Class of 2014 ClInICal RoTaTIon PRoGRaMINTRODUCTIONThemissionofUniversityofPikeville-KentuckyCollegeofOsteopathicMedicine(KYCOM)includespreparationofourgraduatesforcompetencyintheworldofprimarycaremedicine.AsuccessfulKYCOMgraduatewill,aftercompletionoftheprogram,demonstratesufficientknowledge,skillsets,experience,values,andbehaviorsthatmeetestablishedprofessionalstandards,supportedbythebestavailablemedicalevidence,thatareinthebestinterestofthewell-beingandhealthofthepatient.Thematurationprocessfromclinicalyearsthreetofour,

andultimatelysuccessfulgraduationisthesharedresponsibilityoftheindividualstudent,KYCOMandthehosthospitalsandphysiciansthatprovidetheclinicalexperiences.

Duringthethirdandfourthyears,atotalof21fourweekrotationblockshavebeenprovidedtocomplete80weeksofrotationrequirements,whichinclude:

1. 40weeksofrequiredCorerotations(10blocks)2. 24weeksofrequiredSelectiverotations(6blocks)3. 16weeksofElectiverotations(4blocks)4. 4weeksforCOMLEXLevel2preparation(1block)

Thescheduleincludestwoweeksattheendofthethirdyeardevotedtoclinicalskillsevaluationandamandatoryclassmeeting,andfourweeksofwinterholidaybreaks(twoweeksperyear,perKYCOMcalendar).

ELECTIVE ROTATIONS Sixteen(16)weeksofelectiverotationsarerequired.Thesemaybeselectedbythestudent,subjecttoapprovalbytheAssociateDeanforClinicalSciences.Thesemaybecompletedin2-weekor4-weekblocks(nomorethantworotationscanbeoftwoweeksduration).

GENERAL TIMELINEAllcore rotationsmustoccurduring the third-year atKYCOMCoreSites.Core rotation requirementswillbe completed either consecutively or in separate four-week intervals, depending upon core site schedulingconsiderations.Each studentwill have two4-weekblocksduring the3rdyear inwhich therewill not be ascheduledCorerotation.ThestudentshouldusetheseblockstocompleteSelectiveorElectiverotations.*Selective/elective rotations require completion of both Internal Medicine blocks, and at least one block of each of the following: Family Medicine, General Surgery, Pediatrics and Women’s Health.

COMLEX-USA POLICYGraduationrequirementsfromKYCOMincludesuccessfulcompletionofCOMLEX-USALevel1,COMLEX-USALevel2-CEandCOMLEX-USALevel2-PE.KYCOMrequiresCOMLEX-USALevel1tobetakenuponsuccessfulcompletionofyearsoneandtwoofosteopathicmedicaleducation.KYCOMrequiresCOMLEX-USALevel2-CEandCOMLEX-USALevel2-PEtobetakenaftersuccessfulcompletionofCOMLEX-USALevel1andyearsone,twoandthreeofosteopathicmedicaleducation.

Selective Rotation Requirements

Allstudentsarelimitedtoamaximum of four attempts to pass each required COMLEX-USA examinationinordertomeetgraduationrequirements.Failure of four attempts on any COMLEX-USA examination will result in dismissal from KYCOM.

COMLEX-USALevel1Beforeentryintothethirdyearofosteopathicmedicaleducation,allstudentsmustsitforCOMLEX-USALevel1.NostudentmaybeginthethirdyearofstudyuntilCOMLEX-USALevel1istaken.Delay(s)fromthisscheduleprevent the startof the thirdyearof study,and requirepre-approval from thePromotionandMatriculationCommittee.WhenCOMLEX-USALevel1istaken,thestudentwillbepermittedtoenterthirdyearclinicalrotationsatthebeginningofthenextscheduledrotationperiod.

FailureoftheCOMLEX-USALevel1examinationwillresultinaplanofstudydeterminedbytheAssociateDeanforClinicalSciencesandtheAssociateDeanforBasicSciencestoassiststudentsinpreparingforaretakeoftheexamination.Thisplanmayincludea)counselingthestudentonhowtopreparefortherepeatexaminationwhilealsosatisfactorilymanagingtherequirementsoftheclinicalrotationorb)beingremovedfromclinicalrotationsinordertodevotefull-timepreparationfortheexamination.Ifthestudentisplacedonacademicleavefromclinicalrotations,theAssociateDeanforClinicalSciencesmayrecommend(a)anindependentstudyor(b)enrollmentinanapprovedboardpreparationreviewcourse,atstudent’scost.Followingaretakeoftheexaminationthestudentwillbepermittedtoreturntoclinicalrotationswhileawaitingresults.Whentheretakeresultsareobtainedthestudent’sstatus willbere-evaluatedandprogressofthestudent’sclinicalstudieswillbedetermined.

