Policy Title: Organizational Chart Policy 2

Purpose: The purpose of the organization chart is to identify NATP personnel as to their job titles,

oversight and implementation of the policies and procedures, lines of authority, and communication.

Scope: All NATP personnel

Responsibility: The NATP Administrator is responsible for the content development and

distribution of the organizational chart and Policy.

Policy: The Nurse Assistant Training Program (NATP) will develop and maintain an

organizational chart to include all personnel who perform NATP related duties.

Procedure:

1.0 The organizational chart will include the following elements:

1.1 Information blocks including titles and names of personnel showing lines of authority and

communication for all personnel associated with the training.

1.2 Each block of information will include the job title and name of the individual currently assigned

to that job and lines of authority.

2.0 The organizational chart will be reviewed and up-dated annually, or as necessary due to personnel

changes, job title changes or lines of authority.

3.0 Each time there is a change in personnel, job titles or lines of authority, the NATP

administrator will update the document and re-distribute to staff.

4.0 A copy of the organizational chart will be placed in the Policy and Procedure manual with this

policy.

Nurse Assistant Information Sheet General Information The Nurse Assistant Training Program (NATP) is approved by the California Department of Health Services (CDPH). The contact information for the CDPH is P.O. Box 997416, Sacramento, CA. 95899-7416, (916) 327-2445. The Nursing Assistant Training Program is a 16-week full-time course offered in Fall and Spring. This course is designed for students who have expressed interest in the entry level nursing profession. This course meets Title 22 regulations in taking care of the geriatric population in the long-term care setting utilizing skills in basic care, emergency care, and communication. Upon successful completion of this 6-unit program, and achieve the certificate of completion, are qualified to apply for and take the California Competency Exam, administered by the American Red Cross or the National Nurses Aide Assessment Program. Once the student passes the state competency exam, the California Department of Public Health will issue the certificate demonstrating the graduate is a new Certified Nursing Assistant pending criminal screening.

Admissions Procedure Applicants must complete and submit a Nurse Assistant application to the School of Nursing for consideration. Applications may be obtained at RCC School of Nursing website during application filing periods. Applications must be submitted electronically to [email protected]. Please verify current application filing period dates with Counseling Department of School of Nursing office at (951) 222-8405. Students must take Nelson-Denny Assessment and successfully demonstrate 6th grade reading/English competency. This test is offered by the Assessment Center, free of charge.

The Nursing Enrollment and Evaluations Specialist will evaluate each applicant for admission into the program. Applicants accepted into the program will be notified by e-mail.

Program Requirements Students accepted into the Nursing Assistant Training Program are required to submit health & physical examination, immunization records, American Heart Association (BLS) HealthCare providers CPR card, and complete a background check through Life Scan (fingerprints). Applicants must be free from communicable diseases, infections, psychological disorders, and other conditions that would prevent the successful performance of the responsibilities and tasks required in the training program. Based on guidelines from California Department of Public Health (CDPH), and clinical facility policies; nursing students must have clear criminal background and be fingerprinted to participate in clinical courses. The School of Nursing requires applicants to disclose prior misdemeanor and felony convictions. The applicant must explain the circumstance of the conviction and provide documentation of rehabilitation. The CDPH considers the natures and severity of the offense subsequent acts, recency of the acts of crime, compliance with court sanctions, and evidence of rehabilitation in determining eligibility for certification.

State Certification Requirements All students are required to meet the state requirements for hours of theory and clinical attendance. The California Department of Public Health (CDPH) requires a valid social security number for certification. The California Department of Public Health Licensing and Certification Program Aide and Technician Certification can be reached at (916) 327-2445.

Requirements for Eligibility 1. Be at least 17 years of age 2. RCC Application on file and eligibility to attend RCC 3. Graduation from an accredited high school or equivalent (G.E.D., High School Proficiency Examination) 4. English/Reading competency at 6th grade level or higher

Selection Procedure Selection of the Nursing Assistant Program will be filled on first-come first-served basis of qualified applicants. RCC reserves the right to designate a certain number of spaces for contract agreements and/or to meet grant-designated outcomes. If a student is not offered a space in the program, a new application is necessary in the next application period.

Expenses

Physical examination/lab work cost approximately $100-600 8. Background check/CPR cost approximately $220.00 9. It is recommended that students carry personal health and accident insurance including hospitalization. Policies

are available to college students at reasonable rates. The college provides liability insurance at NO cost to the student.

When planning expenses, students should be aware they may have to limit their hours of employment due to program requirements. Scholarships and grants and available to those who qualify.

