Dr. Mays is a board certified plastic surgeon by the American Board of Plastic Surgery and a member of the American Society of Plastic Surgeons. He completed his medical degree (cum laude) at the University of Louisville School of Medicine and went on to complete a 6-year integrated plastic surgery residency, training under surgeons from such prestigious institutions as Johns Hopkins, NYU, Memorial Sloan Kettering Cancer Center, MD Anderson Cancer Center, and Manhattan Eye, Ear, and Throat Hospital. Dr. Mays is a Key Opinion Leader publishing extensively for some of the leading plastic surgery journals and textbooks. Dr. Mays and Dr. Calobrace are currently completing their textbook on augmentation mastopexy. Dr. Mays performs a range of procedures in the field of plastic surgery with a special interest in face, breast and body cosmetic surgery.

JeuveauTM– Our newest neurotoxin has patients raving over its quick onset (2-3 days), giving patients smoother looking skin on the forehead, around the eyes and mouth.

First, some breast implants, the vast majority being Biocell textured implants sold by Allergan (previously Inamed or McGhan), have been associated with a rare type of lymphoma called BIA-ALCL (breast implant associated-anaplastic large cell lymphoma). I happen to chair the BIA-ALCL task force for the Aesthetic Society in the U.S. and actually gave testimony to the FDA back in March. Allergan voluntarily recalled these implants world-wide, deciding not to sell them or allow them to be used in the future. This recall, however, does not mean women are required to have their implants removed (not that kind of recall) but they should know of this rare risk and should be evaluated on an ongoing basis by having a discussion with their plastic surgeon about if and when any treatment would be appropriate. Smooth breast implants have no association with this lymphoma. Other textured implants have had a few cases of BIA-ALCL, but are much more rare. Due to the rarity, the implants may still be used after considering the risks and benefits. Dr. Mays and I like to see all of our breast implant patients once every one to two years for ongoing evaluation and care. Secondly, there has been a new entity described in social media as “Breast Implant Illness”, even though that is not a medical term and there is no associated diagnosis in the medical literature. Nonetheless, this is a condition with a wide array of symptoms (fatigue, joint pain, headaches, brain fog, rashes, etc) that many very capable and compassionate women believe may be associated with their breast implants, which includes smooth and textured, saline and silicone. The challenge as a plastic surgeon (and one that publishes extensively, lectures throughout the world, and has treated nearly 10,000 breast patients) is at this point, we have no data and no information to help guide us in how best to manage these patients. We are always mandated to treat by science, knowledge and evidence-based medicine, and in BII (Breast Implant Illness), we step away from all of that. Almost all studies show implants have no connection to these symptoms, but there

are a few emerging studies that may point to some rare association. SO WHAT TO DO? Well, Dr. Mays and I do what we always do. Listen to our patients and support them and honor them and believe them. Educate them on what we know — then together make a decision for the right path. Women chose to have implants placed initially with no real indication other than a desire to look and feel better. And so for us, if patients want their implants removed in hopes of feeling better (with no guarantees…but honest hope), then we believe that is our patients’ right. It is our responsibility to provide all of the appropriate information and guidance, but the decision is 100% the patient’s, and we honor that.

It is important to know that we continue to follow our patients closely, and we also continue to be involved in clinical studies such as the post-approval studies, designed over a 10-year period. The vast majority of our patients absolutely love their implants and would never consider removing them. Patients continue to choose breast augmentation, and we believe in their right to do so, for the vast majority this will be an amazingly positive decision. But it is fair for patients to know although we do not understand it, there may be a link to this type of illness

and that is fair. It is also important to remember breast surgery is more than implants. Patients often choose breast lifts, breast reductions, auto-augmentations with their own tissue or use of fat grafting for augmentation. Many women choose these options. We often remove implants or downsize implants and do breast lifts with fat grafting. In cases of BIA-ALCL or concerns of illness, removal of the entire capsule is also warranted. For these procedures, an experienced expert breast surgeon provides the best guarantee of a successful outcome. When consulting with patients a common complaint I get is

L A X I T YThe facelift is designed to remove the laxity of the face by tightening the skin and underlying muscle to give a refreshed appearance. Most of the time when we are addressing the facial laxity, we are also addressing the neck laxity, because the lines become blurred where the face ends and the neck begins. The banding and laxity in the neck is usually due to relaxation of the underlying platysma muscle which directly causes skin laxity because these two structures are closely adherent.

V O L U M E L O S SOnce we have addressed the laxity of the skin and muscle, we need to address the volume loss in the face. The deep and superficial fat pads that disappear with time need to be replaced. The best way to replace the fat loss is with your own fat with a procedure called fat transfer, or fat grafting. During the facelift, fat is harvested from the abdomen and injected back into the areas of volume loss to plump up the skin and restore that youthful appearance.

