Self Care
We all have small accidents at home or suffer from
minor ailments such as coughs and colds. It’s a
good idea to keep your medicine cabinet stocked
up with over the counter remedies and a first aid kit.
Don’t forget to keep your medicines out of the
reach of children. Your medicine cabinet should
contain pain relief (Paracetamol and Ibuprofen),
anti-histamines, oral rehydration salts, anti-diarrhoea
tablets and sunscreen. Always follow the directions
on medicine packets and information leaflets, and
never exceed the stated dose.
Your first aid kit should contain bandages, plasters,
a thermometer, antiseptic, eyewash solution, sterile
dressings, medical tape and tweezers.

often your local pharmacist can provide advice

travel health advice, travel vaccines,
immunisations, family planning advice and
specialist care for patients with long term conditions
such as asthma and diabetes, as well as smears

dietary & lifestyle advice and basic nursing
procedures such as taking blood pressure.
- 11 -
Closure for training
Should you need urgent medical attention when
we are closed for staff training, please telephone
the normal surgery number. This will give you the
option to be connected to Message Link who will
assist with any urgent matters that cannot wait until
the surgery re-opens.
Appointments
You can make an appointment by telephoning the
surgery on 01242 539080, by coming into the surgery
in person, or by using the online booking service.
You may have an appointment with the doctor or
the nurse of your choice, provided that they are
available.
Short urgent appointments are always available for
the same day but we cannot guarantee that you
will always be able to see your own doctor. If you
require an urgent appointment on the same day,
please telephone the surgery first, rather than
coming down in person, since all our urgent
requests are dealt with initially by a telephone
consultation.
If you request an urgent appointment, visit or
telephone call, you will be phoned back by the
triage doctor. He or she will discuss with you your
health concerns, and will assess how best to
manage your health needs: this may be further
- 12 -
advice, a prescription, an appointment at the
surgery or a home visit.
Online Booking
Using our EMIS Access system you can now book
and cancel appointments online. To use this facility
you must register, using a secure ID that the surgery
will issue on request via a registration letter. Please
request this from reception and they will provide
you with the information that you will need.
Alternatively, please e-mail the surgery via our
'contact us' page to request that the registration
information is sent to you at home. Please note that
online booking is only for appointments with a
doctor or the phlebotomist.
SMS Text Service
We are pleased to announce that we now have
the option to send a comment on your blood tests
results via a text message. We can also send you
text reminders about your booked
appointments. To do this we do need to have your
correct mobile phone number on our computer
system. If you wish to take advantage of this
facility, please give your mobile phone number to
reception. We hope that you will find this service
useful.
- 13 -
Home Visits
If you can make an appointment to come to the
surgery it is better to do so. We have more facilities
at the surgery and it is easier to examine and treat
you there. Visits are usually restricted to
housebound patients only. If you cannot come to
the surgery and need a home visit, this can be
requested by telephone. Please try to telephone
before 10.30 am, this helps the visiting doctor to
organise their visits for the day. If you require an
urgent visit please tell the receptionist.
Telephone Advice
If you wish to speak to your doctor on the
telephone, please ask the receptionist to book you
a call, which may be at a different time of day,
depending on the doctor’s own arrangements.
The receptionists have been asked not to interrupt
the doctors during their surgeries except in
emergencies.
Repeat Prescriptions
We want to make it easy for our patients to order
repeat medication, and you can request your
medication by fax or online, by post, in person or
via a pharmacy.
- 14 -
Your request will be processed within 2 working
days of receipt so please allow sufficient time to
order your medicines before you run out.
We regret that we cannot accept telephone
requests for repeat prescriptions. This is to remove
the possibility of mistakes being made in
transcribing telephone requests and also to free up
our telephone lines for patient’s urgent medical
needs.
In person: It will help us to provide a better service if
you return the white, right hand portion of your
previous prescription, indicating which items you
are requesting. If you cannot do this, please
complete one of the yellow request forms, which
are on the reception desk.
Post: Please type, or write clearly your name,
address, date of birth, plus the name of your
medications, the strength and the quantity – for
example, patient number 001, Aspirin 75mg x 28.
Online: Please follow the links on our website
Fax: 01242 541698
E-mail: to [email protected]
To maintain confidentiality if you use fax or e-mail to
request your prescription, you need to only provide
your patient record number (shown on the top right
hand side of your last prescription) plus the name of
- 15 -
your medication, the strength and the quantity – for
example, patient number 001, Aspirin 75mg x 28.
Collection of Prescriptions
Most pharmacies offer a service to collect
prescriptions from the surgery on behalf of patients.
The pharmacies then get the medicines ready for
patients to collect direct from the pharmacy, which
saves patients a trip to the surgery. If this service
would help you, please contact your local
pharmacy to find out if they can offer this.
Clinics & Services

