PA Board of Barber Examiners Allegheny County George K. Daniels, Jr., license no. BL052272, of Pittsburgh, Allegheny County, was suspended pursuant

to the Order of the Court of Common Pleas of Allegheny County dated May 9, 2016, which the court

issued under section 4355 of the Domestic Relations Code. The suspension is effective immediately.

Lancaster County Marcus J. Matos, license no. BL054118, of Lancaster, Lancaster County, was suspended pursuant to the

Order of the Court of Common Pleas of Lancaster County dated June 13, 2016, which the court issued

PA Board of Certified Real Estate Appraisers No disciplinary actions were taken.

PA Board of Chiropractic Out of State Maria Ann Giacalone, PA license no. DC006461L and AJ009424, of Cape Coral, Florida, was

indefinitely suspended for a period to run, at a minimum, concurrent with her criminal imprisonment and

Delaware County Kevin Anthony of Clifton Heights, Delaware County, was required to pay a civil penalty of $1,000 and

costs of investigation in the amount of $625.41 because he employed an unlicensed individual to operate a

two thousand dollars ($2,000.00) and pay costs of investigation of two hundred dollars ($200.00), because

she engaged in the practice of massage therapy and/or represented herself as engaged in the practice of

massage therapy, and/or offered and/or held herself out as being engaged in the practice of massage

PA Board of Medicine Montgomery County Stephen Charles Padnes, license no. MD010322E, of Glenside, Montgomery County, had his license to

practice as a Medical Physician and Surgeon temporarily suspended, pending a hearing, on the grounds

that his continued practice within the Commonwealth may be an immediate and clear danger to the public

suspended retroactive to February 24, 2016, for being unable to practice with reasonable skill and safety

automatically suspended for no less than ten years based on pleading guilty to numerous felonies under

Allegheny County Kathleen Sammel, notary commission I.D. No. 1143994, of Pittsburgh, Allegheny County, was suspended

for two months, with such suspension immediately stayed in favor of probation, subject to terms and

conditions as specified by the Secretary; assessed a civil penalty of $250, a probation assessment of $100

for costs of continued monitoring, and must successfully complete a live, in-person, six-hour notary

practice and procedure course approved by the Secretary because she failed to maintain an accurate

notarial registry of all official notary acts; failed to be familiar with the duties and responsibilities of a

notary public; and failed to be of good character, integrity, and ability. (08/04/16)

Bucks County Thomas J. Brandis, notary commission no. 1016252, of Telford, Bucks County, permanently and

voluntarily surrendered his notary commission because he failed to keep and maintain custody and control

of an accurate chronological register of all official acts. (08/04/16)

Lancaster County Charles B. Rettew III, notary commission I.D. No. 1136637, of Manheim, Lancaster County, was

suspended for six months, with such suspension immediately stayed in favor of probation, subject to

terms and conditions as specified by the Secretary; assessed a civil penalty of $500, a probation

assessment of $300 for costs of continued monitoring, and must successfully complete a live, in-person,

six-hour notary practice and procedure course approved by the Secretary because he failed to maintain an

accurate chronological registry of all official notary acts and failed to be familiar with the duties and

responsibilities of a notary public. (08/04/16)

Lisa K. Copenhaver, notary commission I.D. No. 1030845, of Ephrata, Lancaster County, was suspended

for a period of three months subject to the terms and conditions specified by the Secretary; assessed a civil

penalty of $250, stayed in favor of successful completion of a live, in-person, six-hour notary practice and

procedure course during her suspension because she failed to maintain an accurate chronological registry

of all official notary acts, to be familiar with the duties and responsibilities of a notary public; and to be of

good character, integrity, and ability. (08/11/16)

Kathryn R. Unger, notary commission I.D. No. 1170052, of Lancaster, Lancaster County, was suspended

for a period of three months, with such suspension immediately stayed in favor of probation, subject to

the terms and conditions specified by the Secretary; assessed a civil penalty of $250, stayed in favor of

successful completion of a live, in-person, six-hour notary practice and procedure course during the

probation period and a probation assessment of $150 for the costs of continued monitoring because she

failed to maintain an accurate chronological registry of all official notary acts and to be familiar with the

suspended for eighteen months, with such suspension immediately stayed in favor of probation, subject to

terms and conditions as specified by the Secretary; assessed a civil penalty of $500, a probation

assessment of $900 for costs of continued monitoring, and must successfully complete a live, in-person,

six-hour notary practice and procedure course approved by the Secretary because he failed to know

through personal knowledge or have satisfactory evidence that the individuals appearing before him were

the parties who signed the document; to maintain an accurate chronological registry of all official notary

acts; and to be familiar with the duties and responsibilities of a notary public. (08/04/16)

Susquehanna County The Secretary of the Commonwealth accepted a Consent Agreement in which Michael Brainard of

Jackson, Susquehanna County, permanently and voluntarily surrendered his notary commission I.D. No.

1241365, along with any other licenses, registrations certificates or permits authorizing him to practice as

a notary because he failed to require personal appearance; to be of good character, integrity, and ability;

and to be familiar with the duties and responsibilities of a notary public. (08/11/16)

Out of State Carolyn A. McMahon, notary commission I.D. No. 1208251, of Sewell, New Jersey, permanently and

voluntarily surrendered her notary commission because she failed to take and subscribe the constitutional

oath of office, give a surety bond and record the bond, commission and oath of office in the office of the

recorder of deeds for the county in which the notary maintains an office at appointment or reappointment;

and she failed to be familiar with the duties and responsibilities of a notary public. (07/22/2016)

PA Board of Nursing Allegheny County Megan Elizabeth Wegley, license no. RN557576, of Coraopolis, Allegheny County, was indefinitely

suspended for no less than three years retroactive to June 10, 2016, based on violating the terms of a

Tiffany D. Merideth, license no. PN288625, of Pittsburgh, Allegheny County, was placed on probation

for no less than twelve months to run concurrently with her criminal probation and assessed a civil

guilty by a judge or jury of a felony or a crime of moral turpitude in the courts of Commonwealth, the

United States, or any other state or received probation without verdict, disposition in lieu of trial or an

Accelerated Rehabilitative Disposition in the disposition of felony charges, or has been dishonorably

discharged or has been discharged under circumstances amounting to dishonorable discharge from the

military forces of the United States or of any other country; and she is addicted to alcohol or

hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination or has

Bradford County Dean Gregory Parsons, license no. PN275937, of Sayre, Bradford County, voluntarily surrendered his

license to practice as a practical nurse in the Commonwealth of Pennsylvania, because he had a license

suspended or revoked, or received other disciplinary action by the proper licensing authority in another

state, territory, possession or country and violated the Act in that he failed to report to the Board on his

biennial renewal application or within 90 days of the final disposition, whichever was sooner, that a

disciplinary action had been taken against his license in another state. (07/27/16)

Bucks County Nicole Mitchell, license no. PN266921, last known of Warrington, Bucks County, was automatically

suspended based on a conviction under the Drug Act. (02/11/16)

suspended for ten years from the date of her conviction, based on her felony conviction under the Drug

reprimanded, because he had a license suspended or revoked or has received other disciplinary action by