S K I N T E X T U R EBy addressing the laxity and volume loss of the face we have treated 2 of the 3 signs of aging. The third sign of aging is the skin texture. The outer surface of the skin can be improved with resurfacing techniques that can be performed at the time of the facelift or at a later date in CaloSpa with our laser technology. Skincare is a perfect complement to the facelift because it can keep the skin hydrated and refreshed to help maintain that healthy glow.

Some patients fear facial surgery because they are concerned about looking like they have had plastic surgery. Dr. Mays and I spend a lot of our time performing facial

much about it or show many pictures. One of the major

factors, of course, is our clients’ confidentiality and respect

Man up, Bro! Staying healthy and looking fit are top priorities when it comes to lifestyle choices. As the stereotypes around plastic surgery are slowly being broken down, more and more men are turning to surgical and non-surgical cosmetic procedures. According to the American Society of Plastic Surgeons (ASPS) annual statistics, more than 1.3 million cosmetic procedures were performed on men in 2018. This accounts for only about 8% of the total number of cosmetic procedures. This is very similar to the number that was seen for men in 2017.

Over the past five years, there was a 23% increase in liposuction and a 12% increase in tummy tucks among men, and a 30% increase in male breast reductions, according to ASPS. We notice that young men tend to focus more on enhancing their bodies, while older men have more minimally invasive facial procedures to look younger.

The desired results from most plastic surgery treatments are similar for both men and women. However, the approach and technique involved require the surgeon to understand the differences in the bodies of men and women. Each gender has unique features, such as skin thickness, muscle tone as well as the way they store fat.

Successful male cosmetic surgery will also focus on creating or retaining masculine features, which may be sharper or more prominent than those of women. In our office, some of the top procedures for men include liposuction, hair transplants, neck

lifts and blepharoplasty (eyelid surgery). One of the most common surgical procedures is the treatment of gynecomastia which is when men develop extra tissue in the chest area that can make them appear more feminine. The treatment can vary from just liposuction of the chest to making incisions and removing the extra tissue.

Facial rejuvenation is important to men of all ages. Men are inquiring about things that in the past were not discussed such as skincare options, laser hair removal, laser resurfacing, HydraFacials, or injectables like toxins and facial fillers. For many women, the after effects of childbirth, weight changes, and the natural aging process leave the vaginaand surrounding tissues stretched, torn, and lacking elasticity. Many women during puberty or after childbirth may see their labia minora become unusually large or misshapen. Other women develop loss of fullness in the labia majora after menopause.

Vaginal rejuvenation is a term that describes both surgical and non-surgical treatment of the vagina and labia. When treating the vagina, we often perform a vaginoplasty which is more of a functional or reconstructive surgery. It is designed to address the effects of childbirth and aging on the vagina. The surgical treatment typically includes repairing stretched and separated vaginal muscles, tightening and narrowing the vaginal opening and canal, removing excess lax vaginal lining. These procedures can renew self-confidence, and possibly enhance sensation and sexual satisfaction for the patient and their partner.

The anatomy of the female genitalia can vary significantly. Like breast sizes, the desired appearance of the labia will vary with individual preferences. In some individuals the labia minora (the inner folds of tissue) can be quite large and redundant creating both a cosmetic and sometimes even a functional concern. Some women develop hypertrophic or enlarged labia minora after multiple childbirths or as the result of aging. The best candidates for labiaplasty (surgical reduction of labial tissue) are healthy women who have oversized, sagging, or asymmetrical inner or outer labia. Improvements from this procedure include: Relieving irritation and discomfort during sexual activity and athletics (such as bicycling, spinning, running), and while wearing fitted pants or bathing suits. Labiaplasty, can repair damage caused by childbirth or treat redundancy caused by hormonal changes.

Internal vaginal changes after pregnancy can also lead to embarrassing problems such as stress incontinence. Other vaginal changes seen with menopause like dryness and atrophy can lead to painful intercourse. The vaginoplasty and labiaplasty will not specifically address these concerns. Instead, the non-surgical ThermiVa® procedure may be recommended for vaginal rejuvenation.

The non-surgical rhinoplasty uses the same injectable soft tissue fillers that I use for plumping the lips, smoothing the skin or to lift the sagging tissues, products like Juvéderm® or Restylane®.

The procedure can be called many names. The liquid rhinoplasty, the no hassle or non-surgical nose job. The procedure focuses on the small changes that can be achieved with dermal fillers. Dissolvable and reversible, a few tiny injections of the filler can equal big improvements in the nose.

One of the most common issues treated with the non-surgical rhinoplasty is a pronounced bump along the nasal bridge called the dorsal hump. The filler injections can plump up the area around the bump, giving the illusion of a straight nose. Other improvements achieved with fillers include smoothing small indentations, creating a straighter looking nasal bridge, and lifting a drooping

nose tip. The overall appearance can be improved with slight tweaks from filler injections, improving facial symmetry for a more attractive appearance.

