SAGE Eldercare’s MissionSAGE Eldercare is a unique community resource that offers a comprehensive array of programs and services for older adults and caregivers, promoting independence and quality of life.
SAGE Eldercare’s HistoryIncorporated in 1954, SAGE Eldercare is a private, nonprofit organization concerned with the health and general welfare of older citizens and their families. SAGE Eldercare grew out of a two-year study initiated by the social studies department of the Summit College Club. A local council on aging emerged from the club’s commitment to educate itself and members of the community on issues affecting the elderly. Council representation came from civic groups, churches, social agencies and interested residents. These community efforts resulted in the formation of SAGE, the Summit Association for Gerontological Endeavor.
SAGE Eldercare is distinguished among elder service providers in being not only the oldest agency in New Jersey, but also one of the few in the United States that offers a comprehensive array of services under one roof.
SAGE Eldercare is a pioneer in providing community-based care for the elderly, beginning with the introduction of its HomeCare services in 1954. SAGE started one of New Jersey’s first Meals On Wheels programs in 1966 and opened one of New Jersey’s first adult day care programs in 1975. Many of the services and programs provided by SAGE Eldercare have served as models for those used by other public and private agencies. Celebrating its 60th year of operation, SAGE Eldercare offers 14 programs that serve 7,000 older people and their families annually in Union, Essex, Morris and Somerset Counties and beyond.

Spend-A-Day, open since 1975, provides a day of social and stimulating activities for frail or impaired adults in a medically supervised, secure, comfortable setting. Licensed by the Department of Health and Senior Services as an adult medical day care, we combine skilled services with extensive recreational, educational, and group programming to offer a viable option to in-home, nursing home, or assisted living care.

What Aging Means to MeFirst Prize Winner of SAGE’s 60th Anniversary Contest

Bette Davis, a true Hollywood legend, once said, “Growing old is not for sissies” . . . How true it that statement?
For me, growing “oldERR” is accepting the progression of The Circle of Life, thank you Hakuna Matata. Let’s face it, we’re ALL going to grow old . . .. make no mistake about it . . . NEWS FLASH!! There is no Fountain of Youth. What did the Borg say to Captain Picard on Star Trek? “Resistance is Futile.” Yep, I’m a Trekkie, too, but that’s what keeps me young at heart. I enjoy EVERYthing about LIFE.
Aging grants you the right to not worry about drudgery things like work (UGH!) or money matters (can’t I just for once win the lottery) -- pick out any of your own. Sure we still worry about them, but it’s all manageable, truly it is, trust me . . . we finally can “allow” (and that’s the key word) ourselves to realize that we can’t always take care of those “things” this very minute, now, today . . . there will always be a tomorrow . . . OR maybe not, in which case we won’t need to worry about it anymore, would we?
Getting older finally finally allows you to do those things you want to do . . . and YES that cliché of travel does hold merit. Who wouldn’t like to just hop in a car and drive to North Carolina, or Virginia, or Washington, or the Jersey Shore . . . There is still so much more to learn, and to see . . . in museums, at the theatre, or visit an old friend, make a new one, volunteer, be a pet walker.
Senior years should be years of new explorations, like exploring within ourselves, for if you enjoy YOURSELF and your company first, YOU WILL enjoy the pleasures of aging, and all the wonders and happiness that awaits you when you accept that next challenge of Life . . . take the plunge . . . the water is fine, no Jaws . . .
Being a Senior is a Good Thing, I finally get a discount at Dunkin Donuts, at my hair salon, at the cinema . . . At the end of the day, age is just a number. It’s about what you do with your Life that matters . . ENJOY IT! . . . That’s an order, period!
I leave you with a quote by Ann Landers (1918-2002), a well-known advice columnist:

Meals On WheelsHot meals delivered with a warm smile
SAGE Eldercare’s Meals On Wheels was established in 1966 and was one of the first meal delivery programs in New Jersey.
Professionally planned and prepared meals are delivered to individuals age 60 or older who are unable to food shop or cook for themselves, or who may need respite services following a hospitalization. There are no income requirements.
SAGE Eldercare delivers meals to:• Berkeley Heights• Chatham• Madison• Millburn• Mountainside• New Providence• Short Hills• Springfield• Summit
Meals are delivered Monday through Friday and can be scheduled for a short time or indefinitely.
Meals On Wheels volunteers deliver meals and visit briefly with
program participants between 11:15 am and 12:30 pm.
Clients must be home to receive their meals, unless special arrangements are made.
Call 908.273.5554, Monday through Friday, between 7:30 am and 3:30 pm to make arrangements. Delivery can be started within a few business days.
Menus are planned by a registered dietician. All meals include milk and in some cases, we are able to accommodate special diets, including diabetic, pureed, renal and cardiac. All meals are relatively low in fat and sodium.

