In this issue of Health, we are also featuring the enthusiastic young team of Candy Stripers who volunteered here over the summer. The feedback from these students has been terrific and this is just one of the ways in which MedStar Southern Maryland Hospital Center is helping the youth in our community explore healthcare possibilities. In addition to the Candy Striper program, we host Mini Medical School and Stroke Education programs for young people attending nearby schools.

As always in Health, we are proud to present inspiring personal stories that show the special bonds that develop between our physicians and their patients, as you will see in our feature on Dr. Sabloff, with his patient Patrick Forbes. In recognition of Breast Cancer Awareness Month in October, we also present the heartwarming success story of Dr. Tsiapali and her breast surgery patient Valerie Calhoun.

Stroke Education

Dr. Henry J. Wise, Jr. High School students enrolled in science and health courses were recently treated to a Mini Medical School, hosted by ED physicians Michael Antonis, Katy Voss and Cory Wittrock.The day-long session gave students one-on-one time with physicians in small groups, to learn how to operate ultrasound equipment, how the physiology of the body works and how their education during high school related to the day’s lesson. The program’s success may lead to additional Mini Medical School programs. See page 14.

MedStar Southern Maryland Hospital Center’s Community Outreach and Stroke teams presented a fun, free, interactive program at a local elementary school to teach students important information that could possibly help them save a life one day. The fun, interactive program used reading materials, coloring books, a cartoon-style movie and hands-on presentations to inspire students, including teaching first graders how to check their own pulse. See page 14.

Some examples of PromptCare-treated issues are sports injuries, sprains, cuts and scrapes, and most facilities provide basic lab and X-ray service.

“We can treat simple colds and sore throats, as well as simple infections, like ear infections,” explains Nosheen Azam, MD, who is stationed at MedStar PromptCare Waldorf. “When a patient can’t get into their doctor or doesn’t want to wait at an ED, we can treat them for issues like strep, flu and urinary tract infections. We can also provide urine pregnancy tests and basic blood work to determine whether a patient is low on electrolytes or is anemic. PromptCare can also treat patients with IV fluids for mild to moderate dehydration.”

When to rush to an Emergency Department

The way to receive the proper care when you’re ill is to go to the correct doctor who can best address your health care needs. Developing a long-term relationship with your primary care doctor is very important, but when you are unable to see them, we present a guide that shows when to use our MedStar PromptCare facilities or our Emergency Department (ED).

When to go to your primary care doctor Unless you are experiencing a traumatic injury, this should be your first choice. It is important to have a primary doctor and to schedule regular visits, so that physician gets to know you well and can proactively deal with any health problems before those problems advance. Not only does regularly visiting your primary care doctor keep you healthier, which proves to be more cost-effective

over the long-run, your primary care doctor can be a great resource to help you when you are having trouble.

“As primary care doctors, we like to monitor your health, and to guide and steer you in the right direction,” says internist Tara Saggar, MD. “We always want to know first what is going on, and we can help guide patients and get you the level of care you need. We’re the goal keepers who know when we can help you, or where we should send you if, for example, you need imaging or certain tests better suited to a PromptCare facility or to the ED.”

When to visit a PromptCare facility MedStar PromptCare is ideal for a range of illnesses that arise outside of your primary care doctor’s office hours, but are not serious enough for an ED visit.

For trauma involving your face or head, for open fractures, big lacerations, injuries you believe may require a CAT scan and for any life-threatening emergencies, head straight to the ED. There, physicians like Michael S. Antonis, DO, can work on the most complex emergency cases, order more sophisticated lab procedures and tests than PromptCare facilities and treat serious injuries 24-hours a day.

“We really want you to be proactive about your health because a primary care physician establishes a relationship with you and gets to know you, your health and your medications,” says Antonis, echoing the message of Dr. Saggar. “They can always help coordinate care for you, especially when they’re in the MedStar Network. If you do need to come to the ED, it’s easy for us to call and talk to them and consult with them

when they know your history.” As a safety measure, Antonis also recommends everyone type in a list of all medications and dosages into their cell phones, or to take a photo of the labels on your medications and keep that on your phone in case of an emergency.

Antonis estimates around 250 cases per month that come into the MedStar Southern Maryland Hospital Center ED should have gone to either the patient’s primary care physician or a PromptCare facility. Antonis estimates within the next few years, MedStar will be on a completely integrated system which will give the ED, MedStar PromptCare and MedStar physicians instant access to the complete medical records of patients, which enables seamless and accurate ordering, documentation and billing.

