INTRODUCTIONSouthern Illinois University Athletic Department recognizes that the safety of our guests,

staff, and participants are of utmost concern in the operation of events in any of our athletic facilities. This plan has been created to insure the wellbeing of our guests, participants, and athletic facility personnel in the event of an emergency when an event is being held in one of our facilities. When the facility is in normal business day operations, please refer to the building’s Building Evacuation Response Team (B.E.R.T.) Manual.

This plan is intended to provide a guide for emergency operations during an event using one or more of Saluki Athletics facilities, and to assist key officials and emergency organizations in carrying out their responsibilities for the protection of life and property. Personnel with intimate knowledge of unwritten plans or procedures may be unavailable at the very time it becomes necessary to implement them. This written plan will furnish a guide to be referred to as needed, by unfamiliar personnel, while also serving to refresh the knowledge of key individuals and can be used to inform persons who become replacements.

The successful outcome of an emergency situation requires that the plan be followed efficiently and as closely as possible while granting those in charge the authority to exercise the flexibility needed to accomplish the plan’s intent.

The procedures that follow are not a stand-alone plan. This plan must be supported by other county and city emergency plans, the University Disaster Plan, appropriate equipment resources and trained personnel.

We hope it will never have to be used, but will always be prepared to implement it at any time.

Southern Illinois University Arena Location: 1400 Arena Drive. Usage: Sporting events, trade shows, circus, graduations, concerts, etc.Seating Capacity: 8,500 combination of chair back and bleacher seating.Entrance to Arena Floor: 12’ H x 8’W, but only 11’6”H at the lights.Entrance to Upper concourse: Limited to double doors on both east and west concourses.Arena Floor Size: 101’W x 206’L, wall to wall. EMERGENCY TYPESAny of the following emergencies may require the Emergency Plan to be put into effect, upon determination of the Athletic Director and other emergency management personnel:

Criminal emergencies are incidents where the commissioning of a crime results in casualties or potential for casualties. Such emergencies include:

Hostage situations, Terrorist activities, Threat of harm through use of some lethal weapon, Bomb threats, etc., Crowd control problems.

Natural disasters are emergencies created by some natural, unforeseeable event that results in casualties or the potential for casualties. Such events include:

fire, flood, other weather related emergencies (tornadoes, thunderstorms, lightning, etc.), seismological conditions, electrical hazard due to systems failure, power outage, stadium/scoreboard/equipment collapse, any condition related to the structural integrity or potential for a hazardous condition in a

facility, any unforeseeable events such as a plane/helicopter crash, etc.

Medical only emergencies are incidents created by some inherent medical condition that has caused or has the potential to cause casualties. Such conditions would include food poisonings, mass illnesses, conditions where multiple injuries are caused by action of a crowd (crowd rushes

the field, crowd panics, etc.), and any emergency of a medical nature not created by a criminal activity or natural disaster.

AUTHORITY TO ACTIVATE THE PLANThe authority to activate the plan is vested in the Chancellor or Designee.Upon activation, the authority to implement the plan is vested in the Director of Athletics, Director of Public Safety or in his/her absence, one of the following in line of succession:

1. Associate Athletic Director - Facilities2. Director of Athletic Facilities and Game Operations3. Operations Managers4. Highest Ranking Athletic Department employee

Upon activating the plan, the activating official (Chancellor of Designee) becomes the Incident Command Team Leader for the plan and is authorized to make such on-the-spot changes in it as are necessary to further its intent and purpose.

Criteria for Activating the PlanThe Chancellor’s decision to activate the Emergency Action Plan is based upon the presumed existence of one or more of the following considerations, namely that:

Loss of life or major injuries has occurred or is occurring. Loss of life or major injuries is likely to occur unless guests and employees are

vacated from the facility or an area of the facility. Damage to property is likely to take place in areas where guests or employees are

located. Interruption of a primary service to the facility is such that continuation of an event

is impossible.

