1. CONTRACTOR IS RESPONSIBLE FOR VERIFYING EXISTING UTILITY LOCATIONS. DRAWINGS ARE NOT ALLINCLUSIVE. CONTRACTOR SHALL COORDINATE WORK AROUND EXISTING UTILITIES WITH UTILITY OWNERS(THIS WORK IS INCIDENTAL TO THE CONTRACT).
2. THE CONTRACTOR SHALL NOTIFY DIGGERS HOTLINE AND AFFECTED UTILITIES PRIOR TO THE START OF WORK.ANY LOCAL MUNICIPAL UTILITY WHICH IS NOT A MEMBER OF THE DIGGERS HOTLINE MUST BE CONTACTEDSEPARATELY.
3. RADII, ELEVATIONS, AND DIMENSIONS ARE GIVEN AT THE PAVEMENT EDGES, UNLESS OTHERWISE NOTED INTHE PLANS.
4. EROSION CONTROL ITEMS SHOWN IN THE PLANS ARE AT SUGGESTED LOCATIONS. THE ENGINEER MAYMODIFY LOCATIONS TO FIT FIELD CONDITIONS.
5. CALUMET COUNTY HIGHWAY DEPARTMENT IS SPONSORING A PROJECT THAT INCLUDES RESURFACING CTH NWITHIN THE LIMITS OF THIS PROJECT. COORDINATION WITH CALUMET COUNTY MAY BE REQUIRED DURINGCONSTRUCTION AND IMPLEMENTATION.

CONTACT MIKE HARDY AT THE CITY OFAPPLETON TRAFFIC DEPARTMENT AT (920) 832-5580 ONCE ALL CABLEAND WIRE IS INSTALLED FOR INSTALLATION OF THE SIGNAL CABINETASSEMBLY.
ALL-WAY STOP IS TO BE INSTALLED PRIOR TO DE-ENERGIZING TRAFFICSIGNAL. THE TRAFFIC SIGNAL SHALL NOT BE DE-ENERGIZED FOR LONGERTHAN A CONTINUOUS 5-DAY PERIOD.
THE ENGINEER MAY ADJUST THE LOCATIONS OF ITEMS UNDER THISCONTRACT TO AVOID CONFLICT WITH EXISTING UTILITY FACILITIES.
OUTAGAMIE COUNTY HIGHWAY DEPARTMENT SHALL FIELD VERIFY FINALLOCATIONS OF CONCRETE BASES PRIOR TO CONSTRUCTION.
ALL MULTIPLE RUN CONDUITS SHALL BE PLACED AS TIGHTLY AS POSSIBLEIN ONE TRENCH. THE CONTRACTOR WILL BE RESPONSIBLE TO IDENTIFYTHE WIDTH OF EACH TRENCH AND IDENTIFY ONE CONDUIT THAT ISCENTERED IN EACH TRENCH. THIS INFORMATION SHALL BE PROVIDED TOOUTAGAMIE COUNTY FOR FUTURE UTILITY LOCATION NEEDS.
INSTALL THE CONCRETE CABINET BASE AND METER BREAKER PEDESTAL AMINIMUM OF 4 WEEKS PRIOR TO THE START-UP DATE OF NEW SIGNALOPERATION TO ALLOW OUTAGAMIE COUNTY AND WE ENERGIES TOSCHEDULE AND INSTALL NEW METER SERVICE.
ADJUST VIDEO DETECTION DURING CONSTRUCTION AS NEEDED.
INSTALL ALL NEW SIGNAL CABLE AND WIRE, LIGHTING WIRE, ANDGROUNDING WIRES.

