Welcome to christiana care

Welcome to Christiana Care Health System.

We strive to fulfill our mission by ensuring the very best in care, the most experienced health care professionals, excellence and innovation in technology, comprehensive treatment programs and so much more.

This guide serves as an introduction to the many Christiana Care people and services available to meet your needs – and those of your family and your visitors – during your hospital stay. It also includes important information about valuable people and services you can depend on after you leave the hospital.

It is important that you help us give you the care you want and need by:

asking us questions when you don’t understand something or need more information;

encouraging your family (or friends) to ask us questions (with your permission); and

letting us know when we are being helpful (and, when you think we can be more helpful).

It is only by listening to you that we are best able to help you. On behalf of all my Christiana Care colleagues and our medical-dental staff, I wish you the best of health. Please be sure to let us know how we can help you. We very much appreciate your feedback and suggestions.

Wishing you good health,

Robert J. Laskowski, M.D., MBA

President and Chief Executive Officer

for our VisitorsVisiting philosophyChristiana Care’s patient and family centered care philosophy recognizes that visits from family members, close friends and clergy are an important part of your healing process. Our visitation policy provides a safe environment that promotes patients’ physical and emotional well-being, provides adequate time for patient rest and allows time for medical therapies and nursing care. Some units may have more restrictive visiting privileges to ensure that patients receive sufficient recovery time.

Visitors are encouraged to communicate directly with the nurse caring for the patient to discuss visiting arrangements and share information about the patient. Visitors are asked to respect the nurse’s direction in coordinating visiting times according to each patient’s preferences, care needs and the needs of the family.

Guidelines It is recommended that in semi-private rooms visiting be limited to

two visitors per patient at a time.

Family and visitors should maintain a quiet environment to help avoid unnecessary noise.

Family and visitors may be asked to leave the room during tests or treatments or when the doctor or nurse needs to examine the patient per patient preference.

Visitors wishing to stay past visiting hours are asked to discuss the request with the nurse caring for the patient.

All children should be supervised by an adult.

Visitors are not restricted, limited or denied visitation privileges on the basis of race, color, national origin, religion, sex, gender identity or expression, sexual orientation or disability.

As used in these guidelines “family” means any individual who plays a significant role in an individual’s life such as spouses, domestic partners/civil unions, significant others (of both different and same sex), and other individuals not legally related to the patient as per patient preferences. “Family” also includes a minor patient’s parents, regardless of the gender of either parent.

patient safety at christiana care:

SPEAK UPTM*Christiana Care’s Speak Up™ program provides useful information and suggestions to help keep you safe throughout your stay in a Christiana Care hospital and during your recovery time after discharge. It’s part of our commitment to provide you with a safe environment to receive the care you need.

Highlights of the Speak Up™ program include:

Speak up if you have questions or concerns. If you still don’t get an answer or still don’t understand, ask again. Your health is too important to worry about being embarrassed if

you don’t understand something that your doctor, nurse or other health care staff tells you. It’s your body and you have a right to know.

Be the center of your health care team. Don’t be afraid to ask about safety.

If you’re having surgery, ask about marking the area that is to be operated upon to avoid any chance of confusion in the operating room.

For your safety, your health care staff will ask you to tell them what procedure or type of surgery you are having done.

While you are in the hospital, if you (or a loved one), are experiencing a serious change in condition, and feel this change is not being recognized by your caregivers, use the phone in the room and dial 6385.

Pay attention to the care you are receiving. Expect health care staff to introduce themselves when they enter

your room and look for their identification badges.

Notice whether your caregivers and visitors have washed their hands. Don’t be afraid to gently remind someone to do this. Hand washing by you, our staff and visitors is the most important way to prevent the spread of infections.

While in the hospital, you will be wearing an identification bracelet. Please check your bracelet to confirm it correctly identifies you.

Expect our health care staff to check your wristband or to ask you questions, such as your name and date of birth, to correctly identify you before giving you medications, treatments or taking you for a test.

If our staff does not identify you, feel free to remind them.

Page 11: Patient & Visitor Information Guide - 2011

11

Educate yourself about your diagnosis, the medical tests you are undergoing and your treatment plan. Thoroughly read all medical forms and make sure you understand

them before you sign anything. If you don’t understand, ask your doctor or nurse to explain them.

Make sure you are familiar with the operation of any equipment that is being used in your care. If you will be using oxygen at home, do not smoke or allow anyone to smoke near you while oxygen is in use.

Ask a trusted family member or friend to be your advocate. We encourage visitors to support their loved ones.