Stages in the Emergency Action PlanThe following are the stages in the plan:

What follows is a general flowchart in regards to conducting the Emergency Action Plan:1. An emergency situation is reported to the Chancellor by the Director of Athletics, Security,

Safety, Medical, or Associate Athletic Director - Facilities.2. An initial site assessment is conducted by the Medical, Security, Safety, and/or Associate

Athletic Director - Facilities.3. A report is made to the Chancellor and a recommendation made on advisability of

implementing the Emergency Action Plan.4. If plan is activated, Incident Command Post is immediately designated and members of

Incident Command Team proceed to that location.5. Appropriate notifications are made, including local emergency management personnel if

necessary. Local emergency management personnel may assume role of Incident Command Director upon arrival.

6. Employees are notified by appropriate supervisors of emergency situation7. Incident Command Team Leader directs the use of appropriate public address announcement

and video board messages to notify guests of emergency situation and proper procedure. Evacuation (partial or total) as necessary will be conducted by Security personnel and/or

Event Staff as needed. SIU DPS and SIU Game Staff will be responsible for Traffic and Crowd control along with

Carbondale Police Department as needed.8. Liaisons are appointed by Incident Command Team members to help coordinate activities in

the field.9. Members of Incident Command Team make proper notifications to personnel regarding

assignments and delegation of authority.10. Staging areas (vehicle, helicopter, treatment, triage) are set up as needed by directed

personnel. Ongoing assessment will determine if alternate staging areas are required.

Incident Command Team members maintain constant communications with personnel through liaisons/cell phones/radios and continually monitor progress of emergency situation until it is resolved.

Incident Command Post/Communications CenterRole

To locate in one place, if feasible, the Emergency Command Team members and provide an initial meeting point from which to share site assessment information.

To provide a centralized communications link to emergency personnel. To provide most efficient communications and access to personnel and materials at

all locations of the facility affected by an emergencyLocation

EquipmentThe following equipment will be kept on hand for use in the Emergency Command Post:

Directories of relevant telephone and contact numbers Maps and diagrams of facility and surrounding areas Legal Pads, writing instruments

Incident Command Post Personnel and Responsibilities Incident Command Team Leader –Chancellor or Designee, is responsible for the direction of and all final decisions with respect to the emergency. When an emergency is declared, the Incident Command Post Leader notifies the following individuals:

a. Security (DPS, Carbondale Police)b. Associate Athletic Director - Facilitiesc. Director of Athletic Facilities and Game Operationsd. Participants and Officialse. Public Address and Video Board Operatorf. Plant Operations (if required)

Director of Athletics - will be responsible for coordinating communications throughout the facility and with the University. This individual will assign facility and/or Game Management Personnel to assist in the coordination of other functions as needed.

Serves as stand-by for the Chancellor Supervises the following Employees.

o Communications Supervisoro Director of Game Operationso Security and Ushering Supervisorso First Aid Supervisor; and trainerso Concession Stand Supervisor

Security Director -The ranking on-site law enforcement officer will assume this role. Serves as liaisons between law enforcement agencies and the Command team. Coordinate security, crowd control, and traffic contingencies. Will act as a consultant with the Incident Command Team and help coordinate the

site assessment and advise members of the team on alternate staging areas and evacuation routes, etc., as they become necessary.

Serves as Sergeant-at-arms at the Incident Command Post.

Associate Athletics Director – Facilities or designee – Will be responsible for assisting in the coordination of incident response.

Acts on behalf of the Director of Athletics Designates and supervises the Runners requested by the Command Team (if needed).

Medical Commander - Assistant Athletic Director of Athletic Training, Athletic Trainer, or a designee, will assume this role.

Serves as Coordinator of the following areas:a. Ambulance stagingb. Treatment Areasc. Deceased individuals

Works with EMS Chief in formulating a site assessment and making medical provisions.

EMS Chief - The ranking on-site member of the EMS units will assume this role. Works with Medical Commander in formulating a site assessment and making

medical provisions.

Fire/Rescue Chief - The ranking member of the Fire/Rescue units will assume this role. Serves as liaison between firefighting and rescue agencies and the Command

Team. Responsible for containing hazardous conditions, extracting victims, and

assessing structural and HAZMAT risks, as well as assisting the EMS Chief in coordinating emergency transportation.

Site AssessmentEmergency decision making follows a set protocol—the Incident Command Team Leader, Medical Director or a designee assesses the situation with the Associate Athletics Director - Facilities and the Security and/or Safety Director.

