What do you do when your elderly parentsinsist on living in their home. No matterhow“young at heart” they might be, thereoften are times when more supervision

is needed. Preparingmeals, housekeeping,a trip to the grocerystory and sometimeseven personal care andupkeep. Their adultchildren can feel guiltyand overwhelmedlike they aren’t doingenough.

More of the elderlyare insisting in living at home. But you need notworry, home care providers can ease the burdenon the family and provide a friendly smile andcompanionship to those who are in need.

Demand is increasing for home care professionalssince the population of older people are living longer

and at home.“Among caregivers and those who are in need

of the service, its’ hard to find someone who doesn’thave a story to tell,” said Kris Faupel, marketing

Since they arenot providing medical care, there is no licensinginvolved, however, the company does runbackground checks on the home care providersand they are bonded and insured through thefranchise. Faupel said they also want to knowwhat their interests are which will help placethem with a client.

They also are expected to monitor and givefeedback to the family on changes in behavior,appearance, etc. Often families don’t see specificchanges in their elderly parents and caregiverscan offer that perspective.

“We have a spike in caregiver requests afterthe holidays,” Faupel said. As family membersspend time with their parents they might noticea change— the house is not as well takencare of, their appearance has becomemoredisheveled, they are not eating well, there’s asense of isolation, etc. It’s then the family realizesthey need someone to help.

When someone is requesting home careassistance, the provider goes to the home andmeets with an adult child to get the history.

Following that, they are introduced to the

elderly client.First impressions will tell if it is a right match

between both provider and client — how theyfeel with one another and how at ease they are.

Creating the relationship between the clientand the home care provider also gives adultchildren that security, relieving some of theirguilt. They can be reassured that the client has amultilevel of coverage.

Professional home care providers also can behired by a primary caregiver to provide respitecare, temporary care as a break for them. Respitecare can be provided for a few hours, a day, orlonger.

Faupel said many people think a home careprofessional is “babysitting.”“The job is a lot morethan simply babysitting,”And those who haveexperienced their services have a lasting respectfor them.

Is it age or P.A.D.that’s slowing you down?

You’re out walking, or you’re at home, climbingthe stairs. Your legs or feet ache. Maybe they startto cramp. You get relief when you stop movingbecause the pain goes away.

It’s just part of aging, right? Not necessarily.Discomfort in your legs or feet during the day couldindicate Peripheral Arterial Disease, or P.A.D.

An estimated 8 to 12 million Americans arecurrently living with P.A.D., but only 2 million havebeen diagnosed. Even more worrisome, more than50 percent of those with P.A.D. have no signs orsymptoms of the disease.

According to Alan T. Hirsch, M.D., Professorof Epidemiology and Community Health at theUniversity of Minnesota School of Public Health,“P.A.D. is a disease that has huge significance to thepublic and to every American family because it isassociated with a very high rate of heart attack andstroke, and it is the primary cause of amputation.”

The disease occurs when blood flow to the legsand feet is restricted by a build-up of fatty depositsin the blood vessels in the legs. It can cause pain,cramping and discomfort that can make walking— and life —miserable, or individuals may have nosymptoms at all. Ultimately, untreated blockagescan cut off blood flow entirely and lead to the loss

of a leg or foot to amputation.The symptoms of P.A.D. can be tiredness,

cramping or pain in the legs that occurs when youwalk but goes away when you rest; foot or toe painthat disturbs sleep; and/or slow-to-heal wounds orsores on the feet or toes. Even if you do not havesymptoms, you may have P.A.D. or be at increasedrisk for it.

Factors that put you at risk for P.A.D. include:• Age 50 years and older• Diabetes• African-American or Hispanic• Currently smoke or are a former smoker• High blood pressure, or hypertension• High cholesterol• Heart disease, heart attack or stroke“Current national guidelines state that

individuals who are 70 years and older, or thosewho are 50 years of age and older who have knownrisk factors for P.A.D., or individuals who are youngerthan 50 years and have diabetes, should be testedfor P.A.D.,” says Dr. Hirsch.

Doctors can determine if you have P.A.D. witha simple, painless test called an Ankle BrachialIndex (ABI). Additional tests can determine whereblockages are located.