Health Exam and CPR Certification Students will be admitted to the program pending submission of a CPR Card (American Heart Association BLS Healthcare Provider) valid for 1 year, and a completed health examination form with clearance permitting unrestricted functional activities essential to nursing practice in accordance with the American with Disabilities Act (1990). Health Exam and CPR Certification are not required at the time of application.

Background Check and Drug Screen All new and readmitting students are required to demonstrate a clear background check and drug screen prior to enrollment in clinical courses. See “Background Check/Drug Screen” policy for further details. The background check requires that students be able to provide a valid social security number. The process for obtaining the background check is available in the School of Nursing office.

California Department of Public Health The Professional Certification Branch (PCB), Aide and Technician Certification Section (ATCS) is primarily responsible for the certification of nurse assistants by ensuring applicants adhere to the California Health and Safety Code, Section 1337-1338.5. Citizenship and Immigration Status Verification for Certification The Federal Personal Responsibility and Work Opportunity Reconciliation Act requires government agencies to eliminate “public benefits” to individuals who cannot provide proof of their legal status in the United States. “Public Benefit” has been interpreted to include a certificate issues bt a state. This means that all applicant for CNA certification will be required to submit verification of citizenship or legal residence status in the United States. A certificate may not be issued until legal status in the United States has been confirmed by the Immigration and Naturalization service. Disclosure of Social Security Number/Tax ID Number If you fail to disclose your social security number or tax/ID number, your application for initial or renewal certification will be delayed or rejected.

Fingerprint Requirement One requirement to qualify for the certification examination is the submission of Live Scan fingerprints. These are evaluated by the California Department of Justice and the F.B.I. The Riverside Community College District Board of Trustees has adopted policies and procedures and has endorsed practices, which provide for the district and its employees and students to comply with all applicable laws relating to prohibition of discrimination based on gender, age, race, color, national origin, disability or sexual orientation.

Nurse Assistant Training Program

Job Description

Under medical or licensed nursing supervision, the Certified Nursing Assistant provides nursing

care to patients/residents/clients or inmates who are physically ill, disabled, convalescent or

geriatric; assist the physician or nurse with examination and treatments of patients; motivate

and assist patients in developing and utilizing their maximum potential for self-care; and do

other related work.

Provides physical support to assist patients/residents/clients or inmates in performing daily living

activities, including rising out of bed, bathing, dressing, feeding, toileting, walking or exercising,

social actions or rehabilitation programs; turns or re-positions bedridden; records vital signs,

height and weight; measures and records dietary intake and urinary and fecal output;

administers medications or treatments as directed; collects specimens; observes or examines

patients/residents/clients or inmates to detect symptoms that may require medical attention;

assists nurses or physicians in the operation of medical equipment or provision of care;

transports patients/residents/clients or inmates to treatment units, operating rooms or other

areas using wheelchairs, stretchers or movable beds; cleans and sanitizes patients clothing,

rooms, bathrooms, examination rooms, or other patient/resident/client areas; positions or holds

patients, residents, clients for surgical preparations; performs immediate and temporary

interventions in emergency situations; assists with admission, transfer, or discharge activities;

sorts, counts and distributes linens and clothing; assists in serving meals and special diets;

performs other tasks as required and as defined by the California Code of Regulations, title 22,

Division 5, Chapter 2.5, Article 3, 71835 through 72841.

Purpose: To establish the procedure and line of authority through which policies are established and to promote consistency in the development, modification and presentation of school policies and procedures.

Scope: Applies to all institutional policies, the governing principles that mandate actions with broad application throughout the school community

Responsibility: The NATP Administrator and RN Program Director are responsible for the content

development and distribution of the organizational chart and Policy.

Policy:

I. General All school policies fall within a greater hierarchy of laws, statutes and rules. School policies are subject to compliance with laws and regulations instituted by higher governing authorities as follows:

a. Federal laws and regulations b. State laws and administrative rules c. California Department of Public Health d. California Bureau for Private Postsecondary Education

II. Policy Development, Implementation, and Changes/Updates

a. In the interest of shared governance, the RCC SON Curriculum Committee may include faculty, administration, and students.

Procedure:

a. The RN Program Director and Administrator maintain all official school policies. b. The following policies and procedures in this manual have been approved by the

RCC School of Nursing- Nursing Curriculum Committee and full faculty. Any additions and or revisions are required to follow this same process before the policy and/or procedure is implemented and disseminated to students.

Purpose: To establish the duty of the instructors for completion of the students’ record in order to maintain current information on past and present students.

