EDUCATOR’S REPORT

Our conference started off Friday evening with food and fellowship at a restaurant nearby the hotel conference site. We applaud the 35 guests who were able to attend. It was a record attendance for a pre- conference event. Thank you to Ruth Novak for organizing that event. Sarah Bowles, the perioperative manager at Sanford Bemidji, volunteered to be our Fall conference committee chair. She and her team of Cindy Conway, Marta Sjostrum, Danielle Newland and Maria Stay sacrificed their valuable time to give us a great day of education and networking. The conference location was the Hampton Inn and Suites located on the shore of Lake Bemidji. Our conference room had large windows with a spectacular view. The magnificent changing season into fall, was

evident in beautiful Bemidji. We had 112 in attendance. Deb Moengen our current MNDAKSPAN president opened the day with introductions. She also invited everyone to the next MNDAKSPAN conference to be held in Plymouth, MN on Feb. 10 and 11th and the Fall 2017 conference to be held at Breezy Point Resort in Pequot Lakes, Minnesota on September 29 and 30th.

Our first speaker was Janna Galloway RN, MS, CPHQ to present “Changes in Reimbursement: Value Based Purchasing” She is the Director of Quality & Performance Improvement here at Sanford Bemidji Medical Center. Janna has served in various other roles at Sanford. She was the Perioperative Nurse Manager, the Perioperative Supervisor and an OR Charge Nurse. She has held various surgical services positions. She obtained her MSN at University of North Dakota Grand Forks in 2012. Health care services are forced to look at cost and value to the consumer. Affordable health care changes have changed our reimbursement. Value based purchasing enables hospitals to improve income based on patient care improvements and outcomes. Your facilities are compared to national averages to determine payments. She expressed the rules set forth by the federal government are constantly changing. Janna challenged us to be vigilant with best practices and evidence based practices to keep your health care delivery in the highest reimbursement categories. She said the federal government goal is to have 90% of reimbursement tied to alternative payment bundles.

Dr. Craig Bruce was our next speaker with “The Changing Face of Outreach Medicine” Dr. Bruce completed his Bachelors and Master’s degree at University of South Dakota. He went on to obtain his DDS at University of Nebraska. He completed pediatric residency at The Children’s Hospital – Denver, Colorado and general residency at Phoenix Indian Medical Center, Phoenix AZ. He has a passion to serve the Native American population. Dental caries is the most common childhood disease. It is an even greater problem in the Native American population. He gave an overview of the dental surgeries he performs in the OR. He included pictures to explain before and after. Dr. Bruce includes education in his care of these children, especially diet changes.

Our conference service project is the Wolfe Center. It is a project by the Nameless Coalition for the Homeless in Bemidji. A representative briefly explained their mission. $285 was donated to their organization. Bemidji winter weather runs from November to March with frequent below freezing temperatures. Homelessness in northern Minnesota poses a variety of challenges. MNDAKSPAN was pleased to make a donation to this organization.

Barbara Forshier RN,BSN,JD, CAPA, CPAN was our last morning speaker. She presented, “Protect Your Professional License-What Every Nurse Should KnowAbout the Law & Your License.” Barb wears many hats. Barb began her nursing education at Lakewood Community College in White Bear Lake, MN. She continued her education at Augsburg College. Barb then went on to Hamline University Law School in St. Paul. She currently has her own law practice. She also practices as an RN at United Hospital in St. Paul. Legal issues in health care, is one of the most requested topics in the post conference evaluations. She

gave an overview of the types of law and terminology involved in malpractice cases. For example, “foreseeability” suggests we put side rails up because a patient could fall if left down. It is impossible to summarize her expertise in a few short sentences. We all valued her wisdom and recommendations to protect our nursing license.

MNDAKSPAN has a solid reputation for preventing hunger at conferences and possible adding a few extra calories to the diet plan of attendees. We all enjoyed a great lunch organized by the planning committee.

Our first speaker for the afternoon was Dr. Maxwell Gessner. He works at Bemidji Sanford. He has been practicing there since 2008. Dr. Gessner began his medical education at University of North Dakota. He completed his general residency in Akron, Ohio and his anesthesia residency at University of Virginia, Charlotte, along with a fellowship there. Dr. Gessner presented “Post-Anesthesia Emergencies and Complications.” He provided an overview of post op complications. Some studies suggest up to 24% of our patients face some type of complication after anesthesia. Abdominal and orthopedic procedures pose a greater risk. Dr. Gessner reviewed the potential complications in the respiratory, cardiovascular and neuro systems. What a great review of our perianesthesia practice with a skilled MDA.

