The immigrants who arrived in the colony during 1867 were generally healthy, and, with the exception of a few cases of typhoid fever which were landed from the ship Canterbury in December, their illness presented·no very unusual features.

The cases from the Canterbu·ry were immediately removed to this hospital, and were under treatment at the end of the year. There were only two deaths amongst the immigrants, and these were very young children, one from peritonitis, the other from diarrhrea. .

There was a· considerable number of children from Princes Bridge and' Sunbury Industrial Schools admitted. into the insti£ution during 1867, suffering from scarlatina, measles, whooping-cough, ophthalmia, and cancrum oris. This course was adopted in the overcrowded state of these institutions, to isol.ate those children from other children o( the schools, and thus check the spread of contagious diseases. . . · . There were nine deaths in this hospital amongst the children of the Industrial Schools-one from measles,

one from whooping-cough, two from scarlatina, and five from that intractable and fatal disease, cancrum oris ; these deaths occurred in children of weak and delicate' constitutions. .

I cannot conclude these remarks without refernng to the great advantage the Industrial Schools derive from having the Immigration Hospital as a receptacle for many cases that cannot be treated beneficially in those estab" Jishments.

The sickness aud mortality ii;~ the Melbourne Gaol during the.yead867'did-not materially ·differ:frori.t those of the previous year. The gaol itself was remarkably healthy, nearly the wl10le of the sickness having yccurred among prisoners suffering from disease on their admission 'into the gaol.' The sick ,list was greatly increased by the admission of a number oflunatics, for whom there was not sufficient. accommodation at the asylum.

Clas~ I.-Zymotic Diseases.'-The most·numerous diseases in tills class were delii:ium"trcmens, intemperance (a less seve're phase of _delirium tremens), and itch. It 1vas often difficult to distinguish between delirium tremens and intemperance ; both res~ted, from the same cause, and were attended. by most of the same symptoms. The principal difference was the delirium, and this was to some extent present at night in the cases of intemperance. Delirium tremens' has always fornied a considerable item in the diseases of the gaol, and great attention has been paid to its treatment; the various modes of treatment recomm_ended by the best authorities in.Europe have been tried in their turn, without any great differen~e in the result: .My·a1,tention 'has bce'n'arre'sted ·by t!Hruii.iform, indeed almost invariable, post mm·tem appearances observed in fatal cases of this disease, and these, coupled with the symptoms indicative ·of the usual cause of the malauy during" life, have compelled me to conClude, in oppo­sition to most writers on the subjept, that the disease essentially' consists in an inflammation of the membranes of the brain, caused by the ·.il;nmoderate use of 'stimulants: This 'view has induced ine to discard the usual mode

·of treatment adopted in ~his disea~e-viz., the use ofsti~~l:mts, nar8<;~ics;_.&c.-·!l.nd to adopt such measures as arc calculated· to reduce tl1e JDflammatwn. . These may be stated· to consist m cleanng out the stomach and bowels, applying cold to the head, keeping the patient as· quiet as· possible,'iisinli an' unstimulating and nutritio~s diet, and, above all, applying a large number of.]eeches.below.·thc.eaFs·repcatedly, If ne~essary. The success of this treat'Qlent

is remarkable, the mortality from ,that·diseasiJ)n the .. g1J.t,Jl,.during.the y;ear having been reduced to less than I per cent. When this is compared \vith the mortality among .the troops in Great Britain '(15'7 per cent.), and with that of the army in India (13·7 per cen:t.); for the' same disease, the contrast is striking. The number of eases of itch which were 'admitted to the ,_aol in 1867 were 307, showing an increase of nearly lOO on the previous year; and scarcely a day passes without some case being admitted. The low lodging-houses of Melbourne must be infected very largely by this disease. The cases of diarrhcea and dysentery were somewhat less than during the previous year; the mortality also was greatly diminished, that of 1866 being at the rate of nearly 6 per cent., whilst in 1867 it was only a.bout 4 per cent .. There ·were 49 cases, of rheumatism ; these occurred to prisoners mostly whilst in the gaol, and were very slight, the average duration of the disease being only five days.

Class III.-Local Diseases.-The cases of this class of disease were rather less numerous than in I866, but the mortality was considerably greater. There were 135 cases of the various forms of insanity admitted; of these, 3 died, the deaths being recorded under the heads of paralysis, apoplexy, and suffocation respectively. The case of suffocation was one in which a greedy patient had bolted a large piece of meat, which stuck in the glottis, and though medical aid was procured in a few minutes and the meat removed, the result was fatal. The greatest amount of mortality occurred among the eases of apoplexy-they were all cases of serous al?oplexy, the result of previous disease of the brain, probably caused by drunken habits. The fatal case of mening1tis occurred in a 1pan who had been a Ion&' time in the gaol hospital, frequently subject to convulsions. The cases of epilepsy were numerous, but, with the exception of the fatal case, they were very amenable to treatment. The case of softening of the brain occurred in a man aged 33, but was the result of-disease of long standing. ,The cases of ophthalmia were numerous,

. but not generally of a severe type; they were all cured without leaving any permanent change of structure in the eye. The cases of bronchitis were rather less numerous than during the preceding year; the fatal case occurred in a child which was almost in a dying state OJ;J. its admission to the gaol. The cases of dyspepsia (nine-tenths of which were the result of previous intemperance) were considerably less numerous than during the preceding year, and on the whole less severe. A case of true leprosy occurred which was of some interest; the subject of it was a native of Santa Cruz in the West Indies, who had come to this colony from Adelaide with the hope of getting to his native place. He was a frightful object, having• l9st some of his fingers an<} toes, his face was distorted, and his mouth was contracted. ~o treatment was of any use, and at length he died. . Class IV.-Developmental Diseases.-Oue man died of old age, apparently sinking from natural decay without any symptom of disease. ·

REMARKS.

