Secretary of the Commonwealth Carol Aichele announced that 99 disciplinary actions have been taken by the Bureau of Professional and Occupational Affairs’ 29 professional-licensing boards and the Division of Legislation and Notaries of the Bureau of Commission, Elections and Legislation:

PA Board of Accountancy
Franklin County Troy A. Beam, certificate no. CA026596L, of Shippensburg, Franklin County, was assessed a civil
penalty of $40,000 and was revoked based upon his six felony convictions of obstructing and impeding
the IRS laws, attempting to evade and defeat payment of assessed taxes and willful failure to file tax
returns, in federal court. (06/23/14)
Montgomery County Rodney K. Reynolds, license no. CA031573L, of Pottstown, Montgomery County, was suspended
pursuant to the Order of the Court of Common Pleas of Chester County dated June 12, 2014, which the
court issued under section 4355 of the Domestic Relations Code. The suspension is effective

Dauphin County

suspended pursuant to the Order of the Court of Common Pleas of Dauphin County dated June 16, 2014,
which the court issued under section 4355 of the Domestic Relations Code. The suspension is effective

Philadelphia County Hair Connection, license no. BO433255, of Philadelphia, Philadelphia County was suspended for failing

PA Board of Certified Real Estate Appraisers
Delaware County Robert N. Coyle, certificate no. BA000548L, formerly of Glen Mills, Delaware County, was ordered to
pay a civil penalty of $4,000 and was revoked because he pled guilty to the crime of loan fraud which was

PA Board of Cosmetology Allegheny County Esspa Inc. t/d/b/a Esspa Kozmetika Organic Skincare, license nos. CZ106704 and CY106964, of
Pittsburgh, Allegheny County, was ordered to pay a civil penalty of $1,500 for operating a salon without
displaying its salon licenses and failing to file an application to change the location of the salon with the

failing to post a “no smoking” sign, and for failing to have the minimum equipment necessary for a salon.

$3,500 because she engaged in gross incompetence, negligence or misconduct in the carrying on of the

Delaware County Lynne R. Stonis, license no. YM001213L, of Folsom, Delaware County, had a public reprimand placed
on her permanent disciplinary record with the board, her license was suspended for a period of 30 days,
and she must complete five hours of continuing education in Medical Ethics within 60 days from the date
of the order because she engaged in unprofessional or immoral conduct when she misrepresented or
concealed a material fact in obtaining a license or a certificate issued by the board or a reinstatement
thereof by submitting an altered document to satisfy the board's audit of her continuing education
requirements, and made a false or deceptive biennial registration with the board. (07/22/14)
George G. Bocobo, license no. MD042554E, until recently of Villanova, Delaware County, was
suspended pursuant to the Order of the Court of Common Pleas of Montgomery County dated June 9,
2014, which the court issued under section 4355 of the Domestic Relations Code. The suspension is

Lancaster County David A. Lundgren, license no. OM000106, of Marietta, Lancaster County, was suspended from the
practice of oriental medicine for a period of three years and six months, with the first six months served as
an active suspension, because he engaged in sexual behavior with a current patient. (07/22/14)
Lehigh County Maryann Theresa Weisman, license no. MD043771E, of Whitehall, Lehigh County, was temporarily
suspended, pending a hearing, on the grounds that her continued practice within the Commonwealth may
be an immediate and clear danger to the public health and safety. (08/07/14)
McKean County Abaz Sosic, license no. MD069319L, of Bradford, McKean County, was indefinitely suspended because
he was convicted of a felony, a crime involving moral turpitude, or a crime related to the practice of

Out of State Anne C. Aldridge, Pennsylvania license no. MD047235L, of Bear, Delaware, was placed on indefinite
probation based on disciplinary action taken against her license by another state. (06/11/14)
Dale N. Payne, license no. MD031726E, of Tucson, Arizona, was indefinitely suspended for no less than
five years retroactive to March 4, 2014, such suspension to be stayed in favor of no less than five years of
probation to run concurrent with his period of probation in Arizona, because he was unable to practice the
profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol,
or mental incompetence and had a license or other authorization to practice the profession suspended,
revoked, refused or otherwise disciplined by the proper licensing authority of another state. (07/22/14)
William D. Trusnovic, of Jackson, California, was granted a Pennsylvania license immediately placed on
probation to run concurrent with his probation with the Medical Board of California. (06/24/14)

Connor William Smith, license no. MD067771L, of New York, New York, had his Pennsylvania license
automatically suspended based on his misdemeanor convictions under the Drug Act. (05/21/14)
Erik Robert Washburn, of Marina, California, was granted a Pennsylvania Medical Training license
immediately suspended for a period of no less than three years and stayed in favor of probation for no less

