on a non-federally related transaction without holding a current and valid certificate from the State Board

Joshua Rosinski, D.C., license no. DC009549, of Mechanicsburg, Cumberland County, had his

suspension stayed to probation until at least November 16, 2021 because of compliance with a previous

Christopher George Bereznak, license no. DS029539L of Tunkhannock, Wyoming County, was actively

suspended for a minimum period of ten years, retroactive to August 11, 2017, because he was found

committing two (2) counts of retaining funds intended to pay for funeral goods and services when the

funeral director and funeral entity have not provide any funeral goods and services; committing six (6)

counts of failing to provide full and factual representation concerning aspects of the services rendered or

the funeral furnishings; committing one (1) count of failing to comply with the Federal Trade

Commission by failing to provide customers with a Casket Price List, a General Price List, and a

Statement of Funeral Goods and Services; committing one (1) count of failing to give the family

representative or agency official arranging for a funeral a written agreement form statement of funeral

goods and services at the time of arrangement which must be prior to the disposition of the decreased; and

committing two (2) counts of failing to embalm, seal in a container that will not allow fumes or odors to

escape, or refrigerate, human remains held twenty-four (24) hours beyond death. (03/23/20)

$500; and costs of investigation of $1,160.89, because she failed to provide full and factual representation

Philadelphia County permanent voluntary surrendered its license to practice as a restricted business

corporation in the Commonwealth of Pennsylvania, and Christopher Angelo Hawkins, F.D., license and

registration no. FD012325L & FS012325L of Philadelphia, Philadelphia County, and Julian V. Hawkins

Funeral Home, Inc., were levied a joint and several civil penalty of $2,000; and costs of investigation of

$1,000; because they both failed to deposit in escrow or transfer in trust to a banking institution in this

Commonwealth, the entire amount of monies received by the funeral director under a prepaid contract for

funeral services or merchandise, including additional service fees or arrangement fees, and Hawkins

engaged in fraud or deceit in obtaining or attempting to obtain a license. (9/18/20)

Westmoreland County

Kelly L. Corridoni, license no. FD012772L & FS012772L, of Avonmore, Westmoreland County, Kelly

L. Corridoni Funderal Home, Ltd., license no. FR000994L & FB014501, of Avonmore, Westmoreland

County, and Keystone Crematon Alliance, LLC, were jointly and severally levied a civil penalty of

$1,500; and costs of investigation of $819.88; and Keystone Cremation Alliance, LLC must cease and

desist from performing any activity that requires an authorization issued by the Board, because Kelly L.

Corridoni aided or assisted an unlicensed person to engage in an act or practice for which a license is

required; Kelly L. Corridoni Funeral Home did not include the name of its permanent supervisor in

advertising media, and Keystone Cremation Alliance, LLC practiced as a funeral director without holding

PA Board of Massage Therapy

Allegheny County Lili Zhu, of Harwick, Allegheny County, was denied a license to practice massage therapy because she

failed to demonstrate that she satisfies the educational requirements for licensure, and because she

presented false credentials or documents in support of her application. (09/08/20)

Dean Ralph Hager, license no. MSG009715 of Mckeesport, Allegheny County, was levied a civil penalty

of $1,000; costs of investigation of $628.67; and a reprimand was placed on his disciplinary record with

the Board; because he failed to provide massage therapy records immediately upon demand of the Board

or its authorized agents, and he failed to maintain massage therapy records for at least three years from the

last date that services were provided to the client. (9/22/20)

Centre County Julia Lynn Clarke, of Pleasant Gap, Centre County, was denied a license to practice massage therapy

because she failed to prove she could practice with reasonable skill and safety. (09/22/20)

Delaware County

Timothy C Raudabaugh, license no. MSG008355 of Folsom, Delaware County, was levied a civil penalty

of $2,000; and a reprimand was placed on his permanent disciplinary record with the Board, because he

was convicted under Federal law, under the law of any state or under the law of another jurisdiction of a

crime of moral turpitude or of an offense which, if committed in this Commonwealth, would constitute a

Westmoreland County

Gary Wilt, of Latrobe, Westmoreland County, was levied a civil penalty of $1,000; costs of investigation

of $498.42; and must cease and desist from practicing massage therapy, because he practiced massage

Sylvia Noronha, of Latrobe, Westmoreland County, was levied a civil penalty of $1,000; costs of

investigation of $1032.68; and must cease and desist from practicing massage therapy, because she

months, following a period of three months active suspension, the remaining period shall be stayed in

favor of probation; was levied a civil penalty of $1,000; a probation assessment of $600; and must attend

six hours of notary practice and procedure course(s), because he failed to require personal appearance, be

familiar with the duties and responsibilities of a notary public by failing to include the essential notarial

language on three documents, and failed to be of good character, integrity and ability. (8/28/20)

Lehigh County

Deborah A. Mayo, notary commission no. 1104876, of Allentown, Lehigh County, was suspended for 12

months, immediately stayed in favor of probation; levied a civil penalty of $500; probation assessment of

$600; and must complete six hours of remedial education because she failed to require the personal

appearance of four individuals on two occasions and failed to provide a copy of her notary register upon

Andrea Kauss Boughner, LPN, license no. PN258432L, of Coraopolis, Allegheny County, had her license

Christine Spanik Milner, RN, license no. RN333117L, of Brackenridge, Allegheny County, had her

license put on probation indefinitely, based on being placed in the Accelerated Rehabilitative Disposition

program in the disposition of a felony charge. (05/05/20)

Jane Ellen Keenan, RN, license no. RN267428L, of Carnegie, Alleghany County, had her professional

nursing license automatically suspended, based on her felony conviction under the Controlled Substance,

Drug, Device and Cosmetic Act. (06/23/20)

On September 24, 2020, the State Board of Nursing automatically suspended the license of Donald L.

Nardelli, R.N., license no. RN613714, of Fallentimber, Cambria County and Pittsburgh, Allegheny

County, retroactive to January 13, 2020, until he demonstrates that he can practice with reasonable skill

and safety based upon his conviction under the Controlled Substance, Drug, Device and Cosmetic Act.

suspended for two years, and no less than three years concurrently, both suspensions stayed in favor of no

or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs, or other

had her licenses indefinitely suspended and was assessed $562.50 in costs of investigation, based on being

Albert E. Hippert, Sr. and Tylersburg Motors, LLC, license nos. MV055470L and VD031339, of

Tionesta, Forest County and Tylersburg, Clarion County, were levied a $200 civil penalty and placed on

probation for a period of one year, based upon making substantial misrepresentations of material fact,

making a false promise of character likely to induce the sale of a vehicle, and demonstrating

unprofessional conduct or incompetency to operate as a licensee. (05/22/20)

Justin W. Shanley, license no. MV219149 of Enola, Cumberland County, formerly of Lititz, Lancaster

County, was reprimanded and levied a civil penalty of $500 based upon his misdemeanor criminal

conviction for a crime of moral turpitude and making a material misstatement on an application for

Jason S. Layton, license no. MV157914L, of Carlisle, Cumberland County, was revoked based on his

County, was levied a civil penalty of $500 and had his vehicle salesperson license indefinitely suspended,

investigation totaling $585.22 and placed on probation for a period of one year, based upon allowing

another organization to use its license in a capacity for which that individual should not have held a

license, and willfully permitting and individual or salesperson to buy, sell or exchange a vehicle for his