Know what medications you take and why you take them. Keep a current list of all medicines, vitamins, herbs and

supplements you are taking and bring the list with you to any visit to the hospital or your doctor. For your convenience, we have provided a detachable “My Medication List” on page 36. Use this handy form to track all your medications.

Use a hospital, clinic or other type of health care organization that has experience in treating your type of illness.Participate in all decisions about your treatment. You are the center of the health care team. You and your doctor should

agree on exactly what will be done during each step of your care.

Plan your recovery by finding out about your condition, your new medicines and your follow-up care.

You will be given important directions about your follow-up care, including written instructions. If you follow these directions, you will have a greater chance of getting well faster.

* Speak Up™ is a service mark of The Joint Commission.

patient relations Departmentconcerns and QuestionsChristiana Care’s goal is to achieve excellence in every aspect of a patient’s stay or a family member’s visit. If you, a family member or visitor has a concern that cannot be resolved by talking to your physician or nurse, the Patient Relations Department will work with you to facilitate an inquiry and resolve the matter with the appropriate hospital staff members. Patient Relations has offices in both hospitals and is open from 8 a.m. to 4:30 p.m. To contact a patient representative, please call Patient Relations at 302-733-1340 (ext. 1340) at Christiana Hospital or 302-428-4608 (ext. 4608) at Wilmington Hospital.

organ DonationsThe donation of an organ or tissue is perhaps the greatest gift of life and health that we can give. Anyone who is 18 years of age or older may legally state his or her desire to be an organ, tissue or body donor. For more information call Patient Relations at 302-733-1340 (ext. 1340) at Christiana Hospital or 302-428-4608 (ext. 4608) at Wilmington Hospital. advance Directives: individual instructionsWe recognize and respect your right to make health care decisions. To help make these decisions, you have a right to be informed adequately about your medical condition, treatment alternatives and the likely risks, benefits and possible consequences of each.

If you become unable to communicate your wishes regarding medical treatment, your physician will attempt to determine what you would want if you could express yourself. Your physician may rely on previous conversations he or she had with you regarding health care decisions. Your physician also may rely on family members and close friends who talked to you about your wishes concerning health care.

You may choose to formally make known your wishes – while you are able – in an Advance Directive (formerly known as a Living Will or Durable Power of Attorney for Health Care). Advance directives are written instructions that guide the actions of medical staff, family and friends, if necessary. You may also choose to designate someone to make decisions for you should you become unable to make your own decisions. For more information regarding advance directives, call Patient Relations at 302-733-1340 (ext. 1340) at Christiana Hospital or 302-428-4608 (ext. 4608) at Wilmington Hospital. and support you through the process. Care managers assist adult patients with home care, infusion services and medical equipment arrangements at home. They review patient admissions and hospitalizations to determine if the services given are medically necessary and need to be done in a hospital setting, and work with your insurance company so that their care guidelines are met. Call 302-733-2222 (ext. 2222) or 302-428-2208 (ext. 2208) for more information about the discharge process.

aDDitional informationutilization managementHospitals are required by federal law, The Joint Commission and insurance companies to review patient admission and lengths of stay for medical necessity, appropriateness, quality and discharge planning.

Insurance plans have restrictions on what will be reimbursed in a hospital setting. For example, Christiana Care is reimbursed only for acute care that is medically necessary. In addition, preadmission/pre-procedure certification and second surgical opinion requirements must be met to guarantee payment if your insurance requires it. Utilization Management coordinators will assist you in interpreting your insurance requirements and the need for inpatient care. They will help your physician and refer you to a contact person for discharge planning.

If you have questions about Utilization Management, call 302-733-3041 (ext. 3041) at Christiana Hospital, or 302-428-4941 (ext. 4941) at Wilmington Hospital. Our staff members are pleased to answer your questions.

how to make a Gift to christiana careChristiana Care is a private not-for-profit regional health care system that relies, in part, on the generosity of individuals, foundations and corporations to fulfill its mission. You can help Christiana Care in a variety of ways:

Charitable gifts can be made on behalf of a program, medical specialty, capital project or endowment.

Unrestricted gifts are applied where funds are most needed.

Gifts can also be made in memory of, or in honor of, someone such as a member of the family, a friend or a caregiver who has been particularly caring and compassionate.

