Differentiaal diagnosen
• Primair
– distale radius fractuur
– Scaphoid fractuur
– perilunaire dislocatie
– SL dissociatie
– TFCC letsel
– ulna impaction syndrome (meestal secundair of chronisch)
• Secundair
– partiële artrose
– ulna plus syndroom/ulna impaction syndrome
functionele anatomie
• polsgewricht:
– radiocarpaal gewricht
– distaal radioulnair
(DRU) gewricht
– midcarpaal gewricht
• distale radius onderdeel van
het polsgewricht
• SL ligament tussen scaphoid
en lunatum
polsbewegingen

Scaphoid fracturen
Acute scaphoid fracturen
• veel voorkomende fractuur
• vaak gemiste fractuur
• zwelling en pijn van tab.

gipsimmobilisatie ≥8 weken
• indien instabiel: operatie, 6 wk gips
Non-union scaphoid fracturen
• relatief veel voorkomend
• ip chirurgisch behandeling
• behandeling bepaald door:
– stand scaphoid (dislocatie)
– vascularisatie
• diagnostiek:
– pijn radiocarpaal bij PF/DF
– soms functio laesie
– röntgen PA/lat en MRI
Unionrate hoog:
90%