FailuretoachieveapassingscorebyDecember31ofthethirdyearwillresultinremovalfromclinicalrotationsandactionasabove.ThestudentwillbeallowedtoreturntoclinicalrotationsonlyafterapassingscorehasbeenreceivedbyKYCOM.TheAssociateDeanforClinicalScienceswilldeterminestudentplacementinaccordancewithschedulingandcurricularrequirements.

COMLEX-USALevel2-CEandLevel2-PEUponsuccessfulcompletionofCOMLEXUSALevel1,NBOMEwillnotify studentsof their eligibility toregisterforCOMLEX-USALevel2examinations.KYCOMstudentsmayregisterforCOMLEX-USALevel2-CEorLevel2-PEanytimebetweenthisnotificationandDecember1ofthefourthyearofstudy.AllstudentsarerequiredtotakeboththeLevel2-CEandLevel2-PEexaminationsbetweenthestartofthe4thyearandDecember1ofthefourthyearofstudy.ThestudenthastheoptiontotakeLevel2-CEorLevel2-PEinanyorder.FailuretotaketheexamspriortoDecember1ofthefourthyearwillresultinareferraltotheKYCOMPromotionandMatriculationCommitteeforconsiderationofdisciplinaryaction.

FailureoftheLevel2-CEorLevel2-PEmayresultintheremovalofthestudentfromclinicalrotationspendingreviewbytheAssociateDeanforClinicalSciences.Thiswillbedoneinordertodetermineaplanofstudyforaretakeoftheexamination.Thisplanmayresultina)thestudentbeingcounseledonhowtopreparefortheexaminationorb)beingremovedfromclinicalrotations.Ifthestudentisplacedonacademicleavefromclinicalrotation,theAssociateDeanforClinicalSciencesmayrecommend(a)anindependentstudyor(b)enrollmentinanapprovedboardpreparationreviewcourse,atstudent’scost.Followingaretakeoftheexamination,thestudentwillbepermittedtoreturntoclinicalrotationswhileawaitingresults.Whentheretakeresultsareobtained,thestudent’sstatuswillbere-evaluatedandprogressofthestudent’sclinicalstudieswillbedetermined.TheAssociate Dean for Clinical Sciences will determine student placement in accordance with scheduling andcurricularrequirements.

FailuretosuccessfullycompletetheCOMLEX-USAgraduationrequirementswithintwoyearsfromthestartofthestudent’sfourthyearofstudywillresultindismissalfromKYCOM.

OBJECTIVESTheclinicalyearsatKYCOMareatransitionfromtheacademicexperiencetotheworldofintegrative,experience-basedmedicine.In21months,KYCOMaimstoseethestudentsuccessfullyachievecomprehensionandmasteryofthe“SevenCoreCompetencies”asoutlinedbytheNationalBoardofOsteopathicMedicalExaminers,andevaluatedbybothKYCOM’sinterimevaluativetoolsandthesuccessfulcompletionofCOMLEXLevel2-CEand2-PEbeforegraduation.

EducationalandPerformanceGoalsinclude:

1. Comprehensionoftheosteopathicphilosophy,recognitionoftheneedforitsapplication,anddemonstrationofclinicalOMTskills.

2. Comprehensionoftheapplicabilityofbiomedical,clinical,epidemiologic,biomechanical,andthesocial/behavioralsciencestoclinicalsituations,anddemonstrationofapplicationtopatient-centeredcare.

3. Demonstrationofpatient-centeredcare.Toinclude:a. Effectivedatagatheringb. Developmentofeffectivephysician-patientrelationshipsc. Recognitionofage-relatedpreventivehealthissuesd. Sensitivitytoculturalinfluencese. Developmentoftreatmentplansthatarebothevidence-basedandpatientspecific.f. Developmentoftreatmentplanswithbothscientificbasisandintegrationwithosteopathic

philosophy.4. Demonstration of good communication and interpersonal skills that facilitate quality

physician-patient,physician-familyandphysician-healthprofessionalrelationships.5. Demonstrationofprofessionalbehavior.Toinclude:

sTUDenT elIGIbIlITY foR ClInICal RoTaTIons1. DRUG SCREEN POLICYKYCOMrequiresaurinescreenfordrugsandalcoholimmediatelyaftermatriculationwithKYCOMandpriortothebeginningofthirdyearclinicalclerkships.Studentsareresponsiblefortheexpenseinvolvedwiththisevaluation.PositivefindingswillbereviewedbytheAssociateDeanforStudentAffairsand/ortheAssociateDeanforClinicalSciences.Furtherevaluationbyexternalprofessionalconsultantsmayberequired.Apositivetestresultmaybecomegroundsfordismissal.