1. The undersigned BIDDER proposes and agrees, if this Bid is accepted, to enter into an agreement with OWNER in
the form included in the Contract Documents to perform and furnish all Work as specified or indicated in the
Contract Documents for the Bid Price and within the Bid Times indicated in this Bid and in accordance with the other
terms and conditions of the Contract Documents.
2. BIDDER accepts all of the terms and conditions of the Advertisement or Invitation to Bid and Instructions to
Bidders, including without limitation those dealing with the disposition of Bid security. This Bid will remain subject to
acceptance for 90 days after the date of Bid opening. BIDDER will sign and deliver the required number of
counterparts of the Agreement with the Bonds and other documents required by the Bidding Requirements within 15
days after the date of OWNER's Notice of Award.
3. In submitting this Bid, BIDDER represents, as more fully set forth in the Agreement, that:
(a) BIDDER has examined copies of all the Bidding Documents and of the following Addenda, receipt of all
which is hereby acknowledged:

conditions that may affect cost, progress, performance and furnishing of the Work;

(d) BIDDER is aware of the general nature of Work to be performed by OWNER and others at the site that
relates to Work for which this Bid is submitted as indicated in the Contract Documents.

(e) BIDDER has correlated the information known to BIDDER, information and observations obtained from
visits to the site, reports and drawings identified in the Contract Documents and all additional
examinations, investigations, explorations, tests, studies and data with the Contract Documents.
(f) BIDDER has given ENGINEER written notice of all conflicts, errors or discrepancies that BIDDER has
discovered in the Contract Documents and the written resolution thereof by ENGINEER is acceptable to
BIDDER, and the Contract Documents are generally sufficient to indicate and convey understanding of
all terms and conditions for performing and furnishing the Work for which this Bid is submitted.
(g) This Bid is genuine and not made in the interest of or on behalf of any undisclosed person, firm or
corporation and is not submitted in conformity with any agreement or rules of any group, association,
organization or corporation; BIDDER has not directly or indirectly induced or solicited any other BIDDER
to submit a false or sham Bid; BIDDER has not solicited or induced any person, firm or corporation to
refrain from bidding; and BIDDER has not sought by collusion to obtain for itself any advantage over
any other BIDDER or over OWNER.

BIDDER acknowledges that unit quantities contained in some parts of the proposal are not guaranteed and final
payment will be based on actual quantities determined as provided in the Contract Documents.
5. BIDDER agrees that the Work will be substantially complete and completed and ready for final payment in
accordance with the Special Provisions on or before the dates or within the number of calendar days indicated in
the Agreement. BIDDER accepts the provisions of the Agreement as to liquidated damages in the event of failure
to complete the Work within the times specified in the Contract Documents.

