~he ave~'age daily number of warders and prisoners in the gaol during 1873 was somewhat less' than, in the previous ~ year, hemg 421 to 441, The number of cases of siclmess which occurred in 1873 were, however, 2357 against 2074 in 1872,

showing a considerable increase. The mortality was 14--three more than in 1872. Nearly all the sickness which came under observation and treatment was either contracted before admi•sion to the gaol,

or the result of causes cxtemnl to the establishment .. 'fhere is not, probably, in the world, a more healthy place than the Melbourne Gaol; well situated, well built, ventilated· and drained; the inmates property· dieted and clothed, and kept' clear of the e~cesses t~ which their class is liable' outside the walls, are sultiect to very few ailments, and these of a trifling nature. The gaol IS a hosprtal for drunkards, where excellent accommodation, medieal aid, the greatest care, attention, and solicitude are. wasted. on the most,worthless class of beings in the whole community, at a very considerable expense to the State; and, as far ns reformmg these wretch~d creatures, with very little result. 'fheir usual course is to drink till they bring on deJit·inm tremens, get taken up by the pohce, remanded by the magistrates for a weclt.to the gaol, are admitted to the gaol hospital till they are cur~d, and then sent back to court, when they are usually discharged, Such people should be sentenced to a term of hard labor, whrch would amply ·pay for the expense of their treatment,,and would, probably, tend to check the vice ..

Order I.-Miasmatic Diseases.

, The diseases of this order were more numerous, and the number of deaths greater than usual; the increase is maiply perceptible under the heads of cynanche, influenza, dysentery, aud diarrhma, The 4 deaths which occurred resulted, one fron.1 dysentery, in an old man, after an illness of eleven days, and 3 from diarrhma; two of the three occurred in old women who had reached th~ naturnl, term of life, and the third was in a ·child who hail been· wasted by a long pr.,vious ilhiess of hoopiog-cough. ,

Order .2:-Enthetic I)iseases.

The eases of gonorrhma and syphilis were not more numerous than usuai. and were ~ery amenable to treatment. As the classes of people who come into the gaol, especially the females, are those amongst w~10m these diseases usually prevail, it may be asstlll).ed that venereal disease h11s not yet spread to any gr~at extent: in the colony. , . . .

Order 3.-Dietic Diseases .

. These ai•e the most, prevalent diseases, and during 1Si3 they were. more than usually common-146 cases of deliriu'm tremens, and 256 cases of drunkenness and its results, are a sad reflection on the morals qf the colony. 'fhe peculiar treatment adopted for delirium tremens in the· gaol was, as usuaJ, followed by tlie smallest mortality attonding the disease in any part of the world. This is a satisfactmy result in a medical point of view, but whether it is so in a moral point of view is open to question. ' '

'' Order 4.-Parasitic Diseases.

_These diseases were not more common than usual. Ther~ is a great tendency to skin disease amongst ,prisoners, and indeed amongst all classes in the colony-the dryness of the atmosphere pro?ably contributing to this result:

CLA~S II.-CONBT!TU1'!0NAL DISEA!ES.

The.disc~ses of this class were le~s nu~erous··than usual, but the mol'tality was considerable. Three cases 'of phtl;isis occurred; two. of these were in womim whose constitutions were destroyed 'by a long course of drunkenness and exposure, and one in a man who had been an almost constant inmate of the gaol for years, and who was of a scrofulous habit of body. . One old man, aged 7.2, died of cancer of th!l stomach of long standing, ' ·

CLASS III.~LOOAL DISEASES. .. • I I '

Order 1.-Diseases of tl1e Nervous Sustem.

Tbe diseases of this: order were not more numerous or attended with greater mortality than usual. One of the fat,al cases of apoplexy occurred in a young woman who had fallen whilst in a state of intoxication, and pro baby ruptured a blood-vessel on the brain by the fall. ':rhe other was a case of serous apoplexy in a man 77 years old, the result of diseased blood-vessels of long standing. ·

Order 2:-Diseases of tlte Respiratory System.

The number ~f cases ofbr~~chiti~ during 18i3 was greater than usual, prob~bly owing to the cold winter; they were.not usually severe. One case was fatal in a man 64 years .old; ~ho had also extensive disease ?f liver, heart, aud kidneys. '

These· diseases were less numerous and of less importance than usual, the principal one, dyspepsi~, ~as the result of intemperance. ·

died after one day's illness, of abscess of the kidney. ·

REMARKS.

During the last year there was not one case of infectious illness in the Stockade. There were three cases of insanity removed to Yarra Bend. .

Of attempted suicide there was only one case, and that not a determined attempt. Of accidents there was only one of any severity-a man's han.d becoming entangled in a teasing-machine resulting in the

loss of the thumb and third ftnger-lock.jaw was averted by the hypodermic injection of morphia three times a dny for a fortnight. .