Drug Testing on Demand –Anystudentmayberequired tosubmit todrugand/oralcoholtesting-basedonreasonablesuspicion.

2. CRIMINAL BACKGROUND CHECKKYCOMrequirescriminalbackgroundchecksforallstudentspriortomatriculatingtoKYCOMandpriortothebeginningofthirdyearclinicalrotations.Studentswillberesponsiblefortheexpensesinvolvedwiththisevaluation.ThemechanismforevaluationwillbedeterminedbytheAssociateDeansforStudentAffairsandClinicalSciences.InfractionsmaybereferredtothePromotionandMatriculationCommittee.

Mandatory Self-Reporting for Criminal BehaviorAllacceptedandcurrentlyenrolledmedicalstudentsarerequiredtopromptly(within10calendardays)reportanycriminalchargeseverfiledagainstthemtotheAssociateDeanforStudentAffairs.Criminalbehaviorincludesanyfelonyandmisdemeanorviolationsofthelaw,butexcludesminortrafficviolations,suchasparkingtickets.AnychargesthatwerepreviouslydisclosedontheAACOMASapplicationneednotbereportedagain.StudentviolationsofthelawwillbereviewedbytheAssociateDeanforStudentAffairsinthecontextoffutureimplicationsforlicensure,threattopatientsafety,andtheabilitytobeanappropriatememberoftheosteopathicmedicalprofession.Dependingonthenatureandseverityofthecriminaloffense,studentsuspensionordismissalispossible.

3. IMMUNIZATIONSKYCOM,inconjunctionwithrequirementsofallhospitalsaccreditedbytheJointCommissiononAccreditationofHealthcare(JCAHO)and/orHealthcareFacilitiesAccreditationProgram(HFAP),haverequiredthefollowingimmunizations:

a. TBtestingyearlyb. Chestradiography,everythreeyears,ifTBtestisconsideredpositivec. HepatitisBimmunizationestablishedbythreereporteddatesofimmunizationandby

documentedtestingofantibodytiterd. Tetanustoxoidimmunization,everytenyearse. RubellaandVaricellaimmunity,establishedbytworeporteddatesofvaccinationand

documentedantibodytiter.

ALLTBtestsmustbecurrentbyJune1ofeachyearofclinicalrotations.Acurrent,updateddocumentationofTBevaluationmustbedeliveredtotheSecretaryoftheDepartmentofFamilyMedicinepriortothatdate.FailuretoprovideacurrentTBevaluationwillpreventthestudentfromparticipatinginanyclinicalactivitiesforthefollowingyear,whichwillresultindelayand/orfailuretoachievegraduation.

ItisthepolicythatallHepatitisBdocumentationwillbecompletedonJune1ofthefirstyearofschool.This

documentationmustbedeliveredtotheSecretaryoftheAssociateDeanforStudentAffairs.Failuretoprovideacompleteddocumentationwillpreventthestudentfromparticipatinginanyclinicalrotationsforthefollowingyear,whichwillresultindelayand/orfailuretoachievegraduation.

Documentationof anupdated tetanusvaccinationand rubellaantibody is requiredprior tomatriculation toKYCOM.ThismustbeonfileattheOfficeofTheAssociateDeanforStudentAffairs.

4. ACADEMICSa. Allpre-clinicalcourseswillhavebeencompletedwithgradesof“PASS,”“C”orhigher

beforeAugust1ofthethirdclinicalyearofstudy.b. COMLEXLevel1willhavebeentakenbeforeAugust1ofthethirdclinicalyearof

study.

sTUDenT ResPonsIbIlITIes1. REPORT FOR ROTATIONEachrotationbeginsonthefirstMondayofeachblockandendsonthelastFridayoftheblock.Itisthestudent’sresponsibilitytocontacttheclinicalpreceptoroneweekbeforethecommencementoftherotationtodeterminea“firstdaymeetingplaceandtime.”Failuretocontactthepreceptorand/orfailuretofindthepreceptorcanleadtofailureoftherotation.Itistheresponsibilityofeachstudenttobepresentonthecommencementofeachrotation.Onday“one”oftherotation,itistheresponsibilityofthestudenttosendcontactinformationtotheDirectorofClinicalRotationsforattendanceverificationpurposes.

Intheeventofanunavoidabletardinesstotherotation,itistheresponsibilityofthestudenttonotifytheSupervisingPhysicianandtheOfficeoftheAssociateDeanforClinicalSciencesatKYCOM.Ifanorientationisrequiredataclinicalrotationsite,itisimperativethatthestudentparticipateinthatorientationandfollowtheprotocolsestablishedbythatrotation.

