With the aim of providing convenience and prompt service not only to our pensioners but also to

their beneficiaries or legal heirs at the time that financial assistance is needed, the documentaryrequirements in support- of a funeral, death or survivorship claim have been rationalized. This will

FAQs on Survivorship Benefits (PPG No. 207-11b)

1. What were the amendments made on the policies on survivorshipbenefits?

The documentary requirements in support of a funeral, death orsurvivorship claim have been rationalized. This change will result insimplified documentary requirements and immediate reconciliation orprocessing of the survivorship benefit claim.

2. Why were the documentary requirements rationalized by GSIS?

The documentary requirements were rationalized by GSIS as part of thecontinuing efforts of the Board and Management to provide convenienceand prompt service not only to our pensioners but also to theirbeneficiaries or legal heirs at the time that financial assistance is needed.

3. What are the rationalized documentary requirements for funeral,death and survivorship benefits?

The following are the primary documentary requirements:

For Funeral and Death Benefits: A Death Certificate duly certified by theLocal Civil Registrar, with the certified true copy bearing the originalsignature of the Local Civil Registrar or the authorized representative ofthe local Office of the Civil Registrar

For Survivorship Benefits: NSO-Issued Death Certificate

4. The primary documentary requirements for Funeral and DeathBenefits and Survivorship Benefits are not the same? Will this notbe burdensome on the part of the claimant/s?

Previously, the GSIS requires the submission of an NSO-Issued DeathCertificate in support of a funeral, death and survivorship benefit claims.However, securing an NSO-Issued Death Certificate has been reported tobe taking months before it is made available to the requesting party.Hence, it takes several months before a claimant is paid of the saidbenefits.

Inasmuch as Funeral Benefit is intended to immediately help defrayexpenses resulting from the death of a member or pensioner and, similar

to Death Benefit, is a one-time benefit payment, the Management deemedit appropriate to relax the primary document requirement to a DeathCertificate duly certified by the Local Civil Registrar.

Once the documentary requirements for Funeral and Death Benefit claimshave been submitted to the GSIS, the reconciliation of the accountls of thedeceased member or pensioner, in relation to the processing ofSurvivorship Benefit, shall commence.

5. Will there be a new application form for Funeral, Death andSurvivorship benefit claims given that the similar documentaryrequirements for the first two benefit claims?

A consolidated application form for Funeral, Death and Survivorshipbenefit claims will be issued to ensure that the survivors and/or legal heirsare informed of all benefits arising from the death of a member orpensioner.

6. How will this affect the prescription period of Survivorship Benefits?

The filing of the Funeral Benefit claim shall be considered as aconstructive notice for the Survivorship Benefit claim. Once the FuneralBenefit claim is filed, the Survivorship Benefits shall not prescribe.

7. Is there a hierarchy in the payment of Funeral Benefit?

Funeral Benefits shall be paid in this order of priority:

a. Surviving Spouse

b. Any of the following persons who can present receiptls ofexpenses, provided that the surviving spouse, if still living, hasacknowledged that this person shouldered the funeral expenses:

I. Children of the deceased member or pensioner

11. For single deceased member or pensioner, relatives up tosecond degree of consanguinity

iii. Any claimant other than the children

8. In case the deceased member has two (2) subsisting marriages, towhom shall the Funeral Benefit be payable to?

In case the deceased member has two (2) subsisting marriages and bothspouses are claiming the funeral benefit, the said benefit shall be payableto the surviving legal spouse with the earliest date of marriage based onthe NSO-Issued Marriage Certificate. •