Secretary of the Commonwealth – Notaries Public
Allegheny County Anthony P. Iadicicco, notary commission no. 1076854, of Pittsburgh, Allegheny County, was suspended
for six months, with such suspension to be immediately stayed in favor of probation, subject to certain
terms which include his successful completion of a six-hour notary practice and procedure course and was
ordered to pay a civil penalty of $250, which shall be immediately stayed contingent upon the successful
completion of the six-hour notary practice and procedure course, and assessment fees of $300, because he
failed to keep and maintain an accurate chronological register of all official notarial acts and failed to be
familiar with the duties and responsibilities of a notary public. (08/04/14)
Bucks County Betty L. Gabilanes, notary commission no. 1054989, of Fairless Hills, Bucks County, was suspended for
six months, with such suspension to be immediately stayed in favor of probation, subject to certain terms
which include her successful completion of a six-hour notary practice and procedure course and was
ordered to pay a civil penalty of $250, which shall be immediately stayed contingent upon the successful
completion of the six-hour notary practice and procedure course, and assessment fees of $150, because
she failed to keep and maintain an accurate chronological register of all official notarial acts performed by
failing to list separately each notarial service provided or, in the alternative, by failing to list separately
the clerical fees associated with each notarial service and failed to be familiar with the duties and
responsibilities of a notary public by certifying a document which the notary is prohibited by law from
copying or certifying. (08/04/14)
Lehigh County Chad Nemeh, notary commission number 1219799, of Allentown, Lehigh County, was revoked based on
findings he failed to be familiar with the duties and responsibilities of a notary public, he is not of
sufficient ability to hold the office of notary public, failed to keep and maintain custody of an accurate
chronological register of all official acts by that notary done by virtue of his office and failed to give a
certified copy of the register in the notary’s office to any person applying for the same. (05/28/14)
Philadelphia County Denise LaRossa, notary commission no. 1244503, of Philadelphia, Philadelphia County, was suspended
for six months, with such suspension to be immediately stayed in favor of probation, during which she
much attend and successfully complete a six-hour notary practice and procedure course and was ordered
to pay a civil penalty of $250 and probation assessment of $300 because she failed to be of sufficient
ability and failed to be familiar with the duties of a notary public. (07/23/14)

Antoniette M. Black, notary commission no. 1225117, of Philadelphia, Philadelphia County, permanently
and voluntarily surrendered her commission because she failed to know through personal knowledge or
have satisfactory evidence that the person appearing before her was the party who executed an affidavit
and failed to maintain an accurate chronological register of all official acts performed. (08/13/14)
Somerset County Gary J. Pirschl, notary commission number 1129987, of Rockwood, Somerset County, was revoked,
based on findings he failed to know through personal knowledge or have satisfactory evidence that the
person appearing before him as the person described in and who was executing the instrument, failed to
keep and maintain custody and control of an accurate chronological register of all official acts done by
virtue of his notary commission, failed to be familiar with the duties of a notary public and was not of
sufficient ability to hold the office of notary public. (05/28/14)
PA Board of Nursing
Allegheny County Mara Pierce, license no. RN604806, of Pittsburgh, Allegheny County, was indefinitely suspended for no
less than three years, such suspension to be immediately stayed in favor of no less than three years of
probation because she is unable to practice the profession with reasonable skill and safety to patients.

suspended for no less than three years, such suspension to be immediately stayed in favor of no less than
three years of probation, retroactive to April 8, 2013, because she is unable to practice professional
nursing with reasonable skill and safety and because she was convicted of felony charges in the courts of

Amanda Elizabeth Asbury, license no. RN558448, of Pittsburgh, Allegheny County, was ordered to
attend and successfully complete at least six hours of remedial education on the topic of nursing law and
ethics and had a public reprimand placed on her permanent disciplinary record with the board because she
possessed, used, acquired or distributed a controlled substance or caution legend drug for other than an
acceptable medical purpose and misappropriated equipment, materials, property, drugs or money from an

than three years retroactive to April 19, 2013, based on findings she violated the terms of a previous board

County were suspended for no less than five years retroactive to February 11, 2014 for failing to

professional nursing suspended retroactive to April 19, 2012, such suspension immediately stayed in

physical illness or condition or physiological or psychological dependence upon alcohol, hallucinogenic

for no less than three years, such suspension to be immediately stayed in favor of no less than three years

suspended, with such suspension immediately stayed in favor of probation until such time that she
successfully completes her criminal probation, because she received Accelerated Rehabilitative

years, with said suspension stayed in favor of no less than three years probation, because she is addicted

hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination or has
become mentally incompetent, and has been convicted or has pleaded guilty to a crime of moral turpitude
in the courts of this Commonwealth, the United States, or any other states, territory or country.