This is done during normal surgery hours by
appointment. You can discuss this further with your
doctor.
Diabetes & Asthma
We have nurses who are specially trained in
supporting, advising and treating patients with
these conditions. Please book an appointment
through our receptionists.

Our practice nurses can support you in stopping
smoking, including prescriptions. Please make an

our nurses. You will receive a letter with your result
as a matter of routine. When you make your
appointment please tell the receptionist it is for a
smear. For women aged between 25 and 50, a
smear is recommended every 3 years, for those
over aged between 50 and 65, it is recommended
5-yearly.

advice and information on any aspect of family
and child health matters may be obtained.
Immunisations are carried out separately by the
practice nurses.
- 17 -
Child Health
Appointments are sent to you for your baby and
child’s routine health reviews. One of the first of
these is at 8 weeks with your GP and is arranged
soon after your baby is born. There are other
reviews at 9 months and 2 to 2.5 years with the
health visitor. Child immunisations are by
appointment with Practice Nurse.

please call Health Visitors Team on 01242 513368.

and you may telephone for advice if you have an
emergency or need to discuss something. A full
range of methods is offered including the supply

and implant as well as injectable and emergency
contraception. We have practice nurses who are
fully trained in family planning and are available for
advice if you have any difficulties. Appointments
can be made during ordinary surgeries. We have a
dedicated family planning clinic on Tuesdays
between 4.00 – 6.00pm.

We offer travel advice and healthcare to our
patients, which includes advice about tailored
- 18 -
health care vaccination programmes, malaria