Our next lecture was “Post-Operative Complications Related to Surgical Positioning”

Janelle Haugan RN, CNOR and Heather Sledge RN, BSN worked together on this presentation. Janelle began her nursing career as an LPN from Northwest Technical College Bemidji then continued her

education for RN at Northland Community College Thief River Falls. She is a circulator/charge nurse here in Bemidji. Heather began her nursing education at Finlandia University in Hancock, Michigan and obtained her BSN at Bemidji State University. Heather has worked at several different medical facilities and numerous patient care units. Most recently employed at Sanford Bemidji Medical Center for the past 17 years. They shared their expertise in preventing skin issues in perianesthesia. They emphasized the vital importance of pre- operative assessment of skin. Identify the high risk patients! Implement preventative measures! Assess the patients post op!

In honor of the newly certified nurses and the current certified nurses, we celebrated with ice cream. We enjoyed mini blizzards and lifted up the accomplishments of those hardworking nurses reaching their goals of certification. Our certification Director, Kristin Christensen encouraged everyone to consider certification. She provided ABPANC information at her display table and was available for questions throughout the day.

We welcomed Brenda Kingdon RN, CNOR, MSN, MSB, HCA, DNP as our closing speaker. She is widely recognized for her exceptional skills in leading team

building, financial analysis operational assessments, improvements and planning. She has been serving as the Director of Surgical Services in Bemidji since 2013. She has extensive experience that could fill several pages on this document. She presented, “Season of Change in the Nursing Profession.” She shared her love for nursing and her great appreciation for the profession. Brenda inspired us all to be proud of our work. She also shared some humor with her thoughts on changes in nursing. We all left with a greater sense of value in our work as perianesthesia nurses.

Thank you to all who attended our Fall conference “Seasons of Change” in Bemidji. We look forward to seeing everyone in Plymouth on February 10th and 11th.

Appendix D of the Candidate Handbook has a list of Study References. A frequently asked question is: Do I really have to get ALL of these?? The answer is no, BUT it is also not sufficient to study from just one resource. Choose multiple references that are most valuable based on your specific learning needs. Want a certain resource, but don’t want to fork out the cash? Try checking out your workplace or local library. See if a colleague has a copy that you can borrow. Consider buying used or trying the “renting” option offered on Amazon. Or form a study group and share the references amongst one another!

Expand and validate your knowledge and dedication to perianesthesia nursing. Not only will this give you a competitive edge to an employer, but you will attain an extraordinary sense of personal pride and accomplishment!

Implementing Geocoding for clients in a Minneapolis, MN

Public Health Agency (PHA)

Setting of project: A public health agency (PHA) in Minneapolis, MN focused on family health and wellness with a current client base of approximately 800 active and past clients. The US Department of Housing and Urban Development (HUD) in conjunction with Healthy Housing Solutions (HHS) and the city of Minneapolis, MN developed a summative numerical ranking of the 87 neighborhoods (“HUD Healthy Communities Transformation Initiative | Healthy Housing Solutions,” n.d.).

Problem addressed: The present geocoding system is isolated to Minneapolis, MN. Minneapolis is comprised of 87 neighborhoods, each with their own unique environmental characteristics. Geocoding to neighborhoods allows a more granular approach when evaluating community health.

Objectives of project: • Create a spatially referenced client database providing a system for maintaining confidentiality when

geocoding to the city of Minneapolis, MN to neighborhoods. • Develop and integrate a numerical coding into the electronic health record (EHR) for PHA clients

within the neighborhoods of the city limits of Minneapolis, MN • Determine client attributes social, demographic and health related to be included in the spatially

referenced excel database for joining with GIS for graphical mapping• Develop mapping process for transparent overlay of environmental characteristics and PHA client

population.

Intervention or change implemented, if any: Geocodes were developed for the 87 neighborhoods based on alphabetical order and included characteristics tiers top, middle, bottom along with their numerical ranking. These codes replace the Minneapolis geocode entered at time of referral intake into the EHR.

Actions and methods used to solve the problem and meet the objectives: Background research was conducted through email exchange with the city of Minneapolis Health Department GIS department and epidemiologist along with on-line research of the pilot study. The current clients were exported into Excel and addresses correctly formatted for geocoding. One large city map was created including streets and neighborhood boundaries and an alphabetized list of geocodes and neighborhoods. Appropriate coding and tagging was integrated into the electronic health record, PHDoc, for documentation and data extraction.

