Only serious matters requiring immediate attention should be coursed directly to the Advertising Director’s Office. In matters concerning any request or accommodation in behalf of any costumer, the counselor must see to it that such request is within approved policies and procedures in order to avoid embarrassment on the part of all concerned. TO: ALL EMPLOYERS/EMPLOYEESSUBJECT: Issuance of Employees’ SS Number Card The following procedure in the issuance of Employee’s SS number card will be implemented effective April 1, 1976.

1. Prospective job applicants requesting SS numbers will accomplish Personal Data Record, SSS Form E-1, at the Prior Registration Office, 1st floor of the SSS main office in East Avenue, Quezon City or in any Regional or Provincial office.

2. Upon proper accomplishment of the SSS Form E-1, prospective job applicants will be given a number, indicated in the accomplished SSS Form E-1.

3. Duplicate copy of the SSS Form E-1 will be given to prospective job applicants.4. Upon employment, the new employee will present the SSS Form E-1 to his employer who

will use the same SS number in reporting the employee to the SSS thru the R-1A form (Initial & Subsequent List of Employees).

5. Upon receipt of SSS Form R-1A (Initial & Subsequent List of Employees) from the employer, the SSS will issue a computer generated SS number Card to the new employee thru the employer.

The issuance of the employee’s SS number Card after actual employment will provide the employer the opportunity to compare whether or not the number appearing in the records and the number given by the employee are the same. Furthermore, the issuance of the SS Number Card to SSS members upon employment confirms the establishment of the individual’s account with the SSS and denotes his eligibility for the varied benefits as provided by law for participating members.

Employer and employee members are requested to use consistently the correct SS number indicated in the SS Number Card to assure accurate and proper posting of contributions to the member’s account.

(P) Related to the cleanliness and beautification drive being enforced by this Office, the following instructions are hereby given: (P) 1. All heads of offices, specifically bureau directors,chiefs of service, and directors/chairmen of attached agencies are hereby directed to strictly enforce and assume responsibility over the cleanliness and beautification campaign order by the president within their respective office premises and among their staff. Monthly compliance reports to this effect shall be submitted to the Assistant Secretary for Administration. (P) 2. A Department-wide Cleanliness and Beautification contest is being launched starting September 16, 19--. Prizes will be awarded to deserving winners during the DOL Anniversary Celebration on December 8. The panel of Judges of the different offices on October 18, 19--. (P) As this campaign is a continuing regular activity, efforts toward the attainment of the objectives of the drive should be intensified.
