You will be given the opportunity to achieve a $20 weekly discount on your associate medical plan by participating in multiple activities. Spouses are also eligible for the discount by completing the same criteria. Earn TOTAL 400 points by 12/11 in any of the following categories throughout the year. You can earn 200 in one and 200 inanother just so you reach 400:

• CARE: 400 points max • Lifestyle Questionnaire 50 points each• Prevention Screenings & Immunizations 50 points each• Annual Physician Biometric Screening 50 points • Health outcomes 50 points each

(Blood Pressure, Cholesterol, Blood Glucose, Weight (BMI), Nicotine Free)

• Schedule a One-On-One Coaching Session 10 points each• Group Coaching or Workshops 50 points each

• VOLUNTEER: 75 points max• 25 points each, maximum 50 per quarter• Submit Verification for Volunteering in the Community

• Habitat for Humanity • United Way Volunteer (food bank, Boys/Girls Club, etc.) • Toys for Tots, other community drive • Other Community Events (Susan B. Komen, Relay for Life, etc)

At the bottom of the Home page, click on the button that says Go To Trackers and it will take you to the page shown below where you can see your daily activity, challenges, and connect your tracking device.

Google Fit & Fitbit Connection

Log into your account on the wellness portal and select the house icon at the top of the page. Scroll to the bottom and click on the “Go To Trackers” button. Select the option to allow Fitbit or Google Fit to access your account. Google Fit is what you will use for all trackers except the Fitbit. Follow the onscreen instructions to continuing setting this up. Additional steps may be needed on your mobile device to connect your tracker to Google Fit.

Google Fit & Android

1. Set up a Gmail or mock Gmail if you do not have one2. Install the Google Fit app onto your Phone

3. Open the Google Fit app once it is installed and set up yourprofile

4. Your phone will automatically track and transfer yoursteps

that is tied to Google Fit and your tracker.3. Click Allow. Your data should start being read once this is complete!

Google Fit & iPhone Connection

1. Create a Gmail or Mock Gmail if you do not have one.

2. Install the Google Fit app onto your iPhone3. Open the Google Fit app once it is installed and set up your profile4. Open the Health app on your iPhone, click on the profile picture at the top of the page,

scroll down and select Apps under the Privacy heading, then select Google Fit from thelist.

Select “Turn All Categories On.” If there is data you do not want Google Fit to have access to, turn it off here.

Log into your account on the wellness portal and select the house/home icon at the top of the page. Scroll to the bottom and click on the “Go To Trackers” button. Select the option to allow Google Fit to access your account. If you have multiple Google accounts, make sure select the correct one that is tied to Google Fit and your tracker. Your data should start being read once this is complete! NOTE: The tracker will only go back 30 days for points so do this early!

Trust your tobacco cessation treatment to Balanced Wellness.Our online program paired with virtual coaching offers the opportunity to participate at a time that is convenient for you and in the comfort of your home. A workbook is available as an alternative to the online content. You will choose from one-on-one, group support or a combination of both.

As a participant in our twelve week tobacco cessation workshop you will be given access to our wellness portal. With this access, you will have the tools and resources you need to quit for good. The program is designed to support you through weekly tasks that include videos, articles, journaling, and much more! Our valuable self-help resources are tailored to meet yourunique needs.An alternative workbook can be provided upon request.

The program offers a systematic method based on current addiction and behavior change models to help employees develop a plan to quit and provide support you need to remain tobacco-free. We know it’s hard to quit, and our course addresses the challenges of quitting in a sensitive, supportive style.

The program also provides you with the most important resource when it comes to quitting - a coach. Your coach will be available online viathe portal, through virtual (telephonic or computer)one-on-one assistance, and during optional virtualgroup meetings. We are there when you need helpthe most. You are required to complete 6coaching sessions/meetings.

Uploading Documents for Verification1. Click on the pencil icon under the item you want to add

2. On the following screen, choose the date, upload the document from your computer, and click Finish. Your points will be added once the document has been verified.

3. For multiple documents, be sure the date is changed and resubmit.

Personal Information & Mailbox**OPT-in for Important Messages

To access the dropdown user menu, scroll over the profile icon picture at the top of any page. Here you are able to upload a profile picture, update personal information, change your password, view your calendar, or log out of the portal. Beside this menu is your mailbox icon, which you can click on to access your incoming messages or to send messages to someone else.
