THE MADISON FAMILY

HonoreesEach year at its Annual Meeting, Family Service Association recognizes a Corporate Citizen of the Year and a Family Builder of the Year. The awards are given to those who recognize the needs of individuals and families and step forward to develop solutions that help all of us. Through their actions they help Family Service achieve its mission of strengthening the lives of individuals, families, and communities.

Corporate Citizen of the Year--ComcastEach year, Comcast carries out numerous projects to benefi t the community. Non-profi ts, community organizations, and legislators are invited to appear on Comcast Newsmakers to promote programs and events. Comcast employees volunteer year-round for service providers and Comcast donates to local organizations in support of their charitable activities.

Shortly after the Bucks County Emergency Homeless Shelter became a program of Family Service, Comcast offered a team of volunteers to give the Shelter a face-lift. An employee, whose spouse had spent time living at the Shelter as a young child many years ago, prompted the offer. Overall, 172 volunteers including employees, families and friends, participated in Comcast Cares Day in April. The Shelter received a makeover that included painting, landscaping, and cleaning the grounds and vehicles. Comcast donated computers for Shelter residents and wired the Shelter for internet access. Children use the workstations for schoolwork and adults search for employment, permanent housing, and connect with other helpful resources in the community. In addition, Comcast made a generous fi nancial contribution in support of the Family Service mission.

Family Builder of the Year--St. Mary Medical CenterSt. Mary Medical Center and its Community Ministries play a pivotal role in helping vulnerable individuals and families improve their social, emotional, environmental, and economic well-being. In 2012, St. Mary Medical Center received the Healthy Communities Achievement Award which is given to organizations that assess areas of need; build multidisciplinary partnerships; and support their partners with grants and professional services.

Through community meetings and data gathering, St. Mary compiled information detailing the strengths of our community and identifying areas for improvement. They used the information to reduce disparities in health and well-being of residents. Through their support of the St. Mary Breast Health Initiative, Mother Bachmann Maternity Center, the BCHIP Adult Health Clinic, and the National SAFE KIDS Campaign they insure access to health care and safety education in Bucks County. St. Mary Community Ministries, in partnership with local agencies, created emergency housing for families, particularly single mothers and their children, helping them become independent.

St. Mary was one of the fi rst organizations to come forward and say “we will help” when Family Service assumed responsibility for the Bucks County Emergency Homeless Shelter. And help they did. They set up a tutoring room for children and provided safe new cribs, high chairs, and car seats for residents. They donated family-friendly round tables and chairs for the cafeteria to promote parent-child conversations. They offered hours of expert consultation on how to enhance our Volunteer Program. They provided fl u shots to Shelter residents, volunteers, and staff. Today, they are creating a new indoor play area for children and youth of all ages.

Family of the YearThe Family of the Year is a client in a Family Service program or programs who came to us with a challenge and through dedication to their treatment, have strengthened their lives. They have shown a commitment to improving their overall health and wellness and worked to maximize their independence.

The 2012 Family of the Year struggled with the overwhelming crisis of being homeless. In the process of fi nding both temporary and, eventually, stable housing, the family utilized various programs of Family Service to work on individual issues of each family member, meet basic needs, and begin a path towards a more solid future.

Family Service Association has grown in many ways over the last year. But, as we refl ect on that growth, we are reminded of the words of Calvin Coolidge, “All growth depends upon activity. There is no development physically or intellectually without effort, and effort means work.” In this report you will read about some of our activities; the efforts of our staff, volunteers, donors and partner agencies; and the hard work of those we serve as they strive to grow and change.

Much of the growth Family Service experienced in the last year was due to the addition of the Bucks County Homeless Hotline and the Bucks County Emergency Homeless Shelter to our program mix. These vital services assisted more than 2,000 Bucks County residents including those who were about to lose their homes and those who were literally homeless. Individuals and families called the Hotline in hopes of fi nding a solution to an overwhelming crisis and the Hotline staff responded with care and compassion linking them to many community-based resources. Those whose only option was the Shelter, were welcomed by staff that was committed to understanding and assisting people who had experienced the trauma of losing their homes. They received intensive Case Management from a team that we tripled in size and trained to focus on fi nding permanent housing in a timely fashion. They were fed nutritious meals not only by staff but also by loyal volunteers. And volunteer service did not stop there. Volunteers played a signifi cant role in changing the face of the Shelter with new landscaping and picnic benches, a safer playground, brightly painted rooms, and more. Just as the facility was renewed, so was the hope of Shelter residents who found a new path leading to a safe home to call their own.