The number of cases of illness treated in this hospital during the year 1867 was 421. Of these there were' seven deaths-one from J;lhthisis, one from congestion of lungs, one from aneurism of aorta, one from perforation of bowels, one from bronchitis, two from diseased kidneys.

Of fever there was only one case, brought into hospital on the 14th of July. This roan arrived in the stockade on the 18th of June. He was then suffering from the premonitory symptoms of fever.

The stockade was remarkably free from epidemics of any kind. Very few cases of influenza. The number of external ca~es treated was 1,823.

, The returns again show an increase. in the daily average strength ·of the hulks and reformatory, the numbers being, in 1866, 306; and in 1867, 320.

There has been a corresponding increase in the number of cases· of sickness, viz. :-in 1867, 418; and in 1866,379.

Four deaths have occurred during 1867. One adult female, with malignant disease of the lungs, and three c~ildren ; ca!lse of death being, respectively-diphtheria; dentition, wi~h c_erebral congestion; and premature

· b1rth. In th1s last case the child was born under the seventh month, and hved only twenty-four hours. The average duration of cases has been 9 days, as against 7·75 for 1866. A number of chronic foul ulcers, and one case of obstinate psoriasis, have mainly brought about this result.

During the year 1867 the average daily number of children in the Prince's Bridge Industrial School was 420, while the " sick return" for the same period shows 1,844 cases under medical treatment.

CLASS I.-ZYMOTIC DISEASES. Of this nurU:ber 1,365 cases occurred in the Zymotic Class ; ophthalmia alone-which. from the beginning ha~

been the chief drawback to the health of this institution-furnished 754 cases. . Ophthalmia.-On taking charge, on the 1st of March, I found 84 cases· of this formidable disease in hos­

pital, many of them presenting signal marks of its destructive character; during that month 160 new eases were added to the sick list, many of which speedily yielded to the measures· adopted for their cure ; but, in consequence of overcrowding, and other unfavorable circumstances, over which I had no control, many of them had repeated attacks, and were several times placed under medical treatment. As the year advanced, however, although fresh eases were occurring daily, I was enabled to keep the disease within comparatively narrow bounds, and also to lessen its virulence, so that on the 31st of December only 15 cases remained in hospital, none of them presenting the virnlenee or intensity which characterized the disease in the earlier,part of the year, and I am happy to add the injury to sight was very inconsiderable. .

.Measles.-There were 90 eases of measles in .May and June, all of which, except one, terminated favorably. Mumps.-In the months of September, October, and November, there were 120 cases of this disease-all

Wltooping-cough.-Towards the end of the year there were 25 eases of whooping-cough, none of which presented any unusual features.

Fever.-There were 11 cases of continued fever of low type; all had a favorable issue. · <:;:ynanche.-These were eases of ordinary sore throat. -JJiarrhrea and Dysentet:y.-,-There were five deaths from diarrhooa, and one from dysentery, all occurring in

.children of weak constitutions during the period of dentition. . . Scabies-Many of the children 'had this disease on admission; it readily yielded to the treatment adopted.

Cuss H.-CoNSTITUTIONAL DISEASES. Cancrum Oris (T.Jlcerative Stomatis).-The five children who died of this fatal and intractable disease were

young, of scrofulous. habit and delicate constitution. H,ydrocephalus.-The child who died from this disease was sent from Gippsland in a highly diseased state,

'With its head of enormous dimensions. It was an unfit subject for the Industrial Schools. .

CLASS III.-DISEASES OF TirE NERVOUS SYSTEM. Congestion qfthe Brain.-Two delicate children died from this disease during the·period of dentition. Cutaneous Diseases, aonsistinrJ of Eczema, Psoriasis, &a.-Many of the children were affected w~th these.

·diseases on admission ; several of them had repeated attacks during the year.

. CLASS IV.-DEVELOP111ENTAL DISEASES. Marasmus and Debility.-The children who died from these complaints were sickly from their birth •

. The health of prisoners and immigrants at Geelong has been uncommonly good during the past year-one death only occurring in the Gaol and one in the Depot. Both were cases of long standing .disease, contracted outside. ·

The state of the Industrial School, also, in the year 1867, must, I think, be considered highly satisfactory in every respect; the total number of cases of illness · · 1867, 184, with 9 deaths; whilst, in 1866, the total number was 512, with 10 deaths. Five out of the 9 however, occurring in 1867, were from exhaustion after ·measles, of which there was an alarming epidemic in the last quarter of the year 1866 .. We had but one case of measles in 1867; which recovered; consequently but 4 deaths properly belong to the sick list of 1867.

The ophthalmia shows an increase of 3~ cases on the previous year. ~l'his is, however, more apparent thau real, as, during the present year, 1867, I have classed every case of temporary congestion of the eye under the general head of ophthalmia, of which it forms the first stage, whilst in the previous year only confirmed purulent cases were recorded. . . ,