Scope: Applies to all NATP Faculty Responsibility: Administrator, Program Director, Instructors, and Nursing Enrollment and

Evaluations Specialist.

Policy: Student record is an official document that documents the NATP approved coursework in both clinical and theory setting.

Procedure: NATP instructors are responsible to record and complete quizzes, examinations, and attendance records of all the NATP students.

1. The Nursing Assistant Training Program instructors are responsible for completing the following students records:

a. Student theory attendance b. Student skills lab attendance c. Student clinical attendance d. CDPH theory checklist 276-c e. CDPH skills checklist 276-a f. Skills lab evaluation g. Student skills orientation checklist

2. Student records are kept in a locked storage room with limited approved access. The NATP instructor may keep attendance sheets with them during class but should be turned into the RN Program Director upon program completion.

3. The theory checklist (CDPH 276-c) and skills checklist (CDPH-a) must be kept in the students files locked in a cabinet located in RN Program Director office at all times. Skills Orientation Checklist form is to be signed by the student and instructor upon orientation to the facility.

4. Student records are kept for 5 years from the date of enrollment.

Policy Title: Criminal Background Clearance/CDPH283B Dept. Submission

Purpose: Securing a criminal background check prior to enrollment provides the School an important resource, which aids in the evaluation of the applicant.

Scope: Applies to all students

Responsibility: The Nursing Enrollment and Evaluations Specialist is responsible for collecting the CDPH283B application and Live Scan fingerprinting form from the student upon enrollment.

Policy: All students must complete the registration form and Live Scan fingerprinting upon

enrollment. The school shall submit students' CDPH 283‐B, Transmittal 283I and Livescan (Form BCII 8016) to the California Department of Public Health upon completion of the class orientation. Before registering for a Nursing Assistant program, students should be aware of the Disqualifying Penal Code Sections (ATC-98-4) that result in potential denial of their certification. CDPH determines whether you will receive certification.

Procedure:

1. When a student enrolls in the school, the Nursing Enrollment and Evaluations Specialist is the person who enrolls them. The Nursing Enrollment and Evaluations Specialist must make sure that the student fills out an application for the school and the top portions (Sections I- III)

of CDPH 283‐B and the Nursing Enrollment and Evaluations Specialist fills out Section IV. The Nursing Enrollment and Evaluations Specialist will also fill out the coversheet for student roster (CDPH2831) and make sure that the student turns in a completed Live Scan fingerprinting form BCIA8016 or the student may also turn in the Live Scan fingerprinting form at the orientation, which takes place the day before the start of the class.

2. RN Program Directors do not sign CDPH283B, until students successfully complete state and federal NATP requirements and eligible for CNA competency exams. RNPDs review CDPH276A, CDPH276C and student attendance, sign-in sheets (including make-up sessions) to verify successful NATP completion.

violation, must supply the following information to The Nursing Assistant Training Program:

7. California Department of Public Health will evaluate CNA applicants with criminal convictions

for any offence and either grants or denies criminal record clearance by reviewing evidence of good character and rehabilitation provided by applicants or information gathered by CDPH in relation to criteria outlined in Health and Safety Code Section 1337.9(c).

8. A student may not sit for the State Certification Examination until cleared by the CDPH. Nursing Assistant students cannot participate in the mandatory portion of training that requires direct contact with patients/clinical nursing facilities until CDPH 283B and fingerprints through Live Scan method have been submitted to the Department of Public Health, Licensing and Certification Program.

9. Because the applications for certification can be denied in the pre‐screening process, The Nursing Assistant Training Program adheres to the Department of Public Health recommendation that every individual applying for Nurse Assistant certification understands the health and conviction screening requirements before registering for the NATP program.

Students may be denied access to clinical facilities based on offenses appearing on the

criminal record, which may have occurred more than seven years ago. Students must provide schools with information allowing the school (and clinical facilities as necessary) access to the background check. If the students record is not clear, the student will be responsible for obtaining documents and having the record corrected to clear it. If this is not possible, the student will be unable to attend clinical rotations. Clinical rotations are a mandatory part of the CNA program; therefore, the student will be ineligible to continue.

Disqualifying Penal Code Sections If they have been convicted of any of the penal codes listed, CNA/HHA applicants will be automatically denied certification or ICF/DD, DDH, or DDN applicants will be denied employment. All CNA/HHA applicants should review this list carefully to avoid wasting their time, effort and money by training, testing and submission of fingerprints since they cannot receive the required criminal background clearance if they have been convicted of any of these violations.