Common benefits of a non-surgical rhinoplasty include: No downtime, minimal bruising and swelling, short procedure time, results are immediate, changes are reversible, benefits last approximately 12 months and it’s less expensive than surgery. A non-surgical rhinoplasty is not for everyone, however, those patients looking for more dramatic results may not find the benefits of a non-surgical rhinoplasty enough to achieve their needs. The treatment works well on external issues, but a non-surgical rhinoplasty cannot correct interior issues such as a deviated septum or nasal function. Additionally, as the non-invasive rhinoplasty must be repeated every year or two, the patients seeking a more permanent result should consider the surgery.

BRAZILIAN BUTT LIFT (BBL)

The most common surgical option is the Brazilian butt lift (BBL) – contouring the waist and hips with liposuction to sculpt the shape of the buttocks and then adding the fat removed back into the buttocks. Half of the fat survives and produces enhancement in the shape and fullness of the buttocks. This has become an extremely popular procedure that we perform almost every week in our office. This procedure however requires precision and experience or significant risks are possible. For patients with weight loss and sag, buttocks lifts are common and highly effective, occasionally associated with auto-augmentation of the buttocks with the extra tissue or fat grafting. These do have a scar across the posterior belt-line and thus should be positioned exactly to be covered by bathing suits and underwear.

Non-surgical options include the EmSculpt to stimulate the gluteal muscles over a series of treatments leading to a more shapely, lifted booty. Each treatment lasts 30 minutes and a series of 4 are recommended with some yearly maintenance. A new exciting treatment is Scultpra injections for the buttocks. It provides a great option to improve firmness, texture and volume. It works just as it does in facial rejuvenation adding volume, lift and fullness. This usually requires 2-3 treatments.

Beauty is in the eye of the beholder, and it seems all the fitness influencers on Instagram have shown us that being toned is the way to go. Open your social media feed and you will see toning, lifting, tightening, sculpting, pre-workout shakes, post-workout shakes, meal preps, and the list goes on and on. The quest for the toned body is in, weight loss transformations are happening on a daily basis. We celebrate our patient’s weight loss and their body goals. We believe in diet and exercise and maintaining a healthy lifestyle to obtain the best version of ourselves. All these things are great, but what if they can’t take you all the way to that perfectly sculpted body?

Most of us when we look at childhood pictures, we will see a slimmer and trimmer version of our self. We look in the mirror now and we see the effects of real life on our bodies. That once toned abdomen of ours has taken a beating on this journey we call life. Well, the good news is we have ways of getting that toned, slimmer abdomen back. At CaloSpa and CaloAesthetics we have a range of choices from the surgical to non-surgical.

The hardest part for patients is to determine which option is right for them. If we were to split patients up into two

categories, surgical and non-surgical, the best way to tell which category you fall into is to stand in front of the mirror and look at your skin. If you have hanging skin on your lower abdomen that creates a fold at your waistline or there is so much skin you must tuck it into your underwear, you are a surgical candidate. The most common surgery for removing this skin is a tummy tuck, a.k.a., the abdominoplasty. There are many variations of the tummy tuck that we perform to remove the skin and tighten the abdominal muscles at the same time. The outcome after your 4-6 week recovery is a smaller, flatter abdomen.

If you are one of those that does not have the hanging skin but does have some fullness of the abdomen in the form of fat storage covering up that six pack, then you have more options. One option is liposuction, which involves surgery to go in and remove the fat with small cannulas and sculpt the abdominal area. This requires a 4-6 week recovery with activity restrictions to help optimize the result.

Maybe you’re not concerned with pockets of fat or strengthening muscle, but maybe your concern is textural. With 80% of women struggling with cellulite, we are often asked the question of which treatment will best treat the dimples that are so bothersome. Before deciding on a treatment, we must first determine which kind of cellulite is causing these unwanted dimples. Is it primary or secondary cellulite?

Primary cellulite is dimpling on the buttocks and backs of the thighs caused by fiber bands that pull down on the skin, similar to a tufted couch. This tension creates areas of dimpling that creates uneven contour to the buttocks and backs of thighs. For this type of dimpling, we use a treatment called Cellfina® to create a smoothing effect. Cellfina is a single treatment that cuts the fiber bands and releases the tension, creating a smoother appearance to the skin. The treatment can be completed in a single office visit, has mild bruising and swelling, and has long lasting effects.

Secondary cellulite is typically found in multiple areas on the body. In this case, there is no fiber band creating tension. The dimpled or orange peel, appearance in this type of cellulite is a result of skin texture and skin laxity that occurs with weight loss, age, or just genetics. We see this often in the fronts of the thighs, the stomach, buttocks/backs of thighs, and arms. For this type of cellulite, the skin will appear smoother when the skin is pulled taut. To treat secondary cellulite, we use devices to help tighten the skin.

ThermiSmooth® is a common device used to treat these areas, and utilizes radiofrequency heat to tighten skin in the treatment area over a series of treatments. This typically yields little to no downtime, and can be completed as a quick office procedure.• Superficial textural