1. Approximate number of casualties2. Type of casualties (e.g., burn, etc.)3. Accessibility to the scene4. Inherent scene dangers5. Any unusual assistance that may be needed6. Hazardous materials personnel7. Special equipment

At the discretion of the Chancellor and under consultation with the Director of Athletics, Medical, Security and/or Safety Director, the Emergency Action Plan will be put into effect if this initial assessment indicates its necessity.

General Emergency ProceduresTraffic/Crowd ControlThe SIUC Department of Public Safety coordinates traffic and crowd control with the assistance of Athletic Facility Staff. In the event of an emergency, SIUC Police will route and direct all

traffic and direct evacuated crowds to and from the emergency scene. Guests are to follow closely all orders and directions given by Police officers and Game Staff to help any evacuation proceed in a calm, efficient manner.

FireEvery employee upon seeing, smelling or being informed of smoke or fire will initiate one or all of the following:

1. Contact Game Management personnel and your supervisor immediately. Give the exact location of the fire and describe what is burning.

2. Use a cellular phone, or radio to contact a supervisor.

3. Activate a fire pull box.

4. After reporting the fire, use proper extinguishers available in your area. Do this only if you are certain the fire can be contained. Employees should familiarize themselves with the location, type, and proper procedures for operating fire extinguishers within the facility, as well as the location of hoses.

DO NOT DELAY THE SOUNDING OF A FIRE ALARM IN ORDER TO ATTEMPT TO EXTINGUISH A FIRE. NEVER REMAIN IN SMOKE-FILLED AREAS OR FIRE AREAS TO MAKE EMERGENCY PHONE CALLS. CLOSE ALL FIRE DOORS. DO NOT ATTEMPT TO FIGHT A FIRE THAT IS BEYOND YOUR CONTROL.

Upon receipt of an alarm, notification will then be made to occupants within the facility via the video boards, public address system and fire alarm system.

EvacuationThe decision to evacuate the facility prior to the arrival of the Fire Department will be

made by the Director of Athletics and the Chain-of-Command. Upon receiving word that the building is to evacuate, the Director of Athletics or Associate Athletic Director – Facilities will make an announcement on the staff radio to that effect. Immediately thereafter, the Director of Athletics or Associate Athletic Director - Facilities will also direct the reading of a public address announcement, as well as video board message that will instruct all personnel and facility occupants where to go.

SAMPLE ANNOUNCEMENT LANGUAGE“LADIES AND GENTLEMEN, MAY I HAVE YOUR ATTENTION PLEASE. DUE TO DIFFICULTIES IN CERTAIN AREAS OF THIS FACIILTY, WE REGRET THAT THE BUILDING MUST BE TEMPORARILY EVACUATED. WE ARE ASKING THAT YOU FOLLOW THE DIRECTION OF SIU GAME STAFF AND POLICE OFFICERS TO THE NEAREST EXIT IN AN ORDERLY FASHION. PLEASE REMAIN OUTSIDE AS WE EXPECT TO REOPEN THE BUILDING SOON. WE ARE SORRY FOR THE

INCONVENIENCE AND THANK YOU FOR YOUR COOPERATION.” (REPEAT THE MESSAGE)

Once the “All Clear” is given by the Fire Department, the Director of Athletics will notify all departments. Subsequent public address announcements and video board messages will also be made to the same effect.

The Incident Command Team will make a thorough building inspection following the all clear in order to ascertain whether the building sustained any damage. Any such damage will be documented accordingly.

Severe WeatherIn the event of inclement weather, all guests must be advised to seek shelter. In the event

that severe weather threatens an athletic facility and its guests, appropriate announcements via the public address system as well as video boards will be made. Guests will be evacuated to a safe location at the direction of Security Personnel with the assistance of SIU Game Staff. Employees should be prepared to assist with this action. Indoor facilities here will refer to the SIU Arena, Davies Gym, and the Troutt-Wittmann Center, while Outside facilities will refer to McAndrew Stadium, Abe Martin Field and the Cross Country course, SIU Tennis Courts and Charlotte West Stadium. Athletic events held in Student Recreation Center will follow the Recreation Center’s building BERT plan.

