Community Neuro Rehab and Stroke and ESD Team (Neuro-outreach Team)

PLE description overview:

An outpatient and community service for patients with neurological diagnoses and stroke. We are also an early supported discharge service for stroke patients.

Description of patient / client group catered for:

Clients over 16 years of age who have a neuro- disability arising as a result of a stroke or an acquired neurological condition (inherited/non-inherited)

Care provision offered:

Neuro assessment and rehabilitation based on the Bobath concept or Normal Movement approach. This can take place in an outpatient setting or in the patient’s home, workplace or other setting e.g. local gym, swimming pool. We can provide specialist interventions such as splinting and casting, gait re-education using the Biodex unweighing system, Functional electrical stimulation (FES), and specialist upper limb rehabilitation post stroke using the Saebo splinting system.

Complex Discharge Assessment Unit

PLE description overview:

The unit provides an essential discharge assessment and planning role for adult patients from across Central Manchester Foundation Trust.

Description of patient / client group catered for:

The patient group is made up of mainly older adults with complex physical, cognitive and social problems.

Care provision offered:

You will assess and treat adults with complex physical needs from a wide range of disease groups including dementia, orthopaedics, stroke, falls, the majority of whom will have cognitive impairment. You will assess and progress their rehabilitation potential in order to maximise independence prior to discharge home. You will work with the MDT to plan safe and timely discharge.

PLE description overview:

Supported discharge service following up adult patients discharged from the MRI. Patients are seen in their own homes to support initial discharge from hospital and prevent re admission. Short term goals are set to provide rehabilitation to patients in their own homes following hospital admission. The team is multi disciplinary including Physiotherapists, Occupational Therapists and Rehabilitation Assistants.

Adults only. Not condition specific. Patient groups long terms conditions, general rehabilitation, neurological conditions,

Care provision offered:

Assessment of physical and functional ability within home environment and outdoors if applicable. Appropriate delivery of community rehab for up to 6weeks post discharge from hospital. May include exercise prescription, functional activity practice, multi-agency working etc

An in-patient neuro-rehabilitation unit covering rehab to predominantly non-stroke neurological conditions

Description of patient / client group catered for:

Common presentations include traumatic and hypoxic brain injury, multiple sclerosis, spinal cord damage and other neurological pathologies including Guillain-Barre syndrome. Caseload consists of complex patients presenting with mobility, cognitive and communication impairments.

Care provision offered:

Patients’ needs are identified and met through a problem solving and goal setting framework. Comprehensive treatment is delivered through a well established multi-disciplinary team.

Acute hospital-based 12-bedded outpatient area used for the assessment and management of musculoskeletal health disorders and injury.

Rheumatology outpatients presenting with one or more rheumatological conditions – including arthritis (rheumatoid, osteoarthritis, psoriatic, reactive), ankylosing spondylitis (AS), fibromyalgia, hypermobility and systemic lupus erythematosus (SLE). Class-based treatments including chronic conditions class, AS class and AS hydro class. Haematology inpatients & outpatients

Care provision offered:

Initial individual or joint (PT/OT) assessment followed by appropriate treatment. This may include the prescription of a home exercise programme, manual therapy, electrotherapy, cryotherapy, hydrotherapy, acupuncture (normal & electro) and corticosteroid injections.