Geocode and Geographical Information Systems

Geocode defined by the American Heritage® Dictionary of the English Language, Fifth Edition: “To convert addresses, place names, or other conventional locators into geographic coordinates.” and is a subset of GIS spatial analysis, utilized as early as the 1960s. The 1970 to 80’s saw a rise in commercial vendors of geocoding software development. These platforms merge todays approach of organizing spatial information into database structures with the 1960’s approach of separating spatial information to generate maps. By the end of the 20th century geocoding is becoming more user-oriented, especially via open-source GIS software and the 2000’s continues in the rise of GIS software.

Outcomes: A successful system was developed allowing the incorporation of geocodes into the EHR with data extraction and geographical mapping.

Lessons learned (conclusions and recommendations for practice):Geospatial health can link health and illness to geographical location and identified environmental characteristics assisting in evaluation and plans for community health (Noble, Smith, Mathur, Robson, & Greenhalgh, 2012). The project was to develop a number system for geocoding client location into the electronic health record allowing geographical mapping to neighborhoods and comparison to established tiers and a summative numerical ranking of the neighborhoods. Geospatial health is but one more avenue that the nursing informaticist might use to assist in bringing healthcare into alignment with meaningful use and population health.

GOVERNMENTAL AFFAIRS UPDATE

November brings us to an interesting time. For starters, as I compose this column I think about the presidential elections that will occur on November 8th. Truly a life altering event that affects all of us for at least the next 4 years. By the time you are reading this submission, we will have a new president. Did you vote this year? Did you follow with who the ANA endorsed? The ANA has endorsed a candidate for each presidential election since 1984. Did you make an educated decision on your Minnesota, or Dakota constituents? I have to be honest here, looking into our states representatives is entirely new. However, as I learn more about our government I find myself wanting to get involved on some level. For me, that opportunity starts on November 8th but then will continue as I support upcoming events.

Do you find yourself wondering why Minnesota nurses are striking? I personally want to hug each and every nurse that works or worked for the five Allina hospitals that went on strike. Although an agreement was reached in October, a truly challenging time was had by all. Nurses become nurses so they can care for people. To care for people nurses need appropriate staffing and a safe environment to care for patients. One part of the issue is the safe patient standard. We have standards for all kinds of items, like the number of children that can be in a classroom, but we do not have a minimum standard for the number of

patients nurses are assigned to care for at one time. Some

interesting studies are posting about the shortage of nurses and how this is putting the quality of

patient care at risk. The trending research is

proposing that we are seeing an increase in

medication errors, patient falls,

often no lunch break, average time with each patient is down to 10 minutes, as well as the fact that our population is not only aging, but they are more complex patients with 15 or more medications to figure out as well as discharges that are rushed as patients spend less time in hospitals after procedures. As the holidays quickly approach, I wish you all happy and contented Holidays with family and friends.

This Fall has been busy for your Region 3 Director! I have been fortunate to be able to travel to MAPAN’s, INSPAN’s and WVSPAN’s Fall Conferences. The topics that each Component has offered for their education (like your Component) have been timely and captivating topics. I gave the attendees at the WVSPAN Conference an assignment: For your next educational offering, bring along one non-member and let them catch the fever!

National Conference will be here very soon. Make plans now to meet your PeriAnesthesia colleagues in Indianapolis from April 30-May 4, 2017. Come and enjoy Component Night as you meet up with old friends and new attendees. Get in the fast lane and “put the pedal to the metal” for your learning!

Not only will you be taking advantage of your ASPAN membership rights, you will also be helping your component in work toward a working strategic plan and Gold Leaf realization.

As you prepare for the Holiday season, I encourage each of you to maintain (or begin) your activity with your Component. Contact your Leadership Team and volunteer to help. You don’t have to do anything big… Give yourself the gift of involvement with your professional growth and development. Your leaders are looking for help with little things. Perhaps you’re ready for a slightly bigger (and very rewarding) challenge by involving yourself with the ASPAN Board. I encourage you to get your hands in the dressing and get involved. Are you looking for a mentor? ASPAN has the ability to assist you with the “Emerging Leaders.” ASPAN is happy to assist you in this adventure. Involvement

with ASPAN is NOT a life sentence, but a sentence of excitement, networking and support.

Review of Strategic Goals:

The Board of Directors reviewed the components stated goals and action plans after a few minor changes were made at the 8-18-16 meeting. Complete copies of the strategic goals will be posted on the website for your review.

