ABSTRAK
Tanaman kepel (Stelechocarpus burahol [Blume] Hook.f & Thomson) merupakan flora asli
Indonesia bermanfaat sebagai antibakteri, antioksidan, antifungi dan juga sebagai antiseptik luka.
Pada penelitian sebelumnya telah dilakukan uji aktivitas penyembuhan luka terbuka terhadap
perasan buah kepel. Perasan buah dengan konsentrasi 60% mampu mempercepat proses
penyembuhan luka terbuka dengan persentase penyembuhan luka sebesar 59,84%. Adanya
aktivitas antibakteri dan antioksidan pada daun kepel yang mampu membantu proses
penyembuhan luka serta senyawa kimia yang terkandung seperti flavonoid dan tanin, maka
penting dilakukan penelitian lanjutan terhadap ekstrak etanol 70% daun kepel sebagai penyembuh
luka bakar. Penelitian bertujuan untuk mengetahui aktivitas ekstrak etanol 70% daun kepel dalam
formula sediaan salep dengan target sebagai penyembuh luka bakar. Pengujian dilakukan dengan
pemodelan tikus luka bakar dengan pengukuran 4 parameter secara histologi yaitu jumlah
makrofag, kepadatan fibroblast, kecepatan re-epitelisasi dan pengukuran luas luka bakar.
Sebanyak 30 ekor tikus jantan dibagi menjadi 5 kelompok, kontrol positif (Burnazin®), kontrol
negatif (basis salep), salep ekstrak daun kepel konsentrasi 3,25%, 6,5% dan 13%. Pengukuran
parameter jumlah makrofag, kepadatan fibroblast dan kecepatan re-epitelisasi dilakukan pada hari
ke-3, 7, 14. Pengamatan histologi menunjukkan peningkatan jumlah makrofag, jumlah fibroblast
dan ketebalan re-epitelisasi serta penurunan luas luka bakar pada kelompok salep ekstrak daun
kepel secara signifikan dibandingkan kontrol negatif, dan pada konsentrasi 13% ekstrak etanol
70% daun kepel menunjukkan hasil sebanding dengan silversulfadiazin.

BAB 1. PENDAHULUAN
Indonesia merupakan negara yang kaya akan sumber daya hayati.
Keanekaragaman hayati yang tumbuh di hutan Indonesia merupakan aset nasional
yang tidak terhingga nilainya bagi kepentingan manusia. Salah satu manfaat
keanekaragaman hayati adalah pemanfaatannya sebagai obat contoh tanaman
kepel. Tanaman kepel (Stelechocarpus burahol [Blume] Hook.f & Thomson)
adalah flora asli dari Indonesia yang merupakan identitas provinsi daerah
Istimewa Yogyakarta yang biasa dijumpai di keraton-keraton yang ada di Pulau
Jawa (Haryjanto 2012).
Luka bakar adalah suatu bentuk kerusakan atau kehilangan jaringan yang
disebabkan oleh kontak dengan sumber yang memiliki suhu tinggi seperti api, air
panas, bahan kimia, listrik dan radiasi (Moenadjat 2009). Parahnya luka bakar
ditentukan oleh dua faktor yaitu luas bagian tubuh yang terbakar dan kedalaman
luka bakar yang dibedakan menjadi derajat pertama, derajat kedua, dan derajat
ketiga (Corwin 2009). Luka bakar derajat ketiga ini adalah luka bakar ketebalan
penuh, melibatkan kematian epidermis, dermis, dan appendiks kulit. Begitu
kompleksnya proses penyembuhan luka bakar sehingga membutuhkan banyak
biaya untuk menstabilkan kondisi umum tubuh, perawatan luka itu sendiri
maupun untuk mencegah dan mengobati komplikasinya karena sampai saat ini
perawatan luka bakar masih mahal dan komplikasinya menyebabkan morbiditas
dan mortilitas. Diperlukan adanya solusi obat baru untuk membantu masalah ini
secara efektif, aman, murah, dan terjangkau. Salah satu alternatifnya adalah
dengan penggunaan obat tradisional.
Berdasarkan penelitian Sunarni dkk. (2007) daun kepel memiliki
kemampuan sebagai antioksidan. Penelitian lain juga menyatakan bahwa perasan
buah kepel dengan konsentrasi 60% memiliki aktivitas penyembuhan luka terbuka
pada tikus dengan persentasi penyembuhan sebesar 59,84% (Pribadi dkk. 2014) .
Berdasarkan aktivitas daun kepel sebagai antioksidan dan antibakteri, serta
perasan buah kepel sebagai penyembuhan luka terbuka, maka dapat
dimungkinkan bahwa pengujian pada ektrak etanol 70% daun kepel memiliki

aktivitas mempercepat proses penyembuhan luka bakar. Untuk membuktikan
khasiat ini dan berdasarkan data penelitian sebelumnya, maka peneliti
berkeinginan untuk melanjutkan khasiatnya terhadap luka bakar.
Penelitian ini akan dilakukan pada tikus yang diinduksi luka bakar derajat
tiga dengan pengukuran 4 parameter luka bakar yaitu jumlah makrofag,
kepadatan fibroblast, kecepatan re-epitelisasi dan pengukuran penurunan luas luka
bakar. Pengukuran dilakukan dengan menggunakan aplikasi Image Raster 3.0.
Banyaknya parameter yang diukur untuk menunjang data yang bisa membuktikan
daun kepel berkhasiat sebagai pengobatan luka bakar. Diharapkan hasil penelitian
dapat menambah khasanah khasiat tanaman asli Indonesia dan bermanfaat dalam
penyembuhan luka bakar.

BAB 2. TINJAUAN PUSTAKA
Indonesia merupakan negara yang kaya akan sumberdaya hayati. Terdapat
sekitar 30000 spesies tumbuhan berbunga di hutan tropika Indonesia.
Keanekaragaman hayati yang tumbuh di hutan Indonesia merupakan aset nasional
yang tidak terhingga nilainya bagi kepentingan manusia. Manfaat
keanekaragaman hayati adalah kegunaannya sebagai obat (Hidayat 2011). Salah
satu tanaman yang dapat dimanfaatkan sebagai obat adalah tanaman dari famili
Annonaceae yaitu tanaman kepel (Ramadhan dkk. 2015). Tanaman kepel
(Stelechocarpus burahol [Blume] Hook.f & Thomson) adalah flora asli dari
Indonesia yang merupakan identitas provinsi daerah Istimewa Yogyakarta yang
biasa dijumpai di keraton-keraton yang ada di Pulau Jawa (Haryjanto 2012).
Secara tradisional buah kepel digunakan oleh masyarakat sebagai deodoran oral,
dan daun kepel digunakan sebagai asam urat (Anggraeni 2013).
Berdasarkan penelitian Sunarni dkk. (2007) daun kepel memiliki
kemampuan sebagai antioksidan penangkap radikal terhadap isolat flavonoid.
Penelitian lainya Hidayat (2011) mengenai fraksinasi golongan flavonoid dari
daun kepel yang berpotensi sebagai antibakteri terhadap Staphylococcus
epidermidis dengan hasil Konsentrasi Hambat Minimum (KHM) yang diperoleh
sebesar 1.00 mg/ml dan Konsentrasi Bunuh Minimum (KBM) sebesar 2.00
mg/ml. Penelitian sebelumnya Pribadi dkk. (2014) juga menyatakan bahwa
perasan buah kepel dengan konsentrasi 60% memiliki aktivitas penyembuhan luka
terbuka pada tikus yang paling baik dengan presentasi penyembuhan sebesar
59,84%. Berdasarkan aktivitas daun kepel sebagai antioksidan dan antibakteri,
serta perasan buah kepel sebagai penyembuhan luka terbuka, maka dapat
dimungkinkan bahwa pengujian pada ektrak etanol 70% daun kepel memiliki
aktivitas mempercepat proses penyembuhan luka bakar.

panas, bahan kimia, listrik dan radiasi (Moenadjat 2009). Terdapat sekitar 265.000
kematian setiap tahun disebabkan oleh luka bakar. Kebanyakan kasus luka bakar
terjadi di negara-negara berkembang dengan pendapatan rendah hingga

menengah, dan setengah dari kasus terjadi di negara-negara Asia Tenggara,
termasuk Indonesia (WHO 2016). Pada tahun 2012 tercatat 25 kasus luka bakar
derajat dalam (23,8%) dirawat di Burn Unit RSUD Dr. Soetomo dari total 105
pasien luka bakar yang dirawat (Hidayat 2013).
Parahnya luka bakar ditentukan oleh dua faktor yaitu luas bagian tubuh
yang terbakar dan kedalaman luka bakar yang dibedakan menjadi derajat pertama,
derajat kedua, dan derajat ketiga (Corwin 2009). Luka bakar derajat ketiga ini
adalah luka bakar ketebalan penuh, melibatkan kematian epidermis, dermis, dan
appendiks kulit. Luka bakar ini sangat berbahaya dan pasien sebaiknya dirawat
inap bila memungkinkan (Ledbetter 2010).
Proses penyembuhan luka dibagi menjadi beberapa fase yang saling
berkaitan mulai dari fase inflamasi, fase proliferasi sampai fase maturasi
(Novriansyah 2008). Proses penyembuhan luka bakar mempunyai persamaan
dalam fase penyembuhan luka pada umumnya, perbedaanya adalah durasi setiap
fasenya (Tiwari 2012). Fase inflamasi terjadi sejak terjadinya luka sampai sekitar
hari kelima, dan makrofag merupakan sel predominan pada hari ke-3 pasca luka
(Franklin 2007). Jumlah makrofag tinggi pada fase inflamasi dan akan lebih
rendah jumlahnya pada fase selanjutnya sampai luka menutup (Mutiara dkk.
2015). Tingginya jumlah makrofag pada fase inflamasi dapat memfagositosis sel-
sel yang mati lebih cepat dan semakin banyak growth factor yang dilepas, maka
proses penyembuhan luka bakar menjadi lebih cepat (Puspitasari 2015).

