Part 1 (#1-3)

#1: List all homeowners who are on the title of the home.

#2: Leave this section blank because if you do put us down they will send the

check to us and we want it to go straight to you.

#3: Fill in your address

#4: Provide at least one telephone number that is best to contact you at.

#5: Check what time is best for the state inspector to come out to look at your

home after you complete your forms and mail it in.

#1: Provide your address

#3: Check Single Family dwelling

#5: Check yes or no depending on your property.

#6: Check yes or no depending on your property.

#7: Check yes or no depending on your property.

#8: Put the year your home was built.

#9: Check current damage.

Part 3 (#1-2)

#1: Check that you are the current homeowner

#2: Provide one of the following options to provide proof of property ownership.

Part 3 (#3)

#3: It is best if you provide at least 2 of the following options to give proof

that you have Forstex Siding.

Part 4 (#1-3)

#1: Check yes or no if you have filed a previous claim.

All homeowners must sign and date their name.

Tax information

Check no to all 3 tax questions. Since you checked no you do not have to

provide your social security number.

Now all you have to do is have all homeowners sign and date the paperwork!!!