2. ATTENDANCE Promptstudentattendanceisexpectedforthe28daysofeachrotationperiod.Attendanceismandatoryforallclinicalrotations.Amaximumofthree(3)daysforabsenceispermitted,ifapprovedbytheAssociateDeanforClinicalSciences,andmustberecordedonstudentlogsfortherotation.Absenceinexcessofthethree-daystandardwillresultinan“incomplete”fortherotation,untilsuchtimethattheactivityrequirementissatisfied.UnexcusedabsenceconstitutesreferraltothePromotionandMatriculationCommittee,andmayresultinfailureoftheclinicalrotation.

c. Absenceoftheattendingphysicianorthestudent,fromthepracticeforaperiodgreaterthan one day, requires the student to notify the office for clinical sciences, of theinterruptionofactivity.AtthediscretionoftheAssociateDeanforClinicalScienceseitherareadingassignmentoralternatedutyassignmentwillbemade.

d. Reasonsforapprovedabsencemayincludeillness,conferenceattendanceorextendedleave.

e. AnyunexcusedabsencewillbereferredtothePromotionandMatriculationCommitteewitharecommendationoffailurefortheclinicalrotation.

f. ThestudentisnotpermittedtoleavethedesignatedclinicalrotationpriortothescheduleddeparturedatewithoutpriorapprovalofboththeAssociateDeanforClinicalSciences

andtheattendingphysician.DeparturespriortothescheduleddeparturedatewithoutpriorconsentwillbeconsideredanunauthorizedabsenceandissubjecttoreferraltothePromotionandMatriculationCommitteeandpotentialfailureofthatclinicalrotation.

4. SUBMISSION OF STUDENT CASE LOGS AND ROTATION EVALUATIONAll studentsare required to submitacomplete, legibleCaseLog foreachclinical rotationwithinseven (7)calendardaysofthelastdayoftherotation.TheCaseLogconsistsofthestudent’sdailyactivities.e.g.patientcontact,proceduresperformed,readings,conferencesattended,etc.andissubmitted(accompaniedbypreceptorsignature)withinseven(7)calendardaysofthelastrotationday.Failuretosubmitlogswithinseven(7)calendardaysisconsidereda“Failure”andmaybereferredtothePromotionandMatriculationCommittee.ThestudentisrequiredtosubmitaLogSubmittalChecklistwitheachsetoflogstoindicatewhichlogsheetsthestudenthasreturnedtotheClinicalRotationsRecordsOfficer.

a. IllegibleCaseLogsareunacceptableandwillresultinagradeof“Failure”fortheclinicalrotation.

b. Allstudentsarerequiredtoincludetheirlevelofparticipationineachactivity.Levelofparticipation–(1)Observed,(2)Assisted,(3)ManagedUnderSupervision.

c. The teachingphysicianwill sign theLogSubmittalCheckliston the lastdayof theclinicalrotation.

d. ThestudentisrequiredtoevaluateeachrotationthroughtheRotationEvaluationFormandsubmitwitheachsetofCaseLogs.Thisevaluationwillreflectthestudent’sattitudeandobservationsregardingthequalityoftrainingreceivedoneachrotation.Thestudent’sgradewillbereportedas“Failure”untilthisformiscompletedandreturnedtotheOfficeoftheClinicalRotationsRecordsOfficer.

e. Intheeventoflatesubmittalofdelinquentlogmaterials,ifacceptabletotheAssociateDeanforClinicalSciences,therotationgradewillbeconvertedfroma“Failure”toanabsolutemaximumscoreof70%.

5. PERSONAL INSURANCEStudentsarerequiredtohavepersonalhospitalizationinsurancewhileonclinicalrotations.ProofofinsurancemustbeonfileintheofficeoftheAssociateDeanforStudentAffairs.Verificationofcurrenthealthinsurancemustbeprovidedbeforebeginningclinicalrotations.

6. HOUSINGAllhousingneedswhilethestudentshaverelocatedtotheindividualrotationsitesareatthestudent’sexpense.KYCOMisnotresponsibleforstudenthousing.

7. PROFESSIONALISMAsarepresentativeofbothKYCOMandtheosteopathicprofession,itisthestudent’sresponsibilitytomaintainprofessionaldeportmentatalltimes.

a. DRESS KYCOMstudentsareexpectedtodressprofessionallyatall timesandtobeattentivetopersonalhygieneandcleanliness.Itistherightofpatients,peersandhealthcarestafftoexpectasafe,non-offensive,non-infective,andnon-allergenicworkenvironment.Personalappearanceandhygienereflectconcernandrespectforbothstaffandpatientsafety.Itcontributestothedeliveryofqualityhealthcare,andsendsamessagetothepublicthatthehealthcarefacilitymaintainsapositive,respectfulandsafeenvironment.Uncleanandunkemptindividualsprovokediscomfort,andcreateabarriertohealthcareaccess.

