estrogeno e terapia orale con estrogeno + progestinico
- necessari ulteriori dati per studiare differenze nel rischio derivante dall’impiego dei vari regimi di HRT (in particolare vari tipi di progestinici)

SPIEGAZIONE BIOLOGICA DEI RISULTATI:
- l’estrogeno per via orale, e non per via transdermica, è sottoposto ad un
effetto di primo passaggio epatico potendo intervenire sull’equilibrio tra fattori procoagulanti e antitrombotici:
. aumenta concentrazioni plasmatiche di protrombina frammenti 1+2
. diminuisce concentrazioni di Antitrombina
. resistenza alla proteina C attivata
CONCLUSIONI:
Studio multicentrico caso-controllo
- l’utilizzo di estrogeni per via orale, ma non per via transdermica, determina un
aumento del rischio tromboembolico venoso
- i Norpregnani (nomegestrolo acetato e promegestone) appaiono trombogenici
- il progesterone micronizzato e i Pregnani (didrogesterone, medrogestone,
clormadinone acetato, ciproterone acetato, medrossiprogesterone acetato)
appaiono sicuri rispetto al rischio tromboembolico venoso
- necessari ulteriori studi per confermare tali dati

- il rischio non sembra variare significativamente tra i vari tipi di HRT, in particolare
non sono state osservate differenze nel rischio tra HRT contenente CEE o MPA e
altre formulazioni più utilizzate in Europa
- Il rischio associato all’utilizzo di progestinici derivati dal testosterone (NETA, LNG)
sembra lievemente superiore a quello associato a progestinici derivati dal P (MPA,
CPA, CMA, medrogestone, didrogesterone)
