What is the OCFP’s OHT Planning & Delivery of Integrated Care Program?

The OHT Planning and Delivery of Integrated Care Program:

• Recognizes that family physicians are integral to successful system transformation

• Based on active and applied learning resulting from collaboration to build clinical pathways for complex

patients within their patient medical neighbourhoods

• Reflects delivery over an extended period of time

At the outset:

Each approved OHT is anchored in parameters defined by the Ministry of Health

During development & implementation:

OHTs are actively working towards creating clinical pathways and closing gaps

At maturity, OHTs will provide:

• Local solutions for health system navigation

• More timely access to specialists and community resources

• Decreased admin burden, freeing up time for care

• Improved use of technology

❑ Define the vision of their local OHT within the context of their local priority populations in their

attribution area.

❑ Interpret their practice and community level data to define and create plans around

integrated care for the priority patient populations within their OHT

❑ Analyze their patient-level data to proactively identify those who have the greatest needs

and establish a plan for coordinating services from multiple sectors to ensure these needs are

met through a Patient Medical Home/Neighbourhood lens

❑ Develop an approach to the OHT implementation based on analysis of the gaps between

population needs and local resources and reflecting the OHT building blocks that their local

team will be designing with their input, and the general timelines for any changes that are likely

to affect them

❑ Describe the priority populations identified for focus by their local team

Before each session:❑ Determine eligibility of activity for credits based on link

to Learning Objectives❑ Ensure that all speakers complete a Conflict of Interest

(COI) disclosure and send to the OCFP for review❑ If necessary, collaborate with the OCFP to address bias

During each session:❑ Present Disclosure slides (Toolkit, p. 10) ❑ Present overarching Learning Objectives (Toolkit, p. 4)❑ Provide the link to the online evaluation form❑ Ensure that at least 25% time is dedicated to discussion

or interaction.

Promoting a Certified Event

Please use the following language (exactly) on promotion

materials:

• This activity has been certified for up to [insert # of credits based on # hours] Mainpro+® credits.

• The OCFP’s OHT Planning and Delivery of Integrated Care Program overall has been certified for up to 60 Group Learning Mainpro+® credits.

attended/participated❑ Receive their certificate and use the information to
