A. Early Voting Information: Registered voters may cast their

ballots during the 15th through the day prior to an election by personal appearance at any of the Early Voting locations. No photo ID is required and no reason is necessary for Early Voting. All locations are open the following days/hours: Monday, February 13 through Saturday, February 18 from 9 a.m. to 5 p.m. Sunday, February 19 and Sunday, February 26 from 10 a.m. to 4 p.m. Monday, February 20 through Friday, February 24 from 9 a.m. to 7 p.m. Monday, February 27 from 9 a.m. to 7 p.m.

B. EarlyVotingElectionOfficials: Early Voting election

officials have the following responsibilities. 1. Pick up and drop off materials

at 69 W. Washington on designated days.

2. Set up the voting room on the designated date.

3. Arrive at your location by 8:00 a.m. and work until 6:00 p.m.

4. Process voters by completing the necessary forms, saving the voting record on the laptop computer, activating a voter card and provide instructions for using the Touchscreen Voting Units.

5. Complete all the various record keeping forms.

6. Remove the result cartridges and printer from the Touchscreen Voting Units each night. Secure in the Workstations.

7. Secure the equipment and forms each night and at the close of Early Voting.

8. Reattach the printer and reinsert the result cartridges on the correct Touchscreen Voting Units every morning.

EarlyVotingElectionOfficialsAttendance Information:

1. For any employment questions call Human Resources at 312-269-7950. If you can’t report to your site, you must call this number prior to 8 a.m.

2. You must sign in and out on the Time Report everyday in order to be paid correctly.

3. If you are late or leaving early, you must indicate the correct time on the Time Report.

4. You must follow the dress code:

a) Business casual dress Monday through Thursday. No sweat-pants, no baggy clothes. Dress neat, not sloppy, as you are representing the Board.

b) Casual dress on Friday and Saturday. Jeans may be worn.

c) No political items may be worn including buttons, t-shirts, hats, etc.

d) No slogans of any type are permitted on clothing, e.g. Nike, etc.

e) Hats should not be worn in the Early Voting polling place room.

EarlyVotingElectionOfficialsConduct:

1. You must conduct yourself in a professional manner at all times.

2. No profanity/foul language.3. No loud music.4. No use of personal cell

phones during working hours in the voting room.

5. No use of personal laptop computers, iPAD, iPOD or DVD players or headphones during working hours.

6. At no time is food/drink permitted at your work stations, please eat and drink away from the equipment and work materials.

7. You may not bring your child with you to work at your site.

8. If you violate rules and procedures, you may be subject to termination.

JudgesofElection:At every Early Voting location, two of the Election Officials will also serve in the capacity of Judges of Election. One Democratic and one Republican judge will be assigned to each location. The Judges of Election will sign affidavits and give assis-tance when necessary. All Election Officials and Judges of Election are equal in authority at the Early Voting location and are responsible for all duties at the location. The Administrator is in charge of all workers at each location.

C. EarlyVotingAdministrator: The Administrator is in charge of

the Early Voting Site and will have a Board issued telephone, a key chain containing a Workstation key, a Safe key and a Touchscreen Voting Unit key. The Early Voting Administrator must be able to handle all normal duties in processing early voters and will also be responsible for the following:

1. Call the Early Voting Hotline to Check in and out Everyday.

2. ManageElectionOfficials Ensure the election officials are following procedures and acting appropriately.

3. ScheduleBreaksandLunches In order to keep the voting

process running smoothly, the Administrator will schedule the breaks and lunches so that the location is not short-staffed during busy voting periods. The times for breaks and lunches may vary from one day to the next. Ensure election officials are taking breaks accordingly and not taking more time than allowed.

4. VerifyTimeRecordSheets The Administrator must verify

the working hours of the other Election Officials and sign the daily Time Record Sheet.

5. Crowd Control During the Early Voting day,

the Administrator will monitor the flow of voters into and out of the voting room, or del-egate someone for this task. During busy voting periods,

the Administrator should collect voter cards and pass out ballot receipts. The Administrator should also help at the other workstations during breaks and lunches.

6. Periodically monitor the Campaign Free Zone. Ensure the “No Electioneering” cones haven’t been moved and the area is clear of campaign materials.

7. Ensure the Board issued telephone is charged and is answered during work hours.

The Board issued telephone is for official business only. Must not be used for personal calls.

CHAPTER 2: EQUIPMENT SETUP

A. General Setup Information The Early Voting room will be

set up on the designated date. The Touchscreen units will remain in place for the duration of Early Voting. The printer and results cartridges will be removed at the end of each day. The cords and ground covers can remain taped/secured to the floor. The chairs, and extension cords with ground covers should also be arranged and put in place at this time. In addition, all the signs should be posted. A sample Early Voting room setup is on page 7. The Card Activator Device and laptop computers will remain plugged in and in place in the drawers of the Workstations, ensure the Workstations are locked and sealed every evening.

B. Workstations Each location will receive

two Workstations (A & B). Workstation A contains a safe and both have two (2) pull out drawers to hold the laptop and card activator device. Each drawer has an electrical power strip and data line connection. The main power cord pulls out of the workstation and the front also contains the connection for the data line which will then be plugged into the wall jack. The Workstations will contain the supplies and forms as listed on

the Supply List. All material will be locked inside the Workstations each night. A green seal must be used to secure the locked Workstations each night. Record the green seal number on the Daily Ballot Statement.

Electrical Power Cord – when you pull this cord from the workstation listen for clicking sounds. When this sound occurs, the cord will remain in place. When you pull on the cord and do not hear this sound, it will retract. You will keep this cord plugged in through the duration of Early Voting.

C. SetuptheTouchscreenVotingUnits

Set up as many units as your space will accommodate. Do not connect more than three (3) units together with the Daisy Chains. Store the lids in an out of the way place in your voting room. The instructions and diagrams in this chapter will show you how to properly set up this equipment, including how to use the audio ballot controller and headphones for the audio ballot. The Instructions for set up are on pages 12-15. Information on troubleshooting the Touchscreen Voting Units is

on pages 16-17.

D.TouchscreenPrinter A printer is attached to each

Touchscreen Voting Unit and will print a paper version of the voter’s cast ballot as a review and

paper record backup (the voter verifiable paper audit trail or VVPAT). The Early Voting Election Officials are responsible for changing the paper rolls in the Touchscreen Printers. The Touchscreen Voting Unit will inform the voter when the paper supply is low and needs to be changed. See pages 18-19 for instructions on changing the Touchscreen Printer Paper Roll. Be sure that the Touchscreen Voting Unit serial number is indicated on the printer so you can reattach the correct printers to their units the following morning. The printer must be attached to the correct Touch-screen Voting Unit or the totals won’t match.