Purpose: To comply with The Joint Commission, state, and local regulations regarding background checks

for healthcare providers; CAN students must have clear criminal background check and drug screening to participate in placement(s) in clinical facilities. The background check is not a requirement for the CNA program but will be completed after an invitation for admission is received. Background Check/Drug screen are required for registration in clinical nursing courses. The initial background check satisfies this requirement during continuous enrollment in the program; should the educational process be interrupted; a new background check will be required. Students who withdraw from or fail a nursing course will be required to submit a new background check/drug screen prior to re-admission. Scope: Applies to all students. Responsibility: The Nursing Enrollment and Evaluations Specialist is responsible for collecting thee

CDPH 283B application and Live Scan Fingerprinting form from the student upon enrollment.

Policy:

All NATP students must complete the registration form and Live Scan fingerprinting upon enrollment. The school shall submit students' CDPH 283‐B, Transmittal 283I and Livescan (Form BCII 8016) to the California Department of Public Health upon completion of the class orientation. Before registering for a Nursing Assistant program, students should be aware of the Disqualifying Penal Code Sections (ATC-98-4) that result in potential denial of their certification. CDPH determines whether you will receive certification.

1. Students will be required to obtain a clear or approved background check. The School of Nursing will

provide instructions to the student on how the student will apply and pay for the background check. 2. Students will be given a deadline date by which the background check results must be submitted to the

Dean/Director. 3. Students who do not complete a background check/drug screen by the deadline date will not be

allowed to register for classes. Students must provide consent to allow the school and clinical facilities, as necessary, access to the background check/drug screen results.

Drug screen (10 panel) 5. Students may be denied enrollment in the School of Nursing who have felony and/or related

misdemeanor convictions on their record(s). Convictions will be evaluated on a case-by-case basis. 6. Students denied enrollment due to criminal convictions may reapply to the program when it has been

seven (7) years since an offense, or when they receive a dismissal, expungement, or certificate of rehabilitation from the court. However, even with a dismissal, expungement, or certificate, students may still be denied access by clinical facilities, based on the nature of the convictions even though the convictions may have occurred more than seven (7) years ago. Clinical rotations are a mandatory part of nursing education. If a student cannot participate in clinical, he/she cannot complete the nursing program and, therefore, will be denied enrollment into the program. Each clinical facility has different requirements. The requirements of the clinical facilities are final.

7. Students on probation or parole or who have outstanding bench warrants or any unpaid citations,

restitution, etc., will not be permitted to enroll in the program until all outstanding issues are completed. 8. The following convictions, even if they have been dismissed, will prevent the student from being able to

participate in clinical rotations:

Assault/battery

Sexual offenses/sexual assault/abuse

Certain drug or drug related offenses

Alcohol-related offenses (without certificate of rehabilitation)

Other felonies involving weapons and/or violent crimes

Class B and Class A misdemeanor theft

Testing positive for drugs 9. The student understands that Background Check/Drug Screen results, while confidential, will be

reviewed by the Nursing Enrollment and Evaluations Specialist and the Dean of Nursing and may be reviewed by the Clinical Coordinator, Nursing Enrollment Committee members, and clinical agency personnel, as needed.

Procedure:

1. When a student enrolls in the school, the office clerk is the person who enrolls them. The office clerk must make sure that the student fills out an application for the school and the

top portions (Sections I- III) of CDPH 283‐B and the office clerk fills out Section IV. The Office clerk will also fill out the coversheet for student rooster (CDPH2831) and make sure that the student turns in a completed Live Scan fingerprinting form BCIA8016 or the student may also turn in the Live Scan

• General supervision of Nursing Assistant Training Program curriculum. • Lead the full‐time faculty in the division in curriculum development and instructional advancement in the field of health care (nursing) • Assess the instructional needs of students in the program and coordinate the development of a plan to address these needs. • Have a discussion with the instructors at least 2 times per class, once in the theory setting and once in the clinical setting to assess the professional needs of faculty and students and make recommendations to fill those needs.

• Maintain the primary responsibility for scheduling classes within the program following the California Department of Public Health approved Training schedule (CDPH 276 B).

Policy: The nursing assistant training program will follow the California Department of Public Health approved Training schedule (CDPH276-B) with the components of modules per CDPH- C and the clinical hours as listed on CDPH 276A. The Training Program Schedule must be approved by CDPH prior to being used.

