Introduction:Introduction:Introduction:Introduction: HelpAge India (HI) and Oil and Natural Gas Corporation (ONGC) of India have entered

into an agreement to jointly implement the flagship ONGC-HelpAge India Corporate

Social Responsibility (CSR) Project - Varisthajana Swasthya Sewa Abhiyan (VSSA), in twenty

locations across nine States for five years starting from October 2010. The project is

meant for providing dedicated healthcare services to the elderly (those above 55 years)

without any caste, creed or class barriers, bringing qualitative changes to their lives.

Upon signing the project agreement with ONGC, HI has entrusted FourX4 with the

responsibility to help ONGC and HI operationalize the project. The assignment would

help ONGC and HI primarily to roll out the project in the selected twenty locations without

loss of time. The assignment also includes conducting a baseline study of the elderly

population in the identified areas/habitats.

Participatory processes were adopted to identify the locations and habitats, including

community spaces for parking the MMU and offering clinical services. ONGC officials

and important citizens from all locations were consulted and taken into confidence before

household survey was conducted. Panchyat members were requested to examine and

report on the coverage of survey.

All human resources identified for the project are drawn from the local area and have

adequate knowledge on the social, economic and health status of people.

• East Godavari and West Godavari in Andhra Pradesh and Sibsagar in Assam districts

have a lot of agricultural activities. Therefore, the MMU operations there (Kakinada,

Rajahmunday and Assam) should make necessary seasonal changes for ensuring

maximum participation.

• In Gujarat people do not usually like to come out in between 1 to 4 in the afternoon.

This has to be kept in mind while micro-planning the operations.

• During summer Jaisalmer in Rajasthan the temperature reaches 500 at times. Certain

hamlets are inside the deep desert and they live in small groups. This should be kept

in mind while making visits to those hamlets so that the MMU staff does not suffer

health problems due to extreme heat conditions.

• The smaller groups living inside the deep desert in Jaisalmer receive very scanty

medical attention. Therefore, ONGC-HI may consider covering the whole population

in selected hamlets.

• The villages under Tripura MMU, villages in Jorhat MMU and villages in Bokaro MMU

unit are going to operate in conditions that are sensitive from the point of view of

extremist groups being active in those areas. Extreme caution should be exercised for

safety of operations and staff in those areas.

• Further strengthen the contacts that have been generated on the ground level with

various stakeholders like GPs, PHCs/CHCs, local officials, etc., by increasing the

acceptance level and visibility of the project through support from the target

communities.

• Institutionalizing transparency, accountability and good governance. A good number

of local people were trained and deployed in conducting the survey as well as social

mapping processes. They remain at the field level as an important resource for

ONGC and HI and can be used in the monitoring of the project progress, building

networks with other actors like the government hospitals, private health care providers,

etc.

• Encouraging the elders to visit the MMU parking sites within or near their

colonies/hamlets, etc. Utilize the database for micro planning for rolling out the

project activities as well as meeting the needs of all elderly patients, ensuring 100%

coverage.

• Smooth reporting and centralized monitoring. Develop and maintain an appropriate

as well as robust online or off-line MIS. The MIS could be web-based so that better

data integrity, control and use are achieved.

ConclusionConclusionConclusionConclusion This project addresses the need and neglect being faced by elders in India at two

important levels. At the first level, it provides basic healthcare services at their doorsteps,

maintains high level quality and provides a credible and dependable support to the

caretakers of elders at the family and community levels. Often, due to compulsions or

otherwise, at the family level, elders do not get the required level of care, comfort and

support. To such families, and totally neglected families, this project affords an invaluable

opportunity in caring the elders. The stakeholders consulted by us have ensured support

and collaboration in addressing this important social cause. At the second level, this

project could be nurtured and directed towards demonstrating social responsibility of

large corporate entities like the ONGC in supporting causes of the less-privileged.

Implementation of the aforesaid future action points, and information and data presented

by this study would be useful and valuable in implementing this project effectively.

