9.1. PURPOSEThe purpose of this document is to describe the Planning & Scheduling business process to be utilized within Lapindo Brantas, Inc. It is important to note that in some cases deviations from these processes might be acceptable.

9.2. SCOPESThe Planning and Scheduling process as shown above covers the following activities:

9.3. DESCRIPTION OF BUSINESS PROCESS

9.3.1.Long Term Planning (5-Year Planning)

ObjectiveThe objective of a long term planning is to provide a road map of maintenance activities which must be executed over the next 5 years period.

ActivitiesThe five-year maintenance planning will cover the following activities:a. gather data for five-year maintenance planning draftb. prepare five-year maintenance planning draftc. review five-year maintenance planning draftd. revise reviewed five-year maintenance planning drafte. final five-year maintenance planning approval

UpdatingEven the window of long term planning is five years but the planning should be adjusted yearly.

Figure 9.1 shows detail activities of a five-year maintenance planning business process.

9.5A.1 Planner shall collect data for developing the long term maintenance planning.Planner has to start collecting data on May.

The inputs for developing five-year maintenance planning might come from:

o Current long term Production Planningo Expected planned outageso History of unplanned / forced outages over the last 5 yearso History of de-rated performance over the last 5 yearso Projects to be executed within next five years, i.e.:

o Major, Intermediate & Minor Shutdown planning

9.5A.2 Planner shall prepare a five-year maintenance plan draft by reviewing the current 5-year plan and add in the plan for the new 5th year. The preparation must be finished by the first week of June. Planner shall submit the five-year maintenance plan draft to Maintenance Superintendent to be evaluated. The draft shall reach Maintenance Superintendent table not later than the second week of June.

The following activities – but not limited to – might be performed by planner during preparing the five-year plan.

o Compile the planning for the five year period to indicate all the required planned outages (Major, Intermediate, Minor), major modifications needed as well as any other major event (production loss and/or maintenance).

o Compare the dependable production capacity against the current 5-year production plan obtained from the production department. Adjust the planning where possible to try and meet the production plan.

o If the production plan cannot be met, renegotiate the production plan.

o Identify the spares and materials required for the outages which have a lead time of 12 months or more.

o Compile a draft 5-year maintenance plan (Planning of outages, resources required).

9.5A.3 Maintenance Superintendent has to review the five-year maintenance plan draft carefully before he distributes the drafts to:

o Mechanic Supervisoro Electric & Instrument Supervisor

The five-year maintenance plan drafts must be distributed to the related parties not later than the third week of June.

9.5A.4 The Planning team shall discuss the five-year maintenance plan draft in the yearly planning meeting No. 1. All the required changes must be raised at this first yearly planning meeting as there is no other opportunity to make changes. At least the following people must attend this meeting:

o Maintenance Superintendento Production Superintendento Plannero Mechanic Supervisoro Electric & Instrument Supervisor

The yearly planning No. 1 must be held not later than the first week of July.

9.5A.5 The Planner shall update five-year maintenance plan draft as discussed in the yearly planning meeting No 1.

The updated five-year maintenance plan draft must be submitted to Maintenance Superintendent not later than the second week of July.

9.5A.6 Maintenance Superintendent has to review the updated five-year maintenance plan draft carefully before he distributes the updated drafts to:

o Mechanic Supervisoro Electric & Instrument Supervisor

The five-year maintenance plan drafts must be distributed to the related parties not later than the third week of July.

9.5A.7 The Planning team shall discuss the updated five-year maintenance planning draft in the yearly planning meeting No. 2. This meeting shall take place to approve five-year maintenance plan. At least the following people must attend this meeting:

o Maintenance Superintendento Production Superintendento Plannero Mechanic Supervisoro Electric & Instrument Supervisor

The yearly planning No. 2 must be held not later than the second week of August.

9.5A.8 The Planner shall update the updated five-year maintenance plan draft as discussed in the yearly plan meeting No 2.

The final draft of five-year maintenance plan must be submitted to Maintenance Superintendent not later than the end of August.

