The problema#cs of final kissing balloon infla#ons
1) Recent and original concept has been proposed by Dr O. Darremont : Proximal Op#mizing Technique (POT) Hildick-‐Smith et al. Heart 2014 Hildick-‐Smith et al. Eurointerven:on 2010
But POT has not had precise quanOtaOve analysis 2) The use in our technical pracOce of the fractal geometry of arterial bifurca#on (in accordance with the law of the conserva:on of mass)
Murray CD. Proc. Natl Acad. Sci. 1926 Finet et al. Eurointerven:on 2007 Huo et al. Eurointerven:on 2010
3) Several recent and contribuOve publicaOons on FKB op#miza#on
-‐ Final proximal post-‐dilataOon is necessary aTer KB in bifurcaOon stenOng Foin et al. Eurointerven:on 2011
-‐ FKB or sequenOal dilataOon of the SB and MV for provisional stenOng Foin et al. JACC 2012
-‐ POT + FKB post-‐dilataOon with asymmetric balloon inflaOon pressure Mor:er et al. JACC Intervent. 2013
The objec#ves of this bifurca#on bench model study
1) Quan#fy the mechanical effects of the POT
2) QuanOfy the differences between: FKB approach (with its modified protocols) a]er POT and approach with no FKB but consecu#ve POT with SB balloon infla#on (re-‐POT)
concerning different endpoints: -‐ opOmal SBO (side-‐branch osOum) opening, -‐ no strut MAP (malapposiOon), -‐ circular arterial shape, -‐ no excessive arterial overstretch -‐ respect of the final fractal geometry of the bifurcaOon -‐ simplicity of the technique

CONCLUSIONS
Our preliminary results with 25 fractal bifurca#on models with Promus Premier® (Boston) : -‐ 1) quan#ta#vely validated the mechanical effects of POT
-‐ 2) showed by comparison with : -‐ FKB alone -‐ and POT + FKB with symmetric or asymmetric balloon inflaOon pressure that with re-‐POT with SB dilataOon provides the best mechanical results and is the simplest technique in 1-‐stent bifurcaOon strategy.
-‐ SBO almost free of strut (4.9%) -‐ a very low rate of global strut malapposiOon (2.6%) -‐ an almost perfect morphology of the carena and the wall facing the SB osOum -‐ a maintained circular cross-‐secOonal area of the MoV (rela:ve ellip:c ra:o : 1.0) -‐ the almost perfect respect of the linear law of coronary artery bifucaOon geometry (0.66)
