2 Guidelines protocols

3 Anticoagulation

Please complete deanery E-learning package if you have not already done so This is mandatory

There is an educational presentation on the Haematology intranet page (education section)

Guidelines are available on all aspects of anticoag either on the intranet or via Haematology

If in doubt ASK

Warfarin chart doubles as referral form and details of induction are on the form

If patient is to be referred to anticoag clinic referral form MUST be completed in full

Pre-op proformas for patients on long term anticoagulation are produced in pre-op assessment clinic but we can also advise

NB ndash VTE risk assessment please on all patients admitted or day cases

4 Transfusion

Please complete national or deanery E-learning package if you have not already done so One of these is mandatory please contact the trust transfusion practitioner (Tracy Clarke ext 5745) if you have any queries

Competency assessment ndash this is mandatory and dates will be arranged soon after you start for this to take place It will cover all aspects of the correct procedure for blood sampling for transfusion

We keep in stock Red cells Platelets FFP albumin cryoprecipitate anti-D some factor concentrates Beriplex Octaplex Please follow guidelines for their use ( see above)

Blood components must not be given without prescription on patient transfusion record

Follow Trust Maximum Blood Order Schedule (MBOS)

Platelets need to be ordered from Birmingham - please dw Haematologist

Please allow sufficient time (if possible) for these to be preparedissued safely - NONE ARE AVAILABLE INSTANTLY

Avoid unnecessary transfusion at all costs and always consider other possible solutions eg FeB12folatenothing

Verbal consent for transfusion MUST be obtained if at all possible and this and the reason for transfusion clearly documented on the Patient Transfusion Record

Explain risks and benefits to patients and ensure they have an information leaflet

Correct procedures must be followed for patient sampling

The final identity check when taking a blood sample MUST be done at the patientrsquos bedside against a wristband and request form

No other form of checking is acceptable under any circumstances and this must NEVER be omitted URGENTOUTSIDE CORE HOURS The Blood Sciences Laboratory is open 247 though at reduced staffing levels outside core hours Out of hours services are provided by the Microbiology Department For most up to date information please refer to the Pathology Department webpages NORMAL WORKING HOURS Clearly mark all urgent requests lsquourgentrsquo and notify the laboratory by telephone only for VERY urgent requests

Clinical ChemistryHaematology All work sent will be processed as soon as possible with an expectation that urgent results will be available within 1 hour of sample receipt There is no need to contact the laboratory to inform that specimens are being sent For clinical emergencies CSF samples arterial blood gas samples and other samples collected on ice please phone the lab to ensure that these samples are prioritised (contact ext 4040) Specimens marked lsquourgentrsquo without a prior message will be processed with the next batch Transfusion For clinical emergencies and all crossmatch requests a phone call must be made to the laboratory on 5711 to ensure staff can prioritise these appropriately The request card must include the patientrsquos name or (emergency ID) date of birth hospital number consultant ward and time of specimen This is essential for electronic reporting of results Inadequately labelled or illegible samples will be rejected and investigations not performed Results will only be phoned if delay in transmitting results is likely the Biomedical Scientist (BMS) has already agreed to phone or results alert BMS to possibility that immediate action may be required Additional Tests ndash Sample stability permitting tests can be added to previous samples A written request form containing patient information and details of additional tests must be sent to the laboratory Microbiology - Out-of-Hours arrangements - Urgent

If a specimen requires urgent processing contact the Biomedical Scientist on call via the hospital switchboard The Biomedical Scientist does not need to be informed if blood cultures have been collected out of hours

If urgent medical advice is required contact the Consultant Microbiologist on call via the hospital switchboard

Microbiology is open from 0830 until 5pm during the working week and 0830 until 1200 on Saturday morning Specimens received by Pathology for microbiological testing out of working hours will be kept in Pathology reception When the on call Microbiology BMS is called in for an urgent sample they will refrigerate them At this point they will also process any blood cultures received

BLOOD RESULTS Results are available as soon as they have been processed on the computers on the wards using APEX LABORATORY RESULT ENQUIRIES USING lsquoAPEXrsquo double click on the lsquoApexrsquo icon Username apex (enter) pause as typing too fast may lock the terminal If asked for Answerback use the applicable answerback as provided by your colleagues within your work area In an

emergency use PATH or PILL (in capitals) You will go to a new screen asking for a Username and a Password Ask colleagues for this usernamepassword Welcome Screen type 22 (enter) for Enquiry screen Enter the RLQ in Patient Numberenter Surname Type in at least the first 2 letters of the patientrsquos surname (enter) Hit enter again until data shown

The most recent results will be displayed To view earlier records use the Page up and page down keys

NB There may be too many results to fit a single screen use the lsquodownrsquo arrow key to display the further results on this sample lsquouprsquo arrow will return you to top half of results

Cumulative Results can also be shown At the option bar select lsquo5rsquo (enter) Graph- press lsquo2rsquo then enter the result you would like a graph for Graph- press lsquo2rsquo then enter the result you would like a graph for X-RAYRADIOLOGY DEPARTMENT Please ask if you are not sure which test is most important Access is available on every Computer desk top to The Royal College of Radiologists Referral Guidelines Contact numbers

X-ray Reception Appointments 401429994027 Main Viewing Area 1638 Nuclear Medicine 1651 Secretaries 16712990 There is a Radiographer in the department from 800 am to perform plain films From 5 pm to midnight there are 2 Radiographers covering the hospital for urgent cases Plan the routine (pre ward round) work 800 am ndash 500 pm After midnight one Radiographer is on call ndash bleep via switchboard A Radiologist is in the department until 800 pm Monday to Friday After that time the on-call Radiologist can be contacted via switchboard All requests MUST BE CONSULTANT TO CONSULTANT except in an extreme emergency For emergency CT scanning requests out of hours there is a remote reporting service (TMC) which is available via switchboard Criteria for requesting can be found in AE and must be followed When completing a request form please fill in ALL the relevant details ensure the correct patient id sticker has been applied to the form and check PACS for any previous investigations to prevent unnecessary referrals This is a LEGAL requirement Give helpful and appropriate clinical details be honest OUT-OF-HOURS INVESTIGATIONS

