To all incoming clerks,

We would like to take this time to give our deepest congratulations and warmest welcome for being selected to the clerkship program at MedStar Washington Hospital Center. We all sincerely look forward to working with you in the upcoming months. We understand the hard work and dedication you have put into your education thus far and hope to continue to guide you on your path to residency. W e have taken great measures to ensure a strong curriculum for our residents and our externs. We wholeheartedly believe we have one of the best training programs in the nation and hope that your clerkship experience here reflects that.

In order to better prepare you for your upcoming rotation, we have assembled a clerkship manual to be reviewed prior to your arrival. Included in this manual is:

1. Program Overview 2. List of Teaching Faculty 3. Clerkship rules and responsibilities 4. Articles to be reviewed

Required materials: white lab coat, bandage scissors, and pen with note pad

Your clerkship will begin on the first Monday of the month. On the first day of your clerkship, you will

meet in the Graduate Medical Education (GME) office located on the 6th

floor (via the B elevators), Suite 6A-126, promptly at 8am. When driving down Irving St, head south on First St, and park in the hospital parking lot to your right. You will check in with GME, receive ID badges, scrub machine access, computer access, and parking. Once all is completed, have the front desk page the podiatry resident on call (202- 801-1478 or 202-801-1908) and they will meet you. You will receive a tour of the hospital, answer any questions you might have, and then dismiss you to Georgetown University Hospital (GUH). Upon arrival at GUH, please park in Garage 1 (Entrance 2 from Reservoir Rd). Enter the main entrance into the main building and continue down the hallway into the Gorman Building. Take the elevators in the Gorman

Building to the 2nd

floor, turn left and follow the hallway to Gorman 2051. Ask for Allison Jones to check in and receive a badge. Then page the GUH Limb intern at 202-405-3498 to meet them.

Our program manager, Shari Heyward, will contact you 6 weeks prior to the start of your clerkship. If you have any questions or concerns, please feel free to contact her at [email protected]. We wish you the best of luck with your clerkship month!

Sincerely,

Dr. John Steinberg and the MWHC Residents

Student Clerk Responsibilities

1. Each clerk will spend two weeks of their month at MedStar Georgetown University Hospital (one week with the Limb intern and one week in the OR) and two week at MedStar Washington Hospital Center (one week on the inpatient service and one week on the outpatient service).

2. Clerks will be notified either in person or via text when and where we will be

meeting to round the next morning by the resident. The residents will dismiss clerks at the end of the day when all the floor work and cases are done for the day.

3. Clerks will be provided a copy of the inpatient list. It is to the resident’s discretion

to determine how much time is required in advance to prepare for rounds. Depending on the inpatient census, this may require arriving an hour prior to rounds.

4. Clerks are responsible for breaking down dressings to specified patients prior to

the start of rounds, making sure the rooms are stocked with adequate supplies needed prior to rounds, and having supplies readily available during rounds.

-Supplies should include: 4x4 gauze, kerlix, Ace wraps, #15 blades, suture removal kits, betadine swabs and bottle, culture swabs, probes, packing, steri-strips, tape, saline flushes

5. Clerks will be assigned cases on a daily basis, and these cases can be at the

hospital or any one of the various surgical centers that students my go to. The clerks will be notified about what case they will be attending and it is the responsibility of the clerk to prepare accordingly. A copy of the case list for the week may be found at [email protected] (password: ilovefeet!).

6. Clerks will be allowed to participate in cases as long as the clerk can adequately

show that they are competent in using proper sterile technique. You will be responsible for assisting the resident in setting up the OR, which can include pulling gloves and gowns, making sure the tourniquet is available, drawing up local anesthesia if needed, and helping move the patient to and from the OR table.

7. Clerks will be expected to know appropriate methods of charting for patients

including inpatient progress notes, preop notes, and postop notes.

8. Clerks are required to help cover a weekend on call during their time at MGUH

and MWHC. When on call, the clerk will assist the on call resident in seeing consults, staying for late cases, and weekend rounds.

9

9. Clerks are required to attend weekly academics – Wednesdays 4:00pm-6:00pm

with all residents; the 1st & 3rd Wednesdays there is cadaver lab in the autopsy

room from 3:00pm-4:00pm; the 2nd Wednesdays there is grand rounds from 6:00pm-8:00pm. When at MWHC and on office rotations, clerks are also required to attend McGlamry review Tuesdays 6:45am. A copy of the McGlamry schedule may also be found on [email protected], as well as a copy of the textbook.

10. Clerks may be asked to give small presentations or present journal articles

during the month by Dr. Steinberg. Each clerk will be expected to present a 10- 15 minute PowerPoint presentation on a topic preapproved by the residents or Dr. Steinberg on the last Wednesday of the month. Clerks are allowed to use the

medical library on the 2nd floor of MWHC at any time.

11. Clerks are encouraged to make suggestions to the residents about how their

non-surgical time should be used. It is the residents’ responsibility to make sure the clerk is adequately prepared for other rotations, as well as interviews and boards. Clerks may be taken through a series of case workups and mock interviews throughout their clerkship month.

12. Attire: Clerks are expected to wear clinic attire or business casual attire and a

white coat when in clinic and academic meetings. Scrubs will be provided by the hospital and surgery centers for OR cases.

13. Clerks are not allowed to keep scrubs or badges used at the hospital or any of

the surgery centers. Clerks are expected to be present every day for the length of the rotation. Any time off or early dismissal during the clerkship will need to be approved by Dr. Steinberg. Clerkship evaluation forms should be directed to the clerkship coordinator at the last Wednesday academics of the month.

14. A personal vehicle is highly encouraged during the month as sharing a ride with

another student will be logistically difficult given the off site rotations.

15. In case of inclement weather or other travel concerns, the student clerk should

contact the chief resident and the GME office. The student shall be excused from the rotation assignment if there is any concern for the safety of their travel to a location.

16. Most importantly, you are here to learn and we are here to teach. We want you to

get the most out of your rotation so please do not hesitate to ask questions or ask for help when needed.

10

Patient Consent Protocol: No student or resident will perform an invasive exam or procedure without first presenting to an attending and obtaining consent to proceed. In addition, an appropriate chaperone must be present in the room throughout the invasive exam or procedure. Invasive exams include but are not limited to: pelvic exam, rectal exam, breast exam, or any examination of genitalia. Invasive procedures include but are not limited to: injections, nail procedures other than debridement, incision and drainage, biopsy of any tissue
