คล่ืนไฟฟ้าหวัใจคืออะไร EKG (ECG) : electrocardiogram

คือคลื่นไฟฟา้ท่ีเกิดจากการไหลของกระแสไฟฟา้ในหวัใจ เป็นการบนัทกึการเปลี่ยนแปลงของelectrical activity

oAV node : 40-60 bpm

วิธีการตรวจคล่ืนไฟฟ้าหวัใจ LIMB LEADS

วิธีการตรวจคล่ืนไฟฟ้าหวัใจ CHEST LEADS

o V1 : Rt. 4th ICS parasternum

oV2 : Lt. 4th ICS parasternum

oV3 : กึ่งกลางระหว่าง V3-V4

oV4 : Lt. 5th ICS midclavicular line

oV5 : ระดบัเดียวกบั V4 ท่ี Lt. anterior axillaryline

oV6 : ระดบัเดียวกบั V4 ท่ี Lt. mid axillary line

ส่วนประกอบต่างๆของคล่ืนหวัใจ P wave : atrial depolarization

QRS complex : ventricular depolarization

T wave : ventricular repolarization

ส่วนประกอบของกระดาษ EKG ความเร็วกระดาษ = 25 mm/sec

1 ช่องเลก็ในแนวตัง้และแนวนอน = 1 mm

1 ช่องเลก็ในแนวนอน = 0.04 sec

ในแนวตัง้ 1 mV = 10 mm 10 mm(1mV)

Rhythm Sinus rhythm ?o มี P wave น า QRS complex ทกุตวั ? และหวัตัง้ใน lead I, II, aVF และหวักลบัใน aVR

NORMAL SINUS RHYTHM

o rate 60-100 bpm

o atrial rate = ventricular rate

o PP และ PR interval ปกติ

o PR, QRS ปกติทัง้ระยะเวลาและรูปร่าง

Rhythm JUNCTIONAL RHYTHM

RATE 40-60 BPM

IDIOVENTRICULAR RHYTHM

RATE 20-40 BPM

Rate : อตัราการเตน้ o Regular rate?

o Rate ปกติ 60-100 bpm

o 1 ช่องเลก็สดุ = 0.04 sec

o 1 ช่องใหญ่ = 5 ช่องเลก็ = 0.2 sec

การค านวณ rate ในกรณี regular rate Heart rate

300 / จ านวนช่องใหญ่ระหวา่ง QRS ท่ีอยูต่ิดกนั

1500/ จ านวนช่องเลก็ระหวา่ง QRS ท่ีอยูต่ิดกนั

HR = ?

การค านวณ rate ในกรณีท่ี irregular rate o เลือก R wave จดุเร่ิมต้น แล้วนบัไป 30 ช่องใหญ่ นบั QRS ท่ีอยูใ่นช่วงนีแ้ล้วคณูด้วย 10 จะได้ HR ใน 1 นาที

R wave progression ปกติ R และ S wave จะมีขนาดใกล้เคียงกนัใน lead V3-V4

left atrium P wave กว้างมากกวา่ 0.12 sec (3 mm),

notched P wave

Right atriumPeak P wave มากกวา่ 2.5 mm

VENTRICLE ENLARGEMENT

left ventricle S in V1 หรือ V2 + R in V5 หรือ V6 มากกวา่

35 mm S in V1 หรือ V2 /หรือ R in V5 หรือ V6 มากกวา่ 25 mm

right ventricle Right axis deviation + R>s in V1

deep persistent S wave in V5, V6

สงูไม่เกิน 1 mm ในทกุ leads ยกเว้น V2, V3ชาย > 40 ปี มากกวา่หรือเทา่กบั 2 mmชาย < 40 ปี มากกวา่หรือเทา่กบั 2.5 mmหญิง มากกวา่หรือเทา่กบั 1.5 mm

