoft Office Professional compatible (latest version) and an example submission shall be approved by the Airports Authority within fifteen (15) days prior to the Contract start date based on APPENDIX B inventory.

B. The database shall include, at a minimum, all information contained in Appendix B including an update of the inspection scheduling information. The Contractor may suggest additional information to be

included and submit it to the Contracting Officers Technical Representative (COTR) for approval prior to adding it to the database. The database shall also include a section covering inspections for all units included in this inspection program. This section shall, at a minimum, include the following information.

1. Unit number 2. Unit type 3. Location 4. Date of recent inspection 5. Type of recent inspection 6. Inspection Status, i.e., issue certificate, re-inspection required 7. Description of code violation (if applicable) 8. Description of deficiencies and recommended action 9. Re-inspection date 10. Re-inspection status, i.e., issue certificate, re-inspection required 11. Current certificate expiration date 12. Next scheduled inspection date13. Next scheduled inspection type

C. The Airports Authority shall provide the Contractor with additional or updated information to be added in the database; such as new equipment, dates of projected renovations, scheduled major maintenance and/or other related items.

D. The Contractor shall provide the Airports Authority with an updated copy of the database in electronic, printed and computer storage media form (3 1/2" disk, CD ROM) Microsoft Office Professional (latest version) compatible whenever information is added/deleted or updated

E. The Contractor shall provide the COTR with an electronic copy of the Equipment History Database within 5 business days after the Contractor has completed an inspection cycle (routine, periodic or 5 year).

05 MACHINE ROOM/SPACE CHECK CHARTS

A. The Contractor’s Inspector shall review, make entries and sign the Machine Room/Space Check Chart each time the Inspector enters the machine room for inspection. The check charts currently document all inspections, preventive maintenance and corrective repair activities completed on the systems and equipment. The Contractor shall note on the inspection checklist form any discrepancies relating to the check chart entries.

B. The charts are configured to include at a minimum the following information:

1. System, equipment, assembly and part identification. 2. Description of work activity (i.e. weekly PM, monthly PM, annual PM, repair, routine/periodic

inspection etc. etc.) 3. Description of the problem, diagnostics and repair (i.e. reset controller, replaced belt, replaced

control valve “o” ring, added 2 gallons of oil etc.). 4. Time when system/equipment was removed from service and restored to operation. 5. Comment space for recommendations, follow-up activities and other commentary related to

maintenance, service, and warranty. 6. Oil usage log 7. Service personnel name, date and signature

SECTION IX - SPECIAL REQUIREMENTS

01 PERMITS AND RESPONSIBILITIES

A. The Contractor shall, without additional expense to the Airports Authority, be responsible for obtaining all necessary licenses and permits. The Contractor shall also be responsible for all damages to persons or property that occur as a result of the Contractor’s negligence and shall take proper safety and health precautions to protect the work, the workers, the public and the property of others. In addition, the Contractor shall be responsible for all materials delivered and work performed until completion and acceptance of the entire work.

B. The Contractor shall comply with all applicable revisions, additions, changes and/or upgrades to any Federal, state, and municipal laws, codes, and regulations which are in effect on the date of Contract and which affect the performance of the work. The Contractor shall also obtain and pay the costs of any royalties and licenses for any patented or copyrighted items used in the performance of the work.

C. It shall be the responsibility of the Contractor to promptly notify the COTR if an official in charge of compliance with the Occupational Safety and Health Act visits the work site.

02 REGULATION REQUIREMENTS

A. The Contractor, including its on the job site employees and subcontractors, shall comply with all applicable Federal, state, local, Airports Authority and the Airports regulatory, code and procedural requirements. This shall include but not be limited to the Contractor complying with the following Airports Authority requirements:

1. The Airports Authority’s: � Construction Safety Manual � Orders and Instructions � Bulletins � Design Manual

2. The Airport’s: � Advisories � Orders and Instructions � Security, Traffic and Parking Requirements � Safety Procedures including Lockout/Tag-out, Confined Space Entry, Hazardous

Materials, Material Safety Data Sheets etc.

B. The Contractor shall report all incidents and accidents immediately to the Airports Authority in accordance with Federal and State laws and regulations and Airports Authority Orders and Regulations.

03 ASBESTOS CONTAINING MATERIALS/LEAD BASED PAINT

A. Numerous facilities at both Airports were constructed prior to 1981. Therefore, facilities should be presumed to have both Asbestos Containing Materials (ACM) and paint containing lead in their construction.

B. Prior to undertaking any activities that could disturb these materials the Contractor shall obtain prior written approval from the Airports Authority to proceed with such activities.

04 HAZARDOUS/CARCINOGENIC MATERIALS

A. The Contractor, its employees, or subcontractors or their employees shall not bring, produce, use, or store on the job site any hazardous or carcinogenic products without prior written approval by the Airports Authority. All hazardous and/or carcinogenic waste transported or generated on-site at the Airport by the Contractor must be properly disposed off the Airport site by the Contractor as required by law and at no cost to the Airports Authority.

B. The Contractor shall provide the Airports Authority with complete, legible copies of all regulatory notices, violations, citations, etc. received by the Contractor that pertain directly or indirectly to the fulfillment of this Statement of Work.

05 VOC REQUIREMENT

The Contractor shall use on the job site only chemicals and cleaning products that do not exceed the national Volatile Organic Chemical (VOC) limitations rule(s) published by the U.S. Environmental Protection Agency (EPA).

06 HAZARDOUS WASTE

A. The Contractor shall initiate Hazardous Waste Management training and enforcement programs to ensure employees are aware that the domestic drains, and storm drains shall not be used to dispose of gasoline, paint, thinners, hydraulic fluid, solvents, concentrated cleaning agents and other toxic material.