With the addition of these new programs, and the expansion of others, our staff grew by nearly 50%. As a result our Board and leadership team worked to build a stronger infrastructure to support their efforts. A Volunteer Coordinator and new program managers were added to the team. We recruited staff at all levels of the agency with the skills needed to build our new programs and strengthen existing ones. Innovative training strategies were implemented including a customized online e-learning program for behavioral health and human service organizations through which we can now bring training to our staff at any location at any time. Family Service was selected to participate in the 2012 National Council Trauma-Informed Practices Learning Community to enhance our delivery of trauma-informed behavioral healthcare and we continued our agency wide training on Motivational Interviewing.

Recognizing the importance of technology in delivering, managing, and evaluating services, we upgraded computers at our many locations and now provide mobile access to vital data bases. We selected a new Electronic Medical Records program that will expand opportunities for communication with other health care providers and with those we serve. This growth and change was made possible through the efforts of many people.

Our Board and staff recognized that growth for the sake of growth would do little to change the serious problems that are facing people today. They focused instead on growth with a purpose. They said that through our actions we must improve the lives of those who are hungry, homeless, frightened, and seeking hope. They believed that we must provide direction for those who feel that their ability to live in the community is being

compromised by mental illness or substance abuse. They recognized that we must prepare children for school and for healthy transitions from childhood to adolescence and into adulthood. And they decided that we must support parents who are struggling to keep their families strong.

Our volunteers recognized that there are so many tasks, large and small, that change lives forever. So they came to Family Service daily to do everything from stuffi ng envelopes to coordinating fundraising activities, to answering phones, to sharing vital workplace skills with fi rst time job seekers, to equipping children with clothing and supplies for the fi rst day of school, to teaching parents how to properly use car seats. For each volunteer who contributed their time and talents, there were children and families who benefi ted.

Our funders recognized that caring for others requires signifi cant investment and that the burden of cost is best met when shared among the public sector, foundations, businesses and individuals. As a result, the number of funders and the amounts of their donations grew. Families in Bucks County were touched by the generous gifts from all corners of the community.

And the families we served recognized that the power to create true change in their lives rested in their hands. Our talented Board and Staff, our new and expanded programs, the volunteers who touched the hearts of so many, the donors who kept us fi nancially strong, and our hard working partner agencies were the tools. But each person who came to Family Service had to make the choice of using those tools and doing the work…no matter how diffi cult.

We thank all of you who helped us grow with a purpose this year and encourage you to read about the people whose lives you changed. If you remain open to continuing your work as tools of change, we will do our part to use these tools responsibly to improve the lives of the people of Bucks County.

the number of lives you strengthened with your support of Family Service this past year.

7,448=2,641 people received counseling, nursing, and psychiatry services in our Outpatient Behavioral Health program. That is a 34% increased from the previous year.

Although mental illness, child abuse and many of the other issues we address are not exclusive to low-income families, 95% of the people we serve are either low- or extremely low-income. The remaining 5% are moderate-income.

36% of clients in SCOT, Supportive Community-Outreach Team, have a Co-occurring Disorder (both a mental illness and substance abuse addiction).

100% of all residents served in the Older Adult Program maintained stable and safe housing.

97% of parents in our Parent Support Group report having a better relationship with their child as a result of education they received in the group.

The Bucks County Homeless Hotline received 11 calls per day from someone in Bucks County who was homeless or facing homelessness.

The Teen Center helped 404 teens with issues such as abusive relationships, homelessness, thoughts of suicide, self-harm, and low self-esteem.

138 people with HIV/AIDS received 1,696 hours of support services in our Medical Case Management Program.

100% of children in the Parents as Teachers program (PAT) received Initial Health Screenings and Developmental Milestone Assessments insuring proper development or connection to specialized services.

15 fathers participated in the fi rst session of our Fatherhood Initiative as part of the PAT program.

We provided over 600 trips to medical appointments for clients in our HIV/AIDS program.

1,300+ people received food from our food pantry.

Friends of Family Service assist us in increasing awareness of our mission so we can grow in visibility and develop new relationships in the community. They serve as a cabinet for Family Service’s leadership, providing recommendations and information to the Board of Directors.

Growing Healthier Outpatient Behavioral HealthDeanna suffered from deep depression and anxiety. She had a hard time keeping a job. She lost custody of her children more than once. She was addicted to pain killers. People throughout her life asked Deanna “what is wrong with you?” No one ever asked, “what happened to you?”