Procedure: The RN program director will monitor the NATP instructors for implementation of approved training schedule CDPH283B that consists of 50 hours of theory and 100 hours of clinical as approved by the department. The training will be conducted per the approved CDPH 276B nursing assistant daily training program by reviewing the individual student record CDPH 276C for theory, and student skills checklist CDPH276A at least one time per week.

Policy: The Nursing Assistant Training Program will follow CDPH 283B Training Schedule to ensure all clinical modules and 100 hours are met, per Title 22, 71835 in the California Code of Regulations.

Purpose: To provide the benefit of individual attention of the students from the instructors and to comply with the California Department of Public Health Regulation. Scope: Applies to all students Responsibility: Administrator, Program Director, and instructors

Procedure:

A. Ratio of students (clinical setting) not to exceed 15 students to 1 instructor B. All clinical modules have lesson plans available for Instructor use, and Department review

upon request

C. The NATP Instructor is responsible to develop and implement lesson plans

D. The Clinical Instructor provides immediate supervision to students and has no other duties E. The Clinical Instructor provides demonstration of clinical skills at the nursing facility

(Department approved clinical site) and performs student return evaluations of all skills on

CDPH276A (Student Skills Checklist) at the nursing facility

F. All skills evaluated by Department approved Clinical Instructor will be with hand written

documentation, on the date the skill was performed

G. NATP cannot waive any skill on CDPH 276A form (also listed at Title 22, section 71835 of the California Code of Regulations.

H. NATP provides 100 hours of clinical training under immediate supervision of a

Department approved instructor, in a nursing facility

I. NATP maintains attendance sign-in/sign out sheet for every day of clinical, which minimally includes: date and time of clinical, printed/typed name and signature of instructor, printed/typed name of student, and signature of student. Student attendance sheets used by NATP to verify each student completed 100 hours of clinical training.

J. Clinical training is between the hours of 6 a.m. and 8 p.m.

Purpose: The purpose of clinical training is to enhance and add relevance to theoretical knowledge through which students develop the essential core competencies of professionalism, communication, and technical skills.

Scope: Applies to all NATP Students Responsibility: Administrator, Program Director, Instructors

Policy:

A. CLINICAL EXPECTATIONS

Students are expected to demonstrate growth in their clinical performance as they progress

throughout the program. Students must achieve certain learning outcomes

and competencies that demonstrate a safe level of clinical performance. Clinical performance is based

on the demonstration of safe nursing skills that ensure quality, safe, patient-centered nursing care, as

well as the display of professional behaviors and attitudes in all clinical settings. Students are expected

to be prepared for clinical practice and must understand their limitations and boundaries to prevent

avoidable errors. While the nursing faculty is available to assist students in their clinical journey,

students are expected to assume responsibility for the nursing care they provide to patients.

B. CLINICAL EVALUATION

1. Student performance will be evaluated during each clinical experience by qualified clinical nursing faculty. Formal clinical evaluations will occur

at the end of every module and as required to ensure that students achieve student learning

outcomes.

The results of the Clinical Competency Assessment will be considered in the student’s overall

clinical evaluation.

2. The nursing faculty may, at any time during the clinical rotation or clinical experience,

identify a less-than-satisfactory clinical performance or behavior demonstrated by a student

(See Clinical Performance Definitions below).

3. Students who do not demonstrate consistent clinical growth over the course of the training

program may be unable to achieve their student learning outcomes which may result in the

student's inability to progress in the course.

1. Satisfactory Clinical Performance: The student consistently demonstrates quality, safe, competent, professional nursing skills or behavior(s) in the clinical setting or lab.

The student is prepared, inquisitive, and engaged in learning opportunities that are available

during clinical experiences. The student is able to identify areas that need of improvement and

seeks guidance from the clinical faculty or another appropriate member of the health care

team. The student assumes responsibility and accountability for their own behavior and the

care

they provide for patients and the community. The student participates as an active member of

the intra/interprofessional healthcare team, communicating collaboratively and collegially

with other members of the team. Required clinical assignments and activities are completed

in a timely manner.

2. Needs Improvement: The student is able to demonstrate a satisfactory level of competency but

in order to do so may require more guidance or assistance from the clinical faculty than other

students performing satisfactorily at the same semester level. The student may lack consistency

in preparation, planning, and performance on required assignments. Students who earn a ‘Needs

Improvement’ in any student learning outcome(s) will be required to complete remediation as

determined by the RN Program Director and Instructor in order to achieve a ‘Satisfactory’

clinical performance. Failure to complete the required remediation in the designated time frame

may result in the student earning a ‘Minimal’ clinical performance or may interrupt the

student’s progression in the program. Students who do not demonstrate consistent clinical

growth in all student learning outcomes throughout the duration of the course, and therefore

continue to receive a ‘Needs Improvement’ clinical rating, despite remediation, in any student

learning outcome category at the end of the course will receive a ‘Minimal’ clinical

performance in that student learning outcome. The student must earn a ‘Satisfactory’ clinical

performance evaluation by the end of program or the student will earn a ‘Minimal’ clinical

performance in the identified area(s).