Reporting Procedure On receipt of severe weather watches or warnings from the weather alert radio, or via the

NOAA Website the Associate Athletic Director - Facilities will alert the Director of Athletics via telephone/radio or verbal communication.

Severe Weather / Tornado WatchA severe thunderstorm WATCH is an alert for a thunderstorm with damaging winds of

either sustained speed or gusts of 58 miles per hour or greater, and/or hail of ¾ inch or more in diameter. A tornado WATCH means conditions are favorable for a tornado to occur.

1. Notify the Director of Athletics via radio, telephone or verbal communication, who, in turn, will contact the Associate Athletic Director - Facilities.

A. The Associate Athletic Director - Facilities will advise the officials, coaches and participants by the most expeditious means possible.

2. Notify the Director of Athletic Facilities and Game Operations via radio, telephone or verbal communication. The Director of Athletic Facilities and Game Operations will relay the information to employees and other building staff.

3. In the event the Director of Athletics and/or Associate Athletic Director - Facilities are not available, please refer to the Chain-of-Command.

Severe Weather / Tornado WarningSevere weather is IMMINENT, third parties have observed or reported the weather

event in the immediate area, or observers can see and experience the immediate storm effects.

A severe thunderstorm WARNING is issued when a severe thunderstorm is sighted or detected by radar. A tornado WARNING means a tornado has been spotted in the area or detected on radar.

1. Notify the Director of Athletics via radio, telephone or verbal communication who will, in turn, contact the Associate Athletic Director - Facilities.

A. The Associate Athletic Director - Facilities will assign an individual to a designated observer posts (Weather Alert Radio, NOAA Radar, Visual Observation area) and ask other members of the Chain-of-Command to stand by to assist with relocation of guests, staff and participants if the order is given.

2. Notify the Director of Athletic Facilities and Game Operations via the radio, telephone or verbal communication of the situation at hand.

A. The Facility Operations Staff will inform subordinate staff personnel by the most expeditious means possible.

3. The Incident Command Team, upon hearing the relocation instructions via the radio system must report to the Incident Command Post.

4. The Associate Athletic Director - Facilities will make follow-up telephone/radio calls to building security personnel to ensure that staff have been prepared for the relocation.

5. Director of Athletics will issue the relocation order via the Public Address System and video boards to all zones possible. (Sample announcement to follow)

Lightning is the most consistent and significant weather hazard that may affect intercollegiate athletics. Within the Unites States, the National Severe Storms Laboratory (NSSL) estimates that 100 fatalities and 400-500 injuries requiring medical treatment occur from Lightning strikes every year. While the probability of being struck by lightning is extremely low, the odds are significantly greater when a storm is in the area and the proper safety precautions are not followed. Prevention should occur long before any intercollegiate athletic event or practice. In accordance with these findings of the NSSL and NCAA, the following procedures are to be implemented at Southern Illinois University – Carbondale.

1. In situations where spectators are present and a lightning detection device is available, the Associate Athletic Director - Facilities will operate, or designate one person to use the detection device, the designee or Associate AD will alert all managers to the threat of lightning. At first sight of lightning, scrupulous observation and caution should occur.

2. Each coaching staff or trainer should carry a cell phone for monitoring weather conditions and in some cases, call for transportation assistance. For example, such person detects lightning while on the golf course and then calls the pro shop for golf carts to be sent to a particular hole. As a matter of courtesy and policy, the coach or trainer who first detects lightning, should attempt to contact all other coaches conducting a practice or contest within the same proximity to put them on alert for lightning. All coaches and team trainers are ultimately responsible for the safety of the members of their designated sport.

3. Please refer to the SIU Athletic Team Lightning Policy Chart to determine where the nearest shelter is for the particular sport, the maximum amount of time to seek shelter, what lightning detection device is available, and who is to coordinate the evacuation.

4. If the weather forecast is for storms, contact the Associate Athletic Director - Facilities for updated information on lightning in the area before any athletic practices or event.

5. When in doubt, or if unable to seek shelter, as identified on the attached chart, then the Alternate Safety Procedure should be followed.