Keep out-of-hours blood tests and X-ray requests to those that will alter your immediate management The remainder can be done as a routine the following day

There are haematology and separate biochemistry on-call technicians who will need to be contacted via switchboard after 500 pm and before 900 am If you are in the hutted wards you will need to phone a porter to take out-of-hours bloods to the Lab During the day there are routine portering facilities for collection of blood samples

On-call Radiographer can be contacted through switchboard

If you want to get an investigation done quickly it is most effective if you yourself speak to the person concerned This applies particularly for CT or other urgent X-ray procedure requests

Enter either patientrsquos RLQ or name (enter) PHARMACY Open Mon-Fri 830-530 Sat Sun and Bank Holidays 10am-1230pm Outside of these times the On-call Pharmacist is available via switchboard EDS requests take time to be dispensed and wherever possible should be at the Pharmacy before 5 pm Mon-Fri and before 1230 on Saturday Sundays and Bank Holidays if the medication is required on the same day In all cases endeavour to get the EDS to pharmacy as early in the day as possible to ensure patient flow is an efficient as possible Most wards have a pharmacist visit each day The pharmacist will confirm medication histories and complete medicines reconciliations check medication charts and advise where necessary They are more than happy to answer any medicine related questions Please ensure you read and act on confirm actions from the advice written in green by the pharmacist This advice can be found on the In-patent Medication Chart (IMC) or in the patientrsquos notes Pharmacy Phone numbers General Enquiries 4016 Medicines Information 4017 Dispensary 4424 Dispensary Pharmacist 2200

Be aware of what is an acceptable abbreviation when prescribing and what is not Read the Trustrsquos Medicine Code in relation to Prescribing ndash Section 2 and The Trustrsquos Prescribing Guidelines Both on the Medicines Optimisation page of the intranet Link above This will save you a lot of time in the long run as you will be asked to make amendments where your practice does not meet the standards required

Establish Allergy status Challenge allergies Note the reaction as this could be an intolerance NOT a true allergy Do this BEFORE prescribing and record the details including the reaction on the IMC and communicate this to all staff IF it is a life threatening anaphylactic reaction

Do not writerdquo IV Oral SCrdquo ndash oral as route options this is not good practise It should be one or the other especially for drugs where doses are not equivalent for example for Morphine

When clerking patients take care that details are accurate

Do not assume the medication they have brought in is their own

If the patient is re-admitted from a WVT community bed you DO NOT need to re-write the In-Patient Medication Chart (IMC)

IF your patient is discharging to a WVT community bed please ensure that an EDS is completed AND the In-patient Medication Chart has AT LEAST three days of administration left to record If there is not sufficient room the chart MUST be re-written

If you see the annotation OA written in green by a pharmacist this is highlighting to you that the dose written on clerking is not the same as the dose prior to admission

Pharmacists will also currently annotate page 4 of the IMC any medication not prescribed that the patient was on prior to admission The IMC is currently (June 2016) under review so this page number may change

Do NOT abbreviate the word UNITS when prescribing Insulin or any other medication This is a NEVER EVENT if the dose is incorrectly administered as a result

Beware concomitant prescribing of SC Low Molecular Weight Heparin (LMWH) and the new oral anticoagulants (NOACs)One at a time WVT formulary medicines for LMWH are Enoxaparin Rivaroxaban is the NOAC

FY1 medical staff are not permitted to prescribe for Out Patients on FP10 prescription pads A systematic review highlights seven drugs drug classes are responsible for almost half of all SERIOUS

Medications errors Namely- Methotrexate Warfarin NSAIDs Digoxin opiates acetylic salicylic acid Beta blockers Take extra care

Record a medication list doses routes etc on the Medication History form of the admission booklet Ideally use TWO sources and record the initial sources you have used

Discharging Patients ndash Top Tips

Start the Electronic Discharge Summary (EDS) as early as possible in the patientrsquos stay and up-date as you have more information

Check ALL information written for you by the pharmacist in green in the notes or on the IMC This will help you get the correct information and prescription correct first time

Ensure the patientrsquos destination on discharge is correct If a patient is going to one of WVT Community Hospital beds pharmacy will not dispense any items that they hold as stock Therefore if the destination changes from a Community Hospital bed to home and the discharge is complete it will require a re-work to dispense any items not dispensed ( Bromyard Leominster Ledbury Bromyard Hillside)

If a patient requires a Controlled Drug when doing an EDS you will need to write a ldquopaper prescriptionrdquo This is located on the Trust Intranet There is also guidance on the Medicines Optimisation intranet page on CDs available and their pack sizes Link to CD prescription httpnwwherefordshirenhsukHHTHomepageaspx On the reverse of the CD prescription is guidance on how to complete a CD prescription and meet all the legal requirements

A complete list of medications being taken by the patient is required on the EDS even if nothing is required to be dispensed The medications ldquoStopped Changed needs to be completed so that all information for the GP is clear and not ambiguous in any way Detail medications stopped changed doses increased or reduced or suspended while in hospital

Antibiotic Guidelines These are available on the intranet and also available to download as the Microguide app Prior to all prescribing the ALLERGY STATUS MUST BE ESTABLISHED Where a patient states they have an allergy the allergy MUST be challenged and the reaction established to rule out a possible intolerance as opposed to an allergy For all antibiotic prescriptions document the indication and duration and review date on the prescription ALL antibiotic prescriptions MUST have a documented review within 72 hours Remember ASPIRE to the principles of good antimicrobial stewardship when prescribing