Successful implementation of this project of ONGC can be a source of inspiration for

further or similar activities by large corporate. It can source the beginning of a new

beginning.

HelpAge India (HI) and Oil and Natural Gas Corporation (ONGC) of India entered into

an agreement to jointly implement the flagship ONGC-HelpAge India Mobile Medicare

Unit programme - Varisthajana Swasthya Sewa Abhiyan (VSSA) as a Corporate Social

Responsibility (CSR) Project - in twenty locations1 across nine States comprising three

major regions of India for five years starting from September 2010. The project is aimed

at providing health care services to all elderly persons (those above 55 years of age) of all

faiths, castes and classes. The responsibility of implementing and managing the project

rests with HI who has tremendous experience and expertise in bringing invaluable

qualitative changes in the lives of elderly persons through dedicated health care services

at their doorsteps.

Upon signing the project agreement with ONGC, HI entrusted FourX4 with the

responsibility to help ONGC and HI to identify the most suitable locations, understand the

health care problems of the aged in the identified villages/hamlets, locate suitable human

resource for the project as well as establish first level contact and rapport building with the

communities in the identified areas. The assignment would primarily help ONGC and HI

to roll out the project in the selected twenty locations without loss of time. The assignment

also included conducting a baseline study of the elderly population in the identified

areas/habitats.

FourX4 began the assignment by contacting the officials of ONGC at the given locations

(List is attached as Appendix-II). Based on the list of contacts provided by ONGC

Corporate Office in Delhi, the field visits were planned. The field visits began after

discussing with the concerned officials (called First Person of Reference-FPR) and arriving

at a suitable time schedule based on the convenience of ONGC officials at each site.

Several rounds of telephonic conversations were done to arrive at mutually convenient

dates and plan of action.

The scope of work included the following:

• Maintaining regular contact with HI Head Office (HO) team

• Establishing contact with ONGC officials in 20 projects

• Identifying the villages in each location in consultation with local ONGC officials

and HI team (wherever necessary)

• Visiting 20 locations and the identified villages in each location in order to prepare

detailed operational plan

• Active consultation with the major stakeholders in the identified villages to elicit

their opinion regarding inclusion of their village in the project and soliciting their

active participation during the implementation period of the project, i.e. five years

• Conducting Baseline Survey in each location consisting of a number of villages or

Gram Panchayats (GPs) by recruiting local investigators/surveyors in order to

arrive at the exact number of elders in each village and location (combination of

many GPs/villages)

• Contacting and identifying suitable manpower from the locality for each MMU

• Identifying central locations in consultation with HI and ONGC for establishing the

project office where the MMU staff would gather, stocks would be kept, records

would be maintained and MMU would be parked during non-functioning days

and hours

We had estimated an elder population load @1000 per village and in total 20,000 elders for all the 20 locations.

The actual number of elders per village in each location is estimated to be approximately 2500 to 3000 and the estimated number of elders would be 60,000 from all 20 locations. See Table 1 as Appendix-III.

The key processes and methodologies adopted to complete the assignment as per the

agreed timeline included the following:

each of the selected 20 locations started with a visit to the concerned ONGC

office. The objective of the visit was to meet with the officials of ONGC, know their

priorities and in consultation with them identify the specific villages of GPs for

operationalizing Mobile Medicare services for the elderly. This was successfully

achieved in all locations except for Kolkata. Kolkata couldn’t suggest any focus

villages because they do not have any regular operational area as yet.

Together with the location identification, the focus was also to establish initial

contacts with key stakeholders of the proposed MMU project, collect basic

demographic information and initiate survey processes for conducting the baseline

survey. The key stakeholders with whom communications were established

included:

understanding of the ONGC’s operational areas under that project or asset.

The approach was to ask ONGC officials to let us know their priorities so that

the villages finally selected are in synergy with their need and liking.