6. Alternate Safety Procedure- If no safe structure is within a reasonable distance, find a thick grove of small trees surrounded by taller trees or a dry ditch. Assume a crouched position on the ground with only the balls of the feet touching the ground, wrap your arms around your knees and lower your head. Minimize contact with the ground, because lightning current often enters victims through the ground rather than by a direct overhead strike. MINIMIZE YOUR BODY’S CONTACT SURFACE AREA, AND MINIMIZE CONTACT WITH THE GROUND! DO NOT LIE FLAT! If unable to reach safe shelter, stay away from the tallest trees or objects (such as light poles or flag poles), metal objects (such as fences or bleachers), individual trees, standing pools of water, and open fields. Avoid being the highest object in a field. Do not take shelter under a single, tall tree.

Indoor Relocation Procedures/ Safe Shelters Relocation Procedures/Safe Shelters – SIU Arena Tornado- The SIU Arena is an approved tornado shelter and will not be evacuated. Guests in upper level will be relocated to the lower level of Lingle Hall, the Donald Boydston Center and the main floor with the assistance of the ushers and DPS. Guests will be kept from underneath center-hung scoreboard. Teams should seek shelter in their locker rooms.

Relocation Procedures/Safe Shelters – Abe Martin Field Tornado- Guests should be instructed to seek shelter in the men’s and women’s bathroom located to the west of the stadium. Teams should be directed to seek shelter in the clubhouse.

Earth Quake- Stay calm, crouch on floor between bleachers and cover head and neck with hands and arms or try to move out of bleacher slowly and carefully. Once the shaking stops exit cautiously out of bleacher area.

The Threat is Received by TelephoneReceiving a bomb threat via the telephone calls for calm and reserve on the part of the

receiver. Remain calm and get as much information from the caller as possible. To help you stay calm and get the information needed, use the BOMB THREAT INFORMATION SHEET that is attached. The employee receiving the call will then notify the Director of Athletics.

Upon arrival, the Director of Athletics will:

1. Notify the SIU Police Department (911) as well as any SIU Police officer on site.2. Notify the chain-of-command of the situation.3. Institute search procedures immediately.4. Complete investigation and documentation of the event (write an incident report).

Evacuation ProcedureThe Director of Athletics, with the Chain-of-Command, may authorize an evacuation in

case of a bomb threat. In the event that an evacuation is issued, the fire alarm should be sounded.

SIU Police Department authority supersedes any other authority.

Upon ordering the evacuation of the facility, the Director of Athletics will notify the Incident Command Team to initiate evacuation procedures.

The Director of Athletics will make, or direct to be made, the necessary announcement via the Public Address and video boards. The verbiage for the announcement should be similar to the following:

SAMPLE ANNOUNCEMENT LANGUAGE“LADIES AND GENTLEMEN, MAY I HAVE YOUR ATTENTION PLEASE, WE REGRET THAT WE MUST INTERRUPT THIS EVENT, HOWEVER, AS A RESULT OF THE FIRE ALARM SOUNDING, YOU MUST LEAVE THE BUILDING IMMEDIATELY. WE ASK THAT YOU FOLLOW THE DIRECTION OF THE POLICE AND GAME STAFF TO THE NEAREST EXIT. PLEASE TAKE YOUR TIME. WE WILL ADVISE YOU WHEN AND IF YOU CAN RETURN TO THE BUILDING. THANK YOU FOR YOUR COOPERATION.” (REPEAT THE MESSAGE)

The announcement will be repeated every few minutes until it is confirmed that all occupants have exited the facility. Once all attendees are evacuated, all employees shall meet in a designated area as specified by their immediate supervisor and be accounted for. The Director of Athletics, under the direction of the SIU Police Department, will ensure that entrance into the facility is restricted until the bomb threat is alleviated.

Explosion

Any employee aware of the explosion will notify the Director of Athletics or supervisor immediately.

The Director of Athletics will then:

1. Ask the notifying person about injuries, exact location of the blast, whether there is a fire, etc.

2. Notify the SIU Police Department (911)3. Notify Chain of Command and convene the Incident Command Team.4. Prepare for the arrival of emergency medical services personnel.

The Incident Command Team, Plant Facilities Manager and the Electrician will:

1. Respond immediately to the incident command post, and direct representatives to the scene.

2. Check the area for fire, gas leaks, “hot” electrical wires or other threats to lives. All gas and electricity should be “killed” to the affected area if possible.