Depression มากกวา่หรือเท่ากบั 0.5 mm

inverted T wave มากกวา่หรือเท่ากบั

1 mm ร่วมกบั prominent R wave หรือ

R/S ratio>1Isoelectric line

Infarction Ischemia : inverted T wave

Injury STEMI : ST elevation

NSTEMI : ST depression, inverted T wave

Infarction : Q wave

Sinus tachycardiaPacemaker >> SA node ผ่านทาง normal conduction system

EKGRate > 100 bpm

Regularity : regular

P wave : uniform, upright in lead IIPR interval : คงท่ีและอยูร่ะหวา่ง 0.12-0.20 วินาที

Sinus tachycardiaCAUSES

o Stress, exercise

o Cafeine, smoking, alcohol

o Drugs >> adrenaline, atropine, dopamine

o Congestive heart failure

o Shock

o Myocarditis

o Pulmonary embolism

o Acute myocardial infarction

o Chronic lung disease

o Increase cardiac output >> fever, hyperthyroidism, anemiao obesity

Supraventricular tachycardia o Regularity : regular

o Rate : 130-210 bpm

o P wave : มกัซ่อนใน QRS complex ท าให้มองไมเ่หน็ P wave ถ้าเหน็จะเป็นลกัษณะหวักลบัใน lead II

o PR interval : วดัไมไ่ด้ถ้า P wave ซ้อนกบั QRS complex หรือถ้ามองเหน็จะเป็น RP<PR

o QRS complex : < 0.12 sec

SVT

Atrial fluttero Caused by a macro-re-entry circuit in the right atrium

o The re-entry circuit results in an atrial rate of 200-400 bpm(typically 300 bpm)

Atrial flutter EKGo Flutter waves / ‘saw-tooth pattern’ best seen in leads II, III, aVF

and V1

oAbsence of an isoelectric baseline

Atrial flutter

Atrial fibrillation Occur when there are multiple, small area of atrial myocardium continuously discharging and contracting

Atrial rate usually > 300 bpm

The ventricular rate is limited by refractory period of the AV node or accessory pathway

Atrial fibrillation o Ventricular rhythm จะ irregular

oRate : atrial rate > 300 bpm, ventricular rate อาจช้ากวา่ 60 bpm หรือ มากกวา่ 100 bpm

o No P wave PR interval : วดัไมไ่ด้

o QRS : <0.12 sec

Atrial fibrillation

Multifocal atrial tachycardiao Irregular rhythm caused by at least 3 different site of atrial ectopic foci

o EKGP wave : 3 or more different shapes

Changing P-P,P-R and R-R interval

Atrial rhythm usually between 100-180 bpm

Multifocal atrial tachycardia : MAT

QRS complex < 0.12 sec QRS complex > 0.12 sec

Ventricular tachycardia Occurrence of 3 or more depolarization from a ventricular ectopic pacemaker at the rate faster than 100 bpm

Ventricular tachycardia

Ventricular fibrillation Totally disorganized depolarization and contraction of small area of ventricular myocardium

No effective ventricular pumping activity

Commonly in severe ischemic heart disease

Torsade de pointes A specific variant of polymorphic VT in which the QRS axis swing from a positive to a negative direction in a single lead

Sinus bradycardiaRepresent suppression of sinus node discharge rate Physiologic >> in well controlled athletes, during sleep, vagal stimulation

Pharmacologic >> B-blocker, digoxin, opioids, calcium channel blocker

Pathologic >> acute inferior wall MI, increase ICP, carotid sinus hypersensitivity, hypothyroidism

Sinus bradycardia

Atrioventricular block oเกิดจาก supraventricular impulse ไม่สามารถผ่านลงสู ่ventricle ได้ เน่ืองจากมีความผิดปกติของการน ากระแสไฟฟา้ท่ี AV node

o แบง่เป็น 3 ชนิด1st degree AV block

2nd degree AV block3rd degree(complete) AV block

1st degree AV block

2nd degree AV block Mobitz I (Wenckebach)