• Panchayat leaders & functionariesPanchayat leaders & functionariesPanchayat leaders & functionariesPanchayat leaders & functionaries – almost in all locations except Bokaro

(Jharkhand), we found elected representatives of GPs/villages and established

contact with them for site identification and getting facts and figures about the

GPs and villages. In Bokaro (since the Panchayat elections haven’t been held

for long) traditional and community leaders were contacted to help the team

understand local characteristics and dynamics.

• Local Health CentresLocal Health CentresLocal Health CentresLocal Health Centres – the officials of the government health system were

contacted to inform them about the project, seek their cooperation while

providing health care services for the aged, jointly organizing camps for

elderly persons and also to gather information on suitable local candidates for

the position of Doctor, Nurse and other staff.

administrative requirement to inform the civil administration, however, it goes

a long way in establishing good relationship with officials from the very

beginning of a project. This ensures that should the project require any help in

future there shouldn’t be any official reluctance while providing the same.

• NGOsNGOsNGOsNGOs – an informal understanding with local civil society organisations,

although we found only a few in the selected locations, would ensure that the

project gets their necessary support when necessary.

The identified candidates were grouped based on the basis of the available

openings in the project. HI was given list of human resource for each location for

completing the HR processes.

B. Consultation with Local Leadership: Consultation with Local Leadership: Consultation with Local Leadership: Consultation with Local Leadership: Establishing contacts with the Panchayats was

the most crucial step undertaken by us in facilitating the operationalization of the

project. The objective was to appraise and win the confidence of the GP/village

leadership for the successful initiation of the project and develop a broad

understanding on the project amongst the local leadership and general

population. It was observed that the local leadership and the community at large

was welcoming this move as taking care of the aged is increasingly becoming a

major issue! In the case of Bokaro, in the absence of formal PRIs, contact was

established with local important citizens and traditional community leaders who

have influence over the community. Contacts of local leaders were provided by

ONGC’s local office in every location.

In all the cases the leadership welcomed the initiative undertaken by HI & ONGC

and appreciated the working model of project and assured their support in

facilitation of the project.

After initial discussion with GP members, basic information on demography and

infrastructural facilities were collected. The key parameters on which information

collected were:

� No. of Villages (including paras or hamlets) in the GP

� Population details village wise

� No. of HHs in the GP

o No. of BPL HHs

o No of HHs with Job cards – Discussion was held on old age people

participation in NREGA work

o No. of HHs enrolled under Health Insurance programme

� No. of old Age pensioners & pending cases

All the above information is consolidated into a Gram Panchayat information

format for future references. These will also be fed into the overall database for

use during project implementation. The following table gives an idea of the

population (general & elderly) in all locations, region-wise and the total project of

19 MMUs.

The above table gives us very interesting facts and figures. Although, the number

of units are equal in western and eastern India the population is significantly

higher in the east and per GP population is almost double in the east (7969) in

comparison to the other two regions (west-4713 & south-4168). On an average, it

was observed that 13% of the total population constitutes the elders. This average

is subjected to increase, as generally, the people do not consider population within

the age bracket of 55-60 as aged. Another interesting factor is that the proportion

of elderly population is highest in western India followed closely by south India and

a bit distantly by eastern India. The average MMU general population in south,

east and west are 38115, 36434 and 32320 respectively, which shows a higher

percentage of elder population in the south than in the other two regions.

It is worth mentioning here that during consultation with GP members on the

population of people of 55 yrs & above, an assessment of the patient load on

each MMU was also done. The estimation of elderly population has been done by

taking 13% as the estimated population of elders (As per Census of India 2001,

population of 60 yrs & above constitute around 10% of the total population.

Therefore, the population of 55 yrs & above has been taken as 13%).

The actual working hours for a MMU in a day will be 8 to 10 hours. On an

average 2 hrs will be involved in commuting between different villages, on

average 3-5 locations within a village. The average working hour per MMU that

would effectively go in for attending patients comes to 6 hrs, another two hours for

daily data compilation, reporting and preparing for the next day’s schedule in

terms of stock replenishment, etc.