IdentificationPhysicians diagnose P.A.D.in several ways:

• Physical Exam of the legs, feetand toes to discover weak orabsent peripheral pulses or othersigns of disease.• Doppler (Ultrasound)testperformed on the legs todetermine where the blockage isoccurring.• Ankle•brachial index (ABI)performed to compare bloodpressure in the arms with bloodpressure in the legs to diagnoseasymptomatic P.A.D.

Treatment optionsMild P.A.D. can usually betreated with lifestyle changes ormedication.• Lifestyle changes: Stop

smoking, eat healthy, exerciseregularly.• Medication: Lower bloodpressure, reduce cholesterol,lower the risk of blood clotsand/or manage diabetes.

ProceduresWhen P.A.D. has progressed inits severity, further treatment isrecommended to restore bloodflow to the leg and foot, therebyreducing pain and lowering therisk of losing a leg or foot toamputation.• Endovascular proceduresinclude atherectomy,angioplasty, stenting and lastlysurgery.

One of the more difficult conversations is the onewhere we talk about death. But it’s necessary,particularly for family caregivers who are helping anill or aging parent.

And often you will find the parent eager toparticipate in the conversation.

Talk about plans for funerals so you fully understand theirwishes. That includes how the body will be laid to rest, whatkind of service is desired and how to pay for funeral and burialarrangements.

Also understand local options for hospice care. This isavailable in Ventura County through a number of differentcommunity-based agencies that emphasize dealing withsymptoms of an illness rather than with cures. Hospice caremeets the needs of terminally ill individuals and/or their families,including in-home options.

Finally, don’t forget yourself. You might want and need helpin coping with the passing of a loved one. Many local supportgroups and agencies are there to provide assistance you need.You don’t have to go through this alone.

When you begin to look for a physician, ask friends forreferrals and then— if possible, depending on your care plan— arrangefor a meeting with the physician where you talk with them about yourexpectations— and theirs.

When medical treatment is necessary, you must be willing to stepup and ask all the questions you can so you, and your parent, are fullyinformed of the diagnosis and treatment, including any medication.

The California Health Insurance Counseling and Advocacy Program(HICAP) offers assistance with Medicare and private insurance operations.Counselors with the program have regularly scheduled hours at mostsenior citizen centers in the county as well as at hospitals. The SocialSecurity Administration also can provide answers to many of yourMedicare and Social Security questions.

Visit hospitals, rehabilitation centers or nursing centers, if possible.Ask questions about the operation; be sure to understand if they acceptMedicare or MediCal, or your insurance. Talk about billing procedures.

Check out nursing home facilities. Make sure it is a place where youand your loved one will be comfortable and be sure to spend time with theresidents as well as the staff.

Keep your brain fitIf you find yourself wondering where you left

your glasses— again— or struggling to recalla name or a word you’ve used a million times,you’re not alone. But if you’re worried that yourlapses are destined to be a permanent part ofold age, take heart. It’s never too late to improvebrain function.

Here are some simple exercises you can do tokeep your brain fit.

• Take a guided tour of a museum or anothersite of interest. Pay careful attention to what theguide says. When you get home, reconstructthe tour by writing an outline that includeseverything you remember. Memory activities

that engage all levels of brain operation, suchas receiving, remembering and thinking, help toimprove the function of the brain.

• Choose a song with lyrics you enjoy butdon’t have memorized. Listen to the song asmany times as necessary to write down all thelyrics. Then learn to sing along. Once you’vemastered one song, move on to another.Developing better habits of careful listening willhelp you in your understanding, thinking andremembering. Reconstructing the song requiresclose, intentional focus and an active memory.

Can we still be happy in old age?Biologist LewisWolpert notes in his just-

published book, “You’re Looking VeryWell: TheSurprising Nature of Growing Old,” that youngpeople who hold negative stereotypes about theold (regarding them as feeble and helpless) are ata higher risk of suffering heart disease or a strokewhen they grow older themselves.

Nobody dies of just old age in America. In fact“old age” is no longer permitted as the cause ofdeath on death certificates.

It is widely assumed that aging isprogrammed into our genes, like ourdevelopment from infant to adult. ButWolpertnotes that genetic research now persuasivelyindicates otherwise. No normal human genespromote aging.

