Introduction to Cause & Effect Diagram
Cause and Effect diagram, AKA, Ishikawa diagram , are causal diagrams created by Kaoru Ishikawa (1968) that show the causes of a specific event. Common uses of the Ishikawa diagram are product design and quality defect prevention, to identify potential factors causing an overall effect. Each cause or reason for imperfection is a source of variation. Causes are usually grouped into major categories to identify these sources of variation. The categories typically include• People: Anyone involved with the process• Methods: How the process is performed and the specific requirements for doing it,
such as policies, procedures, rules, regulations and laws• Machines: Any equipment, computers, tools, etc. required to accomplish the job• Materials: Raw materials, parts, pens, paper, etc. used to produce the final product• Measurements: Data generated from the process that are used to evaluate its
quality• Environment: The conditions, such as location, time, temperature, and culture in
which the process operates

Root Cause Identification by Why-Why Analysis
1. Loop holes in recruitment and selection processes of faculties resulting into lower teaching standards.
2. Loop holes in selection processes of students due to which non-deserving students get the seats.
3. The new students are not well oriented about the culture and image of college, in the outside world, which they are supposed to maintain.
4. College alumni found unfit for the good job profiles which results into disinteresting job profiles for new students.
5. Students not being trained for final placements due to which companies loose interest eventually and lesser companies visit campus over the years.
Root Causes
• Just the qualification of faculties (Phd., Pg etc) must not be enough. Faculties must be tested through real sample lecture (where students and experience faculties both rate the candidates).
• Students and teachers, both are the representatives of institutes and must be made aware that there actions reflect the quality of institute they belong to. New students must be oriented properly so that they have a sense of belonging towards the institute and would set up a better brand image.
• The college must understand the “Garbage in-Garbage out” phrase and must allot the seats only to the deserving students, who would, after the completion of course, would earn good name for the college. A thorough check of each applicant must be done through personal interviews, GDs etc.
Recommendations
• The final year students must be informed that their actions in the professional world will have direct impact on their junior’s placement season. This recognition will drive them to perform well and earn the institute a good name.
• Throughout the session of the course, the students must be regularly trained for the placements through mock GDs and personal interviews. The career development cell must maintain a proper communication with the alumnus and the students so that the alumnus can share their valuable experience which will help the upcoming batch.
Recommendations
We learned an organized way to identify the various causes of a problem through the Cause and Effect Diagram.
We also learnt that the easily identified reasons for the problem, might not be the actual cause and hence a through analysis (using why-why and other tools) must be done to find the root causes.
The identification of root causes results into more focused actions which in turn provides better results.
The above learning helped us to identify the root causes or the low brand perception of the colleges and we also recognized that, we as students, can also work for the betterment of our institute’s brand image.