BAB 3. METODE PENELITIAN
3.1 Determinasi dan Pengambilan Bahan Uji
Bahan yang digunakan adalah bagian daun dari tanaman kepel
(Stelechocarpus burahol [Blume] Hook.f. & Thomson) yang diperoleh dari Balai
Penelitian Tanaman Rempah dan Obat (BALITTRO). Determinasi tanaman
dilakukan di “Herbarium Bogoriense”, Bidang Botani Pusat Penelitian Biologi,
LIPI, Cibinong, untuk mengidentifikasi jenis dan memastikan kebenaran tanaman
tersebut.
3.2. Persiapan Hewan Uji
Tikus yang digunakan sebagai hewan uji diaklimatisasi terlebih dahulu
selama 7 hari dalam kandang hewan Fakultas Farmasi UHAMKA. Selama
aklimatisasi tikus diberikan makan dan minum setiap hari. Hewan percobaan yang
digunakan sebanyak 30 ekor tikus putih (Rattus norvegicus) jantan Sprague
Dawley dengan berat 150-200 g. Tikus dibagi menjadi 5 kelompok dengan jumlah
masing-masing kelompok terdiri dari 6 ekor.
3.3 Pembuatan Ekstrak Etanol 70% Daun Kepel
Pembuatan ekstrak etanol 70% daun kepel dengan maserasi 1,2 kg serbuk
simplisia daun kepel dengan etanol 70%. Pada 6 jam pertama dilakukan
pengadukan secara perlahan. Selanjutnya didiamkan selama 18 jam, wadah
diletakkan ditempat yang sejuk dan terlindung dari cahaya. Kemudian hasil
maserasi disaring, ampasnya dimaserasi kembali dengan etanol 70% dan
diperlakukan dengan cara yang sama, remaserasi dilakukan sebanyak tiga kali.
Maserat yang diperoleh dipekatkan dengan vacuum rotary evaporator dilanjutkan
dengan waterbath dengan suhu 40ºC hingga diperoleh ekstrak kental.
3.4. Uji Karakteristik dan Penapisan Fitokimia
a. Uji Organoleptis
Pemeriksaan organoleptik meliputi pemeriksaan bentuk, warna, bau, dan
rasa terhadap ekstrak etanol 70% daun kepel (Depkes 2008).
b. Penetapan Susut Pengeringan.

Tara botol timbang dangkal bersumbat kaca yang telah dikeringkan pada
suhu 105 °C selama 30 menit. Timbang 2 gram ekstrak kental dalam wadah yang
sudah ditara. Ratakan zat uji sampai setinggi lebih kurang 5 mm perlahan-lahan
dengan menggoyang. Buka sumbat dan biarkan sumbat ini di dalam oven,
keringkan di dalam oven pada suhu 105 °C selama 30 menit atau hingga bobot
tetap, pada waktu oven dibuka botol segera ditutup dan biarkan dalam desikator
sampai sampai suhunya mencapai suhu kamar sebelum ditimbang. Timbang pada
selang waktu 1 jam sampai perbedaan antara dua penimbangan berturut-turut
tidak lebih dari 0,25% (Depkes 2008).
c. Penapisan Fitokimia
Penapisan fitokimia dilakukan terhadap ekstrak etanol 70% daun kepel
meliputi adanya fenol (FeCl3), flavonoid ( Shinoda dan ammonia), tanin (gelatin
dan FeCl3), saponin ( test busa), alkaloid ( Mayer, Bouchardat, Dragendorff) ,
steroid/terpenoid (Liebermann Burchard) (Hanani 2014)
3.5. Pembuatan Konsentrasi Ekstrak Etanol 70% Daun Kepel
Berdasarkan hasil orientasi daun kepel dengan konsentrasi 13%
menunjukkan aktivitas penyembuhan yang paling baik. Maka dari itu dibuat
variasi dengan menurunkan konsentrasi uji sebagai berikut:
Konsentrasi Uji I : 1 x 13% = 13%
Konsentrasi Uji II : ½ x 13% = 6,5%
Konsentrasi Uji III : ¼ x 13% = 3,25%
3.6. Pembuatan luka bakar derajat III dan perlakuan terhadap hewan uji
Tikus diaklimatisasi selama 1 minggu dan dicukur rambutnya di daerah
punggung bagian atas. Kemudian sisa rambut dibersihkan menggunakan Veet®.
Tikus dianestesi menggunakan ketamin dengan dosis 40,08 mg/kg BB secara IM.
Plat logam khusus dengan diameter 1,5 cm x 1,5 cm dipanaskan sampai 100ᵒC
kemudian logam panas ditempelkan ke daerah punggung bagian atas tikus selama
30 detik. Sesudah pembuatan luka, tikus diberikan analgetik secara oral. Ekstrak
daun kepel konsentrasi 3,25%, 6,5%, 13%, burnazin®, dan basis salep diberikan
dengan mengoleskan secara merata pada daerah luka pada masing-masing
perlakuan sebanyak 2 kali sehari setiap pagi dan sore (Nazrun 2005).

3.7. Pengambilan Sampel Histologi
Spesimen biopsi kulit atau jaringan diambil yaitu pada hari ke 3, 7 dan 14.
Setiap pengambilan meliputi daerah luka dengan mengambil bagian dari otot dan
jaringan lemak subkutanius masing-masing hewan, agar pada saat pengamatan
bagian-bagian kulit terlihat jelas. Jaringan diambil menggunakan pisau bedah
yang telah dibersihkan menggunakan etanol. Sebelum dilakukan pengambilan
sampel dilakukan anestesi terhadap hewan uji menggunakan ketamine injeksi.
Spesimen difiksasi dengan larutan Buffer Neutral Formalin 10 %.
3.8. Pembuatan Preparat Histopatologi
Jaringan difiksasi dengan menggunakan larutan Buffer Neutral Formalin
atau BNF 10% dibiarkan pada suhu kamar selama minimal 24 jam.Jaringan
dipotong-potong dan dimasukkan ke dalam wadah spesimen yang terbuat dari
plastik. Dilakukan proses dehidrasi dengan konsentrasi alkohol bertingkat yaitu
alkohol 70%, 80%, 90% alkohol absolut I, alkohol absolut II, masing-masing 2
jam. Kemudian dilakukan penjernihan (clearing) untuk menghilangkan sisa
alkohol dengan menggunakan xylol. Lalu dilakukan proses pencetakan atau
parafinisasi menggunakan parafin sehingga tercetak di blok-blok parafin,
disimpan pada lemari es. Blok-blok parafin kemudian di potong tipis setebal 6-8
µm dengan menggunakan mikrotom. Hasil potongan diapungkan dalam air hangat
bersuhu 60°C (waterbath) untuk meregangkan agar jaringan tidak berlipat.
Sediaan kemudian diangkat dan diletakkan pada gelas objek untuk dilakukan
pewarnaan Hematoxyllin dan Eosin (HE). Prosedur pewarnaan Hematoxyllin-

Eosin (HE) dilakukan dengan cara deparafinasi, sediaan preparat pada objek
direndam dalam xylol I dan xylol II selama masing-masing 2 menit. Kemudian
redehidrasi dengan perendaman secara berturut dalam alkohol absolut, alkohol
95% dan alkohol 80% masing-masing selama dua menit lalu dicuci dengan air
mengalir. Pewarnaan dengan Hematoksilin dilakukan selama 8 menit, selanjutnya
dibilas dengan air mengalir, serta diwarnai dengan Eosin selama 2-3 menit.
Sediaan yang diwarnai eosin dicuci dengan air mengalir lalu dikeringkan. Sediaan
kemudian dilakukan dehidrasi lanjutan, dimasukkan ke dalam alkohol 95% dan
alkohol absolut masing-masing sebanyak 10 kali celupan, lalu ke dalam alkohol
absolut II selama 2 menit. Selanjutnya ke dalam xylol I selama 1 menit dan xylol
II selama 2 menit. Sediaan kemudian diteteskan dengan etilen dan ditutup dengan
gelas penutup dan selanjutnya diperiksa di bawah mikroskop (Mustaba 2012).