A Allergy status - Check BEFORE prescribing

S Samples - Have appropriate Samples been taken BEFORE antibiotics commenced

P Policy - Antibiotic choice in accordance with guidelines

I Indication for use ndash Ensure this is recorded on the IMC

R Route ndash Insure your directions are clear Calculate and document the Infection Severity Score(ISS) to determine if IV or Oral treatment is appropriate and Review daily to ensure prompt stepdown to oral treatment

E End review date Document on the IMC

Medicines Related Guidelines These are currently available under the Medicines Optimisation section of the intranet Please establish you can locate these guidelines before you start prescribing- Use the link below Medicine Optimisation home page then therdquo Quick Linksrdquo box on left hand side of the page The fourth one down httpnwwherefordshirenhsukMedicinesManagementHHTHometabid2714Defaultaspx Controlled Drugs (CDs) You will have been issued with a CD aid This will assist you to ensure you prescribe all CDs according to legal requirements It will also tell you what we stock at WVT and what the pack sizes are In addition to Schedule 2 CDs prescriptions are required for Schedule 3 CDs on Discharge This includes Midazolam Temazepam and Tramadol Warfarin From Monday 3

rd August 2015 warfarin dosing while patients are ldquoIn-Patientsrdquo within WVT was switched from the

traditional time of 600pm to 200pm This is to prevent patientrsquos INRs being left to the on call teams to check and prescribe the dose for transfer to a community bed or discharge Note Patients can revert to their 600pm dosing when they return home Methotrexate If a patient requires this WEEKLY medication either orally or Sub-Cut it cannot be prescribed by an FY1 or FY2 This requires a Consultant registrar or Clinical Nurse specialist Obtaining medicines Out of Hours Wards can borrow medicines and there is a stock list of all medication held at each WVT location on the Medicines Optimisation page of the intranet Medications in original packs only can be borrowed (they donrsquot have to be full packs) Strips of medication or individual vials cannot be supplied to another ward There is an Emergency Medicines Cupboard ndash Keys can be accessed at switch board via the Clinical Site Manager ndashNOTE this stock is NOT to be used for discharging patients For a clinical emergency or advice there is a Pharmacist on-call Contact via switchboard

POSTGRADUATE MEDICAL CENTRE (Ext 40255194)

Get to know the staff This is the place to go if you need help or advice on any aspect of your job All trainees post is delivered to the Centre for your collection The Clinical Tutor is based at the Centre The Centre is open 9 am - 5 pm Monday to Thursday and 9 am ndash 4 pm on a Friday Centre Manager Mrs Helen Pryce If you wish to access the Centre out of these hours (ie to use the library) you will need to apply to the library for an out of hours card A Junior Doctors Forum is held once every other month in the Postgraduate Medical Centre Chaired by the Clinical Tutor and Foundation Programme Director The Centre Manager will inform you by e-mail of any future forums CLINICAL SUPERVISION Each junior doctor in the Trust is allocated an Educational Supervisor it is usually the Consultant for whom they are working This person should be the first port of call for problems In addition there is a Clinical Tutor who is available to provide support and discuss any difficulties MEDICAL MANPOWER OFFICE (Ext 5789 ndash 8am to 5 pm) Their role is to ensure there is adequate medical staffing on each day and co-ordinate rotas annual leave and study leave Also Kay Price is available to discuss other matters which arise day to day Please contact them if you are sick or unable to come in to work Please ensure that you familiarise yourself with individual arrangements for different departments ID BADGES [with photo] Available from Main Reception on Tuesdays amp Thursdays between 12 and 1pm THE MESS The Mess is situated on the 2

nd floor above the main entrance next to the on-call rooms It is the place to go to relax

3 Anticoagulation

Please complete deanery E-learning package if you have not already done so This is mandatory

Guidelines are available on all aspects of anticoag either on the intranet or via Haematology

Warfarin chart doubles as referral form and details of induction are on the form

If patient is to be referred to anticoag clinic referral form MUST be completed in full

Pre-op proformas for patients on long term anticoagulation are produced in pre-op assessment clinic but we can also advise

4 Transfusion

Please complete national or deanery E-learning package if you have not already done so One of these is mandatory please contact the trust transfusion practitioner (Tracy Clarke ext 5745) if you have any queries

Competency assessment ndash this is mandatory and dates will be arranged soon after you start for this to take place It will cover all aspects of the correct procedure for blood sampling for transfusion

Blood components must not be given without prescription on patient transfusion record

Follow Trust Maximum Blood Order Schedule (MBOS)

Please allow sufficient time (if possible) for these to be preparedissued safely - NONE ARE AVAILABLE INSTANTLY

Avoid unnecessary transfusion at all costs and always consider other possible solutions eg FeB12folatenothing

Verbal consent for transfusion MUST be obtained if at all possible and this and the reason for transfusion clearly documented on the Patient Transfusion Record

Explain risks and benefits to patients and ensure they have an information leaflet

Correct procedures must be followed for patient sampling

The final identity check when taking a blood sample MUST be done at the patientrsquos bedside against a wristband and request form

No other form of checking is acceptable under any circumstances and this must NEVER be omitted URGENTOUTSIDE CORE HOURS The Blood Sciences Laboratory is open 247 though at reduced staffing levels outside core hours Out of hours services are provided by the Microbiology Department For most up to date information please refer to the Pathology Department webpages NORMAL WORKING HOURS Clearly mark all urgent requests lsquourgentrsquo and notify the laboratory by telephone only for VERY urgent requests