3. Notify the SIU Police Department and Incident Command Team of the condition of the scene (i.e. needs for ambulances, presence of life-threatening conditions, etc….)

5. Write an incident report.

The remainder of the building will be evacuated according to the evacuation procedure.

ThreatsIn the event an employee receives information regarding threats of any kind, that employee should contact their supervisor or SIU Police immediately and tactfully, to avoid arousing the

suspicions of other employees or guests. SIU Police will be notified and consult with Director of Athletics if needed, in making a decision if emergency plan needs to go into effect.

Suspicious Package/Device1. Contact the Director of Athletics, Associate Athletic Director - Facilities, or

Director of Athletic Facilities and Game Operations and advise of the circumstances. Be prepared to describe the suspicious package/device and provide the location of the package/device.

2. Do not move, touch or handle the object.3. Do not use two-way radios.4. Do not turn off light lights or touch switches.5. Keep loud noise to a minimum.6. Restrict use of telephone to only urgent business.7. Secure the location.8. The Director of Athletics and Chain of Command person responsible will

communicate the situation and procedures to be followed by the staff should an evacuation be necessary.

Anthrax or Biological/Chemical ContaminatesAnthrax Letter

If a letter is opened and it states something about being contaminated with anthrax, the following procedures should be followed:

1. The employee who handled the letter/package should stay in place and put the letter/package into the trash can and close the bag.

2. Notify the Director of Athletics or appropriate chain of command, who, in turn will instruct someone to notify the SIU Police Department and state name, location and cause of concern. He will also instruct notification to any Jackson

ANTHRAX LETTER/PACKAGE OR CALL

1) Stay in place and put the letter/package into the trash can and close bag if possible.

2) Notify the Director of Athletics or appropriate chain-of-command.

3) NO one is to leave the potentially contaminated area.

4) Everyone else should evacuate the building.

5) Wait for the Haz Mat team to arrive.

3. Anyone near the potentially contaminated area/employee should be quarantined in the same location as the employee who opened the letter/package. Everyone else should evacuate the facility immediately and under the direction of DPS and Facility Staff.

4. Quarantine the affected area. 5. Wait for the Haz Mat Team to arrive.

a. Haz Mat Team will decontaminate employees.b. Haz Mat Team will evaluate the room and make decisions on decontamination of the area.

6. The Emergency Management Agency of the Fire Department will take the letter/package and give it to the FBI to be tested.

7. Complete the documentation of the event (write an incident report).

Anthrax CallIf a call is taken and the caller states that anthrax has been put into the Facility’s

ventilation system (HVAC), the following procedures should be followed:

1. The employee who handled the call should note the exact time and message, any distinguishing features of the voice, and, if possible, the telephone number off the caller ID (if applicable). Ask the caller where the anthrax was discharged.

2. Notify the Director of Athletics or appropriate chain of command, who, in turn will instruct someone to notify the SIU Police Department (911) and state name, location and cause of concern. He will also instruct notification to any SIU Police Officer on site.

3. Evacuate the building immediately according to the evacuation procedure. Gather in the green area north of Lingle Hall

4. Wait for the Haz Mat Team to arrive.a. Haz Mat Team will evaluate the situation to determine if decontamination

should occur.5. The FBI will investigate the incident.6. Write an incident report.

Both the human immunodeficiency virus (HIV) and the Hepatitis-B virus (HBV) have been transmitted in occupational settings by contact with an open wound, non-intact skin, mucous membranes to blood or blood contaminated body fluids. Blood is the single most important source of HIV and Hepatitis-B in the workplace setting. Exposure can come from contact with liquid or semi-liquid blood, human blood components, pathogenic microorganism, human body fluids, vaginal secretions, cerebrospinal fluid, synovial fluid, pleural fluid, pericardial fluid, amniotic fluid, saliva, any body fluid that is difficult or impossible to differentiate between body fluids, and any unfixed tissue or organ in any form. The risk of Hepatitis-B infection following a needle stick or cut exposure to blood is directly proportional to the probability that the blood contains Hepatitis-B, the immunity status of the recipient, and on the efficiency of transmission. The risk from exposure of non-intact skin mucous membranes is likely to be far less that from an open wound or sore.