BAB 4. HASIL DAN PEMBAHASAN
A. Hasil Determinasi Tanaman Kepel
Determinasi merupakan tahap awal dalam penelitian untuk mengetahui
identitas dan memastikan kebenaran jenis dari tanaman yang akan diteliti.
Determinasi tanaman dilakukan di “Herbarium Bogoriense” Pusat Penelitian
Biologi - LIPI Cibinong, Bogor menyatakan bahwa tanaman yang digunakan
dalam penelitian ini adalah Stelechocarpus burahol (Blume) Hook.f. & Thomson
dengan nama daerah kepel dan suku Annonaceae.
B. Hasil Ekstraksi Daun Kepel
Hasil ekstrak daun kepel yang diperoleh dilihat pada tabel di bawah ini.

Daun kepel yang digunakan dalam penelitian ini yaitu daun segar. Daun
kepel sebanyak 7 kg yang diperoleh dari BALITTRO dibersihkan dengan cara
dicuci dengan air mengalir sampai bersih agar terpisah dari kotorannya, kemudian
dikeringkan dengan cara diangin-anginkan dan terlindung cahaya matahari
langsung. Pengeringan yang dilakukan bertujuan untuk mengurangi kadar air yang
terkandung dalam simplisia sehingga dapat mencegah timbulnya bakteri dan
jamur yang dapat menurunkan kualitas simplisia. Simplisia daun kepel kering
diperoleh sebesar 2,1 kg, kemudian dibuat serbuk dengan menggunakan blender
dan diayak dengan ayakan no mesh 40. Penyerbukan dilakukan untuk
memperkecil ukuran partikel simplisia. Ukuran partikel yang kecil akan
memperbesar luas permukaan simplisia sehingga pelarut yang digunakan mudah

masuk kedalam pori-pori simplisia dan senyawa aktif yang tertarik lebih
maksimal dan memudahkan proses ekstraksi.
Diperoleh berat serbuk kering sebesar 1,2 kg kemudian diekstraksi dengan
metode maserasi. Metode maserasi dipilih karena sederhana baik secara
pengerjaan maupun peralatan. Selain itu metode ini baik digunakan pada senyawa
yang mudah terurai pada metode penyarian yang menggunakan pemanasan.
Maserat yang didapat kemudian dipekatkan dengan menggunakan vacum rotary
evaporator pada suhu 50ºC sampai didapatkan ekstrak kental yang mudah
dituang. Tujuan dari pemekatan adalah untuk mengurangi kadar air dan
mengurangi sisa pelarut. Pemekatan dilanjutkan kembali menggunakan waterbath
dengan suhu 50°C untuk mendapatkan ekstrak kental. Ekstrak kental daun kepel
yang diperoleh sebesar 134,96 g.
C. Hasil Karakteristik Ekstrak Etanol 70% Daun Kepel
Untuk mengetahui karakteristik ekstrak etanol 70% daun kepel dilakukan
uji organoleptis, penetapan susut pengeringan dan rendemen ekstrak etanol 70%
daun kepel.

Selain identifikasi serbuk dan ekstrak daun kepel, ekstrak etanol 70% daun
kepel juga dihitung rendemen yang diperoleh serta dilakukan penetapan susut
pengeringan. Berikut hasil rendemen ekstrak dan susut pengeringan:

Rendemen dihitung untuk mengetahui persentase zat aktif yang didapat setelah
dilakukan proses ekstraksi. Sedangkan hasil penetapan susut pengeringan yang
diperoleh sebesar 8,9226% yang dilakukan untuk mengetahui jumlah senyawa
yang hilang selama proses pengeringan.
D. Hasil Uji Penapisan Fitokimia
Hasil penapisan fitokimia dilakukan untuk mengetahui kandungan kimia
yang terdapat pada ekstrak etanol 70% daun kepel yang meliputi uji flavonoid,
alkaloid, saponin, tanin, steroid, dan triterpenoid. Hasil penapisan fitokimia dapat
dilihat pada tabel.

Hasil penapisan fitokimia ekstrak etanol 70% daun kepel menunjukkan
hasil positif pada pengujian flavonoid, saponin dan tanin. Sedangkan pada
pengujian alkaloid, steroid dan terpenoid menunjukkan hasil yang negatif.
Hasil Evaluasi Sediaan Salep Ekstrak Etanol 70% Daun Kepel
Evaluasi yang dilakukan terhadap sediaan salep meliputi uji terhadap
bentuk, bau, warna, dan homogenitas. Semua kelompok konsentrasi salep 3,25%,
6,5%, dan 13% ekstrak etanol 70% daun kepel terdistribusi homogen. Warna
kuning berasal dari vaselin flavum sebagai basis salep sedangkan perubahan
warna salep yang semakin pekat disebabkan oleh penambahan konsentrasi
ekstrak , dengan bau khas dari ekstrak daun kepel. Sediaan uji dibuat dalam
bentuk salep karena mudah dioleskan dan mempercepat proses penghantaran
senyawa aktif. Pemilihan vaselin flavum sebagai basis salep karena bersifat
hidrokarbon sehingga tidak mudah hilang jika terkena air sehingga dapat
memperpanjang kontak antara bahan obat dan kulit (Sentat 2015).
Jumlah makrofag diperoleh dari preparat histologi dengan pewarnaan
Hematoxyllin-Eosin melalui perhitungan 10 lapang pandang pada daerah luka
menggunakan mikroskop cahaya dengan perbesaran 400x. Sediaan uji yang
diberikan pada tikus adalah ekstrak etanol 70% daun kepel dengan konsentrasi
3,25%, 6,5%, 13%, Burnazin®)
sebagai kontrol positif dan vaselin flavum sebagai
kontrol negatif. Burnazin®)
sebagai kontrol positif karena merupakan gold
standard dalam perawatan luka bakar topikal yang mempunyi zat aktif yaitu
Silver Sulfadiazine (SSD). SSD menghambat replikasi DNA dan merusak dinding
sel bakteri. Kandungan perak dalam SSD juga berfungsi sebagai antibakteri yang
membuat luka bersih dari mikroba sehingga regenerasi jaringan menjadi tidak
terganggu (Atiyeh 2007; Almeida dkk. 2000).
Data perhitungan jumlah makrofag pada tikus terhadap waktu
penyembuhan luka diolah secara statistik. Hasil tabel ANOVA terhadap jumlah
makrofag pada hari ke-3, 7, dan 14 menunjukan adanya perbedaan yang bermakna
pada setiap kelompok (p<0,05). Untuk mengetahui perbedaan bermakna antar
kelompok dilanjutkan dengan uji Tukey.

Pengamatan hari ke 3 menunjukan jumlah sel makrofag pada kelompok
konsentrasi uji 13% sebanding dengan kelompok kontrol positif, dan menunjukan
perbedaan bermakna dengan kelompok kontrol negative (gambar 1). Hal ini
karena makrofag menjadi sel predominan pada hari ke 3 pasca luka.

Makrofag adalah sel yang efektif untuk proses fagositosis, makrofag
memfagositosis organisme patogen, benda asing dan sel-sel yang tidak berguna
lagi. Makrofag yang berada di jaringan berasal dari sel monosit darah yang
bermigrasi ke jaringan ikat. Apabila terjadi peradangan, jumlah monosit yang
bermigrasi ke jaringan ikat mengalami peningkatan sehingga makrofag akan
teraktivasi (Franklin 2007; Dwintanandi dkk. 2016).

Fase inflamasi dimulai segera setelah terjadinya luka dan umumnya
sampai hari ke-5 setelah luka. Tujuan utama fase ini adalah menghilangkan
jaringan mati dan mencegah infeksi oleh agen mikrobial patogen. Neutrofil
berperan penting untuk memfagositosis benda-benda asing seperti bakteri. Bakteri
yang tidak terfagositosis oleh neutrofil akan difagositosis oleh makrofag yang
mempunyai daya fagositosis lebih hebat dibandingkan neutrofil. Makrofag akan
menjadi sel predominan setelah hari ketiga pasca luka. Makrofag akan
memfagositosis bakteri pada daerah luka, akan meningkat jumlahnya pada fase
inflamasi dan akan menurun jumlahnya pada fase proliferasi (Gurtner 2007;
Febram dkk. 2010).