E. SetuptheLaptopComputers1. The laptop computers will be

placed in the right drawer of each Workstation and remain there throughout Early Voting. The power cord plugs into the back of the laptop computer and the mouse plugs into the USB port on the right side.

2. Plug the card activator device into the USB port on the left side of the laptop and place in the left drawer of the Workstation. The card activator device will remain plugged into the laptop for the duration of Early Voting. The pictures on page 20 will show you how to set up and use this equipment.

3. The drawer has a power strip and data line to plug in the laptop. Another data line will need to be plugged into the front of the workstation and into the wall jack. If you place the laptop in the right drawer, use the top connection, if you place the laptop in the left drawer, use the bottom connection.

4. Open the laptop cover and press the power button located above the keyboard in the center of the unit. Instructions will be provided to login to the Board’s system.

5. At the end of the day, log out of the system and power down the laptop. However, do not unplug the power connection or turn off the power strip.

6. Place the card activator device inside the same drawer as the laptop so that you may close the drawer and secure until the following day.

7. Ensure the Workstations are locked and sealed at the end of each day. Store the laptop case in the Workstations.

Set up the Card Activator Device The Card Activator Devices will

be inside the laptop computer carrying case. The instructions and diagram on page 20 will show you how to set up this equipment. The Card Activator Device will remain in the Workstation, ensure the Workstations are locked and sealed at the end of each day.

F. LogintoBoard’sSystemonLaptopComputers1. Login information will be

provided in a separate document for security purposes. Refer to these laptop computer login instructions when logging in to the Board’s system.

2. You are ready to process voters after you complete the login procedures (screen-shot 1 on page 27) and you are at the EV Voter Search Screen (screen-shot 2 on page 27). You can search for all voters from this screen. The voters found in a search will be coded with a colored circle (screen-shot 3 on page 27). Green indicates voters who have not cast a ballot. Yellow indicates Vote By Mail voters where the Board has not received a valid voted Vote By Mail ballot. These voters may cancel their ballot and early vote. Gray indicates that the voter must show a picture ID. Red indicates voters who have cast a ballot, or Vote By Mail voters where the Board has received a valid voted ballot. Pink indicates voters who have cast a provisional ballot.

A. The touchscreen should be placed on the floor with the storage covers facing up. The storage covers are marked “Audio”, “Leg Assembly” and “Power Cord”. Also, the yellow and black holes will be facing up.

NOTE: If easier for the election officials, this step could be performed with the unit on a table.

B. Remove each cover and remove the contents. Replace the covers. Unit should remain on the floor or table. Place power cord and audio head-phones to the side.

NOTE: The leg assembly is a one-piece unit.

A. Pull the leg assembly apart so that the upper brace is fully extended and the lower brace is bent upward. AT THIS POINT, THE UPPER AND LOWER BRACES ARE NOT CONNECTED.

B. Place the black bottom ends into the black slots. Place the yellow bottom ends into the yellow slots. Press the legs into the slots until you hear a “click”.

C. Once the legs are securely in place, insert the pin located on the upper brace into the hole located on the lower brace. The braces should resemble an “X”.

D. Extend the lower portions of each leg until the retention pins “click” into place. Make sure the legs are completely extended.

E. Two election officials should carefully roll the unit on its side with the main handle facing up and then lift the unit off the floor or table and onto its legs.

A. Unfasten the four latches and remove the lid. Each latch has a silver locking tab, which must be pushed to the left for the latch to be unlocked.

B. Remove the privacy panels and curtain from the unit and set aside.

NOTE: THE PRINTER IS ATTACHED TO THE TOUCH-SCREEN UNIT.

C. Gently raise the screen and printer and place the position-ing bar in the second position. Printer cable should already be plugged into both the printer and touchscreen.

D. Attach the left privacy panel to the left side of the touchscreen. Attach the right privacy panel to the right side of the printer. You will hear a click when the privacy panels are completely inserted. Using the Velcro, attach the privacy curtain to the top portion of the privacy panels. The privacy curtain should be attached so that the flap covers the back of the touchscreen and printer.

A. Plug the power cord into the right side of the touchscreen then into either a wall outlet or extension cord. Make sure the cord will not cause a voter to trip and fall. Place ground covers over cords on the floor. Use tape to secure the cord to the floor, if necessary. Once the unit is plugged in, the green AC ON light must be lit, if not, plug into a different outlet. The AC ON light is located above the plug on the right side of the unit.

B. Verify that a red seal is on the Cartridge Ports (results cartridge) door. If the seal is missing, immediately call the Board at 312-269-1603.

The verification process for all seals should be completed on Set-up Day and Opening Day with the Seal Accountability Form.

Follow these steps when a voter requests an audio ballot:

A. Lift the audio controller away from the unit. Plug the headphones into the bottom of the controller.

B. Place a set of headphone covers on the headphones. After the voter uses the headphones, remove and dispose of the used covers and replace with new covers.

C. The headphones can be placed on the back of the unit when not in use by a voter.

G. After the zero proof report finishes printing, the screen will display the message “Waiting For First Voter”. Touch “Start Voting”.

H. You will notice two green lights on the front of the touchscreen, one on the printer and one below the screen. If the green light on the printer is flashing you will have to correct the problem or call the Board at 312-269-1603.

I. Verify that the public counter located at the bottom of the screen shows 0. If not, call the Board at 312-269-1603.

J. The front of the touchscreen should be facing a wall, so that the voter can have privacy while voting.

TOUCHSCREEN PAPER CHANGING INSTRUCTIONS

1. Retrieve a new paper roll, a black sleeve and the roll of scotch tape.

2. Remove the right privacy panel and curtain.

3. When changing the paper roll, do not remove printer from unit.

4. Break the red seal, A) twist off the wing nut and B) remove the printer cover. C) Tear or cut the paper between the two rollers (on the face of the printer). D) Push the yellow locking tab and slide the used paper roll and black sleeve off the yellow reel. Record the serial number on the Touchscreen Printer Service Statement.

5. Roll the loose end around the completed paper roll and tape using the labels found with the Touchscreen Printer Service Statement. Write your location number, date, serial number of Voting Unit, public counter number and election official initials. Place the used paper roll inside Envelope 47EV.

6. Unlock the green paper release lever by pulling it up (this will release the pressure on the paper roll). The green light on the printer will flash when the paper release lever is unlocked.

CHAPTER 3: MORNING PROCEDURES BEFORE EARLY VOTING BEGINS

A. ElectionOfficialWorkstations Open the Workstations and pull

out both drawers. The Voter Cards should be by the Card Activator Device. Set out all the forms you will be using as listed on the Supply List. Be sure that the Voting Instructions are visible so that you can instruct the voters on using the Touchscreen Voting Units. Verify that all the signs are posted. See the diagram on page 7 for table layout for the workstations.