KYCOMmaintainsa“NO TOLERANCE”policyfordiversionsfromthemandatoryguidelinesbelow:1. AtalltimesastudentmustbeclearlyidentifiedasaKYCOMstudent.2. ShortwhitelabcoatswithKYCOMidentificationareexpectedtobewornunlessspecifically

instructedotherwisebythehealthcarefacilityorpreceptorphysician.3. Scrubsuitsaretobewornintheoperatingroom,procedurerooms,duringcallhours,and

atthediscretionofindividualpreceptorphysicianand/orhealthcarefacility.4. Clothing,atalltimes,mustbeneat,cleanandfreefromoffensiveodors.Clothingmustbe

professional,consistentwiththestandardsforaprofessionalenvironment,andnotattractundueattentionorserveasadistractiontoothers.Clothingthatcontainsunprofessionaloroffensivewritingorcaricaturesmaynotbeworn.Studentsshoulddressinanon-provocativemanner that demonstrates respect for patients, fellow students, and staff. It must alsobe appropriate to the typeofworkbeingperformedand take into account thepotentialexpectationsofpatients,stafforfellowstudents.

5. Open-toedandcasualshoes,suchassandalsandflip-flopsarenotconsideredprofessionalattire.

6. Jewelry,neckwear,scarvesandaccessoriescanbeworn,however,mustberemovedifeitherpreceptororhealthcarefacilityconsiderthemtointerferewithduty,orapotentialforinfectionandpossibleharmtopatients,stafforselfexists.

7. Tattoosandbodyartcanoftenbeperceivedasaffrontsorthreatstoreligionandfamily.Tattoosandbodyartshouldbecovered topreventpotential foroffensivereactionsfrompatients,peersorstaff.

8. KYCOMstudentsmustbephysicallyclean,wellgroomed,andtakestepstopreventand/oraddressproblemsofoffensivebodyodor.a. Avoidexcessiveuseoffragrances–scentedchemicalsposeathreatforallergicand/or

adversereactionsbypatients,peersandhealthcarestaff.b. Hairstyleandlength(includingmustachesandbeards)mustbeclean,neatandcontrolled.

Hairshouldnotinterferewithdutiesorposeathreat toinfectionforpatient,peerorhealthcarestaff.

b. SEXUAL HARASSMENTAny incident of suspected sexual harassment shall be reported immediately to the SupervisingPhysician,AssociateDeanforClinicalSciencesandtheAssociateDeanforStudentAffairs.AnystudentinvolvedinsexualharassmentshallbereferredtotheAssociateDeanforStudentAffairsforfurtheraction.

c. STUDENT/PATIENT RELATIONSHIPSThe relationshipbetweenanosteopathicmedical student andapatient shall alwaysbekeptonaprofessionalbasis.Astudentshallnotdateorbecomeintimatelyinvolvedwithapatientduetotheethical and legal considerations. Conduct in such an unprofessional manner shall be consideredimproperbehaviorandwillbegroundsfordisciplinaryaction,includingdismissalfromKYCOM.

d. KYCOM PHARMACEUTICAL AND INDUSTRY REPRESENTATIVE POLICYIntroductionKentuckyCollegeofOsteopathicMedicine(KYCOM)operatesasanot-for-profitosteopathicmedicaleducationalinstitutionengagedineducatingosteopathicmedicalstudentsandadvancingosteopathicmedicaleducation.KYCOMhasbeengrantedaccreditationbytheAmericanOsteopathicAssociation’sCommissiononOsteopathicCollegeAccreditation.Ourmissionincludespreparationofourgraduatesforcompetencyintheworldofprimarycaremedicine.AsuccessfulKYCOMgraduatewill,aftercompletionoftheeducationalprogram,demonstratesufficientknowledge,skillsets,experience,values,andbehaviorsthatmeetestablishedprofessionalstandards,supportedbythebestavailablemedicalevidence,thatareinthebestinterestofthewell-beingandhealthofthepatient.

3.Pharmaceuticalcompaniesmayoffergiftstophysiciansfromtimetotime.Thesegiftsshouldbeof limitedvalueandbeappropriate topatientcareor thepracticeofmedicine.Giftsunrelatedtopatientcareareinappropriate.Theuseofaproductorservice-basedsolelyonthereceiptofagiftshallbedeemedunethical.

4.Whenaphysicianprovidesservicestoapharmaceuticalcompany,itisappropriatetoreceivecompensation.However,itisimportantthatcompensationbeinproportiontotheservicesrendered.Compensationshouldnothavethesubstanceorappearanceofarelationshiptothephysician’suseoftheemployer’sproductsinpatientcare.