Hasil analisis dengan Uji Tukey pada pengamatan hari ke 7 dan 14
menunjukan jumlah makrofag pada kelompok konsentrasi 13% sebanding dengan
kelompok kontrol positif dan kelompok konsentrasi 6,5%, dan menunjukan
perbedaan bermakna dengan kelompok kontrol negatif dan konsentrasi 3,25%.
Pada pengamatan hari ke 7 jumlah makrofag semua kelompok uji lebih rendah
dibandingkan dengan jumlah makrofag pada hari ke 3, kecuali pada kelompok
kontrol negatif jumlah sel makrofag lebih tinggi dibandingkan hari ke 3. Hal ini
menunjukan bahwa proses inflamasi pada kelompok kontrol negatif masih terus
berjalan pada fase proliferasi. Kelompok konsentrasi uji 13% dan kelompok uji
lainya jumlahnya lebih rendah dari hari ke 3 yang artinya salep ekstrak etanol
70% daun kepel telah bekerja sebagai anti-inflamasi sehingga proses inflamasi
tidak terus berjalan.
Tingginya jumlah makrofag pada kelompok kontrol negatif menunjukan
terjadinya inflamasi yang berkepanjangan, karena pada luka bakar memiliki
pertumbuhan mikroorganisme lebih banyak. Tidak adanya bahan aktif pada
kelompok kontrol negatif sangat memungkinkan masih adanya mikroba dan
kerusakan jaringan yang harus difagositosis oleh sel makrofag pada daerah luka
(Sura dkk. 2013). Maka proses penyembuhan luka pada kelompok kontrol negatif
akan berkepanjangan, sehingga terjadi penundaan fase proliferasi. Pada kelompok
konsentrasi uji 13% dan kelompok konsentrasi uji lainya jumlah sel makrofag
lebih rendah menandakan bahwa fase inflamasi berakhir dan digantikan dengan
fase proliferasi.
Pada fase proliferasi makrofag juga dibutuhkan untuk menghasilkan
growth factor seperti fibroplatic growth factor, transforming growth factor-beta
(TGF-β), DGF yang untuk merangsang migrasi fibroblas ke daerah luka.
Makrofag mensekresi faktor pertumbuhan dan sitokin yang akan berperan dalam
proses proliferasi. Fungsi makrofag pada fase proliferasi adalah mengaktivasi
fibroblas dan meningkatkan migrasi fibroblas yang berperan dalam pembentukan
jaringan dan memproduksi kolagen (Mulia 2012; Hesketh et al. 2017).

Pemberian salep ekstrak etanol 70% daun kepel dapat mempercepat fase
inflamasi pada luka bakar. Hal ini diduga berhubungan dengan senyawa metabolit
sekunder yang terdapat dalam ekstrak daun kepel yang berperan dalam
penyembuhan luka seperti flavonoid, saponin dan tannin yang berfungsi sebagai
antioksidan dan antimikroba yang mempengaruhi penyembuhan luka (Saroja dkk.
2012). Kandungan tannin dan saponin mempunyai kemampuan sebagai pembersih
atau antiseptik. Saponin dapat memicu vascular endothelial growth factor
(VEGF) dan meningkatkan jumlah makrofag bermigrasi ke area luka sehingga
meningkatkan produksi sitokin yang akan mengaktifkan fibroblas di jaringan luka
(Reddy dkk. 2011). Kandungan flavonoid sebagai antioksidan, antimikroba, dan
antiinflamasi pada luka bakar. Jadi dapat dikatakan salep ekstrak daun kepel 70%
dapat membantu mempercepat penyembuhan luka bakar dilihat dari tingginya
jumlah makrofag pada fase inflamasi.
Kepadatan fibroblast mengalami puncak pada hari ke-7, di mana jumlah
fibroblas tertinggi diperoleh kontrol positif, kemudian diikuti oleh konsentrasi
13%, 6,5%, 3,25% dan kontrol negatif (Tabel 7).

Hasil analisis uji Tukey pada pengamatan hari ke-3 menunjukkan rata-rata
kepadatan fibroblas pada semua kelompok zat uji berbeda bermakna dengan
kontrol negatif. Pada gambar 3 dapat dilihat kepadatan fibroblas pada semua
kelompok zat uji masih berjumlah sedikit karena fibroblas belum berperan dalam
fase inflamasi. Fibroblas berperan pada dua fase yaitu fase proliferasi dan fase
maturasi (Prabakti 2005).

Fibroblas berperan pada dua fase yaitu fase proliferasi dan fase maturasi
(Prabakti 2005). Pengamatan hari ke-7 menunjukkan rata-rata kepadatan fibroblas
pada konsentrasi 13% tidak terdapat perbedaan yang bermakna dengan kontrol
positif dan konsentrasi 6,5%. Perbedaan yang bermakna hanya terjadi antara
kontrol negatif dan konsentrasi 3,25%. Meningkatnya jumlah sel fibroblas akan
meningkatkan jumlah serat kolagen yang akan mempercepat proses penyembuhan
luka. Proliferasi fibroblas dipengaruhi oleh beberapa growth factor yang
dihasilkan oleh makrofag seperti Platelet Derived Growth Factor (PDGF),
Fibroblast Growth Factor (bFGF), Transforming Growth Factor (TGF-β) dan sel
radang, Interleukin-1 (IL-1), Tumor Necrosis Factor (TNF) (Robbins 2007).
Pengamatan hari ke-14 menunjukkan rata-rata kepadatan fibroblas pada
konsentrasi 13% tidak terdapat perbedaan yang bermakna dengan kontrol positif

dan konsentrasi 6,5%. Pada hari ke-14 ini menunjukkan bahwa proliferasi dari
fibroblas menentukan hasil akhir penyembuhan luka. Fibroblas akan
memproduksi matriks ekstraselular yang kemudian akan digantikan oleh kolagen.
Fibroblas akan segera menghilang segera setelah matriks kolagen mengisi kavitas
luka dan pembentukan neovaskular akan menurun melalui proses apoptosis
(Gurtner 2007).
Peningkatan jumlah fibroblas diduga karena efek kandungan senyawa aktif
yang berasal dari ekstrak etanol daun kepel. Hasil ekstraksi etanol 70% daun kepel
mengandung beberapa kandungan senyawa aktif yaitu flavonoid, saponin, dan
tanin. Kandungan tersebut dapat membantu proses penyembuhan luka dengan
mekanisme yang berbeda-beda. Kandungan flavonoid berfungsi sebagai
antioksidan, antimikroba, dan juga anti inflamasi pada luka bakar (Harborne 2000
dan Park et al. 2010). Flavonoid dapat membantu penyembuhan luka dengan
meningkatkan pembentukan kolagen dan meningkatkan jumlah fibroblas (Ambiga
2007). Kandungan saponin dan tanin berperan dalam regenerasi jaringan dalam
proses penyembuhan luka (Reddy 2011). Saponin dapat memicu Vascular
Endothelial Growth Factor (VEGF) dan meningkatkan jumlah makrofag
bermigrasi ke area luka sehingga meningkatkan produksi sitokin yang akan
mengaktifkan fibroblas di jaringan luka (Kimura 2006). Kandungan tanin berguna
sebagai astringen atau menghentikan pendarahan, mempercepat penyembuhan
luka, dan regenerasi jaringan baru (Reddy 2011 dan Nafiu 2011). Selain itu,
kandungan tanin dapat mempercepat penyembuhan luka dengan meningkatkan
penutupan luka serta meningkatkan pembuluh darah kapiler juga fibroblas (Sheikh
2011).
Kelompok yang diberikan salep ekstrak etanol daun kepel konsentrasi 13%
menunjukkan hasil yang lebih baik dibandingkan dengan kelompok yang
diberikan konsentrasi uji lain dengan nilai rata-rata ketebalan re-epitelisasi
13,65µm ± 0,77. Pada hari ke-7 dan hari ke-14 dari ketiga kelompok yang
diberikan salep ekstrak etanol daun kepel, hanya salep ekstrak etanol daun kepel
konsentrasi 13% yang sebanding dengan kontrol positif. Fase proliferasi luka
dimulai pada hari ke4 hingga hari ke-14, di mana terjadi proliferasi sel epitel

untuk menutup luka yang dapat dipengaruhi oleh adanya ujung bebas dari lapisan
epidermis yang telah robek dan beberapa faktor pertumbuhan lain, proliferasi sel
epitel terjadi dengan adanya aktivitas mitosis sel-sel epitel yang berada di dekat
tepi luka, kemudian sel-sel epitel yang sudah matang bergerak dari tepi luka
menuju dermis, sehingga sel epitel bermigrasi dan menyatu di bagian tengah luka
(Handayani 2006).
Pembentukkan re-epitelisasi pada luka bakar yang diberikan salep ekstrak
daun kepel ditingkatkan oleh adanya zat aktif yang terkandung didalamnya, antara
lain saponin, flavonoid dan tanin. Berdasarkan penelitian yang telah dilakukan
oleh Sunarni (2007) daun tanaman kepel mengandung senyawa flavonoid yang
digunakan sebagai antioksidan. Dalam hal ini antioksidan dapat membantu dalam
proses penyembuhan luka yang berperan untuk mengatur kadar ROS (Reactive
Oxygen Species) dalam tubuh, maka apabila kadar ROS meningkat akan
menyebabkan stress oksidatif sehingga kadar antioksidan berkurang dan tidak
dapat melindungi jaringan normal selain itu tidak mampu mengontrol kerusakan,
dengan ini diperlukan antioksidan eksogen dari daun kepel. Selain itu flavonoid
juga dapat membantu proses inflamasi yang berlangsung lebih singkat sehingga
proses reepitelisasi pada penyembuhan jaringan akan terjadi lebih cepat (Sunilson
et al. 2011). Kandungan saponin yang dimiliki pada bagian tanaman dapat
membantu merangsang pembentukan sel epitel yang baru dan mendukung proses
reepitelisasi sehingga mempercepat proses penyembuhan luka (Prasetyo dkk
2010). Kelompok pemberian saponin ini dapat meningkatkan kecepatan migrasi
sel-sel keratinosit pada proses re-epitelisasi lebih tinggi dibandingkan dengan
kelompok kontrol positif, sehingga dapat disimpulkan bahwa saponin tidak hanya
meningkatkan proliferasi sel epidermal namun juga mempercepat proses migrasi
keratinosit yang berperan penting dalam proses re-epitelisasi (Kim YS et al.
2011). Kandungan senyawa tanin berguna sebagai astringen, mempercepat
penyembuhan luka dan regenerasi jaringan baru (Reddy 2011).
Berdasarkan persentase penyembuhan luka didapatkan bahwa konsentrasi
13% mulai dari hari ke 2 sudah menunjukkan adanya perbedaan bermakna
terhadap kontrol negatif. Sedangkan untuk konsentrasi 6,5% dimulai pada hari ke

