Introduction:

We all want to protect our loved ones from harm, and help them live long and happy lives. But it can be difficult to know what steps to take tosafeguard those closest to us. So, we’ve consulted with your OAA BenefitsConsultant, Mumby Insurance Brokers, to identify three precautions thatshould top your to-do list:

First, make their wellbeing a priority. It has been said that you havenothing if you don’t have your health. So, make sure your family eat abalanced diet, exercise regularly, follow doctors’ advice and have check-upstwice a year.

Second, protect their home. Install smoke detectors, test them weeklyand replace the batteries every six months. Buy fire extinguishers, andmake sure everyone in your family knows how to use them. If you ask your local fire department for help inspecting your home for fire hazards,you might even prevent a fire from happening.

Third, safeguard their future with insurance. Income Protectiondisability coverage can help you support your family with an income evenif you’re unable to work. The benefits from Term Life coverage can helpyour dependents cope, if they were to lose your financial support withoutwarning. Products:Income Protection Insurance

OVERVIEW The cornerstone of any financial plan is the protection of your earning powerif an accident or illness keeps you from your job. Your greatest asset is yourability to earn an income.

DETAILS Income Protection Insurance offers Members $100 of monthly incomereplacement benefit per unit.

The maximum total monthly benefit you can apply for is the LESSER of:• $4,500 per month (45 units), or • 50% of your monthly gross Earned Income, less any other disability

insurance in force or applied for, and less any Earned Income continued by your employer or partnership (rounded to the next highest $100).

Note: The minimum coverage you can have in force is five units.

(Earned Income means income earned from your employment or profession,after business expenses but before income taxes. If your income fluctuates, use an average of your income over the previous three years.)

To qualify for benefits you must prove to our satisfaction that you meet thedefinition of Total Disability or Partial Disability and are under the care of aqualified physician at all times.

IN THE CASE OF TOTAL DISABILITY, benefit payments begin following thewaiting period. If disability is due to an accident, benefits are payable for theduration of the disability. If disability is due to illness and occurs before age63, benefits are payable up to age 65. If disability is due to illness and occursat age 63 or older but before age 70, benefits are payable for up to 24 months.

IN THE CASE OF PARTIAL DISABILITY, benefit payments begin following the waiting period or following 90 consecutive days of disability, whichever is greater. If Partial Disability follows a period of Total Disability, the 90-dayrequirement may be waived.

Benefits are payable up to age 65, in an amount equal to the full insuredbenefit reduced by the ratio of the Earned Income received while disabled tothe Earned Income before disability. If the sum of your Earned Income whiledisabled plus benefits received from all sources (excluding investment income)exceeds your average Earned Income over the 24 months preceding disability,benefits will be reduced on a pro rata basis. Coverage may be extended toage 70 if still receiving Earned Income.

Automatic Cost of Living AdjustmentEach January 1, if you have been receiving income replacement benefits forthe preceding six months, your monthly income will be adjusted for inflationusing the Consumer Price Index, subject to a maximum escalation of 5% per year.

DEFINITIONS OF DISABILITY TOTAL DISABILITY – You will be considered totally disabled if, due to sickness orinjury, you are unable to perform the normal duties of your regular occupationand you are not employed in any other gainful occupation. After your waitingperiod plus the first 60 months of benefits, you must be unable to engage inany occupation for which you are reasonably suited by training, education orexperience, and you must not be engaged in any type of gainful occupation.

PARTIAL DISABILITY – You will be considered partially disabled if, due tosickness or injury, you are unable to perform all of the normal duties of yourregular occupation, but are able to perform some of those duties and arereceiving Earned Income from this or any other gainful occupation (providingyour Earned Income is reduced from its pre-disability level as a result of thedisability). Manulife Financial must receive satisfactory proof of your disabilitywithin 90 days from the date disability begins.

Products:Office Overhead Expense Insurance

OVERVIEW If your business is your family’s source of income, it makes good sense toensure that your business can continue in the event of your Total Disability.Office Overhead Expense Insurance can help pay the bills for your sole practiceor partnership when an accident or illness leaves you temporarily disabled.

DETAILS Reimbursement will be provided for incurred office expenses such as rent,salaries of your regular office staff (not including your salary or salaries paid to relatives or fellow Members), utilities, leased or rented equipment andother fixed expenses which are normal and customary in the operation ofyour office.

The maximum benefit payable will be your average monthly overheadexpenses during the six month period immediately preceding disability. In the event of joint occupancy or partnership, only your share of the officeoverhead expenses will be used in determining the amount of benefitpayable. If you die while in receipt of benefit payments, your estate willreceive an additional three months of benefit payments in a lump sum to help finalize the affairs of your office.

Choose from waiting periods of seven days or 14 days, and from benefitperiods of six months or 12 months. Maximum coverage is 35 units of $100 monthly benefit, or $3,500 a month.

OVERVIEW Member & Spouse Term Life Insurance is priced affordably to help you securethe maximum financial protection for your family’s future well-being.

DETAILS You can choose the amount of Term Life coverage you want to apply for inunits of $35,000. Your maximum coverage is 16 units, or $560,000 perperson through age 65. Beginning at age 66, coverage reduces by $3,500 perunit each year through age 72. OVERVIEW No matter how careful you are, accidents happen – especially with today’sfast-paced lifestyles. Member & Spouse Personal Accident Insurance isdesigned to provide you with an economical safety net.

DETAILS Members and Spouses insured for Term Life are eligible to apply for up to$200,000 of additional protection against the consequences of a seriousaccident. The value of one unit is $25,000 until age 65. Starting at age 66,coverage reduces by $2,500 per unit each year and ends at age 75. Q&A:The OAA Plan has the answers you’re looking for…

Who is eligible?

All applicants must be residents of Canada and under age 60 years of age.

Spouses and children of Members and employees participating in MemberTerm Life are eligible for Spouse Term Life and Child Life Insurance respectively.(Eligible children must be unmarried, dependent on the member for support,over 14 days and under 23 years of age or under 19 years of age if not in full-time attendance at a school or university.)

Members participating in Member Term Life and spouses participating inSpouse Term Life are eligible to apply for Personal Accident Insurance.

Is a medical exam required?

A short statement of health and other particulars of insurability are required ofall applicants. Manulife Financial may request a physician’s report, a medicalexamination or medical test of any applicant, which will be made at no expenseto the applicant. You will be notified directly if one is required.

When does the insurance take effect?

Insurance will take effect on the date the completed application and requiredpremium are received by Manulife Financial, subject to the final approval of itsunderwriters. In the event the applicant is not insurable, a full refund of premiumwill be made.

Is there a Certificate of coverage?

Each insured Member will receive an individual Certificate specifying thecoverage provided and outlining the main Group Policy provisions.

What happens to my insurance if I move or change jobs?

Unlike coverage you may have through your employer, this protection stayswith you if you change jobs or residence anywhere in the world, as long asyou continue to meet eligibility requirements.

Can the beneficiary be changed?

Beneficiary designations may be changed at any time by providing writtennotification to the insurer, unless the beneficiary designation is irrevocable.Beneficiary designations are subject to applicable provincial law.