3. Minimal Clinical Performance: The student demonstrates severe inconsistencies in their

level of clinical performance or behavior(s), requiring a significant amount of guidance or

supervision from the clinical nursing instructor when compared to other students performing

satisfactorily at the same semester level. Despite assistance, the student is inconsistent in

identifying areas for growth or shows minimal improvement when given repeated

opportunities to demonstrate competency. The student is inconsistent in assuming

accountability and responsibility for their own nursing actions and clinical judgment. A

minimal performance may also be earned by a student who knowingly violates or neglects

any designated clinical agency policies and/or procedures.

a. Students who do not show consistent clinical growth in each student learning

outcome by the end of the NATP course will earn a minimal clinical performance

for that particular outcome.

b. A minimal performance may also be earned by a student who knowingly

violates or neglects any designated clinical agency policies and/or

procedures.

c. Since all course learning outcomes are equally important for demonstrating a student's

clinical competency, earning a minimal clinical performance for any of the course

learning outcomes will result in earning an overall minimal performance for the

clinical rotation.

d. At the end of the course, an earned minimal clinical performance grade in any of the

course learning outcomes will result in a minimal clinical performance as the final

clinical grade.

e. The student who has earned their first minimal clinical performance grade at the end of

the clinical rotation will progress to the next

rotation/semester with a detailed remediation contract developed with semester-level

faculty. The remediation contract may include, but is not limited to, nursing skill

practice exercise, simulation scenarios, case studies, written papers, and use of many

other clinical resources available to students. All clinical contract requirements must

be met and submitted to the RN Program Director in order for the student to earn a

satisfactory clinical grade in the next rotation or subsequent semester.

f. All students receiving a minimal clinical performance will be required to meet with the

RN Program Director and Administrator, as appropriate.

g. If a second minimal clinical performance grade is earned at any other time in the

program, the student will earn an unsatisfactory clinical grade and will be ineligible to

progress in the NATP program.

4. Unsatisfactory Clinical Performance: The student fails to demonstrate quality, safe, competent,

professional nursing skills or behavior(s) in the clinical setting or lab, despite repeated

opportunities for improvement and additional guidance or assistance from the clinical nursing

faculty and nursing staff. The student repeatedly lacks preparation or knowledge necessary for

providing safe nursing care. The student is unable to identify areas that need improvement and fails

to seek necessary guidance from the instructor or other members of the health care team. Because

they do not assume responsibility and accountability for their own behavior and the care they

provide for patients and the community, the student places the patient's safety in jeopardy. The

student fails to participate as an active member of the inter/intra professional healthcare team

which results in a communication break-down, thus jeopardizing patient safety. The student lacks

the appropriate level of clinical reasoning and judgment on which to base their clinical decision-

making. The student fails to utilize nursing informatics systems to effectively communicate,

manage and analyze data, prevent errors, and support decision- making. The student lacks self-

regulated learning behaviors and fails to seek additional learning opportunities or experiences.

Required clinical assignments and activities are inconsistently completed, not completed in a

timely manner, and/or do not demonstrate higher-level thought processes, despite repeated and

documented opportunities for improvement. A student may earn an unsatisfactory clinical

performance in the following ways:

a. The student's clinical performance is at an unsatisfactory level. Repeated

documentation and remediation strategies demonstrate that the student is unable to

meet student learning outcomes.

b. If an unsatisfactory clinical performance grade is earned:

i. The student will be ineligible to progress in the program and may receive an "F"

grade in the course, according to college policy.

ii. The student will meet with RN Program and a Contract for Readmission

and an Exit Interview Form will be developed in collaboration with the

student.

iii. The student will be required to meet with the RN Program

Director/Administrator, School of Nursing.

5. Unsafe Clinical Performance: Overall clinical performance is considered unsafe when a student's

action(s) or pattern(s) of behaviors reflects a substantial departure from that which is expected of

students at the same level under similar circumstances and when the student's actions have or could

have resulted in physical or emotional jeopardy to the patient. If at any time a student's clinical

performance is evaluated by the clinical nursing instructor to be unsafe or grossly negligent, the

student will be dismissed from the clinical area, receive an "F" grade for the course and be ineligible

for readmission to the program. A student who withdraws from any nursing course due to or following

an unsafe clinical performance incident will also be ineligible for readmission into the program.