7 dan konsentrasi 3,25% dimulai pada hari ke 10. Pada hari ke 14 persentase
kontriksi luka pada kelompok uji yang diberikan salep ekstrak daun kepel pada
konsentrasi 13% sebesar 92,32% dan kelompok positif sebesar 95,31%. Hal ini
membuktikan bahwa salep ekstrak daun kepel dengan konsentrasi 13% yang
paling cepat menyembuhkan luka bakar dengan persentase yang sebanding
dengan kelompok positif (Burnazin®).
Proses penyembuhan luka mencakup beberapa fase yaitu fase inflamasi,
fase proliferasi dan fase maturasi. Diawali dengan fase inflamasi terjadi segera
setelah luka dan berakhir 3–4 hari di mana terjadi permeabilitas membran sel
sehingga pada fase ini akan terdapat peradangan, kemerahan, nyeri disertai
dengan adanya bula. Sel pertama yang membantu dalam proses penyembuhan
luka adalah neutrofil yaitu sel darah putih yang berfungsi mempertahankan tubuh
terhadap infeksi bakteri. Pada fase inflamasi ini terjadi peristiwa hemostasis
(penghentian perdarahan), dibantu oleh benang-benang fibrin yang saling
bertautan sehingga sel-sel darah merah beserta plasma (platelet) akan terjaring dan
membentuk scab (keropeng). Pencegahan agar luka tidak terkontaminasi
dilakukan oleh makrofag (sel darah putih) yang bekerja membersihkan luka dari
partikel mikroskopik yang tidak diinginkan seperti bakteri dan sel-sel mati.
Tahap awal fase proliferasi dibantu oleh fibroblas yaitu sel yang
menghasilkan kolagen. Pada fase ini kolagen akan bekerja menghubungkan
jaringan-jaringan pada luka bakar untuk membantu mengembalikan kekuatan
jaringan kulit dan mempercepat penyembuhan luka bakar. Proses terlepasnya
keropeng ini bersamaan dengan proses keringnya luka. Hal ini menandakan sudah
terjadinya pertumbuhan sel-sel baru pada kulit sehingga membantu mempercepat
lepasnya keropeng dan merapatnya tepi luka (Aponno et al. 2014). Tahap terakhir
pada proses penyembuhan luka bakar adalah fase maturasi (remodeling), terjadi
pada saat terlepasnya keropeng dan terlihat jaringan kulit yang baru. Pada fase ini
sel yang masih berperan aktif adalah fibroblas dan kolagen yang akan membantu
memberikan elastisitas, kelenturan, dan kelembaban kulit (Sentat 2015).

BAB 5. KESIMPULAN DAN SARAN
5.1 Kesimpulan
Ekstrak etanol 70% daun kepel konsentrasi 13% dalam bentuk sediaan
salep memiliki aktivitas penyembuhan luka bakar yang sebanding dengan
kelompok positif (Silver sulfadiazin).
5.2 Saran
Disarankan melakukan penelitian lanjutan terhadap senyawa aktif
terpurifikasi dari daun kepel yang berkhasiat sebagai penyembuh luka bakar.

BAB VII RENCANA TINDAK LANJUT DAN PROYEKSI HILIRISASI
Hasil Penelitian Penelitian ini dilakukan bertujuan untuk mengetahui
aktivitas Ekstrak etanol 70% daun kepel
(Stelechocarpus burahol [Blume] Hook.f. &
Thomson) terhadap pengobatan dan penyembuhan
luka bakar tikus. Dari 4 parameter histologi yang
diukur didapat data yang mengarah kepada
penyembuhan luka bakar tikus. Jumlah makrofag
dari semua kelompok perlakuan pada hari ke 3 (fase
inflamasi) lebih tinggi dibandingkan kontrol negatif,
dan jumlah makrofag lebih rendah pada fase
proliferasi. Konsentrasi 13% memiliki aktivitas
terhadap kepadatan fibroblas dalam mempercepat
penyembuhan luka bakar dan menunjukkan ketebalan
re-epitelisasi yang secara statistik sebanding dengan
Burnazin® (silver sulfadiazine 1%). Salep kepel juga
memberikan efek terhadap penurunan luas luka
bakar. Dari ketiga konsentrasi uji, salep ekstrak daun
kepel dengan konsentrasi 13% paling cepat dalam
penyembuhan luka bakar dengan persentase
kesembuhan pada hari ke 14 sebesar 92,32%. Jangka
pendek hasil penelitian diharapkan dapat
memberikan informasi kepada masyarakat bahwa
Ekstrak etanol 70% daun kepel (Stelechocarpus
burahol [Blume] Hook.f. & Thomson) dapat
digunakan sebagai obat luka bakar.
Rencana Tindak Lanjut Perlu dilakukan penelitian lanjutan terhadap bahan
baku ekstrak yang lebih terpurifikasi dengan
melakukan berbagai metode pemisahan. Ekstrak
terpurifikasi ini diharapkan dapat memberikan efek
yang lebih optimal karena senyawa-senyawa

juga untuk mengetahui senyawa aktif yang paling
berperan dalam proses penyembuhan luka bakar.
Selanjutnya ekstrak terpurifikasi ini distandarisasi
dengan melakukan sejumlah pengujian parameter
spesifik dan non spesifik seperti pengukuran cemaran
mikroba, cemaran logam berat, penetapan kadar
senyawa zat aktif dll. Sehingga syarat ekstrak untuk
pemenuhan unsur keamanan dan kualitas bahan baku
obat tradisional bisa terpenuhi.

ABSTRACT
Context: One of the benefits of kepel plant is as a wound antiseptic. Research continued on burns in rats with extract formulas in the form of ointments. The process of healing burns by observing the histopathological parameters. Aims: To determine the activity of Stelechocarpus burahol ethanol extract ointment on the number of macrophages, fibroblast density, reepithelialization , and burn wound surface in rats. Methods: The number of macrophages and fibroblast density were measured by observing the cells in 10 fields of view under a 400x microscope magnification. The thickness of the re-epithelialization was measured using the Image Raster 3.0 application. The measurement of the burn area used the Macbiophotonic Image J program. Results: Histological observations showed a increase in the number of macrophages, fibroblasts, re-epithelialization and a decrease burn area in the extract ointment group significantly compare to the negative control, and at a concentration of 13% showed comparable results with silver sulfadiazine. It can be concluded that kepel leaf ointment extract can accelerate the healing of burn wound with the best results at a concentration of 13%.
Conclusions: Kepel (Stelechocarpus burahol ) leaves ethanol extract ointment at a concentration of 13 % accelerates the healing of burns by observed macrophages, fibroblast density, re-epithelialization and burn area. INTRODUCTION
Indonesia is a country with rich biological resources. Indonesia’s forest biodiversity is a national asset which have priceless benefits to human beings. One of those benefits is its use as medicine, i.e. kepel plant. Kepel (Stelechocarpus burahol [Blume] Hook.f. & Th) is a native Indonesian plant which is the symbol of Special Region of Yogyakarta and can be found in the palaces in the region (Haryjanto, 2012).
Burn wound is a form of tissue damage or tissue loss caused by exposure to heat sources i.e. fire, hot water, chemical substances, electricity, and radiation (Moenadjat, 2009). The severity of the wound is determined by two factors, first is the width of the surface area exposed, and second is the depth of the burn, which categorized into first-degree burn, second-degree burn, and third-degree burn (Corwin, 2009). Third-degree burn is a full depth burn involving the epidermis, dermis, and appendix part of the skin. The healing process of burn is

