Johns Manville Fire
On May 16th, 2003, a fire at the Johns Manville Plant #3on Columbus Ave., in Defiance, Ohio, broke out in the pipe insulation manufacturing area of the facility. When the fire occurred, there was over 100 employees in the plant. Everyone was evacuated. The fire was immediately too big to fight by theemployees and in a matter of a few hours, the manufacturing plant was destroyed. Had it not been for the plants’ Emergency Action Plan the outcome could have been much worse. Not one person was injuredin the fire.

Fire SituationWhen FIRE is discovered:- Activate the nearest Fire Alarm (if installed)- If fire alarms are not available, notify the site personnel about the fire by
one of the following means
- Voice - Radio
- Phone Page- DIAL 911 Notify your local Fire Dept.
Upon Notification of the fire , OCCUPANTS MUST:
- Leave the building using the escape route
- Assemble at the designated area
- Remain outside until the authorities (facility personnel or fire officials)
announces that it is safe to reenter

Fight the Fire ONLY if :
- Notification has been made to evacuate the building
- The Fire Dept. has been notified
- Escaping the area is possible by backing up to the nearest exit
- The Fire is in the Incipient Stage ( Small in Size)
- The Fire Extinguisher(s) is in working condition and the personnel are trained to operate the extinguisher(s)

Once the Fire Dept. arrives, a designated official needs to makecontact with the fire ground officer.
The following information needs to be exchanged :
- Is everyone out of the building and accountable?- What is on fire?- Where is the location of the fire?- What has been already done?- What does the fire dept. need to do to fight the fire?

Tornados : When a warning is issued, have the employees seek shelter. Consider the following: - Establishing a designated area within the facility- Small interior rooms on the lowest level without windows- Hallways on the lowest floor level away from doors &
windows- Rooms constructed with reinforced concrete, brick or block with no windows.- Stay away from outside walls and windows- Have a means of communications with you- Be huddled down use your arms to protect your head & neck- Remain sheltered until the tornado threat has past

- Take a initial survey of the damage
- Determine your needs
- Medical Attention for the injured

Small Spill or Leak
- Contact your supervisor immediately- Follow the guidelines of the MSDS on the material- If toxic vapors are present secure the area and evacuate
personnel if needed- Have proper PPE being worn to provide protection if you
are trained to handle the chemical- Notify local authorities if necessary

Large Spill or Leak- Follow the procedures set up for a small spill or leak - Notify your Supervisor of the situation immediately- Evacuate the area if necessary- (If trained to do so) contain the spill or leak with materials
and equipment that is available on site without endangering ones self
- If anyone is injured or contaminated, isolate that person(s) from others
- Notify local authorities of the incident :Fire Dept, EMA, clean up company, etc.

- Meet face to face with officials- Advise accountability of employees and of any injuries- Provide MSDS on the chemical(s) involved - Provide the following information - What is the chemical(s) - Is it a Liquid, Solid or Gas - The quantity of the release - What has been done so far

• Keep the caller talking as long as possible. Let them speak, but ask as many questions as possible, while someone else calls 911 and relay info while you talk to the caller…
Asked the following questions :- Where is the bomb located? - Which building? - Which floor? - Which room?- What time is the bomb to go off?- What is your name or organization that you are involved?

If the bomb is indicated of being outside : Follow the Severe Weather Procedure.