9.5A.9 After five-year maintenance plan approval, the plan must be distributed to:

o General Manager East Javao Maintenance Superintendento Production Superintendento Mechanic Supervisoro Electric & Instrument Supervisor

The approved five-year maintenance plan must be distributed to the related parties not later than the second week of September.

9.5A.10 The approved five-year maintenance plan shall be printed out and placed on a notice board:

o inside each control roomo inside each workshopo inside the offices of the Maintenance Supervisorso inside the planning office

9.5A.11 The Maintenance Planners must ensure that the orders are placed in enough time for all the critical long lead items (specific spares) as identified in step 9.5A.2. If not, these items will not be available when required which will result in costly and unnecessary delays.

Form 721-MMS-00-X-009 and Form 721-MMS-00-X-010 might be used as a reference for compiling Five-Year Maintenance Planning Plan and Five-Year Maintenance Budget Plan respectively.

9.3.2.Yearly Planning

ObjectiveThe objective of yearly planning is:

to identify the expected peak workloads problems in early period and resolve the problems in advance.

to create the baseline and to provide a road map of maintenance activities which must be executed over a year.

Figure 9.2 shows detail activities of a yearly maintenance planning business process.

9.5B.1 Planner shall excerpt all maintenance activities plan for next year from existing five-year maintenance plan.

Planner has to start excerpting yearly maintenance planning not later than the end of September.

Form 721-MMS-00-X-009 might be used as a reference for compiling Yearly Maintenance Plan.

9.5B.2 Planner must list all monthly maintenance activities for one year (12 months) period. Planner has also to list the types and descriptions of monthly maintenance activities.

The types of maintenance activities could be: Preventive Maintenance (PM) Corrective Maintenance (CM) Predictive Maintenance (PdM) Planned Shutdown(SD) Inspection by Government / MIGAS (INS) Other type of maintenance (OTH)

Each identified monthly maintenance activities must be properly described by planner.

9.5B.3 Planner has to allocate required human resources for each monthly maintenance activities. The following items shall be allocated for each monthly maintenance activities.

Estimated number of maintenance personnel involved in Skill level of required maintenance personnel Estimated hours required for each personnel Estimated total man-hours required for each personnel to

accomplish each monthly maintenance activities Estimated required labor unit cost for each personnel Estimated total cost for each personnel

For each identified monthly maintenance activities, planner shall summarize monthly required man hours and labor costs.

9.5B.4 Planner has to allocate required materials and / or tools for each monthly maintenance activities. The following items shall be allocated for each monthly maintenance activities.

Description of estimated required materials / tools Quantity and unit cost of each required material / tool Estimated total cost of all required materials and / or tools.

For each identified monthly maintenance activities, planner shall summarize monthly required material costs.

9.5B.5 Planner shall identify whether there are any imbalances in allocating human resources. If this is happened, planner shall re-do human resources planning as described in 9.5B.3. Otherwise, Planner may compile summary of yearly maintenance plan draft before he delivers to Maintenance Superintendent.

9.5B.6 Planner must compile summary of yearly maintenance plan. This compilation must show the following information for monthly maintenance activities.

9.5B.7 Maintenance Superintendent has to review the yearly maintenance plan draft carefully before he approves the draft and distributes the drafts to:

The approved yearly maintenance plan must be distributed to the related parties not later than the end of October.

9.5B.8 Yearly maintenance plan must be reviewed at the third week of November. The purpose of this meeting is to review the yearly planning. The following fellows must attend the review meeting.

9.5B.9Planner must update yearly maintenance plan as per outcome of yearly review meeting. Planner shall finish up the updated yearly maintenance plan not later than the fourth week of November.

9.5B.10 Maintenance Superintendent has to review updated yearly maintenance plan carefully before he endorses the updated plan and distributes the plans to:

Updated yearly maintenance plan must be distributed to the related parties by the first week of December.

9.5B.11 As soon as monthly and yearly maintenance plans are endorsed, Planner must update the database.

9.5B.12 Planner shall ensure that the long lead resources are ordered in advance.

9.3.3.Monthly Planning

The purpose of a monthly planning is primarily to plan all maintenance activities in enough details during a month window period.

Figure 9.3 shows detail activities of a monthly maintenance planning business process.