LDI:

Kristin and Andrea attended the ASPAN sponsored Leadership Development Institute in September. They enjoyed a weekend filled with great information about how to improve local component’s membership experience through things like Facebook, Twitter, and other social media. If you are not already a follower of MNDAKSPAN on Facebook, check us out!

They also learned about new educational opportunities that ASPAN is offering to its members like 1 credit CEU’s on line.

Financial Review:

Andrea reported on the financial status of the organization, our assets exceed our liabilities, and the Board made some financial decision about improving the interest rates on our funds currently being held in CD.

Reimbursement Policy:

The Board held a lengthy discussion about the reimbursement of any member attending the National Conference and other national/international events. A decision was made to place tighter controls to expenses in an effort to control costs.

National Conference:

In the past, the President and President Elect have been the Representative Assembly members during the ASPAN National Board meeting. A decision was made that any member of the Board can represent MNDAKSPAN at RA with preference going to the current President as one of the members.

2017 Winter Conference:

Plans are being finalized for the winter 2017 Conference. It will be held at the Crown Plaza in Plymouth. We will again be offering a $5.00 Friday pre-conference event for MNDAKSPA members. For non-members the fee was raised to $25.00 as a way to encourage membership. A drawing will take place for any member who recruits a brand new member to MNDAKSPAN. There will be a Vendor Reception and a Poster presentation following the Friday Conference. If you or someone you know would like to present a poster on a research project or a process improvement, please contact a board member to reserve your spot. An evening “Meet and Greet” event will be held on Friday evening, location yet to be determined.

2017 Fall Conference:

Plans are underway for our Fall 2017 Conference event and the Board has selected Breezy Point on September 29-30th 2017 in the Brainerd Lakes area. More details to come. If you are interested in participating in the planning committee, please contact a Board Member.

Certification Scholarships:

There were 2 certification scholarships awarded this testing cycle. 1 was a recert and 1 was a new certification that passed both tests.

New Business

Get a Member Campaign:

The Board approved a “Get a Member” campaign for the 2017 Winger Conference. Any current MNDAKSPAN member that gets a new member to sign up from now until the Winter Conference in Plymouth will be entered into a drawing for a free, 1 year membership to MNDAKSPAN. So grab your friends and get them to sign up!

Open Position:

Kari Motis, Director of Membership has stepped down from her position. If you are interested in learning more about serving on the Board, please contact a Board member.

Site Visits:

The Board approved a new opportunity for any hospital, free standing surgery center or any place that Peri-Anesthesia nurses are taking care of patients in our region to have a site visit. If you are interested in having a member of the Board come and speak on standards/guidelines, certifications and the benefits of membership, please contact Deb Moengen, President of MNDAKSPAN.

Recruit new members to MNDAKSPAN and win!Tap into your network of colleagues and help MNDAKSPAN grow! As a current member, each new member you recruit will grant you one entry into a drawing for a free MNDAKSPAN membership. As you recruit more members, your opportunity to win increases. A new member is considered someone who has not held membership with MNDAKSPAN for the last two years and joins as an Active Member.

Recruiting TipsDo you have a vast network of potential members but aren’t sure how to approach them or what information to provide? See below for a list of recruitment tips.    

• Identify the best prospects. Review your network of professional colleagues and identify who could benefit from ASPAN/MNDAKSPAN membership.

• Know the benefits of membership. Review the Membership tab at aspan.org and Membership Awards and Renew/Join tab at mndakspan.org to provide prospects with more information about membership benefits.

• Give examples. Provide your colleague with examples of how your ASPAN/MNDAKSPAN membership has benefited you in your career. A positive testimonial can be a great recruitment tool!

• Invite your prospects to upcoming events. The winter conference is just around the corner…February 10th and 11th in Plymouth, MN.

• Close with a plan of action. Walk them through the process of applying for membership. • Follow up. Check back in a few weeks to see whether they have joined and ask if they have

any questions.

Contributions and Feedback to PAR Excellence

The key aspect of better representing MNDAKSPAN activities throughout the year is through member submissions to the newsletter. Even if it is just a short paragraph, an entire article, or a single photo, we warmly welcome all contributed submissions to the newsletter.

Ideas to consider:

• submitting an article or editorial • sharing photos from conferences or other member activities • debuting an excerpt from your research or evidence based practice project • sharing a successful program, strategy, or initiative Please feel free to contact the Director of Publication, at [email protected], with submissions, questions, comments or suggestions. Submission deadlines are the two weeks preceding the earliest publication date.