Rather, we age because randommoleculardamage accumulates in our cells over ourlifetime, and our cells’ ability to repair thedamage declines as we grow older. According toDarwinian natural selection, once we are past theage of reproduction and caring for our offspring,we are useless anyway, so our cells’ repairmechanisms become unnecessary.

Experiments with laboratory animals indicatethat, with treatment, cell repair can be extended,increasing the human lifespan as much asfivefold. It’s debatable whether anyone wouldwant to live that long.Wolpert quotes GeorgeBernard Shaw that a person would require an“insane self-conceit” to believe that “an eternity ofhimself would be tolerable even to himself.”

The author, 81, who teaches at UniversityCollege, London, admits to occasional episodesof depression. But he notes that the most

common age for depression is just 45.His formula for successful aging includes

restricting calorie intake and keeping physicallyand mentally active. Having a religious faith,keeping a pet and living in the country allcontribute to healthy, happy aging, he says.

Age management is already a moneymakingbusiness in America, with private doctorscharging up to $1,275 a month per patientto monitor medically supervised lifestyleregimens. Jeffrey Leake, a senior doctor at the

Cenegenics Medical Institute in Las Vegas, toldthe London Sunday Times that the steep cost isa strong incentive to ensure adherence to theprogramming.

For those without that kind of money tospend, Cambridge University researcher Aubreyde Grey advised Times reporter Lois Rogersthat the short answer to healthy aging is: Don’tsmoke, don’t get fat —- and don’t retire.

When it comes to money, some families just don’tlike to share information. That’s when it becomeseven more important to sit down with your lovedones and ask the tough questions so you fullyunderstand their financial picture, long before theybecome unable to help with their own finances.

Start with a general discussion to find out their wishes onwho they want to control their finances as they grow older andless able, or interested, in helping. Then find out where they keeptheir financial documents and if you need access or rights, such asbecoming a joint holder of a safe-deposit box.

Be sure to fully understand all their assets. That includespensions, benefits, Social Security and investments, as well as theirdaily bank accounts.

Start the conversations early. Don’t try to do it all at once. Andbe sure they know exactly what you are doing at all times.

end of life issues. It’s important to have a viable will and a lawyercan help to sort through issues of living trusts, a durable power ofattorney for property management and for healthcare as well as anadvance healthcare directive.

POLST directive is simple but powerful legal document, preventing confusion

Physician Orders for Life-Sustaining Treatment(POLST) is an availableoption terminally ill

patients now have to direct medicalpersonnel on the type of treatment(if any) they would want toward theend of their lives.

According toMary Carr,executive director of theVenturaCountyMedical Association (VCMA),POLST is amedical and legal formprinted on bright“pulsar pink”cardstock paper and is recognized byemergency room physicians andmedical personnel around thecounty.

After a discussion betweena patient and treating physician,this fairly simple but extremelypowerful legal document is signedby both parties and gives terminallyill patients control over end-of-lifedecisions.

POLST was first developed inOregon in 1990 due to concerns thatDo Not Resuscitate (DNR) orders andadvanced healthcare directives didnot insure that the patients’wisheswere being honored.

It is a muchmore direct formand prevents confusion as to howa terminally ill patient should betreated by amedical facility. The

heavy card stock paper form isdurable and can travel alongwiththe patient if they are transferredfrom one care facility to another.

Designed for people who havea chronic progressive illness, serioushealth condition, or aremedicallyfrail, a completed POLST form detailsspecific end-of-life decisions thata patient wants honored. Thesedecisions can include the use ofcardiopulmonary resuscitation,antibiotics, IV fluids, the useof intubation andmechanicalventilation, or artificial “tube”feeding. In addition, POLST outlines

the patients’wishes regardingwhether or not they want to betransferred out of their residence to afacility for medical treatment.

“It (POLST) should go hand-in-handwith an advanced healthcaredirective,”Carr said. “Patients knowwhat their desires are and theyunderstand the importance ofdiscussing end-of-life issues withtheir physician. A patient shouldbegin talking with their physician ifthere is a possible terminal disease inthe next 12 to 24months.”

Age is not a consideration indeciding if POLST is right for your

situation. The document can beused by both terminally ill adult andpediatric patients, Carr said.

POLST differs from an advancedhealthcare directive because itrequires a physician’s involvementalongwith a signature. It can preventunwanted ormedically ineffectivetreatment, reduce suffering by thepatient and family and insure thatpatients’wishes are honored duringan extremely difficult and stressfultime.