B. LogintoBoard’sDatabasewithLaptopComputers

Turn on the laptops and login. Follow the instructions listed in Chapter 2, Section F. Set the card activator device and voter cards in the left side drawer. The laptop computer is ready when you are at the EV Voter Search screen (screen-shot 2 on page 27). If you cannot get to this screen, call one of the numbers listed on the front cover for the Computer Support Hotline.

C. FirstMorningOnly/OpenPolls On the first morning, remove

results cartridges from the safe and insert in the Touchscreen Voting Units. Turn the power on, and when the units instruct you to open polls, break the yellow seal on the “Polls Opened/Closed” door and turn the knob to “Open.” Seal the door with the red seal. Use the Polls Open/Closed Serial Number Statement

to record the yellow and red seals used for this process. The Polls will remain open and the door locked until 7:00 p.m. on the last day of Early Voting.

D. InsertResultsCartridgesintheTouchscreenVotingUnits

Remove the results cartridges from the safe, insert them in the correct Touchscreen Voting Units and lock them in the units with the red seals. Record all the seal number information on the Daily Ballot Statement.

E. ReattachTouchscreenPrinters Beginning on the morning of day two, remove the Touchscreen printers from the Workstations and reattach them to their corresponding Touchscreen Voting Units. Plug the printer cables into the Touchscreen Voting Units and turn on the power. If attached properly, the green light will appear at the bottom of the printer.

F. RecordthePublicCounterNumbersontheTouchscreenVotingUnitsontheDaily Ballot Statement

When the Touchscreen Voting Units have warmed-up and display the “Ready for Next Voter” screen, record the Public Counter numbers. The Public Counter numbers will be dis-played on the lower left of the screen. Every Touchscreen Voting Unit Public Counter number must be recorded on

a Daily Ballot Statement. This number must match the closing number for the same unit from the previous day. On the morning of the first day, the Public Counter must read “0.”

G. Set out the No Electioneering Cones

The blue “No Electioneering” cones should be placed 100 feet from the entrance to the Early Voting room by using the 100-foot string to measure the correct distance. For additional informa-tion see page 22, “Maintain the Campaign Free Zone.”

H. CollectPollwatcherCredentials Collect pollwatcher credentials

from those who wish to observe during the voting day. These credentials need to say “Early Voting” and the correct date must be indicated. Credentials are placed in the 47EV envelope. For additional information and samples of the Early Voting Credentials, see pages 23-25, “Who’s Allowed in the Early Voting Polling Place.”

Electioneering is prohibited within 100 feet of an early voting polling place. Such behavior includes the soliciting of votes, wearing candidate buttons or badges, hanging campaign posters, distributing sample ballots or campaign literature, and talking about candidates, political parties and/or referenda.

Each early voting polling place will have two 18" blue cones with the wording “No electioneering beyond this point” printed on each cone. A 100 foot string will be provided for you to measure and mark off the 100 foot area.

The 100 feet is measured from the room where voting is conducted. However, the following applies:

A. If the early voting polling place room is located on the ground floor, then the cones shall be placed 100 feet from each entrance to the polling place room. If the 100 feet ends within the building, the cones shall be placed at the exact spot where the 100 feet ends.

B. If the early voting polling place is located on a floor above or below the ground floor, then the cones shall be placed 100 feet from the nearest elevator or staircase used by voters on the ground floor to access the floor where the polling place is located. If the 100 feet ends within the building, the cones shall be placed at the exact spot where the 100 feet ends.

Electioneering is allowed, by law, beyond the Campaign Free Zone during the time that an early voting polling place is open. Individuals have the right to engage in politi-cal discussions, pass out literature, etc. on any early voting polling place property while the polls are open, as long as they are beyond the Campaign Free Zone.

Remember: Only the area within the Campaign Free Zone is the responsibility of the Election Officials. You are not required to monitor or patrol the area outside of the Campaign Free Zone.

WHO’S ALLOWED IN THE EARLY VOTING POLLING PLACE

Voters, while in the process of voting, are allowed in the early voting polling place without any type of credential. All other individuals must have a credential, an Election Board ID or law enforcement ID in order to enter the early voting polling place. They include:

Board Employees have official identification to show the early voting election officials.

The Board Investigator will check on early voting location during the voting days. The Investigator should sign the Investigator Sign in Sheet every time they visit your location. Other agents from the Board of Elections or State Board of Elections may appear at your early voting polling place. All Board of Elections’ agents must present proper identification badges.

OtherLawEnforcementOfficers

After displaying their official ID, representatives of the United States Attorney’s Office, the Illinois Attorney General’s Office, the Cook County State’s Attorney’s Office, the Chicago Corporation Counsel’s Office and the State Board of Elections, are entitled to:

In any location where election materials are handled, the representative shall surrender to the election officials a copy of a letter of authority from the Board of Elections containing the name of the representative. No representative from any other office is authorized to handle election materials. The letter of authority should be returned in Envelope 47EV.

A voter may bring children under the age of 18 while they vote, as long as the children are not being disruptive.

ChicagoPoliceOfficers

Chicago Police Officers are assigned to periodically visit early voting locations. If a police officer is needed for an election-related matter, call the Board, at 312-269-1603. If there is an issue involving public safety, call 911.

MediaandExitPollsters

Members of the news media are issued a special media credential that authorizes admission to early voting locations to photograph, with the permission of the early voting election officials and voters. Media personnel may not, in any way, interfere with the conduct of the election. Media credentials are also included in your supplies.

Credentialed media may:n Film activities in the polling

place, so long as they do not interfere with the balloting process.

n Film any and all election officials or pollwatchers.

n Ask the election officials to report how many ballot applications have been completed.

n Film voters, so long as the filming does not violate the secret ballot. The media should respect the requests of any voters who ask not to be filmed.

Members of the news media and other authorized organizations presenting “exit polling” credentials to the early voting election officials may interview voters outside the location, even if they are within the Campaign Free Zone—so long as they are not inside the room where voting is being conducted.

POLLWATCHERS

A pollwatcher is a person who represents a candidate, political party, citizens’ organization, or a proponent or opponent of a question of public policy. The following are examples of who might be a pollwatcher:

Although these individuals may use these terms to describe themselves, they are all POLLWATCHERS. All pollwatchers must be registered voters in the State of Illinois. A pollwatcher is required to have a credential in order to stay in the early voting polling place and observe.

Number of Pollwatchers allowed in an early voting polling place at the same time are:

Two (2) pollwatchers per candidate;

One (1) pollwatcher per political party at a primary

election, two (2) pollwatchers per political party at a general election;

One (1) pollwatcher per qualified citizens’ organization;

One (1) pollwatcher per proponent or opponent of a

question of public policy.

An early voting credential is issued by the Board of Elections and stamped or signed by a candidate whose name appears on the ballot, or the presiding officer of a citizens’ organization. The credential must be signed by the pollwatcher and given to the Early Voting election officials when the pollwatcher enters the early voting polling place for each day of Early Voting. Return all credentials in Envelope 47EV.