Christiana Care employees or volunteers may not accept an outright gift from a patient or visitor in appreciation for their services. However, you can make a gift through Christiana Care’s Office of Development to recognize a nurse, a physician or a team of health care providers. A form for prospective donors to use when making a gift to Christiana Care is available at nursing stations and information desks throughout the system.

your health care teamThe care you receive at Christiana Care is truly a team effort. We will collaborate with you and your family to develop an individualized and comprehensive plan of care marked by a dedicated medical and nursing staff, efficient support personnel and the latest technology.

Your health care team practices safety when handling blood or body fluid because any fluid has potential for transmitting infection. For the safety and well-being of Christiana Care patients and staff, this might include wearing gloves, gowns, masks, goggles or a face shield in certain situations. Christiana Care practices approved hand washing, another important step in stopping the spread of infection.

medical staffThere are more than 1,200 physicians on staff at Christiana Care who provide personalized professional care for our patients.

Your attending physician will be primarily responsible for your care during your hospital stay. He or she will admit you and may issue orders for your care and treatment, consult other physicians to help in your care, determine which tests and examinations are necessary to diagnose your condition, prescribe treatments and medications you will receive and determine when you are ready to be discharged.

Your physician may issue visiting and dietary restrictions if necessary.

hospitalistsHospitalists are physicians employed by Christiana Care who specialize in caring for patients in the hospital; a hospitalist may manage your care during your hospital stay. Your primary care doctor will share your medical records, history and other information with your hospitalist to ensure that he/she understands your current health condition and preferences.

Hospitalists will examine you and perform or coordinate any necessary exams, tests or treatments. They are available to answer questions or address concerns presented by you or your family members. They can also arrange for you to have a consultation from other medical specialists.

Upon your discharge from the hospital, your hospitalist will prepare a detailed report on your health findings and treatment plans for your primary care physician. Your regular doctor or primary care physician will continue caring for you after you are discharged from the hospital. Please ask a member of your care team if you would like more information on hospitalists.

Pharmacists work with your doctors and nurses to ensure you get the most out of your medication therapy. Our pharmacists screen prescriptions for interactions with other drugs or foods you might be receiving. They also look for duplicate therapies, drug allergies and opportunities to avoid side effects and streamline your therapy to make it more effective and less costly. Pharmacists are important members of the health care team and are available to patients and their families by being present on many of the nursing units at Christiana and Wilmington hospitals.

our commitment to DiVersity anD inclusion

Christiana Care is committed to meeting the health care needs of the increasingly diverse communities of Delaware and other communities we serve.

Our goal is to foster an inclusive culture that respects the diversity of our employees and patients.

We want all patients and families who seek health care at Christiana Care Health System to feel confident that we will honor and value their unique cultural identities.

cultural competenceAs a health care provider it is our goal to understand and respond effectively to the cultural and language needs of our patients as we care for them. At Christiana Care we expect that all of our staff and physicians will care for you in this way. Culturally responsive care requires us to:

Treat each and every person with equal respect and dignity regardless of culture, ethnicity, race, social status or any other aspect of their identity.

Have a working knowledge of the important customs, values, and health beliefs, for a wide range of cultural groups. Have the skills and use the available resources to communicate well with all patients to explore customs, values, and health beliefs and exchange clinical information that is complete, accurate and timely.

Invite you to help us learn more about your culture-based beliefs and needs so that we individualize your care in ways that are medically safe for you. If we are not doing so, please “SPEAK UP!” just as you would about other aspects of your care and safety. We intend to ask you about your beliefs and practices and to include them in your care where that is safe for you. If we don’t do so, tell us things such as:

About where “home” was before coming to the USA,

About any health care procedures that might not be acceptable to you,

About any traditional or home remedies you use to improve your health,

About traditional or alternative healers who are meaningful to you,

28

Page 29: Patient & Visitor Information Guide - 2011

About spiritual or religious practices that may affect your ability to follow treatment recommendations,

About particular food or liquid choices that are important to you,

About your wishes for emotional support.

We welcome your partnership in providing you the best care possible!

If English is not your preferred language and you need a language interpreter to communicate with your nurse, physician or any staff member, we will provide you with a qualified medical interpreter. There is no charge for this service. One way we do this is with special Cyracom interpreter telephones. All of the telephone interpreters are qualified for health care interpretation. Services are available 24 hours per day, seven days per week, in more than 150 languages. This service works best when used with a special dual handset telephone or via a speaker phone. These phones will be brought to you by our staff. We can make outgoing calls to you and accept your telephone calls to us with assistance from an interpreter who speaks your language.

Our sign language interpreting services also are available 24-hours a day, 7 days a week, without charge. Our staff will page a sign language interpreter to assist you.