Once signed by the patient andphysician, POLST becomes a validorder in anymedical setting andmust be followed by healthcareproviders. The law also providesimmunity for anymedical stepstaken to implement the orders onthe POLST.

Walking through the emotional journey of agingAging isn’t easy once you hit a point in life.

Some dread turning 40 and for others it isn’t untilthey qualify for the senior menu, that they reallyfeel the sting.

In my 10 years of working in senior care, Ihave never had anyone tell me they were happyto be in their 70s, 80s or 90s.With the worryof decreasing physical ability or increase offorgetfulness comes the biggest fear, which isdepending on others. Since many adult childrendon’t live close or have very busy lives, manyseniors face these fears alone. In an attempt tokeep some independence or not to be perceivedas a “burden”on family, the decision is made tolook into assisted living.

The decision to move into an Assisted Livingcommunity is not only a decision of locationand services provided, but that of emotionalcomfort. For many seniors their sense of securityand independence is tied to their home, andwith its loss they will have the sense of losingthemselves. This emotional struggle is at the

forefront of every decision to move to AssistedLiving or to try and stay home. The emotions feltare different based on who is experiencing them.

For the senior, there are stages of emotionsthrough the process, much like that of grief. Firstis the feeling of anxiety and fear of the unknown.If you have never lived in Assisted Living, youwon’t have much to draw from. The belief thatindependence is lost and you are moving to“ahome” is the biggest misconception. Tip: Spendtime with residents of communities you like,before you make a decision.

Next is the feeling of being overwhelmedand loss of control. Big decisions of where andwhen and what to do with a home and all one’sbelongings is too much for a senior to process atonce. Breaking it into steps and asking for helpfrom family and friends is a must.

As the family member, usually the adult child,look for signs that your senior loved one needssome assistance. Some examples are: is the housenot being kept the way it always has been, is the

person losing weight, are there more leftoversor half eaten meals in the fridge than usual, or isyour loved one less socially active than before.Tip: Start conversations sooner than later andhave a plan for when help is needed. Some of thefeelings of loss that are experienced by mom anddad carry over to the adult child, so it is importantto be empathetic, but still stay positive.

Others can be of help in walking through thisemotional journey of aging. This includes friends,clergy, respected medical professionals and otherseniors who have walked the road you are onnow. Emotions can sneak up on us when we leastexpect it. The best counter attack is to do yourhomework early and know what your options are,and take your friends and family into the agingbattle with you.

Plan for the inevitable; get organizedWhile you can never plan for the emotional

response from a death, you can ease some of theanxiety by taking a few steps to help organizeand execute plans once someone has passed.

Here are a few suggestions:• Discuss everything.While it is impossible

to know for sure, there are reports that nearly25 percent of life-insurance policies are neverclaimed. A big reason for that has to be that lovedones don’t know what policies were in place. Ifyou find yourself digging for documents for yourloved one talk with your family about everything.Store important information (will, policies, etc.) inone spot and let family members know where itis. A Life.doc or binder with all of your importantlegal, financial and insurance information is agreat way to ensure your loved ones will know

where to turn.• Help parents put together a living-will

and power-of-attorney documents in advanceof health problems. If dementia sets in at a laterdate you’ll be glad you took the time to getorganized since you may be responsible for therecare. Compile all of their medical and insuranceinformation in one binder that is accessible andeasily update, with the latest information.

• Resist the temptation to control and learnto lean on others for help with those thingsyou aren’t very good at. The process of gettingthrough the first few weeks of a loved one’s deathis much easier to bear.

• Determine divisions. The last thingthat grieving relatives need is fights over whogets Grandma’s favorite brooch. If your loved

one did not specify how to divide up thepossessions, allow some time to pass and thenget together and figure it out. An example ofthis is family members planning a weekend atthe beach to mark the one-year anniversary ofher grandmother’s death in 2007. Not only didthey honor her memory, but they went throughher items and reminisced.Waiting a year gaveeveryone time to settle his or her emotions,and holding the get-together in a place hergrandmother loved gave everyone perspective.

3. Delegate.Whatever the task that is neededto do, let each family member in on the action. Ithelps avoid the inevitable “I am doing everything”fight and helps bring together family members ata time when you need each other the most.
