Date of Notification: November 17, 2014 Revision Date: N/A Plans Affected: Mercy Maricopa Integrated Care This notice is to clarify the registration process for CPR/First Aid and C.P.I. classes as there have been some questions about the correct procedures. Each agency/employee should email Janet Bond directly at [email protected] to register for any class on the schedule. This is the only step needed to complete the registration process. She will send you an email response, confirming that she received your communication. You may register employees one at a time, or in the form of a participant list. There is no cap to the number of participants for this training. However, if a registered employee is unable to attend a training they are signed up for, an email should be sent to Janet cancelling that employee's registration. Employees may be registered at any time prior to the class; even same day registrations are accepted. Once the employee has completed the training, each agency's designated person will go into the Relias system to confirm that the employee(s) has completed the training. Each class roster and copies of the completion cards with each invoice will be emailed back so that you do not have to track down your employees to get their cards copied. ALL training sessions are conducted at Quality Care Network, 5326 E. Washington St., Bldg. B, Phoenix, AZ 85034. Janet is in the process of securing a date for the next C.P.I. training, and will let you know just as soon as the date is confirmed. PLEASE NOTE: There is a cap of 20 participants for this class, as well as purchase their workbooks in advance, so each agency will be invoiced for the number of employees registered. If you have an employee who cannot make the training for some reason, you may make a last minute substitution without being required to register in advance. Please make sure that those registered will be attending. Should a class fill up, another date will immediately be secured for an additional training session. CPR/First Aid classes are conducted on every second and fourth Friday of each month. Following is the schedule for CPR/First Aid through the end of this year: 11/28 (day after Thanksgiving, so MIGHT be cancelled if there are no registrations) 12/12 12/26 (again, MIGHT be cancelled if no registrations; day after Christmas) CPR/First Aid class is always scheduled from 11:30-5:00, although it usually finishes early. Participants should enter through the main entrance, sign in and get a visitor badge.
