what is it and why should we care?

What is MAPP?
• A community-wide strategic planning process for improving public health
• Facilitated by public health leadership to help communities prioritize public health issues, identify resources for addressing them, and take action
How is this different from a department strategic plan?
• Community drives the process • MAPP provides the framework for a
community-driven assessment and plan • Community participation leads to collective
thinking • Results tend to be more effective and
sustainable • MAPP process brings diverse interests together
Phase 1: Organize for Success/Partnership Development
• MAPP is initiated by lead community partners coming together in an organized manner to prepare for planning
• Requires high level commitment from partners, stakeholders, and residents
• Health departments must continue to cultivate relationships with these entities and individuals to keep them engaged
• Successful engagement will lead to a successful implementation of a community health improvement plan
Phase 2: Visioning
• A shared vision and common values provide a framework for pursuing long-range community goals
• Vision and value statements provide focus, purpose, and direction

Community Themes and Strengths
• Provides a deep understanding of the issues residents feel are important
What is important to out community? What assets do we have that can be used to improve community health?
Local Public Health System Assessment
• The local public health system assessment is a comprehensive assessment of all of the organizations and entities that contribute to the public’s health
• This is broader than the health department What are the activities, competencies, and capacities of our local public health system? How are the Essential Services being provided to our community?

Community Health Status Assessment
• The community health status assessment identifies priority issues related to community health and quality of life
How healthy are our residents? What does the health status of our community look like? How does the health status compare to other communities?
Forces of Change Assessment
• This assessment focuses on the identification of forces such as legislation, technology, and other issues that affect the context in which the community and its public health system operates
What is occurring or might occur that affects the health of our community or the local public health system? What specific threats or opportunities are generated by these occurrences?
Phase 4: Identify Strategic Issues
• Occurs after a list of opportunities and challenges has been generated from the four assessments
• Participants identify linkages between the MAPP assessments to determine the most critical issues that must be addressed to achieve the vision
Phase 5: Goals and Strategies
• Goals and strategies are formulated that address each of the strategic issues identified in Phase 4
• They provide a connection between the current reality and the vision
• Together, they provide a comprehensive picture of how local public health system partners will achieve a healthy community
• They also can provide the framework for a performance management system
Phase 6: Action Cycle
This process requires a performance management system to monitor progress toward achieving the goals set forth in the community health improvement plan

Essential service system performance measured using the NPHPSP instrument can inform health department PHAB domain performance. MAPP and NPHPSP can be deliberately designed to meet standards in domains 1, 3, 4, 5, 7, and 10.

PHAB Measure 1.1.1 T/L • Document the process for CHA development • Must include various sectors of the community • Document that the partnership meets and
communicates on a regular basis to consider new data sources, review newly collected data, consider assets and resources that are changing, and conduct additional analysis
• Document the collaborative process used
Community Health Assessment PHAB Measure 1.1.2 T/L • Document the identification and description of the
community’s health and areas for health improvement – Data/information from various sources (and how
obtained) – Population demographics – Description of health issues and specific groups with
particular issues and inequities – Description of factors that contribute to specific
population health challenges (social determinants) – Description of existing community assets and resources
to address health issues

assessment were distributed to the community at large and input from the community was sought
• Document the gathering of information, collection of data, conduct of community dialogues, and/or identification of community assets specific to populations and/or geographic areas in the community where health inequities and poorer health indicators were identified

PHAB Measure 1.1.3 T/L • Document how the health department informed
partners, stakeholders, etc. of the availability of the community health assessment
• Document how the health department communicates the community health assessment to the public

– Broad participation of community partners – Information from community assessments – Issues and themes identified by stakeholders in the
community – Identification of community assets and resources – A process to set health priorities
Community Health Improvement Plan
PHAB Measure 5.2.2 L • Must provide a community health improvement
plan that includes: – Desired measureable outcomes or indicators of
health improvement and priorities for action – Policy changes needed to accomplish health
objectives – Individuals and organizations that have accepted
responsibility for implementing strategies – Consideration of state and national priorities

PHAB Measure 5.2.3 A • Must provide a tracking process of actions taken
toward the implementation of the community health improvement plan
• Document areas of the plan that were implemented by the health department and/or its partners

PHAB Measure 5.2.4 A • Must provide an annual report on the progress
made in implementing strategies in the community health improvement plan
• Document that the health improvement plan has been reviewed and revised as necessary based on the report required above
Summary
• MAPP is a great tool to assist with the community health assessment and community health improvement planning process
• PHAB does not require a specific tool to be used • The local public health system assessment is
currently not a PHAB requirement, it too is a tool • Use the concepts from the tools that makes
sense for your community(ies) • Remember to document the process!