Pollwatchers may substitute for each other during the day as long as the number of pollwatchers in an early voting polling place, at the same time, does not exceed the number allowed. Each pollwatcher must present and surrender their own credential to the election officials. If a majority of the election officials determine that the early voting polling place has become overcrowded with pollwatchers so as to interfere with the orderly conduct of the early voting, the election officials shall, by lot, limit pollwatchers to a reasonable number. Call the Board at 312-269-1603 for assistance.

Pollwatcher conduct

Pollwatchers, with proper credentials, may remain in the early voting polling place all day for morning set-up and evening breakdown. They may station themselves where they can see the actions of the election officials. Pollwatchers may not interfere with the orderly conduct of the early voting and may not handle election materials. Pollwatchers may bring violations of the law or improper procedures to the election officials’ attention.

Pollwatchers may call the Board at 312-269-1603 with questions.

To enter an early voting polling place:

A. The pollwatcher must give the election officials a credential each day;

B. The pollwatcher must have a separate credential for each location visited;

C. The pollwatcher may leave and return without an additional credential within the same day.

CHAPTER 4: PROCESSING VOTERS AND ISSUING VOTER ACTIVATION CARDS

WHO MAY VOTE Any voter who completes an EV Application for Ballot (Form 14EV) and who currently resides at the address listed on their active registration record, may vote without signing an affidavit, unless his or her qualifications are successfully challenged or has a saved record indicating a returned voted ballot.

A. ApplicationProcessforVoter(Form 14EV and Laptop Computers)

One election official at your location will be directing the voters to the Workstations and then to the Touchscreen Voting Units. The other election officials will be stationed at the Workstations, by the laptop computers and by the card activator device. The laptop election officials should process the Form 14EV Early Voting Application. The card activator election officials should assist with additional voting forms, voting instructions, activating voter cards and handing out receipts.

Follow the instructions below for each active registered applicant who has not previously voted. Ask the voters to complete Section 1 of the 14EV Early Voting application while they are waiting to be processed. Clipboards and extra pens are available for this purpose.

1. Voter completes Section 1 of the Form 14EV Application for Early Voting Ballot with the following information (see 14EV diagram on page 28).

a. Print Name and residence Address.

b. Print date of birth. This item is not required by law, but is very helpful in finding the voter’s registration record.

c. Check box for party affiliation – NOTE: Only in a Primary Election.

d. Ask the voter to sign in the signature box.

2. Find the voter on the laptop computer using the search screen.

3. The colored dot before the voter record indicates the voter’s status. If the status is “Green”, the voter is eligible to early vote. If the status is “Gray”, the voter is eligible to early vote after showing a picture ID. You can process the voter without additional affidavits if the 14EV applica-tion information matches the information in the voter record. If the color is Yellow, Red or Pink, or, Green with a different address indicated on the 14EV, go to Chapter 5, Voting Forms and Procedures.

4. Verify the date of birth and signature on the voter’s registration record. If there is no signature in the registration record, ask the voter to verify the last four digits of his/her social security number and continue.

5. Initial the “Registration & Signature verified by:” box. The Early Voting election official will then complete Section II and the top right-hand boxes on the form.

6. Using the information on the voter’s screen, you must record the following additional informa-tion on the application (refer to screen-shot 4 to view voter record).

D. Audio feature for Voter Card Activation

1. At the bottom of the voter record, next to the Save button, is a field labeled Act Type, which is short for activation type. The standard default for regular early voters is “Visual.” The dropdown menu also includes the Audio.

2. For an audio activated card, the election official should direct the voter to the touchscreen unit and assist with the audio ballot controller and headphones, if needed. If the voter wishes to use his or her own headphones, the election official should plug the voter’s headphones into the audio ballot controller. After a voter uses the headphones, remove and dispose of the used covers and replace with new covers.

For visually impaired voters, inform the voter that the lower right corner of the voter card is cut so that the voter will know how to insert it into the touchscreen unit. You can offer to insert the voter card for the voter.

Provide the basic instructions for the controller. The Square Button in the middle is the Select Button. The Right and Left Arrows moves forward and backward through the ballot. The Up and Down Arrows move through the list of candidates for a selected office.

4. After voting, the voter must return the voter card to the election

official. Issue a Ballot Receipt (Form 10EV) once the voter has returned the voter card.

NOTE: During the day, one election official should check for fingerprints or smudges on the touchscreens. If necessary, use the anti static wipe, which is located in the black supply bag, to clean the screen. Do not use any other product to clean the touchscreen.

C. NumberandInsertApplicationfor Early Voting Ballot in the Binder

The applications, except for provisional ballot voters, will be consecutively numbered and inserted into the binder. Each laptop should use a binder. Write the Date, Site #, Location and Laptop # (ABS #) on the front of the binder. For each Early Voting day, start a new binder and start with the number “1.” The application for a provisional voter will be placed in the plastic sleeve of the voter’s Provisional Voter Affidavit. If you have voided an Application for Ballot, place it in the Envelope 47EV.

IMPORTANT:Only process voters one at a time so there is no duplication of numbers on the Early Voting Applications for Ballot (Form 14EV).

3. After voting, the voter must return the voter card to the election official. The election official may assist the voter in returning the voter card, if necessary. Issue a Ballot Receipt (Form 10EV) once the voter has returned the voter card.

E.CheckingtheStatusofaVoter Card

You can check the status of a voter card from the main search screen on the laptop computer. At the bottom of the screen is a “Card Status” button. Insert voter card into the card activator device and click on the card status button. If the voter card has not been used, the message will say “Activation #____ Status: Not Voted.” If the voter card has been used, the message will say “Activation #___ Status: Voted.”

If a voter attempts to insert a voter card into the Touchscreen Voting Unit after he or she votes, the following message will appear on the touchscreen:

“The voter card is not valid. Please ask for assistance”

To eject the voter card, the employee must press and release the green button on the back of the touchscreen unit.

Check the status of the card to verify it has been voted.

CHAPTER 5: VOTING FORMS AND PROCEDURES

If a voter decides to vote by mail, packets with ballot applications, instructions and a return envelope are available and included in your supplies. If you find that a voter is only qualified for a provisional ballot, inform the voter that they can register and vote a Grace Period Ballot.

A. Voting by Consolidated Voter’s Affidavit (Form 1)

An affidavit is a sworn statement made in support of a person’s qualifications to vote. Depending on the reason, the election official must complete the affidavit, the appropriate box on the front of the form must be checked and supporting affidavits on the back must be signed and additional acceptable identification might be required. An election official or the designated Judges of Election can sign a supporting affidavit.

Spanish, Chinese and Hindi facsimiles of Form 1 are included in the supplies.