GPS (Guidance, Planning and Support) ServicesPersonalized care plans to get you on the right path
GPS provides caregivers answers on how to provide quality care for the older adults in their lives. Our Care Navigators are registered nurses who specialize in geriatric care and provide personalized care planning, support, and guidance to caregivers and their loved ones.
Basic GPS Service includes:• Assessment of physical and emotional health, support networks,
and community resources• Review of medications and evaluation of regimen• Creation of a personal health record, medication list and a Vial of
Life (emergency info packet)• Evaluation of home safety• Assistance with advanced care planning• Support and resources for caregivers• Coaching on how to communicate better with health care
providers
The Care Navigator provides the family with a written care plan, care options, and available resources. The Care Navigator continues contact with the caregiver for up to one month regarding the progress of care.

Becoming a Certified Dementia Practitioner
If you are a caregiver who cares for someone with Alzheimer’s disease or another type of dementia, SAGE offers educational courses that are related to the CDP certification. There are six parts of training to obtain the certification, all of which help caregivers provide care to patients who are diagnosed with Alzheimer’s disease or a form of dementia. Every two years, ten hours of coursework must be completed to retain the CDP certification.
Currently, the Director of HomeCare and Director of the Spend-A-Day Adult Day Health Center, as well as the majority of staff and Home Health Aides employed by SAGE, have their CDP from the National Council of Certified Dementia Practitioners (NCCDP).

What are the Medicare Insurance Parts A-D?

Hospital insurance that helps cover inpatient care in hospitals, a skilled nursing facility, hospice and home health care.
Medical insurance helps cover medically-necessary services like doctors’ visits, outpatient care, durable medical equipment, home health services and some preventive services. If you don’t join a plan when you are first eligible and don’t have creditable coverage, you are likely to pay a late enrollment penalty.
Medicare Advantage Plan (like an HMO or PPO) is another Medicare health plan choice you may have as part of Medicare; they are offered by private companies approved by Medicare.
Medicare Prescription Drug Coverage is offered to everyone with Medicare. If you don’t join a plan when you are first eligible and you don’t have other creditable coverage, you are likely to pay a late enrollment penalty.

Information and Care Coordination
InfoCare
InfoCare provides social service support to older adults and caregivers through the provision of community referrals and telephone outreach. InfoCare connects seniors with social, educational and clinical opportunities to improve their health and well-being and assists in making linkages to legal assistance and long term planning.
While the department assists callers from across the country, InfoCare is contracted to provide direct social work support to aging adults, particularly those who are frail and isolated, in Summit, Millburn, Short Hills, New Providence and Mountainside.

Information and Care Coordination
Support Groups
P.R.E.P. (People Responsible for Elderly Persons)
Caring for an elderly loved one can be a demanding and stressful responsibility. Help is available at P.R.E.P., a monthly caregiver support group led by a social worker specializing in eldercare. The P.R.E.P. group meets the third Wednesday of each month at 7:00 pm at SAGE. There is no charge, but donations are welcome.
Alzheimer’s Association Caregiver Support Group
The Alzheimer’s Association Caregiver Support Group provides an opportunity for family members and caregivers to discuss the frustrations and trials of caring for someone with Alzheimer’s with others who understand. The group meets at 10:00 am on the fourth Tuesday of every month at SAGE.

Wellness and Prevention
Fall Prevention One third of seniors living in the community fall each year. 50% of these falls happen in a senior’s home. SAGE’s Fall Prevention Initiative educates seniors on how to reduce their chances of falling by modifying their lifestyle and home environments.
SAGE makes presentations throughout the community so participants can learn generally how to prevent falls, as well as understand how they may be specifically at risk. These events can include a range of components, including balance assessments by physical therapists, medication review, exercise demonstrations, and a display of home safety and assistive devices.
Community EducationSAGE hosts programs and events to help educate and provide resources for local residents. Outside professionals are often brought in to discuss topics that are relevant for older adults, their families and caregivers.

Exercise ClassesSAGE’s exercise classes are offered as an off-shoot of the fall prevention initiative, but are open to anyone 60 years of age and older. Current classes are:• Fun-to-be-Fit• Gentle Chair Yoga• Tai Chi
The exercise programs are open to the public and are designed with a gentle approach for all fitness levels to improve strength, flexibility and overall conditioning in an effort to prevent falls.

