Welcome to Buttonwood Park Zoo’s Winter ZooCrew program! We are so happy to have your child joining us. Please read through this information carefully, and let us know if you have any questions! Our staff will be waiting at the Zoo entrance to check your child in for the day. Afternoon pick up runs from 4:00pm – 4:15pm. You may pick up your child at the Zoo entrance. Please do NOT take your child without checking in with ZooCrew staff. Participants will only be released to individuals listed on the registration forms. Identification is required for the first time an individual picks up a participant. Lunches: Campers will need to bring lunch each day or $6 to purchase one of the following lunch options from the Bear’s Den Café. 1. Grilled Cheese 2. Cheese Pizza 3. 2 Chicken Tenders All options come with a beverage (juice or milk) and a choice of fruit or snack. LUNCH OPTIONS: Bring a reusable water bottle instead of bottled water. Pack lunches in reusable containers. Instead of purchasing individually wrapped snacks, buy a large bag & put smaller servings into reusable container Use fabric face masks that can be washed and reused rather than single-use face masks go green for zoocrew! during the week, ZooCrew will be running if the Zoo is open to the public. If the Zoo is closed to the public, ZooCrew will be cancelled and refunds will be issued for that day. Refunds will only be issued if the Zoo cancels programming. If the Zoo is closed, it will be posted on our website and facebook page, and an email will come out as soon as we get word from City Hall.

These will go out to the entire group, even if you don’t attend every day - that way you can follow along with all the fun!

• Participants will be expected to wear masks at all times while indoors or whenever they are within six feet of an animal.

• Materials will not be shared; each participant will be provided with their own set of craft supplies. If they would like, campers are also invited to bring their own craft supplies (scissors, tape/glue, and coloring supplies).

• Out of consideration for all of our BPZOO guests, if a participant is experiencing any of the following symptoms (fever or chills, cough, shortness of breath or di�culty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea), please do not allow them to attend this program. You will be granted a full refund as a sign of our appreciation.

A completed, current Emergency Contact Information, Medical Authorization and Release Agreement and Health Care Record/Immunization Record must be submitted prior to participation.

This form must be signed by a parent or legal guardian. If you registered online, a link to these forms were sent in the registration acknowledgement email you received.
