Influenza virus vaccine/tetanus vaccine
Bullous pemphigoid in elderly patients: 2 casereports
A man and a woman, both aged 84 years, developed bullouspemphigoid shortly after they received influenza virus vaccine[‘Previgrip’] and tetanus vaccine, respectively.
The man developed a blistering eruption with tense blisterson his palms and soles and urticarial lesions on his trunk theday after his vaccination. The woman developed pruritus 24hours after her vaccination and a generalised blisteringeruption developed 1 month later. Histology of skin lesionsfrom both patients showed subdermal bulla with lineardeposition of IgG and C3 along the basement membrane zone.Circulating anti-basement membrane zone antibodies weredetected in both patients. Their symptoms responded totreatment with clobetasol.
Author comment: ‘The mechanism of induction of bullouspemphigoid by vaccination is not clear. Vaccination couldreveal a subclinical bullous pemphigoid by enhancing theautoimmune response: this may be related to bullouspemphigoid "induced" by infections.’Fournier B, et al.