very complex, thus stabilizing the general condition, the healing care, and also preventing and treating the complications is considered costly, especially since the complications could lead to morbidity and mortality. There’s a need to resolve the problems effectively, safely, and reasonably affordable. One of the alternatives is by using traditional medicine.
Based on a research by Sunarni et al., (2007), kepel leaves have antioxidant ability. Another research also stated that kepel leaves’s juice with 60% concentration showed healing process activity in open wound on rats with 59.84% of healing percentage (Pribadi et al, 2014). Based on the kepel leaves’ antioxidant and antibacterial activity, and also including the kepel juice’s role in the healing process of open wound, there’s a possibility that the 70% kepel leaves’ ethanol extract could have the ability to boost the healing process of burn wound. This research aim is to proof the efficacy of kepel leaves in healing burn wounds.
This research is carried out on rats which have been induced with third-degree burn and will be observed by measuring four parameters, the number of macrophages, fibroblast density, reepithelialization speed, and the decrease of burn wound surface area. The measurement will be done by using Image Raster 3.0 application. This parameters measurement will support the data that will prove the efficacy of kepel leaves on burn wound medication. It is hoped that the result of this study could increase the knowledge on the efficacy of native Indonesian plants and useful in the burn wound healing process.
MATERIAL AND METHODS
Materials Materials used in this research are kepel leaves obtained from The Research
Center for Spices and Medicinal Plants (BALITTRO), Vaseline, Silver Sulfadiazine (Burnazin®), Ketamine injection. All chemical used in this study were analytical grade.
Preparation of Extract
The kepel leaves could be determine in “Herbarium Bogoriense”, Botanical
field, Biology Research Center, Indonesian Institute of Sciences (LIPI), Cibinong
with register No. 1592/IPH.1.01/If.07/VI/2017 . Kepel leaves ( 7 kg) was washed
with running water and dried under the sun. The sample was grinded and sieved
with a 40 mesh sieve. Then extracted (1.2 kg) with 8 L ethanol 70% by
maceration. The maceration process was repeated twice for residue at same
duration (48 hours). The macerat was evaporated in vacuum rotary evaporator
and continued with 40ºC waterbath until it attains the form of thick extract. This
extract labelled as KLEE ( Kepel Leaves’ Ethanol Extract)

Preparation of Test Animals
Rats that was used as test animal was acclimatized and given food and
drink daily. There were 30 Sprague Dawley male white rats (Rattus norvegicus)
weighed 150-200 g. The research procedure has been approved by the Health
Research Ethics Commission University of Muhammadiyah Prof.DR.Hamka
with ethical approval letter number : 02/17.10/017
Determination of Extract Characteristics
Organoleptic observations of KLEE include of shape, color, odor, and taste. Determination of Loss on Drying is done by using gravimetric, where 2g of thick extract is weighed in a calibrated and dried it at 105 °C in an oven for 30 minutes until constant weight (Depkes, 2008). Preliminary phytochemical screening was carried out for KLEE by testing several secondary metabolites. This extract were being tested for its alkaloids content using Dragendorff, Mayer and Bouchardat reagents, flavonoid test (Shinoda and ammonia test), tannin test (test with gelatin and FeCl3), saponin test (foam test) and steroid and terpenoid test (Liebermann Burchard test) (Hanani, 2014).
Preparation of KLEE ointment
KLEE ointment with a concentration of 3.25% ; 6.5% ; 13% (w/w) is made by weighing 0.325; 0.65; 1.3 g of KLEE then add vaselin flavum until 10 g and crushed until homogeneous. Generating Third-Degree Burn and Treatment of Test Animals
The rats are anesthetized by using ketamine injection at a dose of 40.08 mg/kgBB intramuscular. A special metal plate with 1.5 cm x 1.5 cm in diameter is heated until it reached 100ᵒC, which then would be paste for 30 seconds on the back part of the rat that was shaved previously. After the wound was generated, the rat is given analgetic medication orally. There are 6 test groups namely KLEE with concentration of 3.25%, 6.5%, and 13%, Burnazin® (positive control), and vaselin flavum (negative control ) is then spread evenly on the wound surface twice daily (morning and afternoon) for each treatment for 14 days (Nazrun, 2005). Dosing and treatment of test animals can be seen in table 1.

Histology Sample Preparations Skin tissue samples are taken from the biopsy of the burn wound and the
subcutaneous fat tissue. The sample is taken on the third, seventh, and 14th day after giving the test sample. Before samples are taken, the testing animal is anesthetized by using ketamine injection. The specimen is then fixated by using Buffer Neutral Formalin 10% solution.
Histopathology Sample Preparations The tissue is fixated by using Buffer Neutral Formalin (BNF) 10% solution
and left at room temperature for 24 hours. The tissue is then cut to pieces and placed in a specimen container made from plastic. Subsequently, it will go through dehydration process done with a graded alcohol concentration, which is 70%, 80%, 90%, for 2 hours each. Later, the clearing process is done by using xylol to eliminate alcohol traces. After that, the molding process is done by using paraffin blocks and stored in the fridge. These paraffin blocks are then sliced thinly around 6-8 µm by using microtom. Afterwards, these pieces are floated on 60°C warm water (waterbath) to stretch the tissue and avoid creasing. These specimens are then lifted and placed on object glass to do the Hematoxyllin and Eosin (HE) staining and later observed under the microscope (Mustaba, 2012). Data Analysis
Obtained data in form of numbers of macrophages, numbers of fibroblast, and collagen density is statistically analyzed by using the one-way ANOVA test with 95% confidence (α = 0,05). Tukey test is then used to observe whether there is significant difference.
RESULTS (AND DISCUSSION)
Kepel leaves’ ethanol extract (KLEE) is obtained through maceration method by using ethanol 70% as the solvent. The maserat is then evaporated by using vacuum rotary evaporator at 50ᵒC until the thick extract is obtained. This 70% kepel leaves’ ethanol extract characteristics are semi solid, has a unique smell, bitter taste, and blackish green color. The phytochemical screening results showed that this extract contain flavonoid, saponin, and tannin. This extract yields 11.25% and the loss on drying is 8.92%. Organoleptic and homogeneity observations of the 70% kepel leaves’ ethanol extract ointment showed homogenous consistency with the color of the ointment darkens as the extract concentration increases.

The parameters observed from the histology samples are the numbers of macrophages, fibroblast density, and reepithelialization thickness by observing 10 field views. The width of the burn wound is measured by processing the image using the Macbiophotonic Image J program.
The burn wound model on rats as test animal is made by inducing third-degree burn which damage the tissue until the dermis. The prepared samples used are 70% kepel leaves’ ethanol extract at the concentration of 3.25%, 6.5%, 13%; Burnazin® as the positive control group and vaseline flavum as the negative control group. The reason Burnazin® was chosen as the positive control group is because it is the gold standard for burn wound topical treatment due to the Silver Sulfadiazine (SSD) as its active agent. SSD inhibits bacterial DNA replication and damages bacterial cell wall. The silver content in SSD also has antibacterial functions that help cleanse the wound thus avoids compromising the tissue regeneration (Atiyeh, 2007; Almeida et al., 2000). The sample preparation in the form of ointment with vaseline flavum base has a hydrocarbon characeristics which is not easily disolved in water, thus prolonged the contact between the medical ingredients and skin (Sentat, 2015).
Burn wound is a form of tissue damage or loss caused by exposure to heat sources, i.e. fire, hot water, chemical substances, electricity, and radiation (Moenadjat, 2009). Generally, the healing process is divided into 3 phases. The early phase or the inflammation phase is started immediately after the injury happened where it eliminates dead tissues and avoid infection. The second phase is the proliferation phase where the balance between the scar tissue formation and tissue regeneration occur. The third phase is the maturation phase that aimed at maximizing the structural strength and integrity of the wound (Gutneer, 2007). The healing process of burn wound has similarities with other wound healing process in general, yet had a different duration for each phase (Tiwari, 2012).
Macrophage cells calculation process is done by taking the image from a light microscope which then observed and counted by using Image Raster 3.0 application. The result of the ANOVA table on the number of macrophages on the third, seventh, and 14th day showed significant difference on every group (p<0.05). On the third day of observation, the number of macrophage cells on the 13% concentration group is higher than the 6.5%, 3.25%, and negative control group, with the lattest has the lowest number ( Table 2). This is due to the macrophages became the predominant cell at the third day after the wound occured. Macrophage is an effective cell for the phagocytosis process where the macrophage phagocytoses pathogens, foreign bodies, and other unnecessary cells. Macrophages in the tissue originates from the monocyte cells in the blood that migrated to the connective tissue. In cases that inflammation happened, the number of monocytes that migrated to the connective tissue will increase, thus the macrophages is activated (Franklin 2007; Dwintanandi et al., 2016).
On the seventh and 14th day of observation, the number of macrophages in the 13% concentration group is equivalent with the positive control group and the 6.5% concentration group, yet showed significant difference with the negative control group and the 3.25% concentration group (Table 2). This result shows that inflammation process in the negative control group is still in progress. The high

number of macrophages in the negative control group indicates a prolonged inflammation due to the growth of more microorganism in the burn wound. The absence of active ingredients in the negative control group might be the reason of the presence of microbes and the number of tissue damage that needs to be phagocytosed by the macrophage in the wound area (Sura et al. 2013). Thus, the wound healing process in the negative control group will be prolonged and lead to the delay of proliferation phase. In the 13% concentration group and also other consentration group, the number of macrophages is lower which indicates the end of inflammation process and mark the beginning of the proliferation process.