Action to Take when Exposed1. Wash the skin contaminated by blood or other body fluid immediately with warm

water and soap (anti-bacterial, if possible) for at least ten (10) seconds. Washing should be completed using the appropriate facilities, such as utility or restroom sinks. Use waterless antiseptic hand cleaner when hand-washing facilities are not available. Follow the manufacturer’s instructions when using the product. Hands should always be washed after gloves are removed, even if the gloves appear to be intact.

2. Immediately notify your supervisor on duty. If your Supervisor is not on duty, contact any facility management individual. If you cannot locate anyone in the building, notify SIU Police Department.

3. Write an incident report.

Needle Stick Exposure1. Obtain permission from the source to do HIV and Hepatitis B

screening. Explain reason for the request.2. Determine if source has received Hepatitis B vaccine.3. Notify and obtain authorization for treatment from Supervisor.4. Call the SIU Police Department.5. Injury/Illness and Worker’s Comp forms may be obtained from Department Head,

Supervisor or Risk Manager, Human Resources or the Administrative Office of SIUC Athletics.

FORMS MUST BE COMPLETED AND RETURNED TO HUMAN RESOURCES WITHIN 24 HOURS OF INCIDENT.

Director of Athletics or designee must complete and review the appropriate forms before returning them to Human Resources.

Employee Accident/InjuryAnytime an employee is injured while on duty, a complete set of worker’s compensation

paperwork must be completed by the employee and employee’s immediate supervisor. If an

employee is sent to a hospital, the hospital needs to be notified that it is a workman’s comp injury. A phone call should be made to Human Resources the first business day.

Robbery of Hold-up1. Offer no resistance.2. Respond to each order from hold-up person in a manner so as not to upset the situation

further.3. Do not offer more assistance than that ordered by the hold-up person.4. Follow hold-up instructions completely and do not make any move to secure your position or

inform the proper officials until your safety is insured.5. Once the hold-up person has left the building, move to a safe location and alert police.6. If possible, obtain license tag of the vehicle and/or direction of the escape route.7. Notify the Director of Athletics, if s/he is unavailable, contact the first available person on

the chain-of-command list.8. Write an Incident Report, filling in important data, such as time, number of persons involved,

description and action taken by sequence.

Power FailureIn the event of a power failure the Associate AD - Facilities, at the direction of the

Director of Athletics, along with Electrician will initiate back-up lighting.

Power OutagesIn case of power outages, an emergency diesel generator, or other type of generator,

supplies power to select lighting fixtures and equipment throughout the facility. The Director of Athletics will announce and/or direct to be announced the power outage and request that guests remain in their place of occupancy. The Director of Athletics will contact the Chain-of-Command either by phone or verbal communication.

Write an incident report to document the power outage.

Natural Gas LeaksSuspicion of gas leaks should be reported to the Director of Athletics of Associate

Athletic Director - Facilities immediately. Communications with the Director of Athletics should occur away from the area. The Director of Athletics will notify the Chain-of-Command either by phone or verbal communication.

Should the gas leak be of a serious nature, the Director of Athletics will notify the SIU Police Department. All guests and staff will follow the direction of the Police and Fire Department, as well as the facility staff.

Write an incident report to document the natural gas leak.

Medical EmergenciesIn case of medical emergencies, the Director of Athletics will be notified immediately.

The Emergency Medical team (ambulance) should be on notified if present at event. The

Automated External Defibrillator (AED)In the case of an apparent heart attack or difficulty breathing, when the Director of

Athletics or designee contacts the medical staff, he should request that they bring an Automated External Defibrillator (AED) machine to the location of the emergency. In the event there is no medical staff on duty, the Director of Athletics will request that a AED be brought to the location by the SIU Police Department. There is also an AED located in the training room and can be retrieved by team trainers.

Document all medical emergencies with a written incident report.

GENERAL INFORMATION/SITUATION RESPONSESAbandoned/Lost Child1. Contact Director of Athletics, Associate Athletic Director – Facilities, or Director of

Athletic Facilities and Game Operations. 2. Contact the Police Officer who is on duty at the event. In the event, the officer is not

available, attempt to contact SIU Police Department.3. Do not leave the child unattended by the police or athletics staff, and find a place for the

child to be comfortable and have a supervisor monitor the child.

