異位性皮膚炎
• 是一種先天過敏體質造成皮膚受不了外在刺激而引發反覆濕疹發作的病症。
• 患者皮膚屏障功能較弱,對外在刺激過度敏感導致免疫系統反應失衡。
• 80%患者五歲以前就發病。(大約有 50%的病患是在 1 歲前發病,另有 30% 的病患則在 1-5 歲間發病)
異位性皮膚炎 (又稱異位性濕疹,英文為atopic dermatitis)
• 一種慢性反覆性的皮膚疾病。
• 好發在嬰兒期或兒童期。
• 患者常常同時伴有血清中E型免疫球蛋白升高、個人或家族有過敏性鼻炎、氣喘或異位性皮膚炎等病史。
• 目前並沒有任何單一的病症或抽血檢查可以直接就診斷異位性皮膚炎。
• 醫師必須根據病患的各項病史與臨床表現綜合判斷後才能診斷

異位性皮膚炎的症狀
• 「癢」是最主要的症狀,特別是夜間。
• 皮膚異常乾燥。
• 長期抓癢及搓揉使皮膚變厚、凸起、變粗,皮膚裂開、有痂皮或失去光澤。
• 過度的搔抓常會使皮膚產生傷口,因而引起感染,病灶會有黃色透明滲出液及痂皮。

物等.
• 過度乾燥
• 接觸刺激性的物質

• 情緒、心理壓力
• 過度紫外線傷害
• 臺大醫院的異位性皮膚炎特別門診中,針對 80名患者進行問卷調查,在我們的初步研究中發現:國內的病友們認為引起異位性皮膚炎惡化的五大原因分別是:皮膚乾燥 (87.2%)、天氣熱
(85%)、流汗 (83.2%)、曬太陽
(70.9%)、壓力大、焦慮 (66.7%)。
避免異位性皮膚炎惡化因子: 皮膚乾燥
最好的解決方法: 使用潤膚保濕產品
1. 選擇低過敏性、無香料的保濕產品
2. 針對異位性皮膚炎設計的產品尤佳
3. 洗澡, 洗手後立即塗抹。
4. 注意部位不同使用劑型可能不同
5. 視職業特性, 氣候(秋冬乾燥季節)增加塗抹次數.
塵蹣 (dust mite)
降低異位性皮膚炎復發機率:減少環境過敏原
減少環境過敏原 (II)
• 使用除濕機以保持室內溼度介於50~65%
• 使用空氣過濾器以減少黴菌滋生。
• 避免接觸二手菸、清潔劑、洗碗精、消毒水等化學物質。
• 少用香水、芳香劑、蚊香、樟腦丸、殺蟲劑、髮膠、汽油味、油漆味等具有刺激氣味物質。

Proactive Anti-inflammatory Therapy Summary
If disease frequently flaring, use
•Clearance protocol with topical steroids.
•Then intermittent anti-inflammatory treatment to frequent flare sites.
•Treating normal-appearing skin.
Summary Topical Therapy
• Simple soak and smear works.
• Education and follow-up in a week.
• Incorporate TCIs to early recurrence for
moderate disease.
• Use twice weekly steroid or three times weekly
tacrolimus oint to NORMAL skin to prevent flares
in moderate-severe patients.

Probiotics
+ Two meta-analyses have demonstrated that
probiotics might reduce the incidence of AD
in infants, with the pooled relative risk ratio ranging
from 0.69 to0.79,
+ A recentmeta-analysis also concluded that
treatment with probiotics significantly decreased
the Severity Scoring of Atopic Dermatitis (SCORAD)
index in children with AD; however, other meta-
analyses showed inconsistent results.

Cyclosporine Common Side Effects

• Transition to NB-UVB or other safer option

• Safe in children ( 0.2-0.6mg/kg q week )

Azathioprine (Imuran)
• 3 RCTs show 25%-39% improvement
• Less expensive than other immunosuppressants
• Can use in children
• Watch blood count and liver function
• Systematic review- one case of lymphoma in
follow-up period (Schram, Arch derm, 2011)
Summary of systemic treatments
• Cyclosporine first line for severe disease,
(prednisolone burst, short term may be used)
• Must transition to NBUVB, MTX, AZA, MMF
• All current systemics limited by toxicities or
inadequate efficacy
• There is a major unmet need in dermatology for
agents to treat moderate-to-severe AD