Policy Title: Health Exam and Screening Requirements Policy 15

Purpose: RCC College maintains a health services for all students to promote health, prevent disease, and to provide care for acute illnesses. The student is responsible for maintaining good health practices. Student health and psychological services are available at all three college campuses. Students needs to be free from any health condition that potentially creates a hazard to self or others.

Scope: This Policy applies to all school students

Responsibility: Program Director, Instructors, and Nursing Enrollment and Evaluations specialist

Policy: Students are required to have a complete health examination prior to starting the NATP program, which ensures that students are in good health and are able to perform unrestricted nursing activities. Annual testing to ensure freedom from TB is required. If the PPD skin test converts from negative to positive during enrollment in the program, immediate medical follow-up is required. All students are required to have evidence of specific immunizations and/or titer levels as proof of immunity. Hepatitis B immunization is required (see American DataBank/Complio) for instructions regarding initial physical examinations, immunizations, and third semester physical examination requirements). Any condition described above which is developed by the student after admission to the program, may be considered sufficient cause for suspension or termination from the program.

PREGNANCY FOR NURSING STUDENTS

Pursing students who are or become pregnant must have medical approval to continue in the nursing program. Nursing students must also accept full responsibility for any risks to self and fetus associated with any class or clinical assignment. In each case of pregnancy, the student will be required to inform the Lead Instructor and the clinical instructor of her pregnancy and to file the Physician’s Clearance During Pregnancy form with the School of Nursing Health Coordinator. The student is required to notify the semester lead instructor for any change in her pregnancy status which may necessitate withdrawal from the program. Following delivery, written approval from the physician for unrestricted activity in clinical nursing practice must be submitted prior to return to class.

PHYSICAL ACTIVITY RESTRICTION

The student is required to report to the faculty any change in health status (examples: broken bone, skin lesions, chest pain, contagious disease, pregnancy, injury, back injury, surgery). The student is required to submit a statement to the semester lead teacher from the healthcare provider stating that he/she may participate in unrestricted functional activities essential to nursing practice. This statement must be on file prior to student having client contact.

Restricted physical activities may prohibit participation in clinical/hospital experience and may delay progress in the nursing program until the restriction is discontinued and unrestricted activities essential to nursing practice may be resumed.

If at any time the student is in the NATP program taking medically prescribed medication that may affect student’s performance, the student is required to inform the clinical instructor prior to providing patient care.

Procedure: Any illness or injury occurring when school is not in session is the responsibility of the student. If hospitalization, diagnostic tests, medication or referrals are required, any expense incurred will be the responsibility of the student. Students are encouraged to maintain private medical insurance. Injuries which occur in class or clinical are to be reported immediately to the instructor. All accidents/incidents require completion of written reports by the student and instructor that are required by the health care facility and/or RCC. Students accepted in the nursing assistant training program will be admitted to the clinical component of the program only after all health requirements have been completed and results have been submitted to the School. These requirements are mandated by the affiliating facilities in order to protect patients/residents, students, visitors and staff of the facility. A student must be in optimal physical and mental condition in the clinical area to ensure the safe and effective care of patients. If a student’s physical condition or behavior is symptomatic of substance abuse/emotional/mental illness, the policy on Alcohol/Drug Abuse, and Mental Disability will be followed.

Purpose: The Nurse Assistant Training Program is a concentrated course of study. In order to facilitate academic success, safe clinical performance, and meet program objectives, students are expected to demonstrate responsible attendance patterns. Successful completion of The Nursing Assistant Training Program is dependent upon 100% student attendance. Students are expected to attend all theory classes and all clinical sessions for which he or she is enrolled. All days missed are made up according to the California Department of Health regulations. The CNA Course has additional requirements of specific hours established by the California Department of Public Health (CDPH). As a student in this course you are required to attend the hours designated by CDPH for the course. You cannot miss any classes and there is only a minimal amount of time that may be missed and still meet the requirements of the course. Scope: Applies to all NATP students

Responsible Party: Instructor

1. The NATP Instructor is responsible to develop and implement make-up assignments (theory and clinical). The Instructor is responsible for scheduling with the student to do the makeup time according to the California Department of Public Health regulations.(APPENDIX 5)

Policy: The Attendance Policy of the Nursing Assistant Training Program is in conformity with the Attendance Policy of Riverside Community College District and the California Department of Public Health.