Clinical ChemistryHaematology All work sent will be processed as soon as possible with an expectation that urgent results will be available within 1 hour of sample receipt There is no need to contact the laboratory to inform that specimens are being sent For clinical emergencies CSF samples arterial blood gas samples and other samples collected on ice please phone the lab to ensure that these samples are prioritised (contact ext 4040) Specimens marked lsquourgentrsquo without a prior message will be processed with the next batch Transfusion For clinical emergencies and all crossmatch requests a phone call must be made to the laboratory on 5711 to ensure staff can prioritise these appropriately The request card must include the patientrsquos name or (emergency ID) date of birth hospital number consultant ward and time of specimen This is essential for electronic reporting of results Inadequately labelled or illegible samples will be rejected and investigations not performed Results will only be phoned if delay in transmitting results is likely the Biomedical Scientist (BMS) has already agreed to phone or results alert BMS to possibility that immediate action may be required Additional Tests ndash Sample stability permitting tests can be added to previous samples A written request form containing patient information and details of additional tests must be sent to the laboratory Microbiology - Out-of-Hours arrangements - Urgent

If a specimen requires urgent processing contact the Biomedical Scientist on call via the hospital switchboard The Biomedical Scientist does not need to be informed if blood cultures have been collected out of hours

If urgent medical advice is required contact the Consultant Microbiologist on call via the hospital switchboard

Microbiology is open from 0830 until 5pm during the working week and 0830 until 1200 on Saturday morning Specimens received by Pathology for microbiological testing out of working hours will be kept in Pathology reception When the on call Microbiology BMS is called in for an urgent sample they will refrigerate them At this point they will also process any blood cultures received

BLOOD RESULTS Results are available as soon as they have been processed on the computers on the wards using APEX LABORATORY RESULT ENQUIRIES USING lsquoAPEXrsquo double click on the lsquoApexrsquo icon Username apex (enter) pause as typing too fast may lock the terminal If asked for Answerback use the applicable answerback as provided by your colleagues within your work area In an

emergency use PATH or PILL (in capitals) You will go to a new screen asking for a Username and a Password Ask colleagues for this usernamepassword Welcome Screen type 22 (enter) for Enquiry screen Enter the RLQ in Patient Numberenter Surname Type in at least the first 2 letters of the patientrsquos surname (enter) Hit enter again until data shown

The most recent results will be displayed To view earlier records use the Page up and page down keys

NB There may be too many results to fit a single screen use the lsquodownrsquo arrow key to display the further results on this sample lsquouprsquo arrow will return you to top half of results

X-ray Reception Appointments 401429994027 Main Viewing Area 1638 Nuclear Medicine 1651 Secretaries 16712990 There is a Radiographer in the department from 800 am to perform plain films From 5 pm to midnight there are 2 Radiographers covering the hospital for urgent cases Plan the routine (pre ward round) work 800 am ndash 500 pm After midnight one Radiographer is on call ndash bleep via switchboard A Radiologist is in the department until 800 pm Monday to Friday After that time the on-call Radiologist can be contacted via switchboard All requests MUST BE CONSULTANT TO CONSULTANT except in an extreme emergency For emergency CT scanning requests out of hours there is a remote reporting service (TMC) which is available via switchboard Criteria for requesting can be found in AE and must be followed When completing a request form please fill in ALL the relevant details ensure the correct patient id sticker has been applied to the form and check PACS for any previous investigations to prevent unnecessary referrals This is a LEGAL requirement Give helpful and appropriate clinical details be honest OUT-OF-HOURS INVESTIGATIONS

Keep out-of-hours blood tests and X-ray requests to those that will alter your immediate management The remainder can be done as a routine the following day

There are haematology and separate biochemistry on-call technicians who will need to be contacted via switchboard after 500 pm and before 900 am If you are in the hutted wards you will need to phone a porter to take out-of-hours bloods to the Lab During the day there are routine portering facilities for collection of blood samples

On-call Radiographer can be contacted through switchboard

If you want to get an investigation done quickly it is most effective if you yourself speak to the person concerned This applies particularly for CT or other urgent X-ray procedure requests

Establish Allergy status Challenge allergies Note the reaction as this could be an intolerance NOT a true allergy Do this BEFORE prescribing and record the details including the reaction on the IMC and communicate this to all staff IF it is a life threatening anaphylactic reaction

Do not writerdquo IV Oral SCrdquo ndash oral as route options this is not good practise It should be one or the other especially for drugs where doses are not equivalent for example for Morphine

When clerking patients take care that details are accurate

Do not assume the medication they have brought in is their own

If the patient is re-admitted from a WVT community bed you DO NOT need to re-write the In-Patient Medication Chart (IMC)

IF your patient is discharging to a WVT community bed please ensure that an EDS is completed AND the In-patient Medication Chart has AT LEAST three days of administration left to record If there is not sufficient room the chart MUST be re-written

If you see the annotation OA written in green by a pharmacist this is highlighting to you that the dose written on clerking is not the same as the dose prior to admission

Pharmacists will also currently annotate page 4 of the IMC any medication not prescribed that the patient was on prior to admission The IMC is currently (June 2016) under review so this page number may change

Do NOT abbreviate the word UNITS when prescribing Insulin or any other medication This is a NEVER EVENT if the dose is incorrectly administered as a result

Beware concomitant prescribing of SC Low Molecular Weight Heparin (LMWH) and the new oral anticoagulants (NOACs)One at a time WVT formulary medicines for LMWH are Enoxaparin Rivaroxaban is the NOAC

FY1 medical staff are not permitted to prescribe for Out Patients on FP10 prescription pads A systematic review highlights seven drugs drug classes are responsible for almost half of all SERIOUS

Medications errors Namely- Methotrexate Warfarin NSAIDs Digoxin opiates acetylic salicylic acid Beta blockers Take extra care

Record a medication list doses routes etc on the Medication History form of the admission booklet Ideally use TWO sources and record the initial sources you have used

Discharging Patients ndash Top Tips

Start the Electronic Discharge Summary (EDS) as early as possible in the patientrsquos stay and up-date as you have more information

Check ALL information written for you by the pharmacist in green in the notes or on the IMC This will help you get the correct information and prescription correct first time