Pharmaceutical Research and Manufacturers of America (PhRMA)2 GuidelinesfromthePhRMAcode,developedvoluntarilybythepharmaceuticalindustry,andadheredtobyKYCOMinclude:1. A conference or meeting is any activity, held at an appropriate location, where (a) the

gatheringisprimarilydedicated,inbothtimeandeffort,topromotingobjectivescientificandeducationalactivitiesanddiscourse,and(b)themainincentiveforbringingattendeestogetheristofurthertheirknowledgeonthetopic(s)beingpresented.

2. Financialassistanceforscholarshipsorothereducationalfundstopermitmedicalstudents,residents,fellows,andotherhealthcareprofessionalsintrainingtoattendcarefullyselectededucationalconferencesmaybeofferedsolongastheselectionofindividualswhowillreceivethefundsismadebytheacademicortraininginstitution.“Carefullyselectededucationalconferences”aregenerallydefinedas themajoreducational,scientific,orpolicy-makingmeetingsofnational,regional,orspecialtymedicalassociations.

3. Any financial support provided by PhRMAcertified companies, should be given to theeducationalactivity(CME)providerwiththeintenttoreducetheoverallCMEregistrationfeeforallparticipants.

4. Nogrants,scholarships,subsidies,support,consultingcontracts,oreducationalorpracticerelated items shouldbeprovidedoroffered to ahealthcareprofessional in exchange forprescribingproductsorforacommitmenttocontinueprescribingproducts.Nothingshouldbeofferedorprovidedinamanneroronconditionsthatwouldinterferewiththeindependenceofahealthcareprofessional’sprescribingpractices.

g. MOBILE TELEPHONES AND HANDHELD DEVICESKYCOM students are welcomed guests at clinical rotation sites. They are given the courtesy toparticipateasamemberofthestaff,however,asguests,shouldbemindfulthatmechanicalsounds,attention toelectronicmessagesanduseofkeyboardswithin theconfinesofexamination rooms,operatingrooms,procedureroomsandatbedsidecanbothbeperceived(bypatientsandstaff)asalackofinterest,andpotentiallydistractpreceptorphysiciansandhealthcarestafffromthedeliveryofsafehealthcare.ThefollowingguidelinesfortheuseofmobiledevicesaremandatedbyKYCOM:1. Nohandhelddeviceistobecarriedintooperatingorprocedurerooms.

aPPRoVeD absenCe anD leaVe PolICIesPERSONAL ILLNESSItisparamountthatthewell-beingofthestudentisconsideredinanyillness.Ifastudentisabsentformorethanonedayduringarotation,theOfficeoftheAssociateDeanforClinicalSciencesofKYCOMmustbenotifiedandthestudentmustbeseenbyaphysicianfordocumentationandforthewell-beingofthestudent.Thestudentshouldnothesitatewhatsoevertoreportanillnessasthewelfareofthestudentandhis/herpatientcontactsisofprimeimportance.

TEMPORARY ABSENCE A“shortperiodoftime”mayberequestedtoattendtopersonalbusiness.Itisdefinedasashortperiodoftimeawayfromtheservicerotationactivitiesthatmaybeneededfromtimetotimetoallowthestudenttoattendtopersonalbusiness(e.g.banking,childcare,etc.).A“shortperiodoftime”shouldbeconstruedaslessthanoneday.Permissionofthesupervisingphysicianand/orofficeofclinicalrotationsisrequired.Nodurationorfrequencyrestrictionsaredefined;however,itisintendedthatthestudentprovideclearreasonforthetemporaryabsence.

EXTENDED LEAVEExamplesincludematernityleave,familyleave,etc.RequestsshouldbedirectedtotheAssociateDeanforStudentAffairs.TheAssociateDeanforClinicalSciencesshouldbecopiedonallcorrespondence.

Aleaveofabsencemaybegrantedfromthecollegeforoneofthefollowingfive(5)reasons:1. Amedicalemergency2. Afinancialemergency3. Afamilyemergency4. Pursuitofanacademicendeavorotherthantherequiredclinicalcoursework5. Conferenceattendance.

Extendedleaveofabsencecanbegrantedonlytostudentsin good standing,asdefinedinStudentResponsibilities,section7.h.ofthismanual.

Studentsthatrequiretimeawayfromrotations,thatwouldjeopardizetheattendancerequirement,mayrequestindividualconsiderationfromtheAssociateDeanforClinicalSciences.

MATERNITY LEAVEUncomplicatedpregnancydoesnotrequireaDean’sletter.Timeawayfromrotationsofuptosixty(60)daysmaybegranted.Additionaldayspriortoorpostdeliverywillneedadoctor’sletter.Inthosecases,anadditionaldoctor’sletterconcerningfitnesstoresumethedutiesofamedicalstudentwillbeneededtoreturntoclinicalrotations.Thedaysawayfromrotationswillneedtobemadeuppriortoreceiptofadiploma.

