Job Purpose/Summary:

The Palliative Care Community Team (PCCT) is part of Hospice Peterborough’s hub of excellence and is the gateway to palliative services for the community. As a key member of the PCCT, the intake coordinator will review and priorize all referrals and insure intake processes are carried out in a timely, accurate and sensitive manner.

Primary Duties and Responsibilities:

As the first point of contact with the PCCT the Intake Coordinator will:

o Ensure referrals are complete, prioritized and classified by the urgency and level of care or support required.

o Ensure care requests and referrals are monitored so that all clients and families receive timely responses with follow up calls to determine if the care needs have been met.

o Communicate regularly with clients to ensure their service goals are being met. o Organize relevant client meetings and visits and ensure that any communication

is completed within a timely manner. o Maintain regular and timely documentation and statistical records. o Manage client files, ensuring information is up-to-date at all times. o Provide appropriate information and reassurance when communicating with

clients directly. o Maintain appropriate documentation of referrals, care coordination plans, client

information and data vital to Hospice care and related support services.

• Provide data management for quality improvement and monitoring including ensuring the appropriate Key Performance Indicators (KPI) are recorded accurately and precisely for reporting, performance and quality purposes.

• Work and communicate with PCCT partners, clients and caregivers to ensure they know and understand the processes and services provided.

• Utilize effective communications strategies as required such as shared care plans and Electronic Health Records (EHR).

• Provide Hospice information to clients, health care professionals and community members.

• Provide information about the spectrum of palliative support services which helps to support smooth transitions of care and service supports from one point of care to the next.

• Support the PCCT team through scheduling, agenda development and meeting preparation through contacting participants, circulating necessary documents and documenting minutes.

Organizational Responsibilities:

• Represent Hospice Peterborough in selected community activities, education opportunities and committees.

• Participate in assigned Hospice activities and committees.

• Complete administrative reports on a timely basis. o Statistical collection for reporting. o Ensure accuracy of performance indicators. o Ensure robustness of data collection.

• Participate with the executive director in an annual performance appraisal.

• Fulfill all responsibilities in accordance with the by-laws, policies and procedures and standards of Hospice Peterborough.

• Willingness to accept other duties as required.

Qualifications:

Post-secondary education

Experience supporting clients in a health setting

Knowledge of community resources

Strong computer literacy including MS Office Suites, database management.

Strong written and oral communication skills.

Outstanding interpersonal, communication, and organizational skills.

Ability to work flexible hours.

Knowledge of community health and social service resources an asset.

Current and original copy of a satisfactory Criminal Records Check, Vulnerable Sector Search and Child Abuse Registry Check is required upon the start of your employment.

Fast- paced, highly detailed environment.

Ability to carry using appropriate lifting techniques.

Ability to: grip, walk, sit, stand, climb stairs; use fine hand movements.