Ensure the patientrsquos destination on discharge is correct If a patient is going to one of WVT Community Hospital beds pharmacy will not dispense any items that they hold as stock Therefore if the destination changes from a Community Hospital bed to home and the discharge is complete it will require a re-work to dispense any items not dispensed ( Bromyard Leominster Ledbury Bromyard Hillside)

If a patient requires a Controlled Drug when doing an EDS you will need to write a ldquopaper prescriptionrdquo This is located on the Trust Intranet There is also guidance on the Medicines Optimisation intranet page on CDs available and their pack sizes Link to CD prescription httpnwwherefordshirenhsukHHTHomepageaspx On the reverse of the CD prescription is guidance on how to complete a CD prescription and meet all the legal requirements

A complete list of medications being taken by the patient is required on the EDS even if nothing is required to be dispensed The medications ldquoStopped Changed needs to be completed so that all information for the GP is clear and not ambiguous in any way Detail medications stopped changed doses increased or reduced or suspended while in hospital

Antibiotic Guidelines These are available on the intranet and also available to download as the Microguide app Prior to all prescribing the ALLERGY STATUS MUST BE ESTABLISHED Where a patient states they have an allergy the allergy MUST be challenged and the reaction established to rule out a possible intolerance as opposed to an allergy For all antibiotic prescriptions document the indication and duration and review date on the prescription ALL antibiotic prescriptions MUST have a documented review within 72 hours Remember ASPIRE to the principles of good antimicrobial stewardship when prescribing

Clinical ChemistryHaematology All work sent will be processed as soon as possible with an expectation that urgent results will be available within 1 hour of sample receipt There is no need to contact the laboratory to inform that specimens are being sent For clinical emergencies CSF samples arterial blood gas samples and other samples collected on ice please phone the lab to ensure that these samples are prioritised (contact ext 4040) Specimens marked lsquourgentrsquo without a prior message will be processed with the next batch Transfusion For clinical emergencies and all crossmatch requests a phone call must be made to the laboratory on 5711 to ensure staff can prioritise these appropriately The request card must include the patientrsquos name or (emergency ID) date of birth hospital number consultant ward and time of specimen This is essential for electronic reporting of results Inadequately labelled or illegible samples will be rejected and investigations not performed Results will only be phoned if delay in transmitting results is likely the Biomedical Scientist (BMS) has already agreed to phone or results alert BMS to possibility that immediate action may be required Additional Tests ndash Sample stability permitting tests can be added to previous samples A written request form containing patient information and details of additional tests must be sent to the laboratory Microbiology - Out-of-Hours arrangements - Urgent

If a specimen requires urgent processing contact the Biomedical Scientist on call via the hospital switchboard The Biomedical Scientist does not need to be informed if blood cultures have been collected out of hours

If urgent medical advice is required contact the Consultant Microbiologist on call via the hospital switchboard

X-ray Reception Appointments 401429994027 Main Viewing Area 1638 Nuclear Medicine 1651 Secretaries 16712990 There is a Radiographer in the department from 800 am to perform plain films From 5 pm to midnight there are 2 Radiographers covering the hospital for urgent cases Plan the routine (pre ward round) work 800 am ndash 500 pm After midnight one Radiographer is on call ndash bleep via switchboard A Radiologist is in the department until 800 pm Monday to Friday After that time the on-call Radiologist can be contacted via switchboard All requests MUST BE CONSULTANT TO CONSULTANT except in an extreme emergency For emergency CT scanning requests out of hours there is a remote reporting service (TMC) which is available via switchboard Criteria for requesting can be found in AE and must be followed When completing a request form please fill in ALL the relevant details ensure the correct patient id sticker has been applied to the form and check PACS for any previous investigations to prevent unnecessary referrals This is a LEGAL requirement Give helpful and appropriate clinical details be honest OUT-OF-HOURS INVESTIGATIONS

Keep out-of-hours blood tests and X-ray requests to those that will alter your immediate management The remainder can be done as a routine the following day

There are haematology and separate biochemistry on-call technicians who will need to be contacted via switchboard after 500 pm and before 900 am If you are in the hutted wards you will need to phone a porter to take out-of-hours bloods to the Lab During the day there are routine portering facilities for collection of blood samples

If you see the annotation OA written in green by a pharmacist this is highlighting to you that the dose written on clerking is not the same as the dose prior to admission

Pharmacists will also currently annotate page 4 of the IMC any medication not prescribed that the patient was on prior to admission The IMC is currently (June 2016) under review so this page number may change

Do NOT abbreviate the word UNITS when prescribing Insulin or any other medication This is a NEVER EVENT if the dose is incorrectly administered as a result

Beware concomitant prescribing of SC Low Molecular Weight Heparin (LMWH) and the new oral anticoagulants (NOACs)One at a time WVT formulary medicines for LMWH are Enoxaparin Rivaroxaban is the NOAC

FY1 medical staff are not permitted to prescribe for Out Patients on FP10 prescription pads A systematic review highlights seven drugs drug classes are responsible for almost half of all SERIOUS

Record a medication list doses routes etc on the Medication History form of the admission booklet Ideally use TWO sources and record the initial sources you have used

Discharging Patients ndash Top Tips

Start the Electronic Discharge Summary (EDS) as early as possible in the patientrsquos stay and up-date as you have more information

Check ALL information written for you by the pharmacist in green in the notes or on the IMC This will help you get the correct information and prescription correct first time

Ensure the patientrsquos destination on discharge is correct If a patient is going to one of WVT Community Hospital beds pharmacy will not dispense any items that they hold as stock Therefore if the destination changes from a Community Hospital bed to home and the discharge is complete it will require a re-work to dispense any items not dispensed ( Bromyard Leominster Ledbury Bromyard Hillside)

