This past year we celebrated our move to Lebanon tenyears ago which allowed us to integrate the researchlaboratory space with the hospital and clinic, so thatdoctors and researchers could collaborate more closely. We’ve experienced tremendous overall increases in clin-ical trials and research funding over the past decade.Productive collaboration in research directly impacts thetreatment we are able to provide our local patients. Ourability to take laboratory discoveries directly to ourDHMC patients is a significant contribution we maketo the region.

Some examples of our current research include: Dr.James Sargent’s work on the influence of popularmovies on teen smoking habits; Dr. Patricia Carney’sinteresting research on the wisdom of obtaining yearlymammograms; and Dr. Madeline Dalton’s investigationsinto the increase of tanning booth usage and how itrelates to increased rates of cancer. In addition, Dr. JohnWennberg of the Center for Clinical Sciences atDartmouth Medical School recently published the latestissue of the US Atlas of Healthcare, used extensively as aresource for providers and policy makers across thecountry. These researchers were covered extensively inthe worldwide press this year because of the impact oftheir work on disease prevention. Our researchers haveattracted over $80 million in funding this past year, 14percent higher than the previous year.

How We Stack Up: NationalPublications Give Us High MarksDHMC encompasses a wide range of services and pro-grams, and our doctors, researchers, and providers of alltypes often receive press recognition about their indi-vidual or department efforts. This past year, our centerwas widely recognized in several national publications.

US News & World Report is popular for its annual “top100” rankings from year to year. In 2001, Dartmouth-Hitchcock Medical Center was ranked in the top 50hospitals for our treatment of Digestive Disorders,Gynecology, Heart Related Diseases, and Orthopedics.

Modern Healthcare magazine ranked us among the top 100 cardiovascular hospitals in 2001.“Top hospitals”are noted for their effective operations, and hence theirability to reduce mortality rates and to save money.

In addition, Hospitals & Health Networks rated DHMC asone of the “Most Wired” in their 2001 survey. The surveyshowed that those hospitals that invested in informationtechnology saw a healthy return for that investment interms of “better control of expenses, higher productivity, and more efficient utilization management.”

We know that the most valuableasset we have is people. We careabout the quality of their lives andregularly monitor their satisfaction.

Nurses play a very important role, and we’re proud of our higher thanaverage level of retention. One ofthe ways we do this is by providingongoing education.

This is a referral hospital, which means that

people come from miles around. When a

child is in the intensive care nursery, it isn’t

possible for parents to get in all of the

time. But doctors believe that a baby needs

to feel a warm body and be held in order

to mature. So, a group of us come in and sit

there and take the baby into our arms and

hum. I used to discuss the Red Sox and

pretty soon the baby would slumber off...

We are grateful to the many emergency services per-sonnel that collaborate with us to provide appropriatetreatment to patients in emergency circumstances. It isthe total dedication and hard work of the emergencymedical service personnel who staff the regional ambu-lance services and serve our local fire and police depart-ments that makes life in our rural area secure.

Emergency Preparedness in the wake of 9-11As we ended Fiscal Year 2001, we anticipated celebrat-ing the tenth anniversary of our move to Lebanon.Theseries of events on September 11 caused us to postponeour facility-wide event until late spring 2002. Instead, the entire medical center readied itself to respond toimmediate community needs and to prepare to assist ifinjuries exceeded the capabilities of New York area per-sonnel and volunteers. We developed a database of hun-dreds of nurses and doctors willing to locate to GroundZero if needed. In fact, several of our paramedics werecalled and spent weeks providing assistance. For hisefforts, DHART paramedic John Hinds was given a flagthat had flown over the Stuyevesant School nearGround Zero on the day of the attack.

Community Collaboration: Sharing a Part of Your Life

“By the end of our

mission at Ground

Zero we’d treated

15,000 patients over

12 days, 24 hours

per day, across five

medical stations.”

Friends, colleagues, patients, volunteers...we crossboundaries constantly. Staff are sometimes patients andboth patients and staff volunteer throughout the medical center and throughout the community. Forexample, many of our pediatric residents—as part oftheir training in the Boyle Pediatrics Program—volunteer at other nonprofit services like Listen, CarterCommunity Center, the Haven, Hannah House, GoodBeginnings,The Family Place, and the Good NeighborHealth Clinic.Their experiences influence the qualityof the training our physicians receive.

DHMC has been extremely supportive of the GoodNeighbor Health Clinic since its inception. Our physicians, nurse practitioners, nurses, and para-medicalprofessionals donate their own time to support thework of the clinic which provides free primary medicalcare to those in need who are without insurance or the means to pay. DHMC also provides the continuityof care for our Good Neighbor patients by covering the costs of their labs, diagnostic tests, and referrals to specialists.

More than 300 volunteers help to coordinate theAudrey Prouty Memorial Century Ride and FitnessWalk to benefit cancer research at Norris CottonCancer Center. They staff the rest areas along the route, register participants, serve food at the barbecue, andprovide support to riders and walkers.

The Dartmouth-Hitchcock Mascoma Pediatric Clinicin Canaan celebrated its 20th anniversary in 2001.Dr.William Boyle started the clinic and still providesservices there on a regular basis. He knows that takingcare of children involves much more than seeing themin his office or during hospital rounds. As part of hisspecialty practice—treating children with chronic ill-nesses like cancer, diabetes, and cystic fibrosis—he hasworked with Upper Valley school systems and socialservice agencies. He’s seen first-hand the importancepediatricians play in the broader community. They arepart of a team that includes a child’s family and friends,as well as other professionals and support people.