9.5C.1 Planner excerpt the following information from yearly maintenance plan: monthly maintenance plan materials having long lead time

Last month backlog and other window opportunities might also be reviewed.

Planner has to start excerpting monthly maintenance planning not later than the first week of preceding month.

9.5C.2 Planner must list all weekly maintenance activities for one month period. Planner has also to list the types and descriptions of daily maintenance activities.

The types of maintenance activities could be: Preventive Maintenance (PM) Corrective Maintenance (CM) Predictive Maintenance (PdM) Planned Shutdown(SD) Inspection by BP MIGAS (INS) Other type of maintenance (OTH)

Form 721-MMS-00-X-010 might be used to start compiling Monthly Maintenance Plan.

9.5C.3 Planner has to allocate required human resources for each weekly maintenance activities. The following items shall be allocated for each weekly maintenance activities.

9.5C.5 Planner must compile summary of monthly maintenance plan. This compilation must show the following information for each daily maintenance activities.

The compilation process must be finished by Wednesday of the first week of the preceding month.

9.5C.6 Planner shall deliver compiled monthly maintenance plan to Maintenance Superintendent. At least, the monthly maintenance plan shall cover the following topics:

List of planned maintenance activities for the following month List of long lead time materials requisition List of next month’s material requisition

9.5C.7 Maintenance Superintendent has to review the monthly maintenance plan draft carefully before he approves the draft and distributes the drafts to:

General Manager East Java Maintenance Superintendent Planner Mechanic Supervisor Electric & Instrument Supervisor

The approved draft of monthly maintenance plan must be distributed to the related fellows before review meeting held on Thursday of the second week of the preceding month.

9.5C.8 Maintenance Superintendent must review material requisition prepared by Planner. If everything is alright, Maintenance Superintendent shall notify Maintenance Supervisor to raise material Requisition.

Form 721-MMS-00-X-005 might be used to order materials having long lead requisition time.

9.5C.9 Maintenance Supervisor must create material request as notified by Maintenance Superintendent through E- Procurement. For the purpose of progress monitoring, the request shall be carried out before review meeting is held.

9.5C.10 Draft of monthly maintenance plan must be reviewed on Thursday of the second week of the preceding month. The following fellows must attend the review meeting.

9.5C.11 Planner must update monthly maintenance plan as per outcome of review meeting. Planner shall finish up the updated monthly maintenance plan by Friday afternoon during the same week when review meeting is held.

9.5C.12 Maintenance Superintendent has to review updated monthly maintenance plan carefully before he endorses the updated plan and distributes the plans to:

The updated monthly maintenance plan must be distributed not later than the third week of the preceding month.

9.5C.13 As soon as daily and weekly maintenance plans are endorsed, Planner must update the database.

9.5C.14 Maintenance Supervisors shall ensure that all required materials for maintenance activities of the month has been ordered.

The reason of a weekly planning is mainly to plan all non critical maintenance activities to be done during the following week.

9.5D.1 Extract list of work orders to be started from next week onwards. Required data to generate work orders might come from:

monthly maintenance planning work requests failure reports closing work orders records emergency repairs records

Planner has to identify and prioritize which work orders have to be executed for the following week schedule. Planner shall determine whether raised work orders could be rectified by:

Preventative Maintenance Predictive Maintenance Corrective Maintenance Contracting third party

Planner must also compile job cards, i.e., required documents to perform maintenance tasks mentioned above. The required documents are as follow, but not limited to:

standard report / checklist for performing maintenance tasks working procedure job safety other related documents

Planner shall perform this task every Monday.

9.5D.2 Weekly preventive maintenance (PM) planning processes are described below. Figure 9.5 shows detail activities of a weekly preventive maintenance planning business process.

9.5D.2.2 If any, Planner must excerpt PM tasks to be performed during the next week period from raised work orders.

9.5D.2.3 Planner has to ensure that all human resources are ready.9.5D.2.4 Planner must also ensure that all required materials for PM are

ready. If not, Planner has to raise material requisition.9.5D.2.5 Planner shall prepare material requisition needed for PM

activities before the requisition is approved by Maintenance Superintendent. Form 721-MMS-00-X-005 might be used to raise material requisition.