If a patient requires a Controlled Drug when doing an EDS you will need to write a ldquopaper prescriptionrdquo This is located on the Trust Intranet There is also guidance on the Medicines Optimisation intranet page on CDs available and their pack sizes Link to CD prescription httpnwwherefordshirenhsukHHTHomepageaspx On the reverse of the CD prescription is guidance on how to complete a CD prescription and meet all the legal requirements

A complete list of medications being taken by the patient is required on the EDS even if nothing is required to be dispensed The medications ldquoStopped Changed needs to be completed so that all information for the GP is clear and not ambiguous in any way Detail medications stopped changed doses increased or reduced or suspended while in hospital

Antibiotic Guidelines These are available on the intranet and also available to download as the Microguide app Prior to all prescribing the ALLERGY STATUS MUST BE ESTABLISHED Where a patient states they have an allergy the allergy MUST be challenged and the reaction established to rule out a possible intolerance as opposed to an allergy For all antibiotic prescriptions document the indication and duration and review date on the prescription ALL antibiotic prescriptions MUST have a documented review within 72 hours Remember ASPIRE to the principles of good antimicrobial stewardship when prescribing

3 Anticoagulation

4 Transfusion

Competency assessment ndash this is mandatory and dates will be arranged soon after you start for this to take place It will cover all aspects of the correct procedure for blood sampling for transfusion

Please allow sufficient time (if possible) for these to be preparedissued safely - NONE ARE AVAILABLE INSTANTLY

Avoid unnecessary transfusion at all costs and always consider other possible solutions eg FeB12folatenothing

Verbal consent for transfusion MUST be obtained if at all possible and this and the reason for transfusion clearly documented on the Patient Transfusion Record

Explain risks and benefits to patients and ensure they have an information leaflet

Correct procedures must be followed for patient sampling

The final identity check when taking a blood sample MUST be done at the patientrsquos bedside against a wristband and request form

No other form of checking is acceptable under any circumstances and this must NEVER be omitted URGENTOUTSIDE CORE HOURS The Blood Sciences Laboratory is open 247 though at reduced staffing levels outside core hours Out of hours services are provided by the Microbiology Department For most up to date information please refer to the Pathology Department webpages NORMAL WORKING HOURS Clearly mark all urgent requests lsquourgentrsquo and notify the laboratory by telephone only for VERY urgent requests

Clinical ChemistryHaematology All work sent will be processed as soon as possible with an expectation that urgent results will be available within 1 hour of sample receipt There is no need to contact the laboratory to inform that specimens are being sent For clinical emergencies CSF samples arterial blood gas samples and other samples collected on ice please phone the lab to ensure that these samples are prioritised (contact ext 4040) Specimens marked lsquourgentrsquo without a prior message will be processed with the next batch Transfusion For clinical emergencies and all crossmatch requests a phone call must be made to the laboratory on 5711 to ensure staff can prioritise these appropriately The request card must include the patientrsquos name or (emergency ID) date of birth hospital number consultant ward and time of specimen This is essential for electronic reporting of results Inadequately labelled or illegible samples will be rejected and investigations not performed Results will only be phoned if delay in transmitting results is likely the Biomedical Scientist (BMS) has already agreed to phone or results alert BMS to possibility that immediate action may be required Additional Tests ndash Sample stability permitting tests can be added to previous samples A written request form containing patient information and details of additional tests must be sent to the laboratory Microbiology - Out-of-Hours arrangements - Urgent

If a specimen requires urgent processing contact the Biomedical Scientist on call via the hospital switchboard The Biomedical Scientist does not need to be informed if blood cultures have been collected out of hours

If urgent medical advice is required contact the Consultant Microbiologist on call via the hospital switchboard

Microbiology is open from 0830 until 5pm during the working week and 0830 until 1200 on Saturday morning Specimens received by Pathology for microbiological testing out of working hours will be kept in Pathology reception When the on call Microbiology BMS is called in for an urgent sample they will refrigerate them At this point they will also process any blood cultures received

X-ray Reception Appointments 401429994027 Main Viewing Area 1638 Nuclear Medicine 1651 Secretaries 16712990 There is a Radiographer in the department from 800 am to perform plain films From 5 pm to midnight there are 2 Radiographers covering the hospital for urgent cases Plan the routine (pre ward round) work 800 am ndash 500 pm After midnight one Radiographer is on call ndash bleep via switchboard A Radiologist is in the department until 800 pm Monday to Friday After that time the on-call Radiologist can be contacted via switchboard All requests MUST BE CONSULTANT TO CONSULTANT except in an extreme emergency For emergency CT scanning requests out of hours there is a remote reporting service (TMC) which is available via switchboard Criteria for requesting can be found in AE and must be followed When completing a request form please fill in ALL the relevant details ensure the correct patient id sticker has been applied to the form and check PACS for any previous investigations to prevent unnecessary referrals This is a LEGAL requirement Give helpful and appropriate clinical details be honest OUT-OF-HOURS INVESTIGATIONS

Keep out-of-hours blood tests and X-ray requests to those that will alter your immediate management The remainder can be done as a routine the following day

There are haematology and separate biochemistry on-call technicians who will need to be contacted via switchboard after 500 pm and before 900 am If you are in the hutted wards you will need to phone a porter to take out-of-hours bloods to the Lab During the day there are routine portering facilities for collection of blood samples

On-call Radiographer can be contacted through switchboard

If you want to get an investigation done quickly it is most effective if you yourself speak to the person concerned This applies particularly for CT or other urgent X-ray procedure requests

When clerking patients take care that details are accurate

Do NOT abbreviate the word UNITS when prescribing Insulin or any other medication This is a NEVER EVENT if the dose is incorrectly administered as a result

Beware concomitant prescribing of SC Low Molecular Weight Heparin (LMWH) and the new oral anticoagulants (NOACs)One at a time WVT formulary medicines for LMWH are Enoxaparin Rivaroxaban is the NOAC