� After initial contacts with the community and collection of relevant data and

information, discussion was held on identification of MMU parking locations in

the GP. The local leadership was requested to identify vehicle parking locations

that would cover the maximum number of elderly population in the village.

Two important factors that were kept in mind while recommending MMU

parking sites were:

� The locations should be public places thereby assuring unquestioned access to

all irrespective of their caste, faith or class.

� All the locations can be accessed by four wheel medium size vehicle (Tata

Sumo, Tata Winger, Mahindra Bolero, etc. type models) throughout the year.

All the MMU parking sites suggested by each GP were physically verified for its

suitability and access. Based on the parking location visit observation, some of the

sites were further discussed and in some cases new sites were identified. The

names of the parking locations Gram Panchayat wise are given in region-wise

reports. However, the number of GPs and parking sites in each location is

summarized in the Table No.2.

Identifying a central location for each project unit was considered crucial for better

logistic and schedule coordination. As far as possible, all coordination sites

(central Site) was identified keeping in view various factors like economy distance

to all identified sites, fuelling convenience, availability of accommodation facilities

for staff, to have adequate office space for storage of medicines, space for seating

arrangements for four persons, storage of records and computer and space for

parking MMU during non-working days and in the nights The only exception is

Jaisalmer under Jodhpur ONGC jurisdiction where all the

villages/hamlets/habitations are far away from Jaisalmer town which is proposed

as the MMU office location because this is the only town in the entire district where

minimum infrastructure is available. Keeping these points in mind also after taking

suggestions from local ONGC officials various locations have been suggested. A

list of suggested locations is given as Appendix-III.

During the consultation with local leadership in each GP, information related to the

vacancies available with MMU units was shared. Other stakeholders like PHCs/CHCs,

officials from civil administration, individuals working in social sector were also informed

of the vacancies. As a result of this, discussion with many interested candidates was held

related to all the positions. Based on these discussions a list of potential candidates was

prepared and given to HI. The list of candidates for each region is available in the

respective regional reports. The suitable candidates for each location form the suggested

list may be interviewed as soon as possible.

Discussion held with all the candidates for concerned positions have raised some

important & critical observations. They have been presented in a tabular format below

(Table No.3).

candidates. Most of the Doctors with whom discussion were held raised their concern related to it.

This is essentially because of a huge Demand-Supply gap in the case of MBBS Doctors. Young

doctors are not at all receptive to the idea of working in this project.

When the team explored the opportunity with retired Doctors they are either engaged with NRHM

on contract basis or are into private practice and therefore, are earning more by investing lesser

time than what they would have to invest in the MMU project. In these highly material oriented

times, it is indeed difficult to motivate MBBS Doctors, who are already in short supply, to work for

such a highly socially relevant project for meager amount

MMU operations will address basic health issues related to aging. Therefore, it

becomes important that effective referral links are established with the mainstream

health system. The mainstream health system at the lower levels functions through

the PHCs and CHCs.

Therefore, in all the MMU locations mapping of the health centres starting from

sub-centre to district hospitals was done. This helped in identifying the following:

� No. of sub-centers operating in identified areas, i.e. Gram Panchayats

� PHCs (if in operation) catering to the rural population of the target GPs

� CHCs under which the operational GPs of the MMU project falls

� Sub-division/Area level hospitals for the concerned project locations

� District/Civil Hospitals

Details of health centers (location, doctor’s name and contact number, etc.) have

been recorded for future reference. Visits to PHCs operating in all the project

location were made and the Medical Officers’ In-charge (MOIC) was appraised

about the MMU project. All the MOICs expressed happiness for the initiative in the

area of geriatric care and assured necessary help in this context. In many cases the

Block/Taluka/Mandal/District level health officials were also informed about this

project. However, FourX4 recommends that while commencing the project these

officials and doctors should be formally communicated about beginning of the

project and their cooperation should be solicited for ensuring better health care for

elderly persons.