During the proliferation phase, macrophage is also needed to produce growth factors such as fibroplatic growth factor, Transforming Growth Factor-Beta (TGF-β). Macrophages also activated fibroblasts and increase their migration which play a role in tissue formation process and produce collagens (Mulia 2012; Hesketh et al., 2017).
Administration of KLEE ointment could speed the inflammation phase on burn wound. It is supposed to be related to the presence of secondary metabolite compounds in the kepel leaves’ extract that helps the healing process such as flavonoids, saponin, dan tannin which functions as antioxidant and antimicrobial agent that affects the wound healing (Saroja, et al. 2012). Tannin and saponin also have the ability as an antiseptic agent. Saponin could trigger the Vascular Endothelial Growth Factor (VEGF) and increase the number of macrophages that migrate towards the wound area thus increase the production of cytokine that activates fibroblast in the wound tissue (Reddy,et al., 2011).
Another wound healing parameter which is fibroblast density could be seen in Table 3. The third day observation showed the mean density of the fibroblast in all of the concentration groups have significant difference with the negative control group. Fig.1 showed that fibroblast density in all of the concentration groups still have a small number due to the fibroblast is yet to have a role in the inflammation process.

Fibroblast start to have a part in both proliferation and maturation phase (Prabakti, 2005). The seventh day of observation showed the mean density in the 13% concentration group does not have significant difference with the positive control and 6.5% concentration groups. Significant difference is only found between the negative control and 3.25% concentration groups. The increase number of fibroblast cell would trigger the increase number of collagen fibers which speed the process of wound healing. The 14th day observation showed the mean density of fibroblast in the 13% concentration group does not show significant difference with the positive control and 6.5% concentration groups. This shows that the proliferation of fibroblast determines the end result of wound healing. Fibroblast produces extracellular matrix which then will be replaced by collagen. Fibroblast will disappear immediately as the collagen matrix fill the wound cavity and the formation of neovascular will decrease through the apoptosis process (Gurtner, 2007).

Reepithelialization thickness as another wound healing parameter does not show any significant results during the third day of observation. On the seventh and 14th day of observation, the 13% concentration group has an equivalent value with the positive control group, where the mean value of the reepithelialization thickness is 13.65 µm ± 0.77 (Table 4). It could be interpreted that wound proliferation started on the fourth day until the 14th day, where the epithelial cell proliferation closed the wound that was affected by the epithelial cells’ mytosis activity in the wound edges, subsequently the mature epithelial cells will move from the wound edges to the dermis, thus epithelial cells migrated and attached together at the center part of the wound (Handayani, 2006).

Figure 2 it can be seen that the positive control group and KLEE ointment concentration of 13% showed thicker epithelial formation compared to all test groups. In the proliferation phase, the thickness of the epithelial layer continues to increase until the wound area closes completely. Epithelium layered in the epidermis which is composed of many layers of cells called keratinocytes. These cells are constantly renewed through the mitosis of cells in the basal layer which are gradually shifted to the epithelial surface (Kalangi, 2013).

The observation of the wound surface area is done by using Macbiophotonic Image J program. Based on the microscopic observation from the first day until the 14th day, it is showed that there’s a decrease in wound surface area. On the first day, the wound appears pale white and is still wet. On the third day, the wound in all test groups appears large and swollen, which indicates that the inflammation process is still in progress (Table 5). The inflammation process role is to prevent the entry of bacteria, eliminate dirts from the wound tissue and prepare the advanced healing process (Gurtner 2007). On the seventh day, the wound appears reddish brown on the positive control group while on the 6.5% and 13% concentration group showed the formation of scab and the wound began to shrink in size. On the 14th day, the wound has dried out and the scabs started to come off.The removal of scab indicates the growth of new cells thus speeds the process and help attaching the wound edges (Aponno et al. 2014).

Wound constriction percentage in the 13% concentration group is 92.32% and in positive control group is 95.31% on the 14th day of observation. This proves that the 13% kepel leaves’ ethanol extract oinment is the fastest in healing burn wound with a percentage that is proportional to the positive control group (Burnazin®).

DISCUSSION
This section should deal with the interpretation, rather than recapitulation of results. It is important to discuss the new and significant observations in the light of previous work. Discuss also the weaknesses or pitfalls in the study. New hypotheses or recommendations can be put forth. Avoid unqualified statements and conclusions not completely supported by the data. Repetition of information given under Introduction and Results should be avoided.

CONCLUSION
Based on this research, it could be concluded that the 70% kepel leaves’ ethanol extract with 13% concentration shows burn wound healing acceleration activity with decreased number of macrophages and wound surface area and also the increase of fibroblast density and reepithelialization thickness.

Abstract. Kepel (Stelechocarpus burahol [Blume] Hook.f & Thomson) is a native flora of Indonesia
useful as an antibacterial, antioxidant, antifungal and antiseptic. Antibacterial and antioxidant activity on
kepel leaf can the healing of wounds and the content of chemical compounds such as flavonoids and tannins,
as the background of this research as a healer for burns wound. This study aimed to determine the activity of
70% ethanol extract of kepel leaf which is formulated in the form of ointment to be more effective and
efficient. Testing is done by modeling burn rat with observations the number of macrophages, and fibroblast
density. The animals used for this study were 30 rats, divided into five groups concentrations 3.25%, 6.5%,
13% kepel leaf extract ointment, negative control (vaseline flavum) and positive control (silver sulfadiazine).
Observations were held on days 3, 7, and 14 histologically. Histological observations showed a decrease in
the number of macrophages and fibroblasts significantly compare to the negative control, and at a
concentration of 13% showed comparable results with silver sulfadiazine. It can be concluded that kepel leaf
ointment extract can accelerate the healing of burn wound with the best results at a concentration of 13%.

INTRODUCTION
Indonesia is a country with rich biological resources. Indonesia’s forest
biodiversity is a national asset which have priceless benefits to human beings.
One of those benefits is its use as medicine, i.e. kepel plant. Kepel
(Stelechocarpus burahol [Blume] Hook.f. & Th) is a native Indonesian plant
which is the symbol of Special Region of Yogyakarta and can be found in the
palaces in the region (Haryjanto, 2012).
Burn wound is a form of tissue damage or tissue loss caused by exposure to
heat sources i.e. fire, hot water, chemical substances, electricity, and radiation
(Moenadjat, 2009). The severity of the wound is determined by two factors, first
is the width of the surface area exposed, and second is the depth of the burn,
which categorized into first-degree burn, second-degree burn, and third-degree
burn (Corwin, 2009). Third-degree burn is a full depth burn involving the

epidermis, dermis, and appendix part of the skin. The healing process of burn is
very complex, thus stabilizing the general condition, the healing care, and also
preventing and treating the complications is considered costly, especially since the
complications could lead to morbidity and mortality. There’s a need to resolve the
problems effectively, safely, and reasonably affordable. One of the alternatives is
by using traditional medicine.
Based on a research by Sunarni et al., (2007), kepel leaves have antioxidant
ability. Another research also stated that kepel leaves’s juice with 60%
concentration showed healing process activity in open wound on rats with 59.84%
of healing percentage (Pribadi et al, 2014). Based on the kepel leaves’ antioxidant
and antibacterial activity, and also including the kepel juice’s role in the healing
process of open wound, there’s a possibility that the 70% kepel leaves’ ethanol
extract could have the ability to boost the healing process of burn wound. This
research aim is to proof the efficacy of kepel leaves in healing burn wounds.
This research is carried out on rats which have been induced with third-
degree burn and will be observed by measuring four parameters, the number of
macrophages and fibroblast density. The measurement will be done by using
Image Raster 3.0 application. This parameters measurement will support the data
that will prove the efficacy of kepel leaves on burn wound medication. It is hoped
that the result of this study could increase the knowledge on the efficacy of native
Indonesian plants and useful in the burn wound healing process.
MATERIALS AND METHODS
Materials
Materials used in this research are kepel leaves obtained from The Research
Center for Spices and Medicinal Plants (BALITTRO), Vaseline, Silver
Sulfadiazine (Burnazin®), Ketamine injection. All chemical used in this study
were analytical grade.
Preparation of Extract
The kepel leaves could be determine in “Herbarium Bogoriense”, Botanical
field, Biology Research Center, Indonesian Institute of Sciences (LIPI), Cibinong
with register No. 1592/IPH.1.01/If.07/VI/2017 . Kepel leaves ( 7 kg) was washed
with running water and dried under the sun. The sample was grinded and sieved
with a 40 mesh sieve. Then extracted (1.2 kg) with 8 L ethanol 70% by
maceration. The maceration process was repeated twice for residue at same
duration (48 hours). The macerat was evaporated in vacuum rotary evaporator and
continued with 40ºC waterbath until it attains the form of thick extract. This
extract labelled as KLEE ( Kepel Leaves’ Ethanol Extract)

