ASQ Questionnaires

Has your child been screened to make sure he/she is meeting all of the early development milestones? The teachers have completed these simple questionnaires for the children attending the center. See your child’s teacher for the individual results! We also ask that each parent fill out the questionnaire as well. This makes sure that we see how your child is developing at both home and school!

If you are an in-home provider and would like to have the children in your care (between the ages of 0-5 years old) screened, talk to Gail or Brittney at our next visit. We can provide you with the questionnaire that will look at your child’s development in the areas of communication, gross motor, fine motor, problem-solving, and personal-social skills.