FY1 medical staff are not permitted to prescribe for Out Patients on FP10 prescription pads A systematic review highlights seven drugs drug classes are responsible for almost half of all SERIOUS

Medications errors Namely- Methotrexate Warfarin NSAIDs Digoxin opiates acetylic salicylic acid Beta blockers Take extra care

Record a medication list doses routes etc on the Medication History form of the admission booklet Ideally use TWO sources and record the initial sources you have used

Discharging Patients ndash Top Tips

Start the Electronic Discharge Summary (EDS) as early as possible in the patientrsquos stay and up-date as you have more information

Check ALL information written for you by the pharmacist in green in the notes or on the IMC This will help you get the correct information and prescription correct first time

Ensure the patientrsquos destination on discharge is correct If a patient is going to one of WVT Community Hospital beds pharmacy will not dispense any items that they hold as stock Therefore if the destination changes from a Community Hospital bed to home and the discharge is complete it will require a re-work to dispense any items not dispensed ( Bromyard Leominster Ledbury Bromyard Hillside)

If a patient requires a Controlled Drug when doing an EDS you will need to write a ldquopaper prescriptionrdquo This is located on the Trust Intranet There is also guidance on the Medicines Optimisation intranet page on CDs available and their pack sizes Link to CD prescription httpnwwherefordshirenhsukHHTHomepageaspx On the reverse of the CD prescription is guidance on how to complete a CD prescription and meet all the legal requirements

A complete list of medications being taken by the patient is required on the EDS even if nothing is required to be dispensed The medications ldquoStopped Changed needs to be completed so that all information for the GP is clear and not ambiguous in any way Detail medications stopped changed doses increased or reduced or suspended while in hospital

Antibiotic Guidelines These are available on the intranet and also available to download as the Microguide app Prior to all prescribing the ALLERGY STATUS MUST BE ESTABLISHED Where a patient states they have an allergy the allergy MUST be challenged and the reaction established to rule out a possible intolerance as opposed to an allergy For all antibiotic prescriptions document the indication and duration and review date on the prescription ALL antibiotic prescriptions MUST have a documented review within 72 hours Remember ASPIRE to the principles of good antimicrobial stewardship when prescribing

Clinical ChemistryHaematology All work sent will be processed as soon as possible with an expectation that urgent results will be available within 1 hour of sample receipt There is no need to contact the laboratory to inform that specimens are being sent For clinical emergencies CSF samples arterial blood gas samples and other samples collected on ice please phone the lab to ensure that these samples are prioritised (contact ext 4040) Specimens marked lsquourgentrsquo without a prior message will be processed with the next batch Transfusion For clinical emergencies and all crossmatch requests a phone call must be made to the laboratory on 5711 to ensure staff can prioritise these appropriately The request card must include the patientrsquos name or (emergency ID) date of birth hospital number consultant ward and time of specimen This is essential for electronic reporting of results Inadequately labelled or illegible samples will be rejected and investigations not performed Results will only be phoned if delay in transmitting results is likely the Biomedical Scientist (BMS) has already agreed to phone or results alert BMS to possibility that immediate action may be required Additional Tests ndash Sample stability permitting tests can be added to previous samples A written request form containing patient information and details of additional tests must be sent to the laboratory Microbiology - Out-of-Hours arrangements - Urgent

If a specimen requires urgent processing contact the Biomedical Scientist on call via the hospital switchboard The Biomedical Scientist does not need to be informed if blood cultures have been collected out of hours

If urgent medical advice is required contact the Consultant Microbiologist on call via the hospital switchboard

Microbiology is open from 0830 until 5pm during the working week and 0830 until 1200 on Saturday morning Specimens received by Pathology for microbiological testing out of working hours will be kept in Pathology reception When the on call Microbiology BMS is called in for an urgent sample they will refrigerate them At this point they will also process any blood cultures received

X-ray Reception Appointments 401429994027 Main Viewing Area 1638 Nuclear Medicine 1651 Secretaries 16712990 There is a Radiographer in the department from 800 am to perform plain films From 5 pm to midnight there are 2 Radiographers covering the hospital for urgent cases Plan the routine (pre ward round) work 800 am ndash 500 pm After midnight one Radiographer is on call ndash bleep via switchboard A Radiologist is in the department until 800 pm Monday to Friday After that time the on-call Radiologist can be contacted via switchboard All requests MUST BE CONSULTANT TO CONSULTANT except in an extreme emergency For emergency CT scanning requests out of hours there is a remote reporting service (TMC) which is available via switchboard Criteria for requesting can be found in AE and must be followed When completing a request form please fill in ALL the relevant details ensure the correct patient id sticker has been applied to the form and check PACS for any previous investigations to prevent unnecessary referrals This is a LEGAL requirement Give helpful and appropriate clinical details be honest OUT-OF-HOURS INVESTIGATIONS

Keep out-of-hours blood tests and X-ray requests to those that will alter your immediate management The remainder can be done as a routine the following day

There are haematology and separate biochemistry on-call technicians who will need to be contacted via switchboard after 500 pm and before 900 am If you are in the hutted wards you will need to phone a porter to take out-of-hours bloods to the Lab During the day there are routine portering facilities for collection of blood samples

Record a medication list doses routes etc on the Medication History form of the admission booklet Ideally use TWO sources and record the initial sources you have used

Discharging Patients ndash Top Tips

Start the Electronic Discharge Summary (EDS) as early as possible in the patientrsquos stay and up-date as you have more information

Check ALL information written for you by the pharmacist in green in the notes or on the IMC This will help you get the correct information and prescription correct first time

Ensure the patientrsquos destination on discharge is correct If a patient is going to one of WVT Community Hospital beds pharmacy will not dispense any items that they hold as stock Therefore if the destination changes from a Community Hospital bed to home and the discharge is complete it will require a re-work to dispense any items not dispensed ( Bromyard Leominster Ledbury Bromyard Hillside)