ATTENDANCE AT PROFESSIONAL CONFERENCESKentuckyCollegeofOsteopathicMedicineiscommittedtoprovidingqualitymedicaleducationforourstudents.Thisexperienceincludesexcellenceinacademicandclinicalmedicine,researchandcommunityservice.Inordertomaximizethisprocess,it isfeltthatparticipationinprofessionalmeetingscangreatlyenhanceastudent’sprofessionalandpersonalgrowth.AttendanceofAOA-sponsorednationalmeetings,osteopathicdivisionalsocietymeetingsandAOA/osteopathicspecialtymeetingswillbeprovisionallyapproved.AllothermeetingsmusthaveanindividualrequestandbeapprovedbytheDeanofKentuckyCollegeofOsteopathicMedicine.

1. StudentsonclinicalrotationswishingtoattendaprovisionallyapprovedprofessionalmeetingwillsubmitastudenttravelrequesttotheAssociateDeanforClinicalSciencesatleast30dayspriortothemeetingindicatingthenameandlocationoftheprofessionalmeeting,sponsoringagency,anddatesofprospectiveabsence.

2. Onlyoneprofessionalconferencewillbeallowedperstudentperyearofclinicalrotations.AnydeviationfromthispolicymustbeapprovedbytheAssociateDeanforClinicalSciencesonanindividualbasis.

3. StudentsmustobtainpermissionfromboththeAssociateDeanforClinicalSciencesandthesupervisingpreceptorandbeingoodstanding,seeStudentResponsibilities7.h.,thismanual.

4. TherequestwillbeforwardedtotheDeanofKYCOMforapproval.5. Astudenttravelrequestwillbedeniedifthestudentisonprobation,hasunremediatedfailures,

hasafailinggradeatthetimeoftherequest,orattheDean’sdiscretion.6. Eachstudentwillberequiredtosubmitaone-pageorgreatertypedreportonthemeeting

andthevalueaddedtothestudent’seducation.ThiswillbeforwardedtotheOfficeoftheDeanwithinoneweekofreturn.

7. ParticipationofprofessionalmeetingsoutsideofnormalKYCOMcurriculumisconsideredaprivilegeandhonor.Studentsmustdocumentattendancetothesemeetings.

LEAVE FOR INTERVIEW POLICYThefollowingpolicyhasbeenadoptedregardingresidency/internshipinterviews:

1. Students will be required to submit an Interview Leave Request Form, signed by thepreceptor.

2. Amaximumofthree(3)daysforabsenceispermitted,ifapprovedbytheAssociateDeanforClinicalSciences,andmustberecordedonstudentlogsfortherotation.Absenceinexcessofthethree-daystandardwillresultinan“Incomplete”forthisrotation,untilsuchtimethattheactivityrequirementissatisfied.Studentsthatrequiretimeawayfromtherotation,thatwouldjeopardizetheattendancepolicy,mayrequestforindividualconsiderationfromtheAssociateDeanforClinicalSciences.

VACATIONS AND HOLIDAYSThewinterholidayeachyearandtheclassmeetingeachyeararetheonlyapprovedleavesfromclinicalrotations.ClinicalserviceattendanceduringreligiousornationalholidaysisatthediscretionoftheSupervisingPhysician,hospitalorclinicfacility.Therearenodesignatedreligiousand/ornationalholidaysapprovedbyKYCOMduringtheclinicalrotations.Novacationtimebeyondwhathasalreadybeendescribed,willbescheduledduringtheclinicalrotationperiod.

GeneRal RoTaTIons InfoRMaTIon Student responsibilities listed below are expected of all KYCOM students, and subject to individual hospital policies:

1. Studentswillwritedailynotesonpatientsduringrounds.2. Studentwillbepreparedtopresenttheirpatientsonrounds.3. Studentswillhaveallpatientchartsonroundsandhavepreparedamaintainedup-to-date

flow chart,withlabs,medications,andotherpertinentdata.4. Students will gather medical histories and conduct physical and osteopathic structural

bloodorbodyfluids–bysharpsinjuryorexposuretomucousmembranes/skin–willtakethefollowingstepsimmediately.a. PERFORMBASICFIRSTAID:IMMEDIATELYcleanthewoundandskinwithsoap

andrunningwater.Flushanymucousmembranesoreyeswithcopiousamountsofwaterornormalsalineforseveralminutes.Bloodshouldbeallowedtoflowfreelyfromthewound.Bloodshouldnotbesqueezedor“milked”fromthewound.