Preparation of Test Animals
Rats that was used as test animal was acclimatized and given food and drink
daily. There were 30 Sprague Dawley male white rats (Rattus norvegicus)
weighed 150-200 g. The research procedure has been approved by the Health
Research Ethics Commission University of Muhammadiyah Prof.DR.Hamka
with ethical approval letter number : 02/17.10/017
Determination of Extract Characteristics
Organoleptic observations of KLEE include of shape, color, odor, and taste.
Determination of Loss on Drying is done by using gravimetric, where 2g of thick
extract is weighed in a calibrated and dried it at 105 °C in an oven for 30 minutes
until constant weight (Depkes, 2008). Preliminary phytochemical screening was
carried out for KLEE by testing several secondary metabolites. This extract were
being tested for its alkaloids content using Dragendorff, Mayer and Bouchardat
reagents, flavonoid test (Shinoda and ammonia test), tannin test (test with gelatin
and FeCl3), saponin test (foam test) and steroid and terpenoid test (Liebermann
Burchard test) (Hanani, 2014).
Preparation of KLEE ointment
KLEE ointment with a concentration of 3.25% ; 6.5% ; 13% (w/w) is made
by weighing 0.325; 0.65; 1.3 g of KLEE then add vaselin flavum until 10 g and
crushed until homogeneous.
Generating Third-Degree Burn and Treatment of Test Animals
The rats are anesthetized by using ketamine injection at a dose of 40.08
mg/kgBB intramuscular. A special metal plate with 1.5 cm x 1.5 cm in diameter is
heated until it reached 100ᵒC, which then would be paste for 30 seconds on the
back part of the rat that was shaved previously. After the wound was generated,
the rat is given analgetic medication orally. There are 6 test groups namely KLEE
with concentration of 3.25%, 6.5%, and 13%, Burnazin®
(positive control), and
vaselin flavum (negative control ) is then spread evenly on the wound surface
twice daily (morning and afternoon) for each treatment for 14 days (Nazrun,
2005). Dosing and treatment of test animals can be seen in table 1.

Histology Sample Preparations
Skin tissue samples are taken from the biopsy of the burn wound and the
subcutaneous fat tissue. The sample is taken on the third, seventh, and 14th
day
after giving the test sample. Before samples are taken, the testing animal is
anesthetized by using ketamine injection. The specimen is then fixated by using
Buffer Neutral Formalin 10% solution.
Histopathology Sample Preparations
The tissue is fixated by using Buffer Neutral Formalin (BNF) 10% solution
and left at room temperature for 24 hours. The tissue is then cut to pieces and
placed in a specimen container made from plastic. Subsequently, it will go
through dehydration process done with a graded alcohol concentration, which is
70%, 80%, 90%, for 2 hours each. Later, the clearing process is done by using
xylol to eliminate alcohol traces. After that, the molding process is done by using
paraffin blocks and stored in the fridge. These paraffin blocks are then sliced
thinly around 6-8 µm by using microtom. Afterwards, these pieces are floated on
60°C warm water (waterbath) to stretch the tissue and avoid creasing. These
specimens are then lifted and placed on object glass to do the Hematoxyllin and
Eosin (HE) staining and later observed under the microscope (Mustaba, 2012).
Data Analysis
Obtained data in form of numbers of macrophages, numbers of fibroblast,
and collagen density is statistically analyzed by using the one-way ANOVA test

with 95% confidence (α = 0,05). Tukey test is then used to observe whether there
is significant difference.
RESULTS AND DISCUSSIONS
The burn wound model on rats as test animal is made by inducing third-
degree burn which damage the tissue until the dermis. The prepared samples used
are 70% kepel leaves’ ethanol extract at the concentration of 3.25%, 6.5%, 13%;
Burnazin® as the positive control group and vaseline flavum as the negative
control group. The reason Burnazin®
was chosen as the positive control group is
because it is the gold standard for burn wound topical treatment due to the Silver
Sulfadiazine (SSD) as its active agent. SSD inhibits bacterial DNA replication and
damages bacterial cell wall. The silver content in SSD also has antibacterial
functions that help cleanse the wound thus avoids compromising the tissue
regeneration (Atiyeh, 2007; Almeida et al., 2000). The sample preparation in the
form of ointment with vaseline flavum base has a hydrocarbon characeristics
which is not easily disolved in water, thus prolonged the contact between the
medical ingredients and skin (Sentat, 2015).
Burn wound is a form of tissue damage or loss caused by exposure to heat
sources, i.e. fire, hot water, chemical substances, electricity, and radiation
(Moenadjat, 2009). Generally, the healing process is divided into 3 phases. The
early phase or the inflammation phase is started immediately after the injury
happened where it eliminates dead tissues and avoid infection. The second phase
is the proliferation phase where the balance between the scar tissue formation and
tissue regeneration occur. The third phase is the maturation phase that aimed at
maximizing the structural strength and integrity of the wound (Gutneer, 2007).
The healing process of burn wound has similarities with other wound healing
process in general, yet had a different duration for each phase (Tiwari, 2012).
Macrophage cells calculation process is done by taking the image from a
light microscope which then observed and counted by using Image Raster 3.0
application. The result of the ANOVA table on the number of macrophages on the
third, seventh, and 14th
day showed significant difference on every group
(p<0.05). On the third day of observation, the number of macrophage cells on the
13% concentration group is higher than the 6.5%, 3.25%, and negative control
group, with the lattest has the lowest number ( Table 2). This is due to the
macrophages became the predominant cell at the third day after the wound
occured. Macrophage is an effective cell for the phagocytosis process where the
macrophage phagocytoses pathogens, foreign bodies, and other unnecessary cells.
Macrophages in the tissue originates from the monocyte cells in the blood that
migrated to the connective tissue. In cases that inflammation happened, the
number of monocytes that migrated to the connective tissue will increase, thus the
macrophages is activated (Franklin 2007; Dwintanandi et al., 2016).

On the seventh and 14th
day of observation, the number of macrophages in
the 13% concentration group is equivalent with the positive control group and the
6.5% concentration group, yet showed significant difference with the negative
control group and the 3.25% concentration group (Table 2). This result shows that
inflammation process in the negative control group is still in progress. The high
number of macrophages in the negative control group indicates a prolonged
inflammation due to the growth of more microorganism in the burn wound. The
absence of active ingredients in the negative control group might be the reason of
the presence of microbes and the number of tissue damage that needs to be
phagocytosed by the macrophage in the wound area (Sura et al. 2013). Thus, the
wound healing process in the negative control group will be prolonged and lead to
the delay of proliferation phase. In the 13% concentration group and also other
consentration group, the number of macrophages is lower which indicates the end
of inflammation process and mark the beginning of the proliferation process.

During the proliferation phase, macrophage is also needed to produce
growth factors such as fibroplatic growth factor, transforming growth factor-beta
(TGF-β), and DGF in which they stimulate the migration of fibroblast towards the
wound area. Macrophages also activated fibroblasts and increase their migration
which play a role in tissue formation process and produce collagens (Mulia 2012;
Hesketh et al., 2017).
Administration of KLEE ointment could speed the inflammation phase on
burn wound. It is supposed to be related to the presence of secondary metabolite
compounds in the kepel leaves’ extract that helps the healing process such as
flavonoids, saponin, dan tannin which functions as antioxidant and antimicrobial

agent that affects the wound healing (Saroja, et al. 2012). Tannin and saponin also
have the ability as an antiseptic agent. Saponin could trigger the vascular
endothelial growth factor (VEGF) and increase the number of macrophages that
migrate towards the wound area thus increase the production of cytokine that
activates fibroblast in the wound tissue (Reddy,et al., 2011).
Another wound healing parameter which is fibroblast density could be seen
in Table 3. The third day observation showed the mean density of the fibroblast in
all of the concentration groups have significant difference with the negative
control group. Fig.1 showed that fibroblast density in all of the concentration
groups still have a small number due to the fibroblast is yet to have a role in the
inflammation process.

Fibroblast start to have a part in both proliferation and maturation phase (Prabakti,
2005). The seventh day of observation showed the mean density in the 13%
concentration group does not have significant difference with the positive control
and 6.5% concentration groups. Significant difference is only found between the
negative control and 3.25% concentration groups. The increase number of
fibroblast cell would trigger the increase number of collagen fibers which speed
the process of wound healing. The 14th
day observation showed the mean density
of fibroblast in the 13% concentration group does not show significant difference
with the positive control and 6.5% concentration groups. This shows that the
proliferation of fibroblast determines the end result of wound healing. Fibroblast
produces extracellular matrix which then will be replaced by collagen. Fibroblast
will disappear immediately as the collagen matrix fill the wound cavity and the
formation of neovascular will decrease through the apoptosis process (Gurtner,
2007).

CONCLUSIONS
Based on this research, it could be concluded that the 70% kepel leaves’
ethanol extract with 13% concentration shows burn wound healing acceleration
activity with decreased number of macrophages and the increase of fibroblast
density.