B. The Contractor is responsible for collecting, accumulating, recycling, and/or off-site disposal of its hazardous and toxic waste off the Airport in compliance with Federal, state and local laws governing hazardous waste storage and disposal.

C. The Contractor shall provide the Contracting Officer and the COTR with documentation of hazardous materials or wastes that are accumulated, handled, generated, or disposed of by the Contractor’s operations. The documentation shall demonstrate the adequacy of the handling and disposal operations used by the Contractor and will demonstrate that the Contractor activities will not result in contamination of Airport properly. The Airports Authority shall provide this documentation upon request during periodic environmental inspections of the Contractor’s premises. The Airports Authority shall be copied on all correspondence with regulatory agencies concerning the Contractors compliance with environmental regulations.

D. If the Contractor generates hazardous waste in an amount that makes it subject to state and EPA hazardous waste requirements, the Contractor shall apply for a Hazardous Waste Generator Identification Number. Hazardous waste shall be shipped off the Airport using the Contractor’s Hazardous Waste Generator Identification Number documented on a complete and properly signed Uniform Hazardous Waste Manifest. The Contractor shall be required to submit an Annual Hazardous Waste Report to the State of Virginia Department of Environmental Quality.

E. The Contractor shall be responsible for developing a Resource and Conservation Act Contingent (RCRA) Plan if the amount of hazardous waste generated places it into a category that requires a plan.

F. The Contractor shall be responsible for notification and reporting required under SARA, Title III regulations.

G. The Contractor shall, at start of Contract, implement a written hazardous waste spill contingent plan listing materials used, spill prevention procedures, containment equipment and procedures to be used in the event of spill, personnel protective equipment requirements, notification procedures, in accordance with the Resource Conservation and Recovery Act (RCRA) and the Occupational Safety and Health Administration (OSHA) regulations.

SECTION XI - CONTRACTOR FURNISHED RESOURCES

01 GENERAL

The Contractor shall furnish all resources (i.e. supervision, labor, materials, supplies and equipment) necessary to fulfill all the requirements and satisfactorily perform all the services described in this Specification in a safe, orderly, timely, efficient and workmanlike manner. The Contractor shall provide any additional resources to fulfill the Contract requirements at no additional cost to the Airports Authority.

02 SAFETY EQUIPMENT

The Contractor shall provide all safety equipment/devices, personal protective equipment and clothing as required for its Inspectors.

SECTION XII - AUTHORITY FURNISHED RESOURCES

01 INSPECTION SUPPORT

The Airports Authority will provide an inspection support team for all safety code inspections through its current Vertical and Horizontal Pedestrian Conveyance Systems Maintenance Contractor. The maintenance Contractor will provide free and clear access to all equipment and perform inspection tests and adjustments for the inspector to witness. Airports Authority maintenance or other Contractor will provide fire detection device testing support. MWAA Staff or the Airports Authority’s conveyance maintenance Contractor will provide security escort services to un-badged inspectors.

02 AIRPORTS AUTHORITY CONTACT INFORMATION

The Airports Authority will provide phone numbers, e-mail and mailing addresses for the Contracting Officer, COTR and other Airports Authority key personnel as are warranted.

SECTION XIII - METHOD OF PAYMENT

The Contractor shall submit a separate invoice for both DCA and IAD on a monthly basis for all services completed to the satisfaction of the respective COTR at the end of the month in which the services were performed. The Airports Authority shall incur no obligation for out of scope work that is not authorized in advance, in writing through an authorized service call order, except for Phase I Inspections at Dulles, as noted in 02.D. These monthly invoices shall be itemized to provide a breakdown of cost for all Contract Base Services or Supplemental work according to the following:

01 BASE SERVICES

A. The Contractor shall invoice the Airports Authority for inspection services at the end of each calendar month in which the work was performed and completed in accordance with the Specification, and shall be paid for actual services performed.

B. The Airports Authority reserves the right to withhold a portion of the monthly payment to the extent the Contractor has not fulfilled the requirements of the Specification for a conveyance unit or the month in which their services were performed.

02 SUPPLEMENTAL SERVICES

The contractor shall be reimbursed for the labor and materials required to complete Supplemental Service tasks in accordance with the fully loaded rates specified in the Price Schedule under Supplemental Services. When materials are provided in the course of performing requested supplemental services, the contractor must submit copies of original invoices with the supplemental services invoice. The contractor is not allowed to “mark-up” shipping, handling or freight charges.

The contractor shall not invoice the Authority for any work unless a Service Call Order under $10,000 is signed by the COTR, over $10,000 is signed by the COTR and counter signed by the Contracting Officer has been received or for any work described in a Service Call Order until after all the work described in the Service Call Order has been completed to the satisfaction of the COTR.

SECTION XIV - CONTRACT PHASE OUT

01 SECURITY DEVICES

Upon expiration/termination of the Contract or discontinuance of employment of any of Contractor personnel working in the Airport, all airport keys, security badging and all other Airports Authority identification shall be surrendered to the COTR.

02 RECORDS AND DOCUMENTATION

Upon Contract termination or the end of the Contract period all records and documentation pertaining to each Airport, including, but not limited to Phase I or II Inspection Reports, periodic and routine AMSE Inspection reports, Databases, Drawings, O&M Manuals, Preventive Maintenance Schedules, Preventive Maintenance Records, Equipment History Data, Machine Room/Space Charts, Vandalism/Abuse Repair Data, Entrapment Data etc. shall be submitted to the Airport COTRs in electronic format on a CD for each respective Airport, and remain the sole property of the of the Airports Authority.