DressAtalltimes,KYCOMstudentswillactasarepresentativeoftheschool,andbeattentivetopersonalhygiene,cleanlinessandpersonaldemeanor.ShortwhitelabcoatswithKYCOMidentificationareexpectedtobewornunlessspecificallyinstructedbythehospitalorpreceptor.Scrubsuitsaretobewornintheoperatingroom,procedurerooms,duringcallhoursandatthediscretionofindividualphysiciansand/orinstitution/facility.Atalltimes,however,thestudentmustbeclearlyidentifiedasaKYCOMstudent.SeeStudentResponsibilitiessection7.a.ofthismanualforclarification.

AttendancePromptstudentattendanceisexpectedforthe28daysoftherotationperiod.Theactivitiesofeachdaymustberecordedonstudentlogsfortherotation.Amaximumofthree(3)daysforabsenceispermitted,ifapprovedbytheAssociateDeanforClinicalSciences,andmustberecordedonstudentlogsfortherotation.Absenceinexcessofthethree-daystandardwillresultinan“incomplete”forthisrotation,untilsuchtimethattheactivityrequirementissatisfied.

AttendancePromptstudentattendanceisexpectedforthe28daysoftherotationperiod.Theactivitiesofeachdaymustberecordedonstudentlogsfortherotation.Amaximumofthree(3)daysforabsenceispermitted,ifapprovedbytheAssociateDeanforClinicalSciences,andmustberecordedonstudentlogsfortherotation.Absenceinexcessofthethree-daystandardwillresultinan“incomplete”forthisrotation,untilsuchtimethattheactivityrequirementissatisfied.

Absenceoftheattendingphysicianorthestudent,fromthepracticeforaperiodgreaterthanoneday,requiresthestudenttonotifytheofficeforclinicalsciences,oftheinterruptionofactivity.AtthediscretionoftheAssociateDeanforClinicalScienceseitherareadingassignmentoralternatedutyassignmentwillbemade.

Lectures and MeetingsItistheresponsibilityofthestudenttobefamiliarwiththehospital’sdidacticlectureschedule,andtodocumentattendancewithinthestudentlog.Didacticprogramsincludebutarenotlimitedto:MorningReport,JournalClub,Morbidity/MortalityConference,GrandRounds,andNoonConference.Absencefromanyoftheseeventsrequiresthesignatureofthepreceptor,andshouldbesentwiththestudentlogreportstoKYCOM.Itisexpectedthatthestudentwillattendaminimumof70%ofeachkindofevent.

GradingThebasisfortherotationgradeisthe“KYCOMStudentAssessmentForm”forSelective/ElectiveRotations,whichiscompletedbytheattendingphysician,anddiscussedwiththestudentbeforemailing.Uponreceiptandreviewofallinformation,theAssociateDeanforClinicalSciencesevaluatesthematerial,andassignsanumericalgrade.Agradeof70%isconsideredpassing,aspertheClinicalRotationsManual,page15.

Class of 2014 Student Assessment Form Numerical Grade Calculation

There are ten (10) spheres that evaluate for the seven core competencies. Nine of the 10 are graded from “5” (lowest score) to “10” (highest score). Sphere two (2) is divided into two subsets, with each graded from “3” (lowest score) to “5” (highest score). Each of the ten (10) spheres are completed by the preceptor physician, and the assigned rotation grade (that appears on the transcript) is calculated by the Associate Dean for Clinical Sciences. Each of spheres 1-10, can earn a maximum of 10 points. The preceptor physician is instructed that point values, < 7 (< 3.5 for each subset of sphere 2) are “FAILING”. The MAXIMUM total for ten (10) spheres is 100 pts.

Sample Calculation

Step 1: Spheres 1 through ten are totaled.

Step 2: The total from step 1 is divided by the maximum earned points, i.e. 100- [10 x NA], where NA = number of spheres checked non-applicable (NA).

Step 3: The calculated value from step 2 is multiplied by 100.

Step 4: The value from step 3 is the total grade earned for the clinical rotation.

Each of the ten (10) spheres are completed by the preceptor physician, and the assigned rotation grade (that appears on the transcript) is calculated by the Associate Dean for Clinical Sciences. Each of spheres 1-10, can earn a maximum of 10 points. The preceptor physician is instructed that point values, < 7 (< 3.5 for each subset of sphere 2) are “FAILING”. The MAXIMUM total for ten (10) spheres is 100 pts.

Sample Calculation

Step 1: Spheres 1 through ten are totaled.

Step 2: The total from step 1 is divided by the maximum earned points, i.e. 100- [10 x NA], where NA = number of spheres checked non-applicable (NA).

Step 3: The calculated value from step 2 is multiplied by 100.

Step 4: The value from step 3 is the total grade earned for the clinical rotation.