The practice has a strict policy regarding
confidentiality and data protection. We will only
release test results to the person to whom they
relate unless that person has given prior permission
for the release of this data or they are not capable
of understanding the results.
If there is any doubt we will telephone you on your
registered telephone number to ensure
confidentiality.
Please telephone our main surgery number (01242
539080) and select Option 1.
Services Not Covered by the National Health Service
A fee may be payable for certain services not
covered by the National Health, e.g. private health
certificates, insurance claim forms, some travel
vaccinations etc. A scale of fees is available for
inspection at reception. If in any doubt, please ask
one of the receptionists.
Patient Registration
Anyone who lives within our practice boundary can
register as one of our patients. If you are unsure if
you live within our boundary, there is a map of the
- 19 -
practice area in the waiting room that you can
view, or please ask for advice from our
receptionists.
If you wish to register, please see one of our
receptionists who will give you a simple form to
complete, or complete the pre-registration form on
our website. It will be helpful if you can include
details of your previous or current GP, and your NHS
number.
Confidentiality
The practice complies with Data Protection and
Access to Medical Records legislation. Identifiable
information about you will be shared with others in
the following circumstances:
To provide further medical treatment for you
e.g. from district nurses and hospital services.
To help you get other services e.g. from the
social work department. This requires your
consent.
When we have a duty to others e.g. in child
protection cases anonymised patient
information will also be used at local and
national level to help the Health Board and
Government plan services e.g. for diabetic
care.
If you do not wish anonymous information about
you to be used in such a way, please let us know.
- 20 -
Receptionists and administrators require access to
your medical records in order to do their jobs. These
members of staff are bound by the same rules of
confidentiality as the medical staff.
Your personal information
We keep medical information about you on paper
and on the computer system in accordance with
the requirements of the Data Protection Act.
You have the right to see your medical records if
you wish. You can now do this online via Patients
Access on our Surgery website. Please ask at
Reception if you would like further details.
An appointment will be required if you wish to view
your physical notes. If you wish to have a copy of
your notes a fee will be payable.
Surgery Standards
Leckhampton Surgery – Our Commitment to you
Our aim at the Leckhampton Surgery is to give you the
best possible service. In order to achieve this we need to
work together.
You will be treated with courtesy and at all times
be given the opportunity to take part in
decisions made about your health.
Treatment will not be given to you without prior
discussion and your informed consent.
Your health records are confidential. You have
the right to see your records subject to any legal
- 21 -
limitations. Please note that patient’s records
may be used by the Health Authority auditors to
verify payments to GPs.
We will ensure that all practice staff are suitably
qualified.
We would like you to understand all the
information given to you about your
prescriptions. If anything is unclear, please ask.
Appointments
We aim to see you within 30 minutes of your
appointment time.
We offer routine appointments within 2 working
days.
Telephone Access (during office hours)
If your need is urgent you will be able to speak
to a doctor or nurse within 30 minutes.
Telephone calls to the doctor may be booked
for a set time during the day. Your doctor will try
to call you as close as possible to the time
arranged.
Visits
The on-call doctor will aim to visit you within 4
hours of your call.
Urgent requests will be dealt with within 1 hour.
Routine reviews will be undertaken by your usual
doctor wherever possible.
- 22 -
To help us achieve these standards:
Please try to keep appointments. If you are not
able to attend, let us know as soon as possible.
If you are late for an appointment you may be
asked to re-book.
Please do not use urgent appointments for
routine problems.
Please avoid telephoning in on Monday
between 9.00 am and 11.00 am unless you need
an urgent appointment.
Attend the surgery whenever possible and only
request home visits when you feel it is really
necessary.
Zero Tolerance to Violence
Members of staff who treat patients have the right
to work free from any form of threats, fear, abuse or
violence. The GP practices in Gloucestershire have
agreed a Zero Tolerance Policy. Depending on the
level of the incident a patient who is verbally rude
or mildly abusive may be given a ‘Yellow Card’ as a
final warning. Any further incident will result in a
‘Red Card’ with the patient being removed from
the Practice list. Following a ‘Red Card’ the patient
may face police charges resulting from the incident
and will not be seen by any local GP Practice. The
patient will have to travel to Gloucester to the
Vaughan Centre to receive any further GP services
in secure conditions for healthcare staff and will not
receive home visits.
- 23 -
Clinical Training
The practice is committed to the further training of
a variety of health professionals. These include
doctors, district and practice nurses, midwives and
health visitors. Sometimes you may be asked if one
of these clinicians could be present during the
consultation with your own doctor, or if a
consultation can be recorded. Your consent will be
sought and you have the right to refuse. GP
Registrars are qualified doctors, but now gaining
further experience in the field of general practice,
usually over a year.
Other important information
Immunisations Parents - please do not bring
your child to an immunization appointment
if you they are unwell and remember to
cancel any appointments that you have
made in advance. This saves nurse time and
frees up appointments for other patients. Re-
book your child another appt when they are
well (the surgery will not send a reminder for
this).
Samples If you are bringing a sample to the
surgery please remember to label it carefully
with your details and remember to enter the
date and time that the sample was taken
(on the enclosed form). Valuable nurse time
is spent having to label patient's samples
and forms.
- 24 -
ECGS Female patients - If you have been
asked by your doctor/nurse to attend for a
routine ECG please do not wear tights as
these will have to be removed for this
procedure.
Nurse Appointments If you are more than 10
mins late for an appointment please do not
expect to be seen by the nurse. The nurses
have a tight schedule and are unable to see
late attendees. Please allow plenty of time
to travel to the surgery so that you are not
late for your appointment.
Patient Feedback
And finally, we aim to provide you with the best
possible care and welcome any ideas for changes
or improvements, so please let us know if you have
any comments or suggestions. We are all human
and make mistakes from time to time but it is our
experience that most problems can be resolved
easily and quickly, often at the time they arise and
with the person concerned. However, if your
problem cannot be managed in this way and you
wish to make a complaint, please contact the
Practice Manager or any of the GP partners.