A. The NATP program conforms to the RCC’s policy on attendance:

All students are expected to attend every session of every course in which they are enrolled. Failure to

do so may indicate lack of serious purpose. Students who fail to attend the first class meeting and/or

week may be dropped from the class. Religious observances and military duty, however, are excused.

The student should inform their instructors prior to such an absence. Course work missed for

unavoidable cause may be made up with the instructor’s approval. Under no circumstances will

absence for any reason excuse the student from completing all the work assigned in a given course.

After an absence, it is the responsibility of the student to consult with the instructor regarding the

completion of missed assignments (RCC College Catalog and Student Handbook). B. Successful completion of the Nursing Assistant Training program requires the student

1. Participate in all class and clinical sessions. 2. Maintain minimum overall 75% grade in theory 3. Satisfactorily complete required clinical hours and skills. 4. Successfully pass all clinical skills.

Procedure: It is an expectation that the instructor will be notified prior to a student’s absence or tardy. When

an unexpected absence or event occurs, students should notify their instructor and explain the incident resulting in their absence as soon as possible. Any time within the program that a student’s lack of attendance and/or punctuality occurs at either a class or clinical session, additional course work may be required. Extended absences may require medical clearance before the student is able to return to class and/or clinical. Religious observances may be accommodated, if possible, and only if course/clinical outcomes can be met. The student may receive an incomplete, failing grade, or may be dropped from the program for absences/tardies that result in an inability to meet the student and graduate learning outcomes.

1. Any absence due to illness or emergency requires an explanation by the student to the instructor. a. A student absent from class for emergency reasons must personally notify the instructor or school at

least one hour prior to the absence occurring or he or she will be recorded as “ABSENT" 2. Absence of more than one classroom day and one (8 hour) clinical day may require the student to repeat

the entire course if the student fails to make arrangements for make‐up with the Instructor.

3. Supervised make‐up will be provided by the instructor for only one excused absence (up to 8 hours) of a theory and clinical day. Emergency absences may also require additional official documentation. Additional absences will result in being dropped from the program.

4. More than 2 day of absences from class days “theory” cannot be made up during the program session. 5. Tardiness is not accepted. Three tardies (late entrances) maybe considered the equivalent of one classroom

absence. Students may be dropped from classes or courses if tardiness becomes problematic. 6. The first occurrence of absenteeism or tardiness will be noted in the student’s file. Formal counseling by the

instructor and/or program director will be implemented after a second occurrence, at which time, a decision shall

be made by the instructor to either retain or dismiss the student from the program. The decision will be made

based on overall performance in the class, and reason for absence/tardiness. A third occurrence will automatically

result in program dismissal.

7. 7. Students who are absent or tardy from clinical and/or CNSL may be unable to meet the student learning outcomes of the course which may result in an incomplete or failing grade for the course.

8. If a student is ABSENT for hospital orientation, he/she WILL be dropped from the course.

Seminar: A student who is absent or tardy misses theoretical content related to the student learning outcomes of the course. It is the responsibility of the student to demonstrate professional behaviors to meet the student learning outcomes of the course. Make-up work may be assigned in accordance with RCCD Positive Attendance & CDPH requirements.

1. All Seminars will be video recorded. Students who miss seminar will be required to watch the seminar on campus in the line of sight of instructor within 3 days and complete required make-up assignment.

2. Videostreamed seminars will be available for student access on Mediasite during the course. Clinical: The faculty plans clinical learning experiences related to the student learning outcomes of the course. It may not be possible to provide these experiences a second time within the available time frame of the rotation. Students may be required to make-up missed clinical outcomes by:

1). Attending a clinical experience on an alternate day that may not correspond with their regular clinical assignment. 2.) Daily record of attendance will be kept. 3.) Tardiness (one minute after start time of class) will be noted for all classes. 4.) A student is expected to have met all state standards and follow policies. Excessive absences are grounds for dismissal from the program. Absences will be deemed excessive anytime being absent interferes with the students ability to meet course objectives, theory or clinical. 5.) If a student must leave class, theory and/or clinical early, he/she must sign out before leaving. Failure to do so may result in student not receiving credit for that day. 6.) It is the students responsibility to contact the instructor for any missed assignments. 7.) It is the students responsibility to fill out a clinical make-up form and contact the clinical instructor regarding clinical make-up. FUFILLMENT OF STATE MANDATED HOURS: All missed clinical time, including holidays, must be