If a patient requires a Controlled Drug when doing an EDS you will need to write a ldquopaper prescriptionrdquo This is located on the Trust Intranet There is also guidance on the Medicines Optimisation intranet page on CDs available and their pack sizes Link to CD prescription httpnwwherefordshirenhsukHHTHomepageaspx On the reverse of the CD prescription is guidance on how to complete a CD prescription and meet all the legal requirements

A complete list of medications being taken by the patient is required on the EDS even if nothing is required to be dispensed The medications ldquoStopped Changed needs to be completed so that all information for the GP is clear and not ambiguous in any way Detail medications stopped changed doses increased or reduced or suspended while in hospital

Antibiotic Guidelines These are available on the intranet and also available to download as the Microguide app Prior to all prescribing the ALLERGY STATUS MUST BE ESTABLISHED Where a patient states they have an allergy the allergy MUST be challenged and the reaction established to rule out a possible intolerance as opposed to an allergy For all antibiotic prescriptions document the indication and duration and review date on the prescription ALL antibiotic prescriptions MUST have a documented review within 72 hours Remember ASPIRE to the principles of good antimicrobial stewardship when prescribing

Get to know the staff This is the place to go if you need help or advice on any aspect of your job All trainees post is delivered to the Centre for your collection The Clinical Tutor is based at the Centre The Centre is open 9 am - 5 pm Monday to Thursday and 9 am ndash 4 pm on a Friday Centre Manager Mrs Helen Pryce If you wish to access the Centre out of these hours (ie to use the library) you will need to apply to the library for an out of hours card A Junior Doctors Forum is held once every other month in the Postgraduate Medical Centre Chaired by the Clinical Tutor and Foundation Programme Director The Centre Manager will inform you by e-mail of any future forums CLINICAL SUPERVISION Each junior doctor in the Trust is allocated an Educational Supervisor it is usually the Consultant for whom they are working This person should be the first port of call for problems In addition there is a Clinical Tutor who is available to provide support and discuss any difficulties MEDICAL MANPOWER OFFICE (Ext 5789 ndash 8am to 5 pm) Their role is to ensure there is adequate medical staffing on each day and co-ordinate rotas annual leave and study leave Also Kay Price is available to discuss other matters which arise day to day Please contact them if you are sick or unable to come in to work Please ensure that you familiarise yourself with individual arrangements for different departments ID BADGES [with photo] Available from Main Reception on Tuesdays amp Thursdays between 12 and 1pm THE MESS The Mess is situated on the 2

Discharging Patients ndash Top Tips

Start the Electronic Discharge Summary (EDS) as early as possible in the patientrsquos stay and up-date as you have more information

Check ALL information written for you by the pharmacist in green in the notes or on the IMC This will help you get the correct information and prescription correct first time

Ensure the patientrsquos destination on discharge is correct If a patient is going to one of WVT Community Hospital beds pharmacy will not dispense any items that they hold as stock Therefore if the destination changes from a Community Hospital bed to home and the discharge is complete it will require a re-work to dispense any items not dispensed ( Bromyard Leominster Ledbury Bromyard Hillside)

POSTGRADUATE MEDICAL CENTRE (Ext 40255194)

Get to know the staff This is the place to go if you need help or advice on any aspect of your job All trainees post is delivered to the Centre for your collection The Clinical Tutor is based at the Centre The Centre is open 9 am - 5 pm Monday to Thursday and 9 am ndash 4 pm on a Friday Centre Manager Mrs Helen Pryce If you wish to access the Centre out of these hours (ie to use the library) you will need to apply to the library for an out of hours card A Junior Doctors Forum is held once every other month in the Postgraduate Medical Centre Chaired by the Clinical Tutor and Foundation Programme Director The Centre Manager will inform you by e-mail of any future forums CLINICAL SUPERVISION Each junior doctor in the Trust is allocated an Educational Supervisor it is usually the Consultant for whom they are working This person should be the first port of call for problems In addition there is a Clinical Tutor who is available to provide support and discuss any difficulties MEDICAL MANPOWER OFFICE (Ext 5789 ndash 8am to 5 pm) Their role is to ensure there is adequate medical staffing on each day and co-ordinate rotas annual leave and study leave Also Kay Price is available to discuss other matters which arise day to day Please contact them if you are sick or unable to come in to work Please ensure that you familiarise yourself with individual arrangements for different departments ID BADGES [with photo] Available from Main Reception on Tuesdays amp Thursdays between 12 and 1pm THE MESS The Mess is situated on the 2

nd floor above the main entrance next to the on-call rooms It is the place to go to relax

chat unwind and sometimes just escape Mess fees pay for cereals bread muffins fruit etc and this is managed by the Mess Presidents Your fee is taken directly from your pay ndash you will need to sign the form in your HR introduction pack In a tiny community like Hereford the Mess is always the centre of activity so enjoy it and keep up the tradition INTRANET A lot of information and particularly clinical guidelines are available on the Herefordshire NHS Internet (nwwherefordshirenhsuk) If you click on lsquoClinical Informationrsquo you will see numerous departments that have produced information and guidelines for many clinical conditions Operational policies can be accessed by clicking onto the Wye Valley NHS Trust site and selecting lsquoPoliciesrsquo Health Work Department The Health Work Department provides a confidential service to all staff including WVT PCT Sodexo and WS Atkins All Health Work Department staff are highly competent individuals incorporating a wealth of experience in the Occupational Health field We are a proactive department and are constantly evaluating our procedures and policies to improve our service provision We have strong links with other Occupational Health services nationally both in the Public and Private sector and are members of NHS Plus The Health Work Department is committed to providing and developing a comprehensive and high quality confidential service to all members of staff in order to preserve and enhance their health at work Sharps Injuries information

The HealthWorkOccupational Health Department is open Monday to Thursday 830am - 500pm and Friday 830am - 400pm