That is, until Deanna met her therapist at Family Service. For the fi rst time in her life, Deanna’s therapist, who is trained in Trauma Informed Care, asked Deanna, “What happened to you?” Unlike others, Deanna’s therapist was interested in Deanna, not just her drug addiction.

Deanna talked about the long history of sexual and emotional abuse she suffered beginning when she was six. Although it was painful and diffi cult, each week she discussed her trauma. Over time, Deanna learned to forgive herself for what happened to her. She learned her drug addiction didn’t exist because she was a bad person—it was the only way she knew how to get through each horrible day of her life.

Deanna learned she couldn’t expect to be a good mom if she was never shown how to be a good parent. Her therapist suggested other ways for her to cope with the pain in her life such as attending support and parenting groups, getting exercise, making meaningful and healthy connections with people, and staying committed to her therapy.

When Deanna was ready to try these tactics, she knew she was ready to kick her drug addiction for good. Deanna worked with her therapist and used recommended resources to stop her drug use.

Deanna no longer feels depressed and anxious, although she still has tough days. But now, when those tough days come along, Deanna does not turn to drugs to get by. Instead, Deanna takes

walks and relies on the support of family. Deanna sees a bright future for herself. She wants to ‘pay it forward’ by helping others. Deanna has asked to join Family Service’s Trauma Informed Advisory Board to help other people who are waiting for someone to ask “what happened to you?”

Growing stronger Family Strengthening Programs

A year ago, Todd found himself in prison after violating parole. While he was in prison, his girlfriend relapsed with her drug use and lost custody of their two year old son Marcus. It frustrated and saddened Todd knowing his son was in foster care and he vowed he would make more responsible decisions and be a better father when he was released.

Todd was released from prison and immediately began taking steps to get custody of Marcus. He asked for a parenting class and was connected with the Fatherhood Initiative, which is part of our Parents as Teachers program.

A Family Service Parent Educator joined the supervised visits Todd had with Marcus. At fi rst, they met once a week. Over time the frequency of the visits grew. Todd told his Parent Educator he wanted to build a good relationship with Marcus and was willing to try all that was suggested to him. He learned discipline techniques and effective use of Time Outs. He learned the importance of reading to Marcus and came to look forward to the time they would cuddle up with books. Todd was so proud that he taught Marcus his colors! Through a

Developmental Assessment, their Parent Educator observed that Marcus had some developmental delays and connected Todd and Marcus to a program to address the issues. Marcus began Occupational Therapy and Speech Therapy. Todd continued to use the tools and techniques at home that Marcus’ therapists used at the offi ce.

Todd read all the information he received in PAT and asked many questions of his Parent Educator. He especially wanted to make sure he was correctly using books and games during his precious time with Marcus.

After six months in Parents as Teachers, Todd was awarded full-custody of Marcus. Marcus’ therapy is going well. Dad and son are a closer, stronger family now as a result of the support and education they received from Family Service.

Growing Confi dence HIV/AIDS Programs

When Carla was asked to join a Healthy Relationships group with other straight women who were living with HIV/AIDS, she thought it wasa good idea. But, she didn’t realize it would help her all the waysthat it did. It actually changed her way of thinking.

Carla thought she was in complete control of her disease, but actuallyshe discovered she was not dealing with living with HIV at all. Yes, she took her medications, practiced safe sex, and went to the doctor, but that was only part of controlling this disease.

To Carla, control meant not telling anyone. She thought being in control meant handling it alone. When she fi rst joined the group, Carla was tense and sat with her arms folded. She didn’t speak much at fi rst, but after hearing the stories of the other women she knew she wasn’t the only one struggling. Carla felt safe to ask questions and open up about her life. The members of the group helped Carla realize that she was strong enough to tell people she was HIV

positive. She learned not only how, but when to disclose her status. She learned to stop and think instead of reacting in fear or haste. Because no one knew she had the virus, Carla lived every day in fear of what would happen if she told anyone.

Carla realizes that knowledge is power. She believes anyone who is HIV positive needs knowledge and confi dence to truly be healthy. Carla made some wonderful bonds with people in this group—support she never would have had in her life if not for Healthy Relationships. Through the courage she found in Healthy Relationships, she told family members and some close friends that she was HIV positive. They are supporting Carla in ways she never imagined.

Carla tells us she will continue to control her disease by taking care of herself and is thankful she no longer has to do it alone.

Growing Older Safely Older Adult Programs

For many years, Alice, who lived in low-income senior housing, took care of her herself, not wanting to rely on family or friends for help. Like many older adults, she did not want to be “a burden.” But, Alice’s years of self-reliance came at a cost; her family drifted away and lost contact with her.

