SSI - Initial Event Report - Near Miss with Mower and Vehicle on Curlee Road -Significance Medium

On August 5, 2016, an SST employee mowing on a Hustler ZTR towards the west on the north

edge of Curlee Road turned south towards the roadway in front of a west bound vehicle on Curlee Road. Only a portion of the mower deck went into the roadway. The oncoming vehicle stopped an estimated 3 car lengths back and allowed the mower to get fully off the roadway before proceeding.

1. FPDP - Safety Notification Reports - C-310 CAAS Exclusion Zone Cone Missing­Significance Medium

On August 1, 2016, the basement tunnel entrance in C-331 was not controlled to prevent access to the C-310 CAAS Exclusion Zone. The recommended locations where cones should be used in addition to flagging/barriers are listed in CP3-0P-0301 Appendix B. The individuals who entered the tunnels were briefed on the areas to avoid area due to the C-310 CAAS LCO, but the lack of a control in the area could have resulted in unauthorized access.

The issue is not a procedure violation and other controls were in place to make individuals aware that the C-310 building was under a CAAS LCO. The controls to prevent access to the Exclusion Zone could be improved upon to ensure employees do not breach the zone unintentionally.

2. FPDP- Safety Notification Reports - Non-Compliant ASHRAE Filters Requires Issuance of Level 4 Criticality Safety Incident Report - Significance Medium

On August 3, 2016, Nuclear Criticality Safety (NCS) group issued Criticality Safety Incident Report# NCS-INC-16-002 regarding evaluation of ASHRAE (manufactured by Flanders) filters that were previously ordered, inspected, accepted, and installed into Negative Air Machines (NAMs ). The surface area of these Flanders filters was greater than the maximum value acceptable in accordance with NCSE 100 requirements. A Level 4 Incident detennination was identified in the report, documenting a non-compliance that does not result in the loss of double contingency or the loss of control that is relied upon for a singly contingent operation.

The incident report credits the controls already in place for the ASHRAE filters that were in service or otherwise spent at the time of the issue identification - deficiency tags on the NAMs that contained the filters and a formal work pause issuance for operation of NAMs and handling of spent/waste filters.

3. FPDP - Radiological Anomalous Condition Report - Discovery of Contamination on Excavated Hydrant Valve and Piping at C-611 - Significance Medium

On August 11, 2016, while conducting spot checks of the excavation and cast iron, hydrant valve body, piping, and fittings removed from the Fire Impainnent Project excavation on the SE comer of C-611, RCTs discovered radioactivity of an unknown origin. RCTs were performing characterization surveys for waste characterization. The excavation is approximately 6-8 feet below grade. Initial measurements of radioactivity indicated the highest readings (81,881 dpm/100cm2 total beta/gamma, nondetectable alpha) were from rust/scale located on the bottom of the valve body, piping, and fittings. Surveys of adjacent soil did not indicate the presence of soil contamination. Alpha readings are inconclusive because of the amount of scale on the pipe and the degree of self-absorption that is occurring. There is no history of contamination in this area and the work history indicated the valve and pipe to be original construction from the l 960' s. The excavated piping was covered and secure, area was controlled until further investigation could be performed to detennine the radioisotopes involved.

On August 12, 2016, a sample of the rust/scale was taken to the C-710 radiochemical lab for analysis. Preliminary gamma spectroscopy results indicated the presence of uranium with trace levels of neptunium and cesium. Gamma spectroscopy is not able to detect technetium, plutonium, or thorium at environmental levels. No Ra-226 or Pb-210 was detected on the preliminary analysis. Follow-up· analyses were perfonned over the weekend and reported on 08/15/2016 that confirmed the presence of uranium and neptunium. The laboratory was continuing its analysis to obtain a more complete radioisotopic profile of the material in order to determine its origin.

RCTs surveyed the excavator and workers. No contamination was found. Excavated piping and valves were covered and secured to prevent any spread of contamination. The Project Manager and Facility Manager were notified to prevent access to the area until preliminary sample results could be obtained. Access to the area was controlled by construction boundaries.

Once preliminary results were obtained from the lab, RADCON posted the area and excavated pipe as a Controlled Area, Contamination Area. A soil sample was collected from the area immediately adjacent to the main water line. The lab was requested to perform a confirmation gamma spectroscopy analysis on the sample. Results were obtained on 8/15/2016. Additional results are pending. An ORPS report was initiated on 8/15 based on confirmed sample results indicating presence of uranium and neptunium.

4. FPDP- Radiological Anomalous Condition Report - Clothing Contamination at C-400 - Significance Medium

When exiting the Contamination Area at C-400, a maintenance mechanic alarmed the personnel contamination monitor. RCTs confirmed the alarm and discovered 32,814 dpm/100cm2 total beta/gamma and 86 dprn/100cm2 total alpha on the left knee of the mechanic's company issued scrub pants. RCTs surveyed the mechanic's skin under the contaminated clothing and no detectable radioactivity was discovered.

The mechanic was performing grinding activities in C-400 Zone 3, cutting and removing floor pans. The employee indicated that the contamination could have resulted from either sparks from grinding or he may have knelt down on the contaminated surface. The mechanic was wearing a single set of FR rated cotton anti-C coveralls. No secondary barrier or knee pads were utilized.

RCTs confirmed that contamination was localized to the left knee of the scrub pants and the mechanic's skin was not contaminated. RCTs confiscated the mechanic's scrub pants and requested a written statement. The project is pursuing use of secondary barriers such as FR knee pads or flame retardant blankets.

5. FPDP - Safety Notification Reports - Mechanic Experiences Shortness of Breath While Using Plasma Cutter in C-400 - Significance Medium

On August 24, 2016, while using a plasma cutter to cut around the anchor bolts of a floor pan in Zone 5 at C-400, a mechanic was standing over the immediate area of focus, observed wisps of

white smoke emitting from the floor anchor bolt during the cutting, and reported a shortness of breath.

The mechanic using the plasma cutter was immediately transported to Medical for evaluation.

The Front Line Manager (FLM) for the activity stopped the work and directed the other personnel working in the immediate area to report to Medical for evaluation, as well. Personnel in the immediate area of the cutting activities were wearing required PPE, including a powered­air purifying respirator with P-100 cartridge. The RCT surveyed smears of PPE in use and did

The Mechanic was provided first aid treatment and released to work with no restrictions. The remaining personnel working near the activity were evaluated and released with no medical treatment administered.

C-400 Deactivation Project issued a Work Pause for all plasma and other forms of flame cutting until further evaluation of the event can be completed.