The categories for Form 1 are as follows:

1. Change of Address - Box # 2 A voter who has moved within

30 days prior to the election and still resides within the State of Illinois. Voter must complete the Affidavit (Form 1), check Box #2 and complete the Supporting Affidavit B on the back of the form or provide two pieces of identification, both with current residence address, one post- marked within the last 30 days.

Acceptable identificationincludes: driver’s license social security card utility bill employee identification card credit card civic, union or professional

association membership card public assistance card library card lease or contract for a

residence photo ID issued by a college

or university, accompanied by either:

– a copy of the applicant’s contract or lease of a residence– any mail delivered to the applicant at his or her current residence address postmarked not earlier than 30 days prior to the date of the election

A verification of registration card is NOT acceptable.

2. Cancellation of Vote By Mail Ballot - Box # 3

If a voter record is marked with a yellow colored dot, this indicates the voter applied for a Vote By Mail ballot. The voter must complete the Affidavit (Form 1), check Box #3 and indicate which category authorizes his or her voting in the Early Voting location.

a. I have submitted my Vote By Mail ballot, or a portion of my torn and/or mutilated ballot, to the election officials for cancellation.

b. I applied for, but never received, my Vote By Mail ballot.

c. I received a notice that the Board rejected my Vote By Mail ballot.

d. I returned my Vote By Mail ballot but the Board did not receive it.

Once this form has been completed, both the voter and an election official must sign the form. Also, a 14EV Application must be completed. Change the Vote By Mail type field to EV – Early Voting – Registered, and save the record.

PLEASE NOTE that if a voter indicates they received a Vote By Mail ballot but did not return it to the Board and they do not return the unvoted Vote By Mail ballot to the Early Voting Election Officials, they must vote by Provisional Ballot. Change the Vote By Mail type field to EVP – Early Voting – Provisional, complete the 276P Affidavit and save the record.

Form 1 or Form 276P must be returned to the Board in Envelope 47EV along with any returned and canceled Vote By Mail Ballots. Do not open any Vote By Mail envelopes. Write the date and location number on the front.

3. Assisted Voter - Box #4 or #5 A voter, who qualifies for

assistance, must make a request to the election officials for assistance.

A voter, who is unable to speak, read or write the English language, must complete the Affidavit (Form 1) check Box #4 and complete the Supporting Affidavit C on the back of the form.

A voter who, because of a physical disability, is unable to mark his or her ballot must complete the Affidavit (Form 1) check Box #5 and complete the Supporting Affidavit C on the back of the form.

5. Federal Offices Only Ballot - Change of Address more than 30 days ago within the City of Chicago - Box #7

A voter who has moved to another address within the City of Chicago more than 30 days prior to the election may vote a Federal Offices Only ballot after completeing the Affidavit (Form 1) and checking Box #7 and an Application for Early Voting Ballot (Form 14EV).

In addition, a voter who has moved less than 30 days, but does not have the IDs to complete Form 1 Box #2, may choose to vote a federal office ballot using Form 1 Box #7.

Or, inform the voter that they can register and vote a Grace Period ballot.

Language Assistance

As required by Section 203 of the Federal Voting Rights Act, the Board of Elections provides election materials in English, Spanish, Chinese and Hindi.

Election officials may provide assistance to foreign speaking voters by using the book “We Speak Your Language”, which provides voting instructions

in 16 languages. If verbal assistance in a foreign language is required, call the Board at 312-269-1603.

The voter may be assisted by a person of his or her choice. If the voter needs assistance, but has no one to assist him or her, two election officials or the designated Judges of Election shall assist the voter.

Both the person giving assistance and an election official must complete the Supporting Affidavit C on the back of the Affidavit, Form 1. The person giving assistance must be handed a copy of Form 1A. The election official must also indicate that the voter received assistance by checking the “Was Assisted in Voting” box on the Early Voting Application for Ballot (Form 14EV).

Intoxication is not a valid reason for receiving assistance in voting.

No voter may be assisted by the voter’s employer or agent of that employer or an officer or agent of the voter’s union.

A voter who uses the audio controller and headphones is not required to complete Form 1.

C. CHALLENGING A PERSON’S RIGHT TO VOTE

A voter may be challenged on the basis of qualifications or identity by an election official, a pollwatcher or by another voter. The challenger must state a specific reason for objecting to a person’s right to vote. Thechallengermustdirecttheobjectiontotheelectionofficialsandnottothepersonbeingchallenged.

Guidelines for Deciding a Challenge

The election officials must be fair and impartial in deciding whether to uphold or deny the challenge. The voter must have the opportunity to present evidence as to his or her identity or qualifications to vote before the election officials decide on the challenge.

Election officials should:

Ask the voter to display current identification with his or her name, address and photograph, if available.

Ask the voter to provide specific information contained on his or her registration record (date of birth or last four digits of their social security number).

Ask the challenger to provide proof to substantiate his or her challenge, if the challenge is not based on first-hand knowledge.

After all evidence has been given, all election officials must vote on the challenge.

If a majority of the election officials reject the challenge, the voter may vote without completing an affidavit.

If a majority of the election officials agree that the voter is not qualified to vote in the precinct, the voter must be advised of the reason for the election officials’ decision and the voter must be permitted to vote a Provisional Ballot.

D. VOTING A PROVISIONAL BALLOT

WhoareProvisionalVoters Provisional Voters are those who

do not qualify for the previously mentioned voting procedures. If the voter does not have a registration record, you must create a “Provisional New” record for the provisional voter. The button for “Provisional New” is on the search screen at the bottom. Click on the button, enter the voter’s information and save. The new record will be displayed and have the Vote By Mail type

field set to EVP – Early Voting – Provisional and the ACT (Activation) Type field set for Provisional. When you save the record, you get a temporary registration number, a Provisional ID number and a Provisional Activated Voter Card.

Any voter who falls into one (or more) of the following categories may vote a provisional ballot:

Check 1st box if: Voter indicates he/she is registered

to vote and you do not locate the record on the computer. (New record)

Check 2nd box if: Voter is challenged by an election

official, pollwatcher or voter and a majority of the election officials agree with the challenge.

CompletingaProvisionalVoterAffidavit,Form276PThe voter and an election official must complete the 3-part Provisional Voter Affidavit (Form 276P) prior to voting. The voter may provide information to the election official to support his or her claim to be a registered voter. Additional documentation supplied by the voter should be placed in the plastic sleeve on the reverse side of the affidavit. Or the voter must submit documents

to the Board of Elections within 7 calendar days of the close of the polls on Election Day supporting why his/her provisional ballot should be counted.

The process for issuing a ProvisionalVoter Affidavit (Form 276P) is asfollows:

1. Write the date, the precinct and the ward on the top portion of the affidavit.

2. Complete Section A of the affidavit, checking the reason for voting a provisional ballot and the precinct, ward and ballot style information which can be determined from the new record located on the laptop computer.

