Ltd., Ulsan, South Korea and delivered on 27 February2008. .The vessel had been enrolled with ABS Marine Casualty Response Inc. for prompt access tocomputerized shore based damage stability and residual structural strength calculation programs. .Theattitude of the master and other interviewed ships personnel was confident and supportive towards theinspection.

Other Inspector Comments:Continuous Synopsis Record was document no.2.

DOC had been issued under the Authority of the Government of the Hong Kong SpecialAdministrative Region of The Peoples Republic of China by Det Norske Veritas on 23September 2010 and was valid until 13 September 2015.

SMC had been issued under the Authority of the Government of the Hong Kong SpecialAdministrative Region of The Peoples Republic of China by Det Norske Veritas on 18September 2008 and was valid until 30 July 2013. The intermediate verification audithad been carried out on 17 March 2011.

Safety Equipment Certificate was for 25 persons and supplemented by Form E.

Safety Radio Certificate was supplemented by Form R.GMDSS area A1 + A2 + A3 basis duplication of equipment and shore basedmaintenance.

IOPP type crude oil/product carrier. Form B attached.

The International Ship Security Certificate had been issued under the Authority of theGovernment of the Hong Kong Special Administrative Region of The Peoples Republicof China by Det Norske Veritas on 18 September 2008 and was valid until 30 July2013. Other Inspector Comments:The vessel carried a world wide set of charts andpublications. A contract with DPM (Singapore) Ltd was in place for automatic neweditions and publications. For the chart corrections the vessel had been enrolled withDigiTrace. The master normally checked 5 - 10 voyage charts randomly.

Initial Operator Comments: Root Cause: The chart correction block was not properlypasted on the chart.

All charts and publications were corrected upto the latest notice to mariners except themissing block on BA 323 as verified by the inspector. Upon investigation it was foundthat although BA 323 was properly corrected and the correction number was written atthe bottom left corner of the chart but the block correction was inadequately pasted onthe chart which caused the block correction to come loose. The block correction waslater found lying loose in the chart drawer. The Master has procured the latest editionof BA 323 at Rotterdam and same was supplied to the vessel prior sailing.The Master has already completed a briefing of the navigating officers in chartcorrection with special emphasis to application of chart block corrections.

Other Inspector Comments:As per operators guidelines all charts were corrected up tolast received NtM's. Latest received and applied weekly notices to mariners were week26/2011.

Inspector Observations: On randomly checking last voyage charts it was found thatchart BA-323 had the block correction as part of notice to mariners 1011/2009 not beenapplied. Master immediately ordered new replacement chart for delivery prior to sailing.Confirmation of delivery was received from the Agents at time of the inspection.

If the vessel is provided solely with paper charts are all charts required for the tradingareas of the vessel on board and are these fully corrected?

Other Inspector Comments:Voyage chart for the passage from Gibraltar to Rotterdamhad been reviewed. The checked voyage charts had been corrected up to last receivedNtM's. The charts were of adequate scale and appropriate for the passage, port and itsapproaches.

The passage planning wascomprehensive and adequately detailed with additional information (no go areas -contingency anchorages - etc) clearly marked on the charts. The passage plan hadbeen verified by the Master and countersigned by all navigation officers.