Patrick Forbes is a character. He’s quick with a compliment, always delivered with a grin and a

gleam in his eyes. In fact, he’s well known in his neighborhood for being someone who will talk to anyone passing by, always able to

make them smile. Part of this bright outlook on life comes from the strength of his character and part of his positivity can be traced to his treatment by MedStar Orthopaedic Institute's Dr. Jeffrey D. Sabloff.

On his own since his early teens, Patrick hasn’t always had the easiest life. He has lost family members, but prefers to talk about his grandchildren, lamenting that they used to ask him for fifty cents when they were younger, and they now want hundreds of dollars to

buy the latest technology. He worked long hours as a mechanic and still, after retiring, studies how to take things apart and put them back together. He has had numerous health problems, including suffering from diabetes. And then, tragedy struck, when Patrick lost his left foot in a painful accident.

Soon afterwards, Patrick began experiencing arthritis and a condition called diabetic cold limb in his right knee, turning to Dr. Sabloff for a knee replacement. After the success of that procedure, Patrick experienced knee pain and stiffening in his left knee, above his prosthetic foot. Obviously, knee replacements with below-knee amputations are extremely rare. Patrick’s case also presented other challenges, said Sabloff. Because Patrick was in a wheelchair, his knees were mostly frozen at a 90-degree angle, which required splints and a great deal of effort to straighten them out over time.

“This was an unusual case, but he was in a situation where, if we didn’t do something, he’d never get up and walk,” said Sabloff, who has been working in orthopaedics here since this hospital opened in 1977. “He worked very hard to get back up and walking.”

After his second knee replacement surgery, Patrick used weights to strengthen his legs, complying with orders from Dr. Sabloff.

“Exercise and I never got along, but Dr. Sabloff pounded it in my head,” laughed Patrick. “I’m still working on strengthening both knees, and every time

Dr. Sabloff sees me, he shakes my hand and he sees me walking.”

Patrick said he appreciates the time and attention Dr. Sabloff has spent on him and says has met many

doctors throughout his life, but Dr. Sabloff will always be among his favorites. “He’s a very good doctor,” says Patrick. “I never thought I’d walk again, but Dr. Sabloff said, ‘we’ll get you out of this chair.’”

There’s a lesson here from the lives of both men.

Knowing the great results Dr. Sabloff has achieved, patients can see seemingly impossible results can sometimes be possible. During his long career, Dr. Sabloff has seen great technological advancements, including state-of-the-art implants and advances in rehabilitation. He is proud of the successes he has seen in orthopaedic medicine, including treatment of knee and hip conditions, joint replacement, shoulder problems, fractures, hand and elbow conditions and foot and ankle conditions, as well as in sports medicine. He is also optimistic about the future of the practice.

From Patrick Forbes, we can all learn how important it is to pair medical treatment with a positive attitude. “I don’t let anything bother me,” says Patrick. “Life is good. I don’t worry about

anything. I’ve got a beautiful life.”

Valerie Calhoun feels like she’s back. After an exhausting battle with breast cancer,

she has thrown her wigs to the back of her closet, she is active in her church again and she is scheduling ice cream dates with her granddaughter. None of this would be possible, she says, without the breast cancer treatment she received at MedStar Southern Maryland Hospital Center and the special attention she received from breast surgeon Dr. Ekaterini V. Tsiapali.

It was during a yearly mammogram last July when Valerie found out she had a tumor. After her mom’s three-time battle with breast cancer decades ago, she was scared. Her primary care physician put Valerie in touch with Dr. Tsiapali, who biopsied the tumor. The initial diagnosis was grim: it was a malignant, aggressive form of cancer, in Stage 3. A lumpectomy followed in September, when Dr. Tsiapali removed the tumor and four lymph nodes.

This understandably difficult time was made a little easier, says Valerie, because Dr. Tsiapali made sure she knew what was happening every step of the way.

“Her bedside manner is the greatest,” says Valerie, of Dr. Tsiapali. “She takes time and explains everything. She gave me so much information and she’s very truthful; this is what it is, these are your options.”

Chemo began last November. Valerie experienced hair loss, along with a loss of energy and appetite. Her husband Thomas stopped working to be there for her, and her church, Woodstream Church in Mitchellville, fervently prayed for her health. Her sister took her wig shopping, her

mother, the original cancer survivor of the family, prayed with her, and her children and grandchildren rallied around her. When chemo treatments finished in February, which Valerie recalls as the toughest part of this battle, she began six weeks of radiation treatments.