3. Have the voter complete Section B. Make sure it is filled out completely.

Form 14EVAll provisional voters must complete the Form 14EV application for Early Voting ballot and check the “Provisional Ballot Voted” box. The completed Form 14EV will be placed in the plastic sleeve of the Form 276P. Do not number the Form 14EV or add it to your blue application binder.

SaveProvisionalRecordIf the voter has an existing registration, change the Vote By Mail type field from EV – Early Voting – Registered to EVP – Early Voting – Provisional, which will change the ACT (Activation) Type from “Visual” to “Provisional”. Save the record and record the activation # on Form 14EV and the Provisional ID # on the top of Form 276P.

CreatinganewLaptopProvisionalRecordA new record will need to be created for a provisional voter without a registration record. Follow the procedures listed below to create a new temporary registration and saved record.

A. From the search screen, click on the “Provisional New” button at the bottom of the screen and a temporary record field will be displayed.

B. Using the information on Form 276P and 14EV, enter the voter’s information into the fields.

1. When entering address, you must use the correct suffix. If you tab from suffix and it is a valid Chicago address, the zip code will be inserted. If it is not a valid Chicago address, a message will be displayed asking if it is a non Chicago address. If it is, click yes and continue. If not, call the Board for assistance with the Chicago address. Complete the remaining fields and save the information (screen-shots B.1. see page 39).

2. When the information is saved, a temporary record will be displayed with Vote By Mail type field set to EVP – Early Voting – Provisional and the ACT (Activation) Type set to Provisional (screen-shot B.2. see page 40).

C. Insert a voter card into the Card Activator Device and save the record (screen-shot C. see page 40).

D. Record temporary registration number on Form 14EV and record the Provisional ID# on Form 276P and then exit the record (screen-shots D. see page 40).

E. Finish processing the Provisional voter. 1. Give the voter the pink copy

of Provisional Voter Affidavit (Form 276P).

2. Remind voter that if they are required to provide the Board of Elections with additional documentation in order for their provisional ballot to be counted, they must do so within 7 calendar days of the close of the polls on Election Day. The Board’s address is included on pink copy of the affidavit.

3. Instruct the voter on the Touchscreen Voting Unit, including the use of the stylus and not removing the voter card until it ejects at the end of voting.

4. Direct the voter to any EV Touchscreen Voting Unit.

5. Voter must return the voter card once they have completed their ballot.

6. Keep the Provisional Voter Affidavit (Form 276P) with the application binder to be returned on the following day in the 47EV.

B.2. After you save the voter’s information, this temporary record will be displayed. C. You will receive a message that the Early Voting Provisional information was saved and the card was activated. Click on ok to continue.

D (a). When the record was saved, it created a temporary VRN (voter registration number) at the top left of the record that you will record on the 14EV application. A. Touchscreenresultscartridge The results cartridges are sealed in the back of each Touchscreen Voting Unit during the day. The results cartridges will be removed each night, placed in a bag with the Touchscreen Voting Unit serial number, and locked inside the safe inside Workstation A. The results cartridges will be reinserted into the correct Touchscreen Voting Units the following morning and secured with a red seal. Record the seal numbers on your Daily Ballot Statement. The power on the Touchscreen Voting Units must be turned off when removing or reinserting the results cartridges. On the Saturday set up day, you will write the Touchscreen Voting Unit serial number on the label on each results cartridge.

B. TouchscreenPrinters The Touchscreen Printers must be

removed each night and secured inside a Workstation. The printers will be reattached to the correct Touchscreen Voting Unit the following morning. The power on the Touchscreen must be turned off when removing and reattaching the printers. Be sure that the Touchscreen Voting Unit serial number is indicated on the printer so you can reattach the correct printers to their units each morning. The printer must be attached to the correct Touchscreen Voting Unit or the totals won’t match.

C. ProvideVotingAssistance If a voter requests assistance

with the actual voting process, you may be required to provide assistance. The voter must qualify for assistance as outlined in box 4 and 5 on the Form 1 Affidavit. If qualified, the voter can either choose someone to assist him/her, two election officials or the designated Judges of Election would be required to assist. A Form 1 Affidavit, using box 4 or 5 and Affidavit C on the reverse side, must be completed when assistance is given. In addition, Form 1A, “Notice to Persons Providing Assistance to Voters” must be given to the voter and person or persons giving the assistance. The person giving assistance must read each office and list of candidates and mark the ballot according to the voter’s choice. For more information, call the Early Voting hotline.

Courtesies

Be alert to the needs of voters with disabilities.

Be considerate of the extra time it might take for the elderly or a person with a disability to complete the voting process.

Provide unhurried attention to a person who has difficulty speaking.

Speak directly to the person who has a disability rather than to a companion who may be along.

Speak calmly, slowly and directly to a person who is deaf or hard of hearing.

Do not shout or speak in a person’s ear.

If full understanding is doubtful, write a note to the person who is deaf or hard of hearing.

Before providing assistance to someone in a wheelchair, ask if you may do so and how you should proceed.

Greet a person who is blind or visually impaired by letting the person know who and where you are.

Voting Aids

Touchscreen Voting Unit with Audio Ballot, Audio Ballot Controller and Headphones

The election officials can activate the audio ballot on the voter card when the voting record is saved. When the voter inserts into the touchscreen voting unit the audio ballot will begin. You can offer to insert the voter card for the voter. Election officials also will assist the voter with the audio ballot controller and the headphones. Disposable covers will be available for the headphones. Be sure to start with clean covers and replace after the headphones are used.

Audio Controller

Provide the voter with the basic instructions for the controller. The Square Button in the middle is the Select Button. The Right and Left Arrows moves forward and backward through the ballot. The Up and Down Arrows move through the list of candidates for a selected office.

CHAPTER 6: ADDITIONAL RESPONSIBILITIES DURING THE DAY

Audio/Visual Ballot (activated on the Touchscreen)

Along with an audio ballot, the voter also has the option of viewing their choices on the screen or having the screen remain blank. The touchscreen must be activated manually when using the audio/visual ballot. When using the audio/visual ballot, the voter must use the screen when making their ballot choices. They cannot use the audio ballot controller if the ballot is displayed on the screen.

This type of assistance is not formal assistance in voting and no affidavit is needed. If the voter requests formal assistance, follow the procedures for Assistance in Voting.

Sip and Puff Device(activated on the Touchscreen)

While using the audio ballot and headphones, a voter may also use a sip and puff device (provided by the voter). The sip and puff device should be plugged into the audio ballot controller, to the right of the port where the headphones are plugged in. The touchscreen voting unit must be activated manually when using a sip and puff device. An election official must assist the voter with the headphones and set the audio controller either in the voter’s lap or attach it to the voter’s wheelchair.