'l'he general amount of illness for the year was under the average as to severity. There were six deaths, one of these being from diseased heart, one fi·om aneurism of aorta, one from dysenterv, two from

Although the average daily number of priso~ers and officers on board the Sacramenta in 1:878 is considerably in excess of that of 1872 (127 to 102), yet the number of cases of sickness is somewhat less--302 in 1873 as P?mpared with 320 in 1872. . . .

There is, as there has been for years past, a great similarity in the character of the sickness. There has been one death. This occurred in the case of a Chinaman who, from a sudden and unexpected fall of earth

in the quarries, sustained compound fracture of the tibia and fibula. The leg was properly attended to within an hour of the accident. The case progressed most favorably. The external wounds had in due time healed and the bones united, when he was suddenly, on the 44th day, aeized with acute 'enteritis. Notwithstanding every e'ffect to bring about a contrary result he died on the 46th day, after 48-hours illness. There was no apparent cause for the suddenness of the entiritic attack.

Chinamen as a rule sink rapidly under attacks of acute disease, at least such is my experience,

REMARKS.

The " Industrial Nursery School,;' which was opened. in La Trobe street in August 1872, chiefly for the younger children, . . was closed on the 30th June 1873. ·

One case of Diphtheria occurred which terminated favorahly. The cases of conjunctivitis, although tedious in many

instances, did not result in any impairrnent'of vision.

The system of wet-nursing the infants outside the institution has been hitherto successful, I must, however, state that

latterly I have had some difficulty in procuring wet nurses.

The above cases do not include those treated external to the hospital.

Five deaths occurred during 1873. Two of these took place among the girls. in the reform'atory :-one from phthisis

pulmonalia, the other from .acnte inflammatory disease of the heart combined with pneumonia.

T-he third was a case of confirmed phthiSis, sent as an invalid from the Nelson, and died. i;n less than a !Jlonth from the time of his admission to the school.

·Of the two remaining cases one. died from chronic disease of the brain, the other from gangrene of the lung.

Several cases of hooping cough occurred towards tre end of the year; none terminated fatally.

Of the two cases of typhoid fever one has completely recovered, an~ the other, still in hospital, is progressing favorably.

In regard to cutaneous diseases,.a large number of sealp cases, chi~fly ringw~rm, were treated external to the hospital

The death was that of a girl, deformed in the chest, and affected with scrofula, or rather with a compound disease of syphilis and scrofula, which broke out periodically in deep excavated ulcers on the thighs. She had extensive tubercular deposits in both lungs, probably congenital.' 'Whenever the ulcers healed over (as they did from time to time) the lung disease became aggravated; and she at length sunk nnder one of the seizures •. She would not have lasted nearly so long as she did, but for the sea air and the sea bathing. •

'('he fever cases, 96 in number, ought rather to be called cases of "febrile disturbance," as very few were confined to bed' beyond a week. · ·

There has been no serious eye disease. The cases under the head of "ophthalmia" were merely inflammation of the ou~r tunics of the eye-that is, of conjunctivitis. They gave me'but little trouble, .

The benefits of the salt water are obvious in the children at this school, and the special advantages of regular sea bathing in all cases of chronic eye affections, in constitutional scrofula, and in general debility of the system, can scarcely be over-estimated. · ,

The duration of sickness has been rcmrukably short in most cases. Chest and throat complaints were few and unimportant. About a do21en operations were performed-chiefly f~r club foot, inversion of the eyelids, and cataract.

. Towards the end of the. year 1873 the sickness materially increased through the arrival of a number of chronic invalids.

One death took place within the twelve months out of 244·cases of illness. The death was that of a broken down drunkard; the causes, disease 9f the heart with general dropsy. • .

There ·was but one operation of any consequence, this was resection of the bones of the leg for a badly ulcerated conical

stump, resulti~g from a previoliB amputation.

There have been four fatal cases :-One from fractut•e of the knee of an infant, who had received the injury from a fall

from its drunken mother's arms, previous to admission to the gaol: Another was that of a Chinese, who was admitted to the

gaol suffering from extensive and neglected pneumonia. '£he third occurred to another Chinese, suffering. from peritonitis,

caused by abscess of the kidneys, before admission to the gaol: The fourth death Wi\8 cau~ed by congestion of the brain, ensuing on para)ysi&, from which he was suffering on admission.

The health of the Artillery Corps has been on the whole good. The cases were slight and of short duration, and such as might be expected. . ·

One very severe case of diphtheria occurred, which resulted in complete l'ecovel·y. In the Barracks Hospital there was one 'death, that of a member of the old" Royal Artillery" who had .been taken into

the Colonial Corps. In this case, symptoms of phthisis had developed themselves in October 1872, and the man was removed to the Alfred Hospital for purer air. After six month's tl·eatmentin that iustitutiou he· was. discharged incut·able on the 30th April, and died twenty-four days afterwards. ·