Information on the ailment profile among the old age people was also collected

from the PHCs. These have been recorded for each location and provided in the

region specific reports. The disease profile of elderly persons at each location has

been given in Appendix-V.

Along with government health centers, information on private clinic and nursing

homes were also collected, which are frequently visited by the local population.

Information on these hospitals have also been provided in the regional reports.

baseline survey is considered very important and fundamental to facilitate a

transparent and efficient project implementation system. The baseline database

would form an extremely important data for measuring the progress of the project

over time as well as impact assessment. For instance, it would help in monitoring

its core elements and evaluation of the differences the project has brought about

to the elderly populace of the focus GPs.

In order to carry out the baseline survey exercise support of the GP leadership was

sought to help identify educated youth from their respective GPs. The identified

educated youth were motivated to conduct the survey within a given time period in

order to facilitate timely launch of the project. The GP leadership was requested to

ensure conduct of the survey in the interest of the health care of the elderly persons

of their respective GPs. They were also requested to ensure that the survey is

conducted in all the constituent habitats/natural villages of their GPs.

The identified candidates were given appropriate training for collecting accurate

and complete information as per a pre-determined survey schedule. They were

provided with a detailed instruction-cum-guidance sheet in order to ensure a

flawless survey.

One of these candidates in each location was later on identified to supervise the

survey process and was delegated required responsibility and was supported with

required resources. In some cases another person from the locality was given the

responsibility to supervise the survey process. Except in Tripura in all other locations

this model was employed to conduct baseline survey. In Tripura a local NGO

working in the area was roped in to conduct the survey which utilized its staff from

the locality to complete the survey.

� Further strengthen the networks that have been generated on the ground level with

various stakeholders like GPs, PHCs/CHCs, local officials, etc. so that the acceptance

level and visibility of the programme get a tremendous boost in the target

communities/areas.

� Utilize the manpower available at the community level in the shape of trained field

investigators and supervisors for social audits and participatory monitoring in order to

ensure effective programme implementation and achieve higher level of transparency

and good governance.

means to encourage each one of them to visit the MMU parking sites within or near

� East Godavari and West Godavari in Andhra Pradesh and Sibsagar in Assam districts

have a lot of agricultural activities. Therefore, the MMU operations there (Kakinada,

Rajahmunday and Assam) should make necessary seasonal changes for eliciting

maximum participation.

� In Gujarat people do not usually like to come out in between 1 to 4 in the afternoon.

This has to be kept in mind while micro-managing the operations in those units.

� During summer Jaisalmer in Rajasthan the temperature reaches 500 at times. Certain

hamlets are inside the deep desert and they live in small groups. This should be kept

in mind while making visits to those hamlets so that the MMU staff does not suffer

health problems due to extreme heat conditions.

� The smaller groups living inside the deep desert in Jaisalmer receive very scanty

medical attention. Therefore, ONGC-HI may consider covering the whole population

in selected hamlets.

� The villages under Tripura MMU, villages in Jorhat MMU and villages in Bokaro MMU

unit are going to operate in conditions that are sensitive from the point of view of

extremist groups being active in those areas. Extreme caution should be exercised for

safety of operations and staff in those areas. Details are explained the regional report.

The project has been very well conceptualized by the ONGC-HI joint initiative and it has

received huge support from all stakeholders. GPs have promised to provide their

continued support for this project as long as they are kept informed. In fact, most of them

have opined that the project should continue for a long period and help create an

environment where elderly persons’ care becomes an integral part of the social life and

institutional health delivery mechanisms. The government health systems also have

pledged their informal support for the project and have expressed their desire to partner

with this ONGC-HI initiative for ensuring better quality of life in the areas that they

operate in. The project team must leverage this goodwill which has already been created

on the ground in order to make this initiative hugely successful, effective and impactful.