Ticket Office/Ticketmaster Computer FailureIn the event of a Ticketmaster computer failure, the following procedures should be

followed:

1. The Ticket Office Manager will call Ticketmaster immediately to ascertain the length of the shutdown. (The Ticket Office has the appropriate telephone numbers.)

2. The Ticket Office Manager should immediately notify the Director of Athletics and Game Management Staff.

3. The Director of Athletics or event supervisor will locate the promoter (if applicable) and take to the Ticket Office to meet with the Ticket Office Manager.

4. The Ticket Office Manager will explain to the promoter (if applicable) the contingency plan to be implemented.

Evacuation ReviewPurpose:

To assess effectiveness of Emergency Plan. To determine changes, if any, that could be made in the Plan and implement changes

decided upon.

Functionaries and Activities: Director of Athletics

Employee ProceduresGeneral Procedures Ticket-sellers and concessions/merchandising personnel are to secure their cash and/or ticket

boxes upon announcement of an emergency situation. After these stations have been secured, all Event Staff personnel manning these stations will await further orders from Event Staff Supervisor, remaining at their posts if not in immediate danger.

Ushers shall remain at their posts to aid Security personnel in directing guests to designated locations. Ticket-takers will remain at their posts to help guests safely leave all appropriate points. Concessions/merchandising personnel should be prepared to assist emergency personnel as directed.

Evacuation ProceduresIn the event of an emergency where a partial or complete evacuation of the facility is necessary, there are important procedures to follow:

1) The game staff will open all gates/doors. All personnel assigned to gates/doors will direct guests to exit the facility.

2) All personnel working in sections to be evacuated will inform guests of the need to clear that particular area. Guests should be directed to the nearest and safest stairway or ramp. Personnel should try to keep this action as calm and orderly as possible. Under no circumstances should anyone other than emergency or Game Management personnel be allowed to enter an evacuated area.

3) Ushers or other directed personnel will man all stairways and ramps. Employees will assist guests in moving carefully, calmly, and orderly. Special attention should be given to handicapped guests.

4) Director of Athletic Facilities and Game Operations will be stationed at the field/court to monitor the situation from the ground. Director of Athletics, Associate AD – Facilities, and Security Director will be located at the designated command post. They will advise employees of any problem areas.

5) Employees should offer all possible cooperation to Game Management, Security, or Medical personnel as needed. If asked about the emergency by a guest, the proper response is, "An emergency situation has developed and this action has been authorized to protect those attending the event." Any information that needs to be released to the public will be done so via the public address system. Under no circumstances should employees discuss the situation with guests. This is done to avoid rumors and a potential panic.

First Aid ProceduresThe following procedures must be followed in case of serious illness or injury:

1. Call First Aid immediately by way of nearest security personnel or supervisor. Do not panic. Give your name, location, and the extent and nature of the injury. Do not move the injured person unless instructed by medical personnel.

2. Make sure your supervisor is notified.3. Help clear area of audience/crowd.4. Note any witnesses that might provide additional information regarding the incident.5. Accompany the injured person to First Aid if they are able.6. Under no circumstances should employees discuss the incident with anyone other than

his or her direct supervisor or persons authorized by Southern Illinois University. Unauthorized personnel include all media personnel—newspaper, television, and radio reporters.

7. Use incident reports to completely document the event, including: Location of the incident Guest's seat location Guest's name, address, and phone number Address and phone number of the facility where incident occurred Detailed description of the incident

*Employees should give all of the above information to their supervisors. All information regarding the incident must be given to the Associate Athletic Director - Facilities.

8. If a guest falls or is injured and does not want first aid attention, you should still obtain the information listed above and give it to your supervisor in writing.

Additional Emergency Notes It is important to remember that we should avoid panic. If employees stay calm and act in an

orderly fashion, our guests will do the same. In the event of a fire, use of elevators is prohibited. Concession and novelty stands will be closed immediately in the event of an emergency.

Personnel should be prepared to accept assignment to other duties by Game Management or emergency personnel.

Guests will be prohibited from driving their cars until all problems have been rectified, unless directed otherwise by operations staff. SIU Police Department will be responsible for directing and controlling exit and entrance of traffic during an emergency situation.