Call the Board at 312-269-1603 for instructions on manual mode.

To the right of the headphone port, you will find two ports labeled “S” and “P”. The voter has the choice which jack they use, so that they decide whether they Sip for NEXT or SELECT or if they Puff for NEXT or SELECT.

D.ChangingtheTouchscreenPrinter Paper Roll

The Touchscreen Voting Units will inform the voter when the paper supply is low. When this occurs, you will need to replace the paper roll. As long as a voter is not in the middle of a voted ballot, you can close down the Touchscreen Voting Unit and change the paper roll when you are not busy processing voters. If the voter has not completed his/her ballot, you would need to change the paper roll immedi-ately. When this occurs, you can fold the privacy screen closed to cover the voter’s ballot informa-tion on the Touchscreen Voting Unit while you are changing the paper roll. The instructions for changing the paper roll are on pages 18-19.

E. Securing the Completed Touch-screenPrinterPaperBallotRolls

You must secure the Touchscreen Paper Ballots contained on the completed Touchscreen Printer paper roll you just removed. Secure the completed paper rolls in the following manner.

1. Roll the loose end around the completed paper roll and tape. Use the labels found with the Touchscreen Printer Service Statement.

2. Write the Location #, date and serial number of the Touchscreen Voting Unit.

3. Record the Public Counter number, as displayed on the bottom left of the screen on the Touchscreen Voting Unit and initial.

4. Place the completed paper ballot roll in the Envelope 47EV and secure in the Workstation, along with the Touchscreen Printer Service Statement.

F. IncorrectlyInsertedVoterCardErrors

If a voter card is not inserted properly into a Touchscreen Voting Unit, an error message may occur. If a voter informs you of this happening, perform the following procedures.

1. Hold down the green Activate button until the unit beeps. If the card is released, the voter can reinsert the card and vote.

2. If that does not work, repeat step one until it beeps. Release the button when it beeps and then immediately (within a fraction of a second), press down a second time and hold until it beeps and release.

3. If the activate button does not work in steps one or two, hold down the power button for approximately five seconds until the Touchscreen Voting Unit turns off. After a few seconds, turn the unit back on. The card will be released and

the voter can use the voter card in a different Touch-screen Voting Unit. When the Touchscreen Voting Unit warms up, it will be ready for use by other voters. If the problem persists, call the Board at 312-269-1603.

After retrieving the voter card, be sure to conduct a status test on the voter card.

G. VoterClaimstoHavetheWrongBallot

If a voter returns and states that an incorrect ballot was pro-grammed for him/her, such as the wrong districts or ballot style/code, you should provide the following assistance. PLEASE NOTE: If the voters believe that they were given the wrong ballot, they are usually correct.

1. Instruct the voter to deselect all the voted offices on the Touchscreen Voting Unit.

2. Hold down the “green” Activate button on the back right-hand side of the Touch-screen Voting Unit until it beeps.

3. Press the “cancel activation” box on the screen and remove the voter card.

4. Return to the workstation and review the voter’s Form 14EV application to confirm that the information matches the voter’s saved record information.

5. If the information does not match, find the voter’s correct record and record the correct information on Form 14EV. Save the record and give the correct activated voter card to the voter. Call the EV Hotline so we can delete the incorrect saved record.

6. If the information matches, check the status of the voter card to verify the activation number matches the saved record.

7. Show the voter the laptop record with the district information along with a candidate’s guide.

8. If the voter is still not satisfied with the information, advise the voter not to vote today and to obtain a copy of the voter’s sample ballot from the Board’s office or website before voting.

9. If the voter chooses not to vote, void the 14EV applica-tion and call the EV Hotline so that we can delete the saved laptop record.

10. Call the Board’s Hotline number for assistance if you encounter any problems with the above procedures.

CHAPTER 7: STATEMENTS AND FORMS TO BE COMPLETED

A. SealAccountabilityForm This form will already be

completed by the Board when the Touchscreen Voting Units are being programmed for Early Voting. The Early Voting Administrator, with the assistance of the Election Officials, will verify ALL Seal numbers on the Set-up Day and Opening Day. On the top of the form, verify the Opening Seals on the Workstations. Then, record the Closing Seal numbers for the Workstations. In the middle section, the Touchscreen Voting Unit Seal Numbers for the Polls Open/Close, Results Cartridge and Printer will be verified by initialing next to the Seal Number. If a Seal Number does not match, you must record the correct number and call the Early Voting Hotline. If you access the printer during set-up, record the new Seal Number in Section 5. This form will be returned in the Envelope 47EV.

IMPORTANT: If seal #’s do not match, call the EV Hotline.

B. Daily Ballot Statement Complete a new form each

day. Store completed form in the Workstation and use it to verify the Public Counter number (number of votes cast) on the Touchscreen Voting Units the following morning. Return the form inside the 47EV envelope. The follow-ing information is recorded each day on a Daily Ballot Statement form.

This statement records the old and new seal numbers on the Touchscreen Printers when the paper rolls need to be changed. D. Correction Statement (typo errors)

This form is used while voters are completing their applications and voting records are being saved on the laptop computer. If the voter’s name is misspelled in the computer, indicate the correct spelling on this sheet. Please note that address changes are recorded on the Form 1, not on this sheet. Return this form every day in the 47EV envelope.

E. Daily Time Report Early Voting election officials

must complete and sign this form every day. The Administrator will verify that the recorded times are accurate, sign the form and return it to the Board inside the 47EV Envelope.

The unused Daily Time records will be kept in an envelope and will remain in one of the Work-stations.

F. InvestigatorSigninSheet Investigator Sign in Sheets will

be in the clear sleeve inside the Workstation. An Investigator Sign in Sheet must be completed each day. The Investigator will sign the form every time they visit the Early Voting Location. Return this form inside the Envelope 47EV.

G. PollsOpen/CloseSerialNumberStatement

This form is used to record the yellow seal numbers and red seal numbers used when you open the polls. This form is used to verify the red seals have not been tampered with when you close the polls.

Keep this form in the Workstation and use when new Touchscreen Voting Units are opened at your location. Return this form on the last day in Envelope 47EV.

H. Final Ballot Statement This form is used to record the

ballots cast on the voting units when you close the polls.

I. UsedSealEnvelope All seals removed and recorded

during Early Voting should be placed in this envelope. This envelope will remain in the Workstation for the duration of Early Voting.

J. ReturnedEarlyVotingMaterialsStatement

This form must be completed anytime materials are returned to the Board.

Section 1 of this form must be completed and signed by the Early Voting Administrator. Section 2 will be completed by the person transporting the Early Voting Materials and Section 3 will be completed by the Board.

After Section 1 is completed, remove the pink copy for your records. Attach the other copies (white and yellow) to the Envelope 47EV.

A. RecordtheTouchscreenVotingUnitsPublicCounterNumbers

At the end of each Early Voting day, the Public Counter num-bers must be recorded for each Touchscreen Voting Unit on the Daily Ballot Statement.

B. DetermineNumberofBallotsCast

Using the Daily Ballot State-ment, perform the following procedures and make the following calculations. Record all the information you obtain in these procedures on the Daily Ballot Statement form. A calculator is included in your supply bag to aid you with this process.

1. Determine the number of ballots cast on the Touchscreen Voting Units by subtracting the starting number from the ending number.

2. Add together the number of ballots cast on the Touchscreen Voting Units.

3. Add together the last number entered on the applications for Early Voting ballots in each binder.

4. Add to the application count any Provisional Voter Affidavits.

5. Verify that the numbers for step 2 and 4 are the same.

6. Sign the Daily Ballot Statement form and store in the Workstation until the following morning to verify the next day’s starting Public Counter information.

The Daily Ballot Statement form will then be returned in the 47EV envelope.

C. VerifyingBallotsCastontheTouchscreenVotingUnits

If the application for Early Voting ballot count plus the Provisional Voter Affidavits does not match the total ballots cast on the Touchscreen Voting Units, the following procedures should be followed.

1. Verify that the final Public Counter numbers were recorded correctly.

2. Verify that the starting Public Counter numbers are the

same as the previous day’s Daily Ballot Statement’s ending Public Counter numbers. Call the Board at 312-269-1603 for these numbers.

3. Rework the calculations to check for possible errors.

4. Inspect the two binders of applications for Early Voting ballots to verify that the applications were consecutively numbered.

5. Verify that the total number of Provisional Voter’s affidavits was added to the final appli-cation for Early Voting ballots count.

D. ResolutionofVerificationProcess

If the new totals in C3 now match the total in C5, continue with the your evening proce-dures. If counts still do not match, call the Board’s office at 312-269-1603 for additional instructions.

E. RemoveandSecureResultsCartridges

Turn the power off on the Touch-screen Voting Units (DO NOT CLOSE THE POLLS), break the seal and remove the results cartridges. Each result cartridge will have its own plastic bag, marked with its corresponding Touchscreen Voting Unit serial number (you will write these numbers on the result cartridges and the bags on the Saturday set up day). Place the results cartridges in the correct plastic bag and lock them inside the safe.

CHAPTER 8: EVENING PROCEDURES AFTER EARLY VOTING ENDS

F. Remove and Secure TouchscreenPrinters The Touchscreen printers must

be secured in the Workstations each night. The power on the Touchscreen Voting Unit must be turned off when removing and reattaching the printers. Unplug the printers from the Touchscreen Voting Units and slide the printers upward until they slide out of the track that holds them to the units. Be sure that the Touchscreen Voting Unit serial number is indicated on the printer so you can reattach the correct printers to their units the following morning.

G. Secure Card Activator Device Do not unplug the Card Activator Device from the laptop comput-er. Set the Card Activator Device in the same drawer as the laptop computer so that they may be stored together.

H. Secure Laptop Computer Log out of the laptop computers,

power them down and push the drawer back into the Workstation by using the yellow lever.

I. Prepare 47EV Envelope for Next Day Pick Up

The 47EV envelope for the morning pick-up should contain:n Time Record Sheetn Investigator Sign in Sheetn Any Pollwatcher

J. Prepare Return of Early Voting MaterialsStatement

The Return of Early Voting Materials Statement must be completed anytime materials are returned to the Board. Section 1 of this form must be completed and signed by the Early Voting Administrator. After Section 1 is completed, remove the pink copy for your records. Attach the other copies (white and yellow) to the Envelope 47EV.

K. InventorySupplies An inventory of your supplies

should be conducted. If you are running short of any forms or supplies, call the Board at 312-269-1603, and the necessary materials will be delivered the next day. Do not wait until the last minute to request more forms or supplies and expect delivery the same day.

L. SecureallForms,SuppliesandEquipment

Return all the unused forms to the Workstations. Remember that all completed material must also be secured in the Workstations and the Workstations further secured with a green seal.

After the last person in line at the close of EV for the day has voted, you must complete your daily evening procedures before closing the polls. Use the Polls Open/Closed Statement for recording the seals on the “Polls Open/Close” door. This door should still be secured with the same red seal from when you opened the polls.

1. Do not turn the power off on the Touchscreen Voting Units.

2. Break the seal on the “Polls Open/Close” door.

3. Turn the switch from the “open” position to the “closed” position.

4. The Touchscreen will indicate “Election Verifying Please Wait“ and will then indicate “Printing Results.”

5. Repeat the above steps for all the Touchscreen Voting Units.

B. Complete the Final Ballot Statement

When the Touchscreens have finished processing the close polls, final ballot totals will appear on the screen. Write down the number of qualified votes, provisional votes and total votes for each Touchscreen Voting Unit on the Final Ballot Statement.

C. RemovePaperBallotRollsandResultCartridges

1. Turn the power off on the Touchscreen Voting Units.

2. Open the Touchscreen Printer cover, cut the paper roll and remove the voted paper roll ballots.

3. Roll the loose end around the paper roll and tape using the labels found with the Touchscreen Printer Service Statement. Write the date, the location, the Touchscreen Voting Unit serial number, public counter number (use the total vote number recorded earlier) and sign your names.

4. Remove the result cartridges, ensure serial numbers are on all cartridges and place in their corresponding plastic bags.

D. Securing Voting Material for Return to the Board

1. Place the bags of Results Cartridges inside the secure bankers bag and secure with the seal that came with the bankers bag.

Write this seal number on the Returned Early Voting Materials Statement.

Place the bankers bag inside the Black transfer bag.

2. Place the paper ballot rolls inside the Black transfer bag along with the bankers bag containing the Results Cartridges. Secure zippers with a red seal.

Write this seal number on the Returned Early Voting Materials Statement.

CHAPTER 9: CLOSING THE POLLS

A. LoadandLockthematerials Breakdown the Touchscreen

Voting Units and place the proper lids back on the Touchscreens. Put the Card Activator Device back inside the laptop computer cases for return to the Board. Put the Daisy Chains, the chairs, ground covers (make sure you pull the cords from the ground covers and remove ALL duct tape from the covers and cords), Blue No Electioneering Cones and the unused forms back inside the Workstations. Place a final green seal on the Workstations and record the serial number on the Daily Ballot Statement. The laptops should be placed back inside their carrying cases. Be sure to remove all the signs that were posted from the walls.

All election officials assigned to the location MUST ENSURE that all items to be returned are secured with the Administrator to be returned to the Board’s Office.

The Administrator is required to take all the completed forms and materials and return them to the Board’s Office, as well as com-plete Sections 1 and 2 on the Returned Early Voting Materials Statement. The Administrators will be informed of the designated location for the

drop-off of returned materials.