Now, she is cancer-free, and sees Dr. Tsiapali every six months. The staff greets her warmly when she returns, happy to see her so healthy, and she is equally happy to see them, remembering every kindness, like her appointment around Mother’s Day, when they gave her a flower.

“I’m telling you, this journey has been awesome, because of the entire staff,” said Valerie. “I’m just grateful. I asked God to direct me to a doctor and a team that could take care of me and He did. It’s such a blessing.”

Valerie’s story has a happy ending, like the majority of current breast cancer patients. The diagnosis is no longer as scary as it was years ago, says Dr. Tsiapali, when treatments were, at times, overly aggressive when they didn’t need to be, and techniques had the potential to cause more problems.

“Today, we schedule needle biopsies, like we did with Valerie, instead of surgical procedures,” said Dr. Tsiapali. “The needle biopsy is less invasive, so recovery is easier. We also have made huge strides with techniques to save breasts and reshape them when tumors are removed.”

Dr. Tsiapali is also able to refer patients to MedStar Southern Maryland Hospital Center’s genetic counselors, so breast cancer patients can undergo testing to see if they are carriers of cancer genes and determine

whether patients who have breast tumors would benefit from chemotherapy, as Valerie did.

The trend, says Dr. Tsiapali, is to individualize treatment to each patient’s unique needs. By taking this tailored approach, she has seen patients over the course of her career recover more quickly than patients did as recently as a decade ago. Her reward for providing treatment, she says, is seeing patients like Valerie return to a happy life after a long, difficult journey through breast cancer treatments.

“Getting to know Valerie has been a pleasure,” says Dr. Tsiapali. “She is easy-going and resilient and it has been wonderful to see her get back to being herself.”

Candy Stripers: The Next Generation of MedStar Associates A class of nineteen high school students joined the Candy Striper program at MedStar Southern Maryland Hospital Center over the summer. This program provides the opportunity for young people interested in medical careers to experience volunteer positions learning various parts of the health care field. Candy stripers typically volunteer two to three days per week for eight weeks, and are given work assignments in numerous departments at the hospital.

“Some want to come to an environment to help others and most want experience in the medical field,” says MedStar Southern Maryland Hospital Center’s Volunteer Coordinator Joanne Johnson. “They’re all enthusiastic about their desire to reach their goals and we help them get as close to that as we can.”

To become a candy striper, the students were required to write an essay about why they wanted the volunteer job, and also to submit grades and two letters of recommendation. MedStar Southern Maryland Hospital Center Candy Stripers also have the opportunity to apply for a $1,000 college scholarship.

The Candy Striper program originated in the 1940’s. Duties have grown from a largely administrative function to a more complex, well-rounded experience at MedStar Southern Maryland Hospital Center. Our Candy Striper program combines service with education. Although volunteers have no clinical duties, they participate in a variety of supportive roles that teach them how a hospital works. They also attend “Lunch and Learn” presentations on healthcare topics or other educational activities, given by physicians, nurses and therapists. This summer, Candy Stripers also experienced MedStar Health’s Mobile SiTEL Lab, where they learned to treat a robotic patient.

MedStar Southern Maryland Hosptial Center is proud to announce the new addition of Gastroenterology (GI) specialist, Dr. Walid Chalhoub. Because of the broad range of

symptoms gastroenterologists treat, the GI specialty is in-demand, since many people experience gastrointestinal conditions, from the most common, such as constipation, to those that are incredibly rare, such as Hereditary nonpolyposis colorectal cancer.

Gastroenterology deals with diseases of the gut, liver, pancreas, stomas, esophagus, colon, gall bladder, kidneys and small intestines. It also deals with conditions such as cancer of the esophagus, cancers of the colon or rectum, Celiac disease, Crohn's disease, cirrhosis of the liver, diverticulitis, gastroesophageal reflux disease (GERD), Hepatitis C, liver cancer, Irritable Bowel Syndrome (IBS), pancreatic cancer, pancreatitis and ulcerative colitis.

As a GI surgeon, Dr. Chalhoub is an expert in minimally invasive surgery. Minimally invasive surgery means using the smallest specialized tools and making the smallest incisions possible during surgery. The benefits of minimally invasive surgery mean less scarring, a decreased risk of infection, as incisions are smaller, and less pain after procedures, as less tissue is affected during surgery. These

procedures also result in shorter hospital stays and quicker recovery times.