Our nurses serve on the front line of patient care, fromeveryday patient care to advanced critical care facilitiessuch as our Level I Trauma Center and Level IVNeonatal Intensive Care Unit (both are highest levelrated).Working in New Hampshire’s leading teachinghospital, our nurses are also actively involved in majorresearch studies, serve as educational resources for othernurses and in executive positions in national organiza-tions.They also share in decision making throughoutour organization as we address the ongoing challengesof 21st century medicine.

Several of our nurses were honored in 2001. GeorgiaAnn Bru (since deceased) of Woodstock,VT, won theMentorship Award from the American Organization ofNurse Executives for being a “true mentor, trustedcounselor.” Marilyn Bedell of West Lebanon, NH, waspresented the Oncology Nursing Society Linda ArenthExcellence in Cancer Nursing Management Award. And Marie Bakitas Whedon of Lebanon, NH, wasnamed New Hampshire Nurses’Association 2001Nurse of the Year.

Mary Hitchcock Memorial Hospital is a member of theDartmouth-Hitchcock Alliance, a group of ten localhospitals, mental health centers, and home health agen-cies in Vermont, New Hampshire, and Massachusetts. Each member is an independently owned and operatednonprofit organization, overseen by its own board ofdirectors, with its own budget, and supported, in part,by charitable gifts raised in each community. In ourcommunities, we each treat illness and improve well-ness.As a group we share knowledge, resources, andsolutions to improve the quality, efficiency, and availabil-ity of health care.

Two new endosurgical suites, designed specifically forlaparoscopic surgical procedures to support theDepartment of Surgery’s minimally invasive program, went on line last spring. DHMC surgeons are amongthe first in the Northeast to utilize voice-activatedequipment, a robotic camera arm, state-of-the-art digi-tal imaging capabilities, ceiling-mounted equipmentbooms, and voice-controlled digital image capturingcapabilities. Using a hands-free headset, the surgeon canuse the voice-activated technology to position up toseven different camera angles. The technology alsoallows for live, two-way videoconferencing between thesurgeons and the other departments and specialists.

DHMC installed the Medication Access Computer(MAC) to retrieve medications to fill prescriptions inour pharmacy. It took three years for the robot to bedeveloped and it now fills more than 90 percent of allDHMC prescriptions or more than 3,000 doses a day.Central pharmacists now spend more time focused onpatient care and clinical functions such as reviewingdrug therapy, and preventing, detecting, and resolvingdrug-related problems. All medication is stocked in barcoded packages, and the robot reads the codes to signif-icantly reduce prescription filling error, enhance patientsafety, and provide inventory control data.

DHART provides community hospitals the ability totransport critically ill patients 24 hours a day/7 sevendays a week via both air and ground transport. The verylatest life-saving technology and a team of dedicatedintensive care staff are at the ready to treat critically illor injured patients as they travel to a facility that canbest care for them.

DHMC’s Children’s Hospital at Dartmouth (CHaD)has adopted a whole child “pain free” approach to care.It is one of many principles of care which keep childrenand their families directly involved in decisions abouttreatment. To soothe children and calm their anxietyduring procedures, a new suite with digital video, audio, and computer simulation is being used to distract andentertain children. Relaxation techniques and seda-tion/anesthesia can be delivered here, too.

We embarked on a five-year plan to enhance ourClinical Information System (CIS).The plan providesfor a continuing transition to online documentation andsubstantive enhancement to our multimedia medicalrecord.

With funds from a research grant received in conjunc-tion with Creare, Inc., and NASA, DHMC this yearinstalled a six atmosphere Hyperbaric Chamber. The useof oxygen under pressure allows more oxygen to be dis-solved in the blood and brought to the areas that needit. In addition to wound healing, other conditionsapproved for treatment include exceptional blood lossdue to anemia, compromised skin grafts and flaps, andthermal burns.

The kidney transplantation program reached a majormilestone this year: the 200th kidney was transplanted.Working closely with the New England Organ Bank toassure that the match between the donor organ and therecipient is correct, the team transplants 20-25 kidneys ayear in patients from New Hampshire and Vermont.

DHMC is New Hampshire’s only Level I TraumaCenter. We have an Intensive Care Unit; surgeons andoperating-room personnel on duty 24 hours a day; car-diac and microvascular surgeons on staff; in-house CTscanning technology and Magnetic Resonance Imaging(MRI); and other resources for helping patients in criti-cal condition. As a Level I Trauma Center, we areactively involved in research to improve trauma care andprevent injury. DHMC’s Trauma Center is just one partof an extensive collaborative network of emergencypersonnel who work together to ensure that patientsalways receive care at the nearest appropriate facility.

Meeting Community Needs in the Upper Valley and ThroughoutOur RegionDHMC serves as the primary provider and communityhospital for the Upper Valley area of New Hampshireand Vermont. Last year a new law went into effect inNew Hampshire requiring health care charitable truststo file an annual plan and report activities that meetcommunity needs. Our community’s needs wereassessed using a variety of tools, including the “UpperValley Human Services Needs and Assets Assessment,2000,” community agency reports, and interviews withcommunity leaders. The assessments identified needsrelated to: cost and access to health, dental and mentalhealth services; substance abuse services; transportation;affordable housing; and availability of jobs.

DHMC has long provided a multitude of communitybenefits, but this was the first time we tallied the overallcosts and value. In Fiscal Year 2001, Mary HitchcockMemorial Hospital and Dartmouth-Hitchcock Clinicin Lebanon and northern New England provided morethan $30 million in community benefits and free or discounted care.

Last year’s plan included four areas of targeted need, which we attempted to meet in the following ways:

• Address the need for indigent access to dental care.• Develop a plan to reduce teen substance abuse

by working with the Upper Valley Prevention Partnership.

• Continue to provide community outreach through health education programs, preventive health screenings and community and school initiatives, and to improve internal infrastructure in this area.