When Alice’s health began to decline, she didn’t ask for help. She stopped walking to the grocery store when her legs became too weak. She lost her appetite. She lost weight quickly and stopped seeing the doctor. Alice stayed in her apartment with only her TV and an occasional visit from her neighbor to keep her company.

Alice stopped paying her bills. Her phone was disconnected and she lost her health insurance. Since she always paid her rent, her landlord never knew about her diffi culties.

Then, Alice lost her cable service.

When Alice’s neighbor realized Alice wasn’t paying her bills, she knew she had to get help for Alice. The neighbor spoke to the Family Service Case Manager in her building.

Alice slowly accepted help from her Case Manager. She agreed to home delivered meals, in-home medical care, and light housecleaning services. Her Case Manager reviewed her bills and had her phone and cable restored.

Alice even allowed the Case Manager to contact her closest family member, a cousin who she had not seen in many years.

The Case Manager reconnected Alice with her cousin. Alice’s cousin visited regularly, often taking Alice to dinner and spending signifi cant time with her on the weekends.

Several months after this reunion, Alice suffered a stroke and passed away. This was a sad event, but Alice did not die alone because of the efforts of the Case Manager to reconnect her with her family.

Growing Towards a Future The Teen CenterWhen Nick fi rst came to The Teen Center he was a pleasant mild mannered young man. He came with friends and seemed to really enjoy the snacks provided. Some of the regulars of The Teen Center were a bit wary of Nick since he was new and unfamiliar, but that didn’t discourage him. He was a regular on Fridays and was often waitingfor the center to open on Thursday afternoons. Nick didn’t open up about why he came to The Teen Center each week, but they heard him often refer to it as “his home”.

Then one day Nick came to The Teen Center extremely angry and agitated. Something had sparked Nick’s rage and The Teen Center was the fi rst place he came to talk about it. One of the counselors talked one-on-one with Nick and he revealed that he had been kicked out of the house by his father. Over the past three months he had been living on the streets. He was doing his best to deal with it on his own, but it eventually overwhelmed him. It was the fi rst time in a long time he trusted an adult enough to discuss what he was going through. He told his counselor that The Teen Center was a warm, safe place where he could get “a meal” two days each week.

We learned that Nick, like many teens struggling with serious issues, only thought about the things he needed to do to survive each day. He did not think about tomorrow, or next week, or his future. During one-on-one counseling sessions, Nick was encouraged to think about the consequences of his actions and to think about his future. The Teen Center staff helped Nick look for jobs and eventually he found one at a

local supermarket. An excited Nick knew the next step was to save some money and fi nd permanent housing, but with his very fi rst paycheck, he bought himself a meal and a cell phone.

A cell phone may seem like an extravagant purchase for a homeless teen, but Nick was thinking ahead. He explained his purchase to his counselor; “I want my boss to be able to call me whenever he needs me to pick up extra shifts at work.” He also has a phone so he can inquire about places to live. His counselor will tell you that planning and looking beyond tomorrow was a huge step for Nick.

Nick is starting to think about his future. He is still a regular at The Teen Center when he’s not working. Nick welcomes the ‘new’ kids to the center. He tells them he knows it is hard being the new one and he makes them feel welcome.

Outpatient Behavioral Health includes individual and family counseling for children, adolescents, adults and seniors. Family Service is licensed by The State of Pennsylvania to provide both outpatient Mental Health and Drug & Alcohol treatment services. Services are offered at several Family Service offi ce locations in Bucks County.

The Family Service Older Adult Programs help older adults live better lives today and in the future. With the population of Bucks County living longer and longer, specialized services for persons over 60 become increasingly important. Through a coordinated approach to information and services, these programs strive to keep seniors independent and in their homes.

The Teen Center is a free and confi dential place for teens ages 14 and up to fi nd help. Teens who come to The Teen Center struggle with a variety of problems including substance abuse, depression, relationship diffi culties, homelessness, diffi culties in school, thoughts of suicide, and more.

The Teen Center strives to create a safe and supportive environment for all teens. Staff address the unique problems faced by teens today by providing counseling, educational and support groups, and referral services.

The Teen Center is a gateway for an increasing number of teens needing professional counseling and adolescent psychiatric services which are readily available through our Outpatient Behavioral Health program.

The HIV/AIDS Program (HAP) is made up of several inter-related programs. We serve men, women, seniors, young adults, and people of all ethnic and cultural backgrounds regardless of income.

Two basic premises guide what we do: everyone should know their status; and, if HIV positive, the sooner you are connected to medical care, the better your chances of living a longer and healthier life.