Dr. Chalhoub attended college and medical school in Beirut, Lebanon. From there, he came to the United States for his residency in Internal Medicine at Mount Sinai in New York City. He became Chief Resident there before accepting an Advanced Fellowship in Gastroenterology at MedStar Georgetown University Hospital Center before bringing his gastroenterology training to MedStar Southern Hospital Center.

He is board certified in Internal Medicine and Board Eligible in Gastroenterology and is a Member of the American College of Gastroenterology, the American Society of Gastrointestinal Endoscopy and the American Medical Association. Dr. Chalhoub is the author of numerous research studies and is fluent in English, French and Arabic, and is proficient in medical Spanish.

Recently, MedStar Southern Maryland Hospital Center’s Community Outreach and Stroke departments

presented a lesson in stroke education to a classroom of first graders at an area elementary school in Upper Marlboro, MD. This was a free service for our community that educated young students on identifying signs of a stroke and actions they can take to help.

The presentation was fun and geared toward the young students as it explored topics such as stroke and its symptoms, which include face drooping, loss of balance or coordination, arm weakness and

speech difficulty.  It then taught kids the actions to take if someone is experiencing a stroke.  The class also taught students about diabetes and the importance of watching sugar and sodium intake. The students were given the opportunity to learn how to check their own pulse and watched a cartoon-style, short movie about a young child helping his grandparent who was having a stroke. They were given a handful of reading materials including a health

Mini Medical School Inspires Future Generations of Medical Professionals

Earlier this summer, 10th, 11th and 12th graders enrolled in advanced science or allied health courses at nearby Dr.

Henry J. Wise, Jr. High School were offered the unique opportunity to attend a Mini Medical School given by MedStar Southern Maryland Hospital Center’s Director of the Emergency Department, Dr. Michael Antonis, DO, along with Dr. Katy Voss and Dr. Cory Wittrock.

Between 80 and 100 students participated in this unique program. Students were split into groups of six, who took turns operating a $60,000 ultrasound machine, provided by Sonosite. They were able to examine their hearts, lungs, liver, gall bladders, intestines and veins, and ask questions of the emergency physicians. Antonis

said the reaction of the students was amazement at both the technology and equipment, and the experience of instantly seeing clear pictures of internal organs and the veins in their arms, while he was impressed with their questions.

“They were all very professional and asked very in-depth questions,” said Dr. Antonis. “They were curious about how we became doctors and how long the process takes. As the first in my family to go to college and then to graduate medical school, I urged them to find mentors in their communities and the school system as they look forward to becoming professionals.”

Dr. Antonis says he is pleased to see plans are in the works to continue the Mini Medical School program each year.  He believes it’s an important component to the education of students, and it’s important for hospital associates to go out in the community and interact.

Whether your pain is caused by injury, arthritis, age, degeneration or fracture, Dr. Hampton, a member of MedStar Southern Maryland Hospital Center’s talented team of orthopaedic physicians, is an expert at diagnosing the source of your joint pain, providing education on causes and presenting treatment options.

At this seminar, Dr. Hampton will discuss causes of hip and knee pain, including arthritis (wearing out of the joint), tendonitis and bursitis (inflammation) and fractures (often found in older patients, as bone density decreases with age). He will also talk about available treatment options, including exercise, pain relief medication, minor surgery and total joint replacement. You’ll learn about the medical issues that can lead to total joint replacement, as well as the steps involved in surgery and recovery, including pre-operative planning, time in the hospital for surgery, post-operative rehabilitation, and what to expect from your new joint. He will also discuss minimally-invasive procedures and after-treatment recovery plans. The seminar will conclude with a question-and-answer session. The seminar will be held on:

Diabetes Awareness Day

People with diabetes face serious risks to their heart, eyes, and kidneys, as well as their overall health—even when they have no symptoms. Prevention, early detection and management are the keys to controlling diabetes.

In recognition of American Diabetes month, the Diabetes Self-Management Program at MedStar Southern Maryland Hospital Center is inviting you to join us for Diabetes Awareness Day. Every day when Dr. Sara Parker arrives at work, she has to re-introduce herself to her colleagues. This is

not because they forget who she is overnight, but because sometimes she is working as an intensivist and sometimes she is working in her capacity as an OB/GYN.

Dr. Parker says when she is in the ICU, where she spends approximately 30% of her time, she enjoys the challenge of taking care of critical patients and making sure patients and their families understand the complexities of the illness.