Furniture Restoration Workshop
Do you have a chair that needs to be re-glued, caned or rushed? Does a table or bookcase need some attention? If you have furniture that you would like to have repaired, the volunteer craftspeople at the SAGE Furniture Restoration Workshop can help.
Since 1961, the workshop has offered fine-quality furniture repair at great prices and serves as a major fundraiser to support SAGE’s programs and services. The volunteers are not trained carpenters or furniture restorers by vocation, but they have gained their skills through years of woodworking and furniture repair as a hobby. Stop by for a visit any weekday from 9:00 am to 12:00 pm. The workshop is located on the lower level of SAGE Eldercare’s offices. For more information, call 908.598.5531.

From basic to highly technological, there are many devices that can be beneficial to older adults, especially those who want to remain in the comfort of their own homes. Following are just a few gadgets that can have a positive impact for aging-in-place (some require wireless access within the home):• Tablets and iPads: many apps can be downloaded to promote brain fitness, track health information, play games to improve cognitive ability and mental agility, view family photos, video chat with family on Skype, or keep up with family on Facebook.• Hearing Aids and TV Ears: bulky listening devices are a thing of the past – they are now tiny, transparent and nearly invisible or even implanted inside the ear; TV Ears allow the user to control the TV volume at a personalized level.• Fitbit: wireless activity trackers to help people lead healthier, more active lives by monitoring steps taken, calories burned, sleep quality and more.
• BeClose: an innovative wireless system that is placed in the home and tracks daily routines;
caregivers can check in at any time by using a private, secure web page and if any disruptions to the normal routine occur, caregivers are alerted by phone, email or text message; no second phone line or broadband connection is necessary for set up.• GrandCare System: similar to BeClose, but is connected to the Internet and communicates with
“ w i r e l e s s ” sensors in the residence; also incorporates socia l izat ion,
entertainment and communication elements by allowing the family to send pictures, messages, emails, reminders, calendar appointments, voice messages, videos, music and more through the system.
• TabSafe: a m e d i c a t i o n m a n a g e m e n t system that reminds, dispenses, alerts and posts information on up
to 13 medicines for one user; with Internet connections, a memory chip in the dispenser allows the pharmacist to interface with the system, which can be managed by the homeowner or a caregiver; all information is HIPAA compliant.• PERS (Personal Emergency Reporting Systems): devices run the gamut from the basic panic button or cord, to a device placed in the home to alert first responders with a “call” for help, to wireless, GPS-enabled devices that can be taken anywhere (GreatCall Responder or MobileHelp).• Home Assistive Devices: There are many devices to help in the home aside from PERS, such as LED lighting, photo-enhanced phone dialers and stove shut-off systems, all of which can help seniors with mild cognitive and motor impairment.

Giving OpportunitiesSAGE Eldercare is a nonprofit organization which relies on the public for support. Donations are accepted in the form of cash, checks, credit cards, securities, as well as vehicles.
All gifts are tax-deductible to the extent permitted by law. Information on SAGE’s nonprofit status can be obtained from the Attorney General of the State of New Jersey at 973.504.6215.
Annual Fund is the foundation of philanthropic support for SAGE. These gifts are an important source of unrestricted funds and underwrite a significant portion of the general operating budget.
1954 Society was established in 2004 in celebration of SAGE’s 50th Anniversary and recognizes major donors to the Annual Fund. Giving levels include: • Champion $10,000 +• Star $5,000 — $9,999• Catalyst $2,500 — $4,999• Advocate $1,500 — $2,499• Enthusiast $1,000 — $1,499• Friend $1 — $999
The Care Fund was created in 2006 to financially support low-income seniors wishing to participate in SAGE Eldercare programs. These donations are strictly for seniors with low income.
SAGE Sustainer program allows donors to become regular givers with a recurring monthly gift. Automatic payments can be
made by credit card or online banking; the process is easy and simple to establish; and, SAGE receives gifts on a consistent basis.
Matching Gifts programs through your employer encourage giving to charitable organizations. Most programs match dollar for dollar and some will even double or triple the gift amount.
Legacy Society members are donors to SAGE’s endowment fund either through direct donations or bequests to SAGE or name SAGE as the ultimate beneficiary of a planned gift. Endowment gifts provide a stable source of revenue to ensure the financial security and are used to support program initiatives and the sustainability of SAGE for the future.
Stocks, bonds, mutual fund shares or other appreciated property donations provide the giver with an income tax charitable deduction for the full, fair market value, no matter what you originally paid, which avoids capital gains tax on any appreciation.
Tribute Gifts can be an appropriate and lasting way to honor a beloved friend or family member or to mark an important milestone such as a birthday or anniversary.