9.5D.2.6 Maintenance Superintendent must review material requisition prepared by Planner. If everything is alright, material requisition is sent to inventory department.

9.5D.2.7 Inventory department will review whether material requisition is approved or not.

9.5D.2.8 Maintenance Superintendent has to monitor the progress of material requisition and ensure that materials are procured on time. If there are any revisions, Maintenance Superintendent shall have material requisition revised by Planer.

9.5D.3 Weekly corrective maintenance (CM) planning processes are described below. Figure 9.6 shows detail activities of a weekly corrective maintenance planning business process.

activities before the requisition is approved by Maintenance Superintendent. Form 721-MMS-00-X-005 might be used to raise material requisition.

9.5D.3.6 Maintenance Superintendent must review material requisition prepared by Planner. If everything is alright, material requisition is sent to inventory department.

9.5D.3.7 Inventory department will review whether material requisition is approved or not.

9.5D.3.8 Maintenance Superintendent has to monitor the progress of material requisition and ensure that materials are procured on time. If there are any revisions, Maintenance Superintendent shall have material requisition revised by Planer.

9.5D.4 Weekly predictive maintenance (PdM) planning processes are described below. Figure 9.7 shows detail activities of a weekly corrective maintenance planning business process.

9.5D.4.4 Planner must also ensure that all required tools for PdM are ready.

9.5D.5 Repair by contractor planning processes are described below. Figure 9.8 shows detail activities of a repair by contractor planning business process.

9.5D.5.1 Planner has to excerpt and identify all repairs that need contractors to fix the problems.

9.5D.5.2 Planner must compile a specification for the work to be done by a contractor. The specification should include at least the following items:

Description of work / services required Required start date Required completion date When the quotation must be submitted

Planner shall submit complete works specifications being performed by contractors to Maintenance Superintendent.

9.5D.5.3 Maintenance Superintendent must initiate the procurement process by completing the necessary purchase requisitions complete with required times and urgencies indicated for the services required. Maintenance Superintendent must also obtain an estimated delivery time for requested materials / services from procurement department.

9.5D.5.4 Maintenance Superintendent must review all proposals proposed by bidders. Maintenance Superintendent has to select the qualified bidders carefully before he recommend / choose the qualified bidders to procurement department.

9.5D.6 Having detail information and requirement of all maintenance activities on hand, Planner has to finalize the maintenance tasks planning for the following week. Planner must ensure that the load of each maintenance technician is balance and not overloaded. Do some adjustment if it is considered necessary. Planner shall not schedule too many tasks to be performed simultaneously on equipment or on a system or on a certain area since it will become crowded.

9.5D.7 Maintenance Superintendent has to review the weekly maintenance plan draft carefully before he approves the draft and distributes the drafts via e-mail to:

Maintenance Superintendent has also to invite fellows mentioned above to attend weekly review meeting held on Thursday.

9.5D.8 Maintenance Superintendent must lead weekly review meeting held every Thursday morning. The following fellows must attend the review meeting.

During the meeting, make sure that all supervisors know exactly what he and his team have to do for the next week. Any problems might be happened regarding the allocation of human resources and / or materials must be resolved.

The meeting shall produce maintenance task lists for the following week.

Have maintenance task lists signed by all Maintenance Supervisors and endorsed by Maintenance Superintendent.

9.5D.9 Planner must update weekly maintenance planning as per outcome of weekly review meeting. Planner has also to raise Work Orders as accompanying of weekly maintenance planning.

Planner shall finish up the updated weekly maintenance planning accompanied with scheduled Work Orders by Friday morning.

Form 721-MMS-00-X-012 might be used to raise scheduled work order.

9.5D.10 Maintenance Superintendent has to review updated weekly maintenance planning (Form 721-MMS-00-X-011) accompanied with

9.5D.11 As soon as weekly maintenance schedule are endorsed, Planner must update the database.

9.5D.12 Supervisor shall perform weekly maintenance scheduling as soon as he receives the documents from planner. He must also ensure that all required materials for maintenance activities of the following week have been ordered. Form 721-MMS-00-X-005 may be used for material requisition if it is required.