8. Traffic
The Department will provide primary traffic control devices (i.e. signs, drums, barricades, etc.) for performing roadway improvements. Any additional costs anticipated by the contractor for performing work on the project and maintaining traffic as further discussed in this section shall be compensated under the Traffic Control bid item or be considered incidental to the work item associated with the cost.
The contractor shall coordinate with the Department for changes to traffic control and lane closures. The contractor will be responsible for providing WisDOT certified flaggers and signs according to applicable standard detail drawings if work occurs outside the signed lane closures provided by the Department.
Perform this work in accordance with the requirements of Section 643 of the standard specifications, and as shown on the plans or as approved by the Department, except as hereinafter modified.
Submit to engineer for approval a detailed traffic control plan for any changes to the proposed traffic control details as shown on the plans. Submit this plan ten (10) days prior to the preconstruction conference.
CTH N & CTH KK Intersection
CTH N will remain open to traffic with a minimum of one through lane open in each direction at all times.
Prior to de-energizing the traffic signals, the Department will install an all-way stop at the intersection of CTH N and CTH KK, to remain in place until the new signal is operational. Portable changeable message signs (PCMS) will be placed one week prior to implementation of the four-way stop control at the intersection by Department forces. Contact Joe Zellmer at (920) 832-5673 at least 14 calendar days prior to de-energizing the traffic signal to make arrangements for the PCMS placement and four-way stop control installation. The contractor shall limit the duration of the planned signal outage as deemed practical. In the event that the signal outage remains in place for more than seven (7) consecutive calendar days, the contractor shall temporarily cover the existing signal heads per the requirements of the MUTCD. Costs for temporarily covering and uncovering the signals shall be incidental to the contract.
Calumet County may close the south leg of CTH N for their resurfacing project for a period simultaneous with the contract work. Contact Brian Glaeser with the Calumet County Highway Department at (920) 849-1463 at least five (5) working days prior to lane closures and implementation of the four-way stop control at the CTH N and CTH KK intersection.
The driveway for the BP Gas Station at N9690 County N is permitted to remain closed during construction. Contact Joe Zellmer at (920) 832-5673 at least 14 working days prior to driveway closure to make arrangements for traffic control device placement and coordination with the property owner. Access to the property along CTH KK must be maintained at all times during the CTH N access closure.
No single lane closures shall occur on CTH KK while the signal is de-energized and under all-way stop control.
Keep sidewalks open unless otherwise shown on the plans or as approved by the Department. Maintain pedestrian access to adjacent properties, businesses, schools, and at bus stops or provide where necessary, as directed by the engineer.
Provide adequate temporary sidewalk and bridging between the curb and right-of-way line over freshly paved concrete or other obstructions in the sidewalk area, as directed by the engineer.
CTH N & Wallace Street Intersection
The Department will provide signs, traffic control drums and barricades as necessary for the following single lane closures:
• Stage 1: Lane closure for CTH N northbound traffic and Wallace Street to set bases beyond the curb and gutter and perform storm sewer, sidewalk and roadway work.
• Stage 2: Lane closure for CTH N southbound traffic to set bases beyond the curb and gutter and perform sidewalk and roadway work.
CTH N shall remain open to traffic with a minimum of one lane open in each direction at all times. Roadway may be reduced to one lane utilizing standard detail drawings and/or flaggers between the hours of 9:00 A.M. and 3:00 P.M., Monday through Friday. Inquire with the Department Highway Engineer for allowable lane closure hours beyond these timeframes.

16. Install Concrete Bases, Cabinet Basement, Item SPV.0060.03
A Description
This special provision describes furnishing and installing prefabricated concrete bases provided by the Department for traffic signal cabinets in accordance with the pertinent provisions of Section 654 of the Standard Specifications, and as hereinafter provided.
B Materials
The Department will provide the precast concrete cabinet basement.
Furnish all other materials in accordance with section 654.2 of the standard specifications, with the following exceptions:
Replace Subsection 654.2(6) with the following:
(6) Use schedule 80 PVC electrical conduit conforming to the electrical conduit specified in State Standard Specifications Section 652.
Append Subsection 654.2 with the following:
(7) Furnish a prefabricated foundation, including hardware, as shown in the plan details.
C Construction
Verify all final base locations with the Outagamie County Highway Engineer Joe Zellmer at (920) 209-9807 before construction. Contact at least five (5) working days prior to proposed field verification date to make arrangements.
Confirm scheduled availability for cabinet basement at least two (2) weeks in advance of scheduled installation date. In addition, contact Outagamie County Highway Engineer Joe Zellmer at (920) 209-9807 three (3) business days in advance to coordinate pickup of cabinet basement. Use an approved expandable foam sealant at conduit entrances and knockouts. Foam shall expand to completely seal out debris and moisture from entering the cabinet basement. Fill excavations or voids with backfill compacted in 8-inch lifts with engineer-approved tamping equipment.
Upon installation of new traffic signal control cabinet and decommissioning of existing traffic signal cabinet, when applicable, remove the existing cabinet basement and dispose of offsite. Backfill resulting void and dispose of existing cabinet basement according to standard specification 204.
D Measurement
The department will measure Install Concrete Bases, Cabinet Basement as each individual base furnished and installed, acceptably completed.
E Payment
The department will pay for measured quantities at the contract unit price under the following bid item:
