•I understand that I must have relevant VA paperwork on file with Davenport University. Relevant paperwork includes: VonApp confirmation page until a Certificate of Eligibility is available, a 22-1995 form if I attended any other college in which I received VA benefits and/or a 28-1905 for Chapter 31 students.•I understand I must have Official Transcripts sent to Davenport University from all of my previous colleges. •I must be pursuing a program of study and making satisfactory progress toward completion of the degree listed above. I understand that the VA will only pay for classes that apply toward an approved educational major. If I decide to change my desired major, I must notify, via email, the Central Registrar’s Office in writing (see below) and attach a Curriculum Update form. • I understand that I must notify, via email, the Central Registrar’s Office (see below) of any change in enrollment. These changes include any increase or decrease in credit hours, delivery option change and/or in seat vs online class changes.•I understand that if I withdraw from or stop attending a class (receive a W, F or NF), Davenport University must report this grade along with the last date of attendance to the VA. Further, I understand that in either case, I could be responsible for the repayment of VA benefits associated with these grades. Repayment could include book stipend, monthly housing allowance and tuition and fees. I must also notify, via email, the Registrar’s Office of any such withdrawal (see below).
• I understand that if I am required to take a developmental class, the class must be taken in seat at a Davenport University campus. *Note—Chapter 31 recipients are approved to take developmental classes online as well as in seat.• I understand that if I attend until the end of a class and take the final exam and receive a failing grade, the F does not need to
be reported to the VA. The VA will allow a repeat of any class in my major that was not successfully completed (F , W or I grades). However, the grade will effect my GPA and could impact satisfactory progress. Satisfactory progress must be maintained as stated in Davenport University’s catalog.
• I understand that I cannot repeat a course that I have already successfully completed. • I understand that if I am called to active duty I am entitled to a “Military Withdrawal”. In this event, I must deliver a copy of
my orders to my academic advisor.• I understand that the VA requirements for full time status may differ from the Financial Aid and/or the Davenport
University’s requirements for full time status. It is my responsibility to know the difference.• I understand that if I am a Chapter 33 veteran, my credit hours will be certified at the beginning of the semester, however my
tuition and fees will be reported at the time of the 100% charge in accordance with University Policy.

Copy of ledger of each student account (i.e. billing statement) showing tuition and fee charges, and payments for all semesters. Printed copy of curricular requirement for each student's academic program(s) of study. Registration document(s) showing class schedule, type of class (residence or distance), and charges for each term (per class). VA enrollment certifications for review during site visit (historical chronological order of student’s VAOnce, 1999’s and 1999b’s). Provide annual amount of tuition and fees of Yellow Ribbon recipients (if applicable). Copy of school catalog(s) available for the periodicity of Fall 2010 to present. Copy of enrollment or billing statement verifying tuition and fees paid by a similarly situated non-VA student. One non-VA student academic file, any degree, for review during site visit.
